Metabolic dysfunction-associated fatty liver disease (MAFLD) is a novel term to describe new diag... more Metabolic dysfunction-associated fatty liver disease (MAFLD) is a novel term to describe new diagnostic criteria for diagnosing fatty liver disease free of alcohol intake and associated viral hepatitis infection. These new diagnostic criteria, make it easier to diagnose than nonalcoholic fatty liver disease (NAFLD). Although we may not have a lot of studies about MAFLD yet, we have plenty of evidence about IR, metabolic syndrome and dysfunction, and NAFLD. In this review, we try to spotlight some important and updated knowledge about MAFLD and its related conditions, to help us better understand and deal with this global problem.

The major corporate collapses and related frauds which occurred in Nigeria and around the world h... more The major corporate collapses and related frauds which occurred in Nigeria and around the world have raised doubts about the credibility of the operating and financial reporting practices of quoted companies in Nigeria. This stirred a number of professional and regulatory organizations to recommend reforms that will improve transparency in financial reporting and thereby increase audit quality and corporate governance practices. Although evidence of corporate governance practices and audit quality exists from developed economies, very scanty studies have been conducted in Nigeria where corporate governance is just evolving. Therefore, this study provides evidence on corporate governance, audit quality, and firm related attributes from a developing country, Nigeria. Logistic regression was used in investigating the questions that were raised in the study. Findings from the study show that ownership by non-executive director has the possibility of increasing the quality of auditing. Evidence also exist that size of the company and business leverage are important factors in audit quality for companies quoted on the Nigerian Stock Exchange. The study suggests that the composition of non-executive directors as members of the board should be sustained and improved upon in order to enhance audit quality.

The aim of this study was to examine the strategy, feasibility and outcome of neo-adjuvant chemot... more The aim of this study was to examine the strategy, feasibility and outcome of neo-adjuvant chemotherapy, with doxorubicin and ifosfamide, in adult patients with 'high-risk' soft-tissue sarcomas. Patients with 'high-risk' soft-tissue sarcomas, defined as tumours 58 cm of any grade, or grade II/III tumours < 8 cm, or grade II/III locally recurrent tumours, or grade II/III tumours with inadequate surgery performed in the previous 6 weeks and therefore requiring further surgery, were randomised between either surgery alone or three cycles of 3-weekly doxorubicin 50 mg/m 2 intravenous (i.v.) bolus and ifosfamide 5 g/m 2 (24 h infusion) before surgery. The type of surgery had to be planned at randomisation. Tumours were to be amenable to surgery by amputation, compartmental resection, wide or marginal excision. If chemotherapy was given, surgery had to be performed within 21 days after the last chemotherapy. Patients received postoperative radiotherapy in cases of marginal surgery, microscopically incomplete resection and no further possibility for surgery, and in cases of surgery because of local recurrence. 150 patients were entered into the study and 134 were eligible, 67 in each arm. The most frequent side-effects of chemotherapy were alopecia, nausea and vomiting (95%), and leucocytopenia (32%). One patient died of neutropenic fever after the first cycle of chemotherapy. Chemotherapy did not interfere with planned surgery and did not affect postoperative wound healing. Limb-salvage was achieved in 88%, amputation was necessary in 12% (all according to the plan at randomisation). The trial was closed after completion of phase II, since accrual was too slow to justify expanding the study into the scheduled phase III study. At a median follow-up of 7.3 years, the 5 year diseasefree survival is estimated at 52% for the no chemotherapy and 56% for the chemotherapy arm (standard error: 7%) (P=0.3548). The 5 year overall survival for both arms is 64 and 65%, respectively (standard error 7%) (P=0.2204). Neo-adjuvant-chemotherapy with doxorubicin and ifosfamide at these doses and with this schedule was feasible and did not compromise subsequent treatment, surgery with or without radiotherapy. Although not powered to draw definitive conclusions on benefit, but with an at least 7 year median follow-up, the results render it less likely that major survival benefits will be achieved with this type of chemotherapy.

