Severe hypertriglyceridemia is a rare cause of acute pancreatitis especially in non-alcoholic, no... more Severe hypertriglyceridemia is a rare cause of acute pancreatitis especially in non-alcoholic, non- gallstones, non-obese, non-diabetic patients and has a few options for its management. Hypertriglyceridemia-induced pancreatitis causes 1 to 14 percent of all cases of acute pancreatitis. The risk of developing acute pancreatitis is approximately 5 percent with serum triglycerides >1000 mg/dL and 10 to 20 percent with triglycerides >2000 mg/dL. This is the case of a 63-year-old female who came to the Emergency Department with acute onset of abdominal pain, located at the mid-epigastric region, radiating to back, 10/10 in a scale of severity, no alleviating or aggravating factor, accompanied by nausea, 5 episodes of bilious emesis. Past medical history includes recurrent acute pancreatitis induced by hypertriglyceridemia (3 episodes last year), arterial hypertension, and dyslipidemia treated with Fenofibrate and Atorvastatin. Tradicionalmente la actividad agraria estuvo asociada a mercados de trabajo rurales pero, en la actualidad, este sector demanda nuevos puestos de trabajo que tienen un origen en tareas típicas de los mercados de trabajo urbanos. Estos cambios también están asociados a transformaciones en los patrones residenciales de la mano de obra ocupada en el sector. The C5 inhibitors eculizumab (ECU) and ravulizumab (RAV) prevent IVH by inhibiting terminal complement; however, some C5 inhibitor-treated patients may experience persistent anemia due to extravascular hemolysis (EVH) caused by C3 fragment deposition and opsonization driven by constitutive activation of the alternative pathway (AP). Inhibition of factor D (FD), the rate limiting enzyme of the AP, acts on the complement cascade upstream of C3. Danicopan (ALXN2040, ACH-4471) is a first-in-class oral small molecule FD inhibitor. In vitro studies with RBCs collected from PNH patients have shown that danicopan not only inhibited hemolysis but also prevented deposition of C3 fragments on PNH RBCs. For several decades, the field of magneto-optics (MO) has demonstrated applications that have imp... more For several decades, the field of magneto-optics (MO) has demonstrated applications that have impact on every day applications such as in optical data storage, magnetic field sensing, crucial for magnetoencephalography and magnetocardiography; and compact and efficient optical isolators, among others. In the past, many of these applications and the devices designed for them have heavily relied on inorganic materials. Organic materials with a high MO response represent an interesting alternative to the inorganic equivalent by not only being a more cost efficient solution, but also by allowing the user to modify a number of variables to control and optimize the MO performance depending on the application and level of performance desired. In this dissertation I discuss the MO properties of novel organic materials, starting with polythiophene, which has been of interest due to the strong relationship between its high MO performance and its lamellar structure and regioregularity. I will also be discussing another material system that provides several degrees of MO tunability: magnetite based nanocomposites. A unique and novel synthetic approach described in this dissertation yields both highly transparent and MO responsive polymer films. I will be describing a systematic approach that indicates a strong influence of the size of the nanoparticle as well as the nanoparticle concentration in the MO performance of the bulk polymer, while maintaining high optical quality with minimal scattering and absorption in the visible and near infrared. Finally, I will be discussing the implementation of both a magnetite nanocomposite and a cobalt ferrite based nanocomposite in a free space magnetic field system and demonstrate the proof-of-principle operation of a sensing system

The article addresses some forms of sexual violence suffered by Latin American women during migra... more The article addresses some forms of sexual violence suffered by Latin American women during migratory transit through Mexico. In-depth, semi-structured, qualitative–phenomenological interviews were conducted with 10 women in different stages of major transmigration states. The findings demonstrate the ubiquity of sexual violence during the migration. Five themes were identified: (a) The migration dimension of “being a woman”; (b) Recognition of contextual factors associated with the migratory process: “Intermediaries, road, shelters and sisterhood”; (c) The costs of migration: Abuse, discrimination, and persecution: “Criminal groups, physical violence, and “La Bestia”; (d) Triggers and supports: “Escape and future expectations”; and (e) God as a support.

Included patients had a diagnosis of SCD or thalassemia, were 2-63 years of age, and were continuously enrolled in Texas Medicaid during the study period. Kruskall Wallis and Chi-square tests were used for continuous and categorical variables, respectively. Results: A total of 201 Texas Medicaid patients met the study criteria (24.4% DFX-DT-Only, 20.4% DFX-FCT-Only, 55.2% Switched). Patients who switched from DFX-DT to DFX-FCT had fewer hospitalizations and emergency department (ED) visits compared to DFX-DT-Only and DFX-FCT-Only groups (P< 0.001). Annual inpatient expenditures were lower for patients who switched compared to DFX-DT-Only and DFX-FCT-Only patients (P= 0.001). Mean annual medical expenditures (inpatient, ED, outpatient visits) were lowest for patients who switched, followed by DFX-FCT-Only patients (+11.5%) and DFX-DT-Only patients (+35.9%). ConClusions: In study patients taking DFX, medical resource utilization and expenditures varied by type of DFX (DT vs. FCT). The patient group that switched from DFX-DT to DFX-FCT had fewer hospitalizations, fewer ED visits, and lower medical expenditures. To optimize patient health outcomes and resource use, health care providers and decision-makers should be aware of differences in these measures by DFX formulation.

