OBJECTIVE: To determine the efficacy of complete GSV stripping in terms of morbidity, nerve injur... more OBJECTIVE: To determine the efficacy of complete GSV stripping in terms of morbidity, nerve injury and recurrence rates. METHODS: 42 patients with uncomplicated unilateral and/or bilateral varicosities involving the great saphenous system were included in this study. Patients with primary and/or recurrent varicose veins associated with active or healed ulcers, patients with bleeding diathesis and those who failed to sign the proforma for regular follow-ups were excluded from this study. Complete stripping of great saphenous vein up to the ankle, together with multiple phlebectomies was performed in all patients. Postoperatively, limbs were examined for complications like bruising, edema, wound site infections and sensory abnormalities. All the patients were followed up for the period of one year for recurrence and improvement in sensory abnormalities. RESULTS: All the patients belonged to CEAP class 2 or greater. STUDY SETTING: Surgical departments of Liaquat University Hospital. STUDY DURATION: Five years from January 2000 to December 2004. SAMPLING: Non-probable purposive. INCLUSION CRITERIA: All patients of cervical lymph node enlargement of more than four weeks duration irrespective of age and sex. EXCLUSION CRITERIA: Cervical lymph node less than 1cm. STUDY VARIABLES: Study variables used were age, sex, symptom, clinical finding, stage and histopathology. DATA ANALYSIS: Data analysis was done by using SPSS version 10. RESULTS: A total of 500 patients with enlarged cervical lymph nodes were studied, amongst them 40 (8%) patients were diagnosed with Hodgkin's lymphoma (32 males and 8 females). Mean age of 40 patients was 23 years (range 9-44 years). Out of these 40 patients, in 8 (20%) patients it was difficult to differentiate lymphoma clinically from tuberculosis on clinical examination. Cervical swelling was the commonest symptom (100%) while fever, night sweats and weight-loss were present in 45% patients, 55% of patients were in stage III & IV (advance disease). Mixed cellularity was the commonest histological variant. All 40 patients after diagnosis and staging were referred to Nuclear Institute of Medical Radiotherapy for further management. CONCLUSION: Although tuberculous cervical lymph node enlargement is very common, but Hodgkin's lymphoma has a definite possibility of occurrence (8% in our study). Therefore all patients with enlarged cervical lymph node should not be treated with anti-tuberculosis therapy on empirical basis. Diagnosis should be confirmed by biopsy, otherwise patients may land up with advanced stage of Hodgkin's lymphoma with poor prognosis.

Através da história, passando pelo poder da Igreja, dos reis e imperadores, o poder de punir (corrigir) é delegado, hoje, no Brasil ao Estado. Não obstante ao elevado grau de atualidade da legislação brasileira com relação ao meio ambiente, muitas são as dúvidas que os operadores da lei têm encontrado ao analisar os casos nessa área. De fato, a complexidade desse assunto passa, também, pela questão social e econômica indo desembocar, em muitos casos, na produção agrícola ou nas profundezas de um lençol freático, por exemplo. Entretanto, não é o próprio meio ambiente o destino final daquilo que denominamos de contaminação, senão o próprio homem. Se de um lado o ciclo de um contaminante parece claro, o próprio homem não pode ser considerado somente como um ser biológico, mas, sem dúvida é um ser social e psicológico. Não bastará à lei manter o homem biologicamente vivo, se este não tiver vida social e psicológica. É necessário, portanto, que a legislação ambiental ao ser aplicada, seja trabalhada na sua complexidade, ou seja, realmente envolvendo lei e meio ambiente, sendo o meio ambiente o homem na sua plenitude, e não somente o homem ou o meio ambiente. Two lithographic approaches have been explored for the microfabrication of cellular patterns base... more Two lithographic approaches have been explored for the microfabrication of cellular patterns based on the attachment of marine bacterium Phaeobacter inhibens strain T5. Strain T5 produces a new antibiotic that makes this bacterium potentially interesting for the pharmaceutical market and as a probiotic organism in aquacultures and in controlling biofouling. The microcontact printing (microCP) method is based on the micropatterning of self-assembled monolayers (SAMs) terminated with adhesive end groups such as CH(3) and COOH and nonadhesive groups (e.g., short oligomers of ethylene glycol (OEG)) to form micropatterned substrates for the adhesion of strain T5. The scanning probe lithographic method is based on the surface modification of OEG SAM by using a microelectrode, the probe of a scanning electrochemical microscope (SECM). Oxidizing agents (e.g., Br(2)) were electrogenerated in situ at the microelectrodes from Br(-) in aqueous solution to remove OEG SAMs locally, which allows the subsequent adsorption of bacteria. Various micropatterns of bacteria could be formed in situ on the substrate without a prefabricated template. The fabricated cellular patterns may be applied to a variety of marine biological studies that require the analysis of biofilm formation, cell-cell and cell-surface interactions, and cell-based biosensors and bioelectronics.

