BACKGROUND: Due to under reporting of ADRs by clinicians, second professional medical under gradu... more BACKGROUND: Due to under reporting of ADRs by clinicians, second professional medical under graduates were sensitized about ADR reporting. METHODS: 'Sensitization of Medical Under graduates for ADR Reporting' (SMUAR Model) was introduced for promoting ADR notification by clinicians. One year prospective study was carried out in a tertiary care hospital with the help of 2 nd Prof MBBS students. The students were asked to collect ADRs from clinical departments. Group of eight students were assigned to visit clinical departments (OPD/IPD) every consecutive month for a year. Naranjo's Scale was calculated for the reported ADRs. RESULTS: 72 ADRs were reported by batch 2010 of our tertiary care teaching hospital from Medicine, Dermatology, Surgery, etc. Maximum numbers of ADRs were reported by Medicine Department. The purpose of introducing the constructivist method was to activate the cognitive domain of the students and promote self learning. A cross sectional qualitative and quantitative study was planned on hundred, second and third professional medical students (III to IX semester) who were screened to participate. Topics discussed with the constructivist method were clinical case discussion, basic and advanced cardiac life support, electrocardiography, and community cardiology. The traditional power point method topics discussed were cardiac surgeries, paediatric cardiology and advanced diagnostic cardiology. Diabetes has been reported to cause an increase in offensive and decrease in defensive gastric mu... more Diabetes has been reported to cause an increase in offensive and decrease in defensive gastric mucosal factors, the imbalance of which can cause ulceration and delay the ulcer healing. Eugenia jambolana has been documented to have both antidiabetic and antiulcer activities. The present study evaluates the effects of ethanolic extract of E. jambolana on gastric ulcer healing and on rat gastric mucosal defensive factors in gastric ulcer with co-occurring diabetes. E. jambolana extract was administered orally in the dose of 200 mg/kg once daily for 10 days. E. jambolana extract increased mucin secretion, mucosal glycoprotein and glutathione levels and decreased the lipid peroxidation in gastric mucosa of diabetic rats. Its treatment also reversed the decrease in life span of gastric mucosal cells as indicated by decreased cell shedding in the gastric juice but found to have no effect on cell proliferation, indicating enhanced defensive status. TO THE EDITOR: The informative article by Weiger and colleagues (1) on complementary cancer thera... more TO THE EDITOR: The informative article by Weiger and colleagues (1) on complementary cancer therapies is an important step in creating dialogue between biomedicine and its alternatives. However, I feel that it repeated certain popular misconceptions about the macrobiotic diet. The authors describe the diet as radical but do not define the scientific baseline from which it is a radical departure. The word radical instead appears to reflect a subjective world-view. I would characterize a diet such as raw-juice fasting as radical because it cannot sustain long-term health and it has never sustained a human community. By these criteria, macrobiotics is not radical. Macrobiotic practitioners argue that their diet resembles traditional diets worldwide, whereas today's U.S. diet is a radical and unhealthy departure from the human norm. The risk that the macrobiotic philosophy may cause rejection of conventional therapy is not inherent in the diet but could apply to any alternative therapy. I would argue that for every patient with cancer who forgoes standard medical treatment in favor of macrobiotics, perhaps thousands forgo healthy dietary and lifestyle changes and instead rely entirely on biomedical intervention to keep them alive and well. What philosophy is influencing them to do that? There is no more risk for malnutrition with macrobiotics than with the typical U.S. diet. Only a minority of Americans eat five or more daily servings of fresh, plant-based foods. In fact, every successive revision of the U.S. Department of Agriculture (USDA)'s food guidelines comes closer to macrobiotic recommendations. Finally, women with reproductive cancer can easily eat macrobiotically without consuming soy phytoestrogens.

Gabapentin has been shown to be effective in various types of neuropathic pain. The primary aim of this study was to evaluate gabapentin as a post-operative analgesic. The study also evaluates the analgesic requirement and safety of gabapentin in post-operative period.Methods: Forty patients undergoing elective laparoscopic cholecystectomy were randomized to receive gabapentin or a matching placebo. The patients of group I received gabapentin 600mg orally 2 hrs before surgery and 12hrs after the first dose. The patients in group II received a matching placebo. Patients in both groups received diclofenac sodium 75mg i.m b.i.d for pain. Background: Tuberculosis (TB) is a common and often deadly infectious disease caused by various s... more Background: Tuberculosis (TB) is a common and often deadly infectious disease caused by various strains of mycobacteria, usually Mycobacterium tuberculosis in humans. A third of the world's population is thought to be infected with M. tuberculosis and new infections occur at a rate of about one per second. The present study aimed to study the prevalence of TB in various organs. Methods: This was both prospective and retrospective study and a total of 14,472 patients of all ages and both the sexes were included in the study. All prospective cases during this period and retrospective cases of past one year i.e. total of 14,472 cases were included in the study. Data was entered in MS-Excel sheet and statistical analysis was done. Results: Out of 14,472 patients maximum number of cases received in the department were of female genital tract (FGT) infections 3634 (25.11%) followed by gastrointestinal tract (GIT) 2246 (15.51%) and then reticuloendothelial system (RES) 1807 (12.48%). 9471 (65.44%) cases were observed with non-neoplastic lesions and 5001 (34.56%) with neoplastic lesions. The number of inflammatory and tubercular cases with non-neoplastic lesions was 4338. Out of 4338 inflammatory cases, 284 cases were reported with TB in various systems. The maximum cases 86 (30.28%) were seen in the third decade followed by fourth decade 55 (19.36%). Youngest patient was 2 year old while the oldest was 80 years old. Incidence of TB was low in 6th and 7th decade of life. Conclusions: The incidence of pulmonary and extrapulmonary TB in our study population was significantly high. Hence further investigation was required to determine a proper diagnostic approach of its morphological pattern so that accurate treatment can be opted.