The study enrolled 30 previously treated boys younger than 12 years with haemophilia B (≤2 IU/dL [≤2%] endogenous coagulation factor IX [FIX] activity). All patients were initially given rFIXFc prophylaxis (50-60 IU/kg) once per week with adjustments to dose (≤100 IU/kg per infusion) or dosing frequency (up to two times per week) as needed. The primary outcome measure was development of inhibitors (neutralising antibodies). Here, we report interim, long-term safety and efficacy data from B-YOND, the rFIXFc extension study. Eligible subjects who completed B-LONG or Kids B-LONG could enrol in B-YOND. There were four treatment groups: weekly prophylaxis (20-100 IU/kg every 7 days), individualised prophylaxis (100 IU/kg every 8-16 days), modified prophylaxis (further dosing personalisation to optimise prophylaxis), and episodic (on-demand) treatment. Introduction: The safety, efficacy and prolonged half-life of recombinant factor VIII Fc fusion p... more Introduction: The safety, efficacy and prolonged half-life of recombinant factor VIII Fc fusion protein (rFVIIIFc) in previously treated patients with severe haemophilia A was demonstrated in the phase 3 ALONG and Kids ALONG studies. Here, we report interim safety and efficacy data from the rFVIIIFc extension study, ASPIRE (ClinicalTrials.gov #NCT01454739). Methods: Eligible subjects could enrol in ASPIRE upon completing ALONG or Kids ALONG. There were four treatment groups: individualized prophylaxis; weekly prophylaxis; modified prophylaxis (for subjects in whom optimal treatment could not be achieved with individualized or weekly prophylaxis); and episodic treatment. The primary endpoint was development of inhibitors. Results: A total of 150 ALONG subjects and 61 Kids ALONG subjects enrolled in ASPIRE. As of the interim data cut (6 January 2014), the median time on study was 80.9 (A-LONG) and 23.9 (Kids ALONG) weeks. The majority of subjects (A-LONG, 92.0%; Kids ALONG , 57.4%) had ≥100 cumulative rFVIIIFc exposure days. No inhibitors were observed. Adverse events were generally consistent with those expected in the general haemophilia A population. Median annualized bleeding rates (ABRs) were low with individualized [A-LONG: 0.66; Kids ALONG: 0.00 (<6 years old), 1.54 (6 to <12 years old)], weekly (A-LONG: 2.03) and modified (A-LONG: 1.97) prophylaxis. There was no change in prophylactic infusion frequency or total weekly prophylactic dose in the majority of subjects from ALONG and Kids ALONG. Conclusion: Interim data from ASPIRE confirm the long-term safety of rFVIIIFc and the maintenance of a low ABR with extended-interval prophylactic dosing in patients with severe haemophilia A.

Humans, for various reasons, seem to have a specific preference for salt which is consumed in excess of need and this has been characterized as an important contributor to hypertension. Salt intake is related to the salt taste sensitivity threshold and the salt good taste level. Gustatory sensibility responds to various physiological mechanisms and salt taste is directly modified by cultural and socio-economical factors. We measured the salt taste sensitivity threshold and salt good taste level of a young student population. Zinc homeostasis is achieved after intake variation by changes in the expression levels of zinc t... more Zinc homeostasis is achieved after intake variation by changes in the expression levels of zinc transporters. The aim of this study was to evaluate dietary intake (by 24-h recall), absorption, plasma zinc (by absorption spectrophotometry) and the expression levels (by quantitative PCR), of the transporters ZIP1 (zinc importer) and ZnT1 (zinc exporter) in peripheral white blood cells from 24 adolescent girls before and after drinking zinc-fortified milk for 27 day. Zinc intake increased (p < 0.001) from 10.5 ± 3.9 mg/day to 17.6 ± 4.4 mg/day, and its estimated absorption from 3.1 ± 1.2 to 5.3 ± 1.3 mg/day. Mean plasma zinc concentration remained unchanged (p > 0.05) near 150 µg/dL, but increased by 31 µg/dL (p < 0.05) for 6/24 adolescents (group A) and decreased by 25 µg/dL (p < 0.05) for other 6/24 adolescents (group B). Expression of ZIP1 in blood leukocytes was reduced 1.4-fold (p < 0.006) in group A, while for the expression of ZnT1 there was no difference after intervention (p = 0.39). An increase of dietary zinc after 27-days consumption of fortified-milk did not increase (p > 0.05) the plasma level of adolescent girls but for 6/24 participants from group A in spite of the formerly appropriation, which cellular zinc uptake decreased as assessed by reduction of the expression of ZIP1.