Laterally structured, self-assembled monolayers (SAMs) of different thiols (HS-R-X, R = (CH 2) 3-... more Laterally structured, self-assembled monolayers (SAMs) of different thiols (HS-R-X, R = (CH 2) 3-16, X = -CH 3, -COOH, -NH 2) on gold have been prepared by inkjet printing. The printer is a modified, low-cost desktop printer (Epson Stylus Photo R200), the ink is a 1 mM solution of the thiol in ethanol/glycerol (6:1). The quality of inkjet-printed large area SAMs obtained in this study is between that of a layer self-assembled from a thiol solution and that obtained by soft lithography, according to cyclic voltammetry, electrochemical impedance spectroscopy, scanning electrochemical microscopy (SECM), and polarization-modulated Fourier transform infrared reflection-absorption spectroscopy (PM IRRAS). For the first time, simultaneous printing of two different thiols in a single print job as an alternative to sequential printing and backfilling is demonstrated. The smallest structures consisting of conductive disks of 40 microm diameter were analyzed as single spots by SECM and as random array electrodes with different average disk-disk distance. Conductive band electrodes with variable bandwidth (300 microm to 1 cm) are presented, as well as a pH switchable band structure. As compared to stamping, inkjet printing allows for simultaneous multiple thiol printing in a single print job with the resolution limited only by the droplet size and the precision of the translation stage.

A prospective, analytical, comparative study using randomized controlled trial (RCT) was conducted at Liaquat University Hospital (Jamshoro) & Fatima Hospital-Baqai Medical University (Karachi), from June 2004 to July 2007. In total 49 patients, who either underwent open excision and secondary healing (group A: 25 patients) or rhomboid excision and Limberg flap (group B: 24 patients), were enrolled in the study. Duration of operation, postoperative pain, duration of hospital stay, postoperative complications, and time to recurrence were noted. The inclusion criteria were all patients with primary or recurrent SPD. The exclusion criteria were: cases with incomplete data and those lost to follow-up. STUDY SETTING: Surgical departments of Liaquat University Hospital. STUDY DURATION: Five years from January 2000 to December 2004. SAMPLING: Non-probable purposive. INCLUSION CRITERIA: All patients of cervical lymph node enlargement of more than four weeks duration irrespective of age and sex. EXCLUSION CRITERIA: Cervical lymph node less than 1cm. STUDY VARIABLES: Study variables used were age, sex, symptom, clinical finding, stage and histopathology. DATA ANALYSIS: Data analysis was done by using SPSS version 10. RESULTS: A total of 500 patients with enlarged cervical lymph nodes were studied, amongst them 40 (8%) patients were diagnosed with Hodgkin's lymphoma (32 males and 8 females). Mean age of 40 patients was 23 years (range 9-44 years). Out of these 40 patients, in 8 (20%) patients it was difficult to differentiate lymphoma clinically from tuberculosis on clinical examination. Cervical swelling was the commonest symptom (100%) while fever, night sweats and weight-loss were present in 45% patients, 55% of patients were in stage III & IV (advance disease). Mixed cellularity was the commonest histological variant. All 40 patients after diagnosis and staging were referred to Nuclear Institute of Medical Radiotherapy for further management. CONCLUSION: Although tuberculous cervical lymph node enlargement is very common, but Hodgkin's lymphoma has a definite possibility of occurrence (8% in our study). Therefore all patients with enlarged cervical lymph node should not be treated with anti-tuberculosis therapy on empirical basis. Diagnosis should be confirmed by biopsy, otherwise patients may land up with advanced stage of Hodgkin's lymphoma with poor prognosis.