Thirty patients with a diagnosis of metastatic adenocarcinoma of the lung were entered on a trial... more Thirty patients with a diagnosis of metastatic adenocarcinoma of the lung were entered on a trial to evaluate the antitumor efficacy of 5-fluorouracil 370 mg/m2 daily for 5 days every four weeks in combination with folinic acid 200 mg/m2, 60 min prior to 5FU. All patients had a good performance status, bidimensionally measurable disease, and weight loss less than or equal to 5% of preillness weight. Of the 29 evaluable patients, only two (7%) had partial responses (95% confidence limits 1-24%). Eleven (38%) had stable disease and 16 (55%) progressed. The two responding patients survived 12 and 60+ weeks. The median survival of all evaluable patients was 25 weeks (range 7-60+) and that of the stable patients was 26 weeks. The principal toxicities observed were diarrhea and stomatitis. Myelosuppression was rarely dose limiting. In contrast to the results of treatment with 5FU and folinic acid in metastatic colorectal cancer and breast cancer, the results of treatment with this combination of agents have been much less encouraging in adenocarcinoma of the lung.

AP cells prepared from 8-11-month-old castrated calves were incubated for 12 h with estradiol (E2, 10 -8 mol/L),progesterone (P4, 10 -8 mol/L), testosterone (T, 10 -8 mol/L), or vehicle only (control), and then for 2 h with Kp10. The amount of GH released in the medium was measured by a time-resolved fluoroimmunoassay. Kp10 (10 -6 or 10 -5 mol/L) significantly stimulated the secretion of GH from the AP cells regardless of steroid treatments (P < 0.05), and E2, P4, and T had no effect on this response. The GH-releasing response to growth hormone-releasing hormone (GHRH, 10 -8 mol/L) was significantly greater than that to Kp10 (P < 0.05). The present results suggest that Kp10 directly stimulates the release of GH from somatotrophic cells and sex steroid hormones do not enhance the sensitivity of these cells to Kp10. Furthermore, they suggest that the GH-releasing effect of Kp10 is less potent than that of GHRH.

Traffic congestion has become a great challenge and a large burden on both the governments and th... more Traffic congestion has become a great challenge and a large burden on both the governments and the citizens in vastly populated cities. The main problem, originally initiated by several factors, continues to threaten the stability of modern cities and the livelihood of its habitants. Hence, improving the control strategies that govern the traffic operations is a powerful solution that can solve the congestion problem. These improvements can be achieved by enhancing the traffic control performance through adjusting the traffic signal timings. This paper focuses on finding various solutions for the addressed problem through the optimization using simulation of traffic signal timings under oversaturated conditions. The system under study is an actual road network in Alexandria, Egypt; where, numerous data have been collected in different time intervals. A series of computer simulation models to represent the actual system as well as proposed solutions have been developed using the ExtendSim simulation environment. Furthermore, an evolutionary optimizer is utilized to attain a set of optimum/near-optimum signal timings to minimize the total time in system of the vehicles, resulting in an improved performance of the road network. Analysis of experimentation results shows that the adopted methodology optimizes the vehicular flow in a multiple-junction urban traffic network.

The main problem, originally initiated by several factors, continues to threaten the stability of modern cities and the livelihood of its habitants. Hence, improving the control strategies that govern the traffic operations is a powerful solution that can solve the congestion problem. These improvements can be achieved by enhancing the traffic control performance through adjusting the traffic signal timings. This paper focuses on finding various solutions for the addressed problem through the optimization using simulation of traffic signal timings under oversaturated conditions. The system under study is an actual road network in Alexandria, Egypt; where, numerous data have been collected in different time intervals. A series of computer simulation models to represent the actual system as well as proposed solutions have been developed using the ExtendSim simulation environment. Furthermore, an evolutionary optimizer is utilized to attain a set of optimum/near-optimum signal timings to minimize the total time in system of the vehicles, resulting in an improved performance of the road network. Analysis of experimentation results shows that the adopted methodology optimizes the vehicular flow in a multiple-junction urban traffic network.