Drug abuse is a major concern in the athletic world. The misconception among athletes and their c... more Drug abuse is a major concern in the athletic world. The misconception among athletes and their coaches is that when an athlete breaks a record it is due to some "magic ingredient" and not because of training, hard work, mental attitude and championship performance. The personal motivation to win in competitive sports has been intensified by national, political, professional and economic incentives. Under this increased pressure athletes have turned to finding this "magic ingredient". Athlete turns to mechanical (exercise, massage), nutritional (vitamins, minerals), pharmacological (medicines) or gene therapies to have an edge over other players. The World Anti-Doping Agency (WADA) has already asked scientists to help find ways to prevent gene therapy from becoming the newest form of doping. The safety of the life of athletes is compromised with all forms of doping techniques, be it a side effect of a drug or a new technique of gene doping.

A cross sectional observational study was conducted at a tertiary care centre. One hundred ninty three patients diagnosed with DM were enrolled in the study but 14 participants didn't turn up with 24 hours urine sample. Thus, 179 people actually participated in the study. All DM patients who attended Out Patient Departments (OPDs) and In Patient Departments (IPDs) of Medicine, Surgery and Orthopaedics, were enrolled. Present study includes the effect of ethanolic extract of seeds of E. jambolana (EJE) against gastric ulcers induced by 2 h cold restraint stress (CRS), aspirin (ASP, 200 mg/kg, 4 h), 95% ethanol (EtOH, 1 ml/200 g, 1 h) and 4 h pylorus ligation (PL) in rats. To ascertain the mechanism of action of EJE, its effect was studied on mucosal offensive acid-pepsin secretion, lipid peroxidation (LPO, free radical) and defensive mucin secretion, cell proliferation, glycoprotein and glutathione (GSH, an antioxidant). Acute and subacute toxicity studies were also conducted for the safety profile of Eugenia jambolana. The present study was conducted to evaluate the role of gabapentin in attenuation of these hemodynamic changes. Methods: Forty patients undergoing elective laparoscopic cholecystectomy under general anesthesia with standardized premedication and anesthetics were randomized to receive gabapentin or a matching placebo. The patients of Group I received gabapentin 600 mg orally 2 hrs before surgery and patients in Group II received a matching placebo. Patient's HR, systolic BP (SBP), diastolic BP (DBP), mean BP (MBP), were monitored before and after 1, 2, 5, and 10 mins of endotracheal intubation. Results: Comparison of SBP, DBP, and MBP at 1, 2, 5 and 10 mins after endotracheal intubation showed statistically significant attenuation in the gabapentin group when compared to placebo. Changes in the HR were not significant. Conclusion: Gabapentin 600 mg, given 2 hrs before induction is effective in attenuating the pressor response to laryngoscopy and tracheal intubation.

There is considerable healing power in a good Physician-patient relationship. In the field of healthcare, patient empowerment has been acknowledged as an alternative to compliance in order to guide the provider-patient relationship. It will help patients' confusion, fear and doubt slowly transform into clarity, relief and assurance. With the positive role of physicians, patients will definitely be relieved of hopelessness, have higher satisfaction, better adherence and improved health. There is no doubt that this small gesture by physicians will be a precious gift to humanity.

A cross sectional observational study was conducted at a tertiary care centre. One hundred ninty three patients diagnosed with DM were enrolled in the study but 14 participants didn't turn up with 24 hours urine sample. Thus, 179 people actually participated in the study. All DM patients who attended Out Patient Departments (OPDs) and In Patient Departments (IPDs) of Medicine, Surgery and Orthopaedics, were enrolled. Proper history about development and duration of DM was taken from the patients. Present study includes the effect of ethanolic extract of seeds of E. jambolana (EJE) against gastric ulcers induced by 2 h cold restraint stress (CRS), aspirin (ASP, 200 mg/kg, 4 h), 95% ethanol (EtOH, 1 ml/200 g, 1 h) and 4 h pylorus ligation (PL) in rats. To ascertain the mechanism of action of EJE, its effect was studied on mucosal offensive acid-pepsin secretion, lipid peroxidation (LPO, free radical) and defensive mucin secretion, cell proliferation, glycoprotein and glutathione (GSH, an antioxidant). Acute and subacute toxicity studies were also conducted for the safety profile of Eugenia jambolana. EJE 200 mg/kg, when administered orally for 10 days in rats was found to reduce the ulcer index in all gastric ulcer models. The present study was conducted to evaluate the role of gabapentin in attenuation of these hemodynamic changes. Methods: Forty patients undergoing elective laparoscopic cholecystectomy under general anesthesia with standardized premedication and anesthetics were randomized to receive gabapentin or a matching placebo. The patients of Group I received gabapentin 600 mg orally 2 hrs before surgery and patients in Group II received a matching placebo. Patient's HR, systolic BP (SBP), diastolic BP (DBP), mean BP (MBP), were monitored before and after 1, 2, 5, and 10 mins of endotracheal intubation. Results: Comparison of SBP, DBP, and MBP at 1, 2, 5 and 10 mins after endotracheal intubation showed statistically significant attenuation in the gabapentin group when compared to placebo. Changes in the HR were not significant. Conclusion: Gabapentin 600 mg, given 2 hrs before induction is effective in attenuating the pressor response to laryngoscopy and tracheal intubation.

It will help patients' confusion, fear and doubt slowly transform into clarity, relief and assurance. With the positive role of physicians, patients will definitely be relieved of hopelessness, have higher satisfaction, better adherence and improved health. There is no doubt that this small gesture by physicians will be a precious gift to humanity.