Através da história, passando pelo poder da Igreja, dos reis e imperadores, o poder de punir (corrigir) é delegado, hoje, no Brasil ao Estado. Não obstante ao elevado grau de atualidade da legislação brasileira com relação ao meio ambiente, muitas são as dúvidas que os operadores da lei têm encontrado ao analisar os casos nessa área. De fato, a complexidade desse assunto passa, também, pela questão social e econômica indo desembocar, em muitos casos, na produção agrícola ou nas profundezas de um lençol freático, por exemplo. Entretanto, não é o próprio meio ambiente o destino final daquilo que denominamos de contaminação, senão o próprio homem. Se de um lado o ciclo de um contaminante parece claro, o próprio homem não pode ser considerado somente como um ser biológico, mas, sem dúvida é um ser social e psicológico. Não bastará à lei manter o homem biologicamente vivo, se este não tiver vida social e psicológica. É necessário, portanto, que a legislação ambiental ao ser aplicada, seja trabalhada na sua complexidade, ou seja, realmente envolvendo lei e meio ambiente, sendo o meio ambiente o homem na sua plenitude, e não somente o homem ou o meio ambiente. Two lithographic approaches have been explored for the microfabrication of cellular patterns base... more Two lithographic approaches have been explored for the microfabrication of cellular patterns based on the attachment of marine bacterium Phaeobacter inhibens strain T5. Strain T5 produces a new antibiotic that makes this bacterium potentially interesting for the pharmaceutical market and as a probiotic organism in aquacultures and in controlling biofouling. The microcontact printing (microCP) method is based on the micropatterning of self-assembled monolayers (SAMs) terminated with adhesive end groups such as CH(3) and COOH and nonadhesive groups (e.g., short oligomers of ethylene glycol (OEG)) to form micropatterned substrates for the adhesion of strain T5. The scanning probe lithographic method is based on the surface modification of OEG SAM by using a microelectrode, the probe of a scanning electrochemical microscope (SECM). Oxidizing agents (e.g., Br(2)) were electrogenerated in situ at the microelectrodes from Br(-) in aqueous solution to remove OEG SAMs locally, which allows the subsequent adsorption of bacteria. Various micropatterns of bacteria could be formed in situ on the substrate without a prefabricated template. The fabricated cellular patterns may be applied to a variety of marine biological studies that require the analysis of biofilm formation, cell-cell and cell-surface interactions, and cell-based biosensors and bioelectronics.

Laterally structured, self-assembled monolayers (SAMs) of different thiols (HS-R-X, R = (CH 2) 3-... more Laterally structured, self-assembled monolayers (SAMs) of different thiols (HS-R-X, R = (CH 2) 3-16, X = -CH 3, -COOH, -NH 2) on gold have been prepared by inkjet printing. The printer is a modified, low-cost desktop printer (Epson Stylus Photo R200), the ink is a 1 mM solution of the thiol in ethanol/glycerol (6:1). The quality of inkjet-printed large area SAMs obtained in this study is between that of a layer self-assembled from a thiol solution and that obtained by soft lithography, according to cyclic voltammetry, electrochemical impedance spectroscopy, scanning electrochemical microscopy (SECM), and polarization-modulated Fourier transform infrared reflection-absorption spectroscopy (PM IRRAS). For the first time, simultaneous printing of two different thiols in a single print job as an alternative to sequential printing and backfilling is demonstrated. The smallest structures consisting of conductive disks of 40 microm diameter were analyzed as single spots by SECM and as random array electrodes with different average disk-disk distance. Conductive band electrodes with variable bandwidth (300 microm to 1 cm) are presented, as well as a pH switchable band structure. As compared to stamping, inkjet printing allows for simultaneous multiple thiol printing in a single print job with the resolution limited only by the droplet size and the precision of the translation stage.

A prospective, analytical, comparative study using randomized controlled trial (RCT) was conducted at Liaquat University Hospital (Jamshoro) & Fatima Hospital-Baqai Medical University (Karachi), from June 2004 to July 2007. In total 49 patients, who either underwent open excision and secondary healing (group A: 25 patients) or rhomboid excision and Limberg flap (group B: 24 patients), were enrolled in the study. Duration of operation, postoperative pain, duration of hospital stay, postoperative complications, and time to recurrence were noted. The inclusion criteria were all patients with primary or recurrent SPD. The exclusion criteria were: cases with incomplete data and those lost to follow-up.