Included patients had a diagnosis of SCD or thalassemia, were 2-63 years of age, and were continuously enrolled in Texas Medicaid during the study period. Kruskall Wallis and Chi-square tests were used for continuous and categorical variables, respectively. Results: A total of 201 Texas Medicaid patients met the study criteria (24.4% DFX-DT-Only, 20.4% DFX-FCT-Only, 55.2% Switched). Patients who switched from DFX-DT to DFX-FCT had fewer hospitalizations and emergency department (ED) visits compared to DFX-DT-Only and DFX-FCT-Only groups (P< 0.001). Annual inpatient expenditures were lower for patients who switched compared to DFX-DT-Only and DFX-FCT-Only patients (P= 0.001). Mean annual medical expenditures (inpatient, ED, outpatient visits) were lowest for patients who switched, followed by DFX-FCT-Only patients (+11.5%) and DFX-DT-Only patients (+35.9%). ConClusions: In study patients taking DFX, medical resource utilization and expenditures varied by type of DFX (DT vs. FCT). The patient group that switched from DFX-DT to DFX-FCT had fewer hospitalizations, fewer ED visits, and lower medical expenditures. To optimize patient health outcomes and resource use, health care providers and decision-makers should be aware of differences in these measures by DFX formulation.

Introduction: The safety, efficacy and prolonged half-life of recombinant factor VIII Fc fusion p... more Introduction: The safety, efficacy and prolonged half-life of recombinant factor VIII Fc fusion protein (rFVIIIFc) in previously treated patients with severe haemophilia A was demonstrated in the phase 3 ALONG and Kids ALONG studies. Here, we report interim safety and efficacy data from the rFVIIIFc extension study, ASPIRE (ClinicalTrials.gov #NCT01454739). Methods: Eligible subjects could enrol in ASPIRE upon completing ALONG or Kids ALONG. There were four treatment groups: individualized prophylaxis; weekly prophylaxis; modified prophylaxis (for subjects in whom optimal treatment could not be achieved with individualized or weekly prophylaxis); and episodic treatment. The primary endpoint was development of inhibitors. Results: A total of 150 ALONG subjects and 61 Kids ALONG subjects enrolled in ASPIRE. As of the interim data cut (6 January 2014), the median time on study was 80.9 (A-LONG) and 23.9 (Kids ALONG) weeks. The majority of subjects (A-LONG, 92.0%; Kids ALONG , 57.4%) had ≥100 cumulative rFVIIIFc exposure days. No inhibitors were observed. Adverse events were generally consistent with those expected in the general haemophilia A population. Median annualized bleeding rates (ABRs) were low with individualized [A-LONG: 0.66; Kids ALONG: 0.00 (<6 years old), 1.54 (6 to <12 years old)], weekly (A-LONG: 2.03) and modified (A-LONG: 1.97) prophylaxis. There was no change in prophylactic infusion frequency or total weekly prophylactic dose in the majority of subjects from ALONG and Kids ALONG. Conclusion: Interim data from ASPIRE confirm the long-term safety of rFVIIIFc and the maintenance of a low ABR with extended-interval prophylactic dosing in patients with severe haemophilia A.

The aim of this study was to evaluate dietary intake (by 24-h recall), absorption, plasma zinc (by absorption spectrophotometry) and the expression levels (by quantitative PCR), of the transporters ZIP1 (zinc importer) and ZnT1 (zinc exporter) in peripheral white blood cells from 24 adolescent girls before and after drinking zinc-fortified milk for 27 day. Zinc intake increased (p < 0.001) from 10.5 ± 3.9 mg/day to 17.6 ± 4.4 mg/day, and its estimated absorption from 3.1 ± 1.2 to 5.3 ± 1.3 mg/day. Mean plasma zinc concentration remained unchanged (p > 0.05) near 150 µg/dL, but increased by 31 µg/dL (p < 0.05) for 6/24 adolescents (group A) and decreased by 25 µg/dL (p < 0.05) for other 6/24 adolescents (group B). Expression of ZIP1 in blood leukocytes was reduced 1.4-fold (p < 0.006) in group A, while for the expression of ZnT1 there was no difference after intervention (p = 0.39). An increase of dietary zinc after 27-days consumption of fortified-milk did not increase (p > 0.05) the plasma level of adolescent girls but for 6/24 participants from group A in spite of the formerly appropriation, which cellular zinc uptake decreased as assessed by reduction of the expression of ZIP1.
