ICU-AW is found in 30-50% of patients and increases to 67% in sepsis-critically ill patients. Prolonged ICU stay, the difficulty of weaning from the ventilator, higher hospitalization costs, and an increase in mortality, as well as long-term morbidity, are associated with ICU-AW. ICU-AW causes skeletal muscle weakness, including respiratory muscles, which results in complications that continue even years after being discharged from the hospital. Aim: To describe the rehabilitation management of ICU-AW and provide the information needed clinically to manage these patients. The authors reviewed all types of articles without time filtering using PubMed and Google Scholar databases with "ICU-AW", "ICU-related weakness", "rehabilitation", and "early mobilization" used as keywords. Results: Rehabilitation management can be done as early as possible, beginning while the patient is still in the ICU. Early mobilization programs require teamwork consisting of doctors, therapists, and nurses. Physical activity and early mobilization in the ICU must be carried out with consideration for safety. Monitoring patient safety before and during mobilization is an essential factor to be considered. Any mobilization program should be the decision of the ICU care team. Implementation of rehabilitation programs and early mobilization improves outcomes for patients with ICU-AW and should be continued until the follow-up period. The rehabilitation management aiming at preventing and treating ICU-AW should be done since the patient was still in the ICU and under careful consideration of safety aspects.

Research paper thumbnail of Implementation of Home-based Cardiac Rehabilitation Program in Patients with Coronary Artery Disease: A Literature Review

Indonesian Journal of Physical Medicine and Rehabilitation, Dec 29, 2023

Introduction: Coronary artery disease (CAD) is the major cause of disability and death worldwide.... more Introduction: Coronary artery disease (CAD) is the major cause of disability and death worldwide. An integral aspect of the management strategy for CAD patients is cardiac rehabilitation (CR) program, where exercise assumes a central role in enhancing functional capacity. This program reduces healthcare burden and improves quality of life due to its benefits in reducing mortality and morbidity. Even though the benefits of CR are widely proven, participation and adherence of CAD patients are low. Home-based CR (HBCR) program is the strategy developed to increase the coverage of CR services. This is equally effective as center-based CR (CBCR) and considered safe. Therefore, this study aimed to conduct a comprehensive exploration of HBCR program, with a specific emphasis on the implementation among CAD patients. Methods: Original articles and systematic or narrative reviews were searched using PubMed, Google Scholar, and Research Gate databases. Further exploration of literature citations was also conducted to meet the inclusion criteria needed to explain the topic. Results: HBCR started with pre-participation screening. Patient assessment was done to identify physical function or personal impairment. Generally, all prescription in HBCR is similar to CBCR. Risk factor management in HBCR provided by home monitoring through home visit or telemonitoring and evaluation is carried out to assess outcome achievement which focuses on behaviour target so this prescription can be used as secondary prevention program. The implementation of the HBCR program in CAD patients is quite similar with CBCR, and begin with pre-participation screening, patient assessment, exercise prescription, management of risk factors, monitoring, and evaluation.
