Objectives The Filipino population is largely underrepresented in the currently available literat... more Objectives The Filipino population is largely underrepresented in the currently available literature on multiple myeloma (MM). Herein, we aimed to determine the clinical profile, treatment, and outcomes of adult Filipinos with MM. Material and Methods The records of 74 patients with MM seen at our institution from 2016 to 2019 were retrospectively reviewed. Results The median age at diagnosis was 54 years, with the majority lumped in the 40–65 years age group. At diagnosis, anemia (hemoglobin <100 g/L) was present in 36 (64.3%) patients, but hypercalcemia (calcium ≥2.75 mmol/L) and azotemia (creatinine ≥177 umol/L) were seen in only 9 (20.0%) and 18 (34.0%) patients, respectively. Novel drugs (bortezomib, thalidomide, and lenalidomide) were used in 54 (84.4%) patients for frontline treatment. Due to a USAID-funded study on blood banks, a national policy was instituted in 1994 that set sta... more Due to a USAID-funded study on blood banks, a national policy was instituted in 1994 that set standards for Philippine blood services, promoted voluntary donation, and led to a ban on commercial blood banks. In this follow-up study, we assess the safety of the supply by determining the residual risk for transfusion-transmitted infections (syphilis, hepatitis B and C, HIV). We also identified unsafe facility practices and generated policy recommendations. A 1992 study found that transfusion-ready blood was not safe using the LQAS method (P> 0.05). We found that the 2012 residual risk became 0 to 0.9 percent attributable to the na-tional policy. We noted poor to fair adherence to this policy. We identified unsafe practices such as use of rapid tests and lack of random blood retesting. Training and use of regional networks may improve safety. Despite improvement in safety, facilities complain of funding

Instead, the same law has encouraged the voluntary non-remunerated donation of blood to ensure adequate, safe, affordable, and equitable supply of blood and blood products for Filipinos. This strategy minimizes delays in treatment but reduces the dose of bortezomib received by the patient. Herein, we aimed to determine the treatment patterns and outcomes in Filipino patients with MM who received reduced-dose bortezomib. Methods The records of 47 adult patients with MM, seen at our institution from 2016 to 2019 and treated with reduced-dose bortezomib, were retrospectively reviewed. Results The median age of the patients at diagnosis was 55 years; 59.6% were male. VCD (bortezomib, cyclophosphamide, dexamethasone) regimen was the most commonly used (70.7%) bortezomib-based treatment. Case History: We present a rare case of a 46-yearold Filipino male, who manifested with a two-wee... more Case History: We present a rare case of a 46-yearold Filipino male, who manifested with a two-week history of intermittent high grade fever, jaundice, crampy non-radiating epigastric pain, which were later accompanied by tea colored urine and acholic stools . The pat ient subsequent ly presented with sudden anemia on his third hospital day with no signs of bleeding or blood loss. Physical examination revealed diffuse jaundice and hepatomegaly with mild epigastric and right upper quadrant tenderness. Laboratory findings showed significant elevation of liver transaminases, bilirubins and triglycerides. Complete blood count (CBC) revealed a drop of hemoglobin from 134 mg/dl to 64 mg/dl. Coomb’s test was negative. Repeat aspartate transaminase (AST) and alanine transaminase (ALT) was decreased. This study examined which socio-demographic characteristics of blood donors are associated with a high-risk status for transfusion transmitted infections (TTI) based on the Donor History Questionnaire (DHQ) and compared the risk status of the donor with their actual HIV status. Methods: A total of 5967 potential blood donors were assessed for their eligibility as a donor and the risk status (high vs. low) were ascertained using the DHQ. HIV screening and confirmatory testing for all donor-respondents, regardless of risk status, was done to compare their risk status with their HIV status. Socio-demographic characteristics were collected and linked with the risk status of the donor. Myeloid sarcoma, characterized by the presence of immature myeloid cells occurring at an extramed... more Myeloid sarcoma, characterized by the presence of immature myeloid cells occurring at an extramedullary site, is a rare manifestation of acute myelogenous leukemia (AML). Spinal cord compression as an initial presentation of AML is very rare with only a few reported cases. We discuss a case of a 22-year-old male who presented with bicytopenia and paraplegia. Workups were consistent with AML with monocytic differentiation. Chromosomal analysis revealed loss of Y and t (8;21). Spinal cord MRI showed intradural extramedullary-enhancing soft tissue lesions at levels T2 to T7 and L5 to S1, suspected to be myeloid sarcoma. Patient, however, succumbed to severe nosocomial infection prior to initiation of chemotherapy and radiotherapy.

In Southeast Asia, studies showed that genetic hemoglobin disorders also contribute significantly to the burden of anemia. The study aimed to estimate the proportion of thalassemia and other hemoglobinopathies versus iron deficiency and other causes in a sample of anemic individuals; describe the characteristics of thalassemic subjects in terms of severity of anemia, adequacy of iron stores, and hematological profile; examine the intake of iron supplements among individuals with varying causes of anemia. A random sample of 101 anemic individuals living in Metro Manila was examined. Hemoglobinopathy was determined using capillary electrophoresis. Iron deficiency was determined using immunoradiometric assay for serum ferritin. A questionnaire was used to obtain information on the use of iron supplements.
