The cross-finger flap was described originally in 1950 and is one of the workhorse flaps for finger reconstruction and can be done as described originally or as a modification in multiple scenarios of finger trauma. The aim of this study is to present our clinical experience and suggest possible modifications of... more The aim of this study is to present our clinical experience and suggest possible modifications of Dufourmental flaps. Materials and Methods: In our study thirty four patients operated for different pathologies with reconstruction done by Dufourmental flap July 2021 to December 2022. Results: Twenty-one of the 34 cases were male and the median patient age was 44.2 years. Nine cases were operated on under general anaesthesia and 25 were performed under local anaesthesia. In 20 cases the pathology was benign 10 cases the pathology was malignant and in rest four cases the defect was post-traumatic two among those were secondary to the dog bite. None of the patients has partial or total flap loss, one patient has margins positive on biopsy. Conclusion: Dufourmental flap although a local flap is very versatile flap and can be safely used in wide spectrum of defects and almost all areas of body. This flap can be easily tailored to meet the specific needs of the patient.

Actual aetiology remains unknown, and classical clinical features, which include swelling, pain and discomfort in the scapular region along with MRI, usually confirm the diagnosis. Methods: In our study, we described 11 patients with elastofibroma dorsi from 2014 to 2021 identified retrospectively treated with marginal excision. The patient’s age, gender, lesion side, tumour size, and location were analysed in addition to intraoperative and postoperative complications. Results: The median age in our study was 58 years, with a male: female ratio of 0.57:1.The tumour was more commonly present on the left side, with pain being the most common symptom. Necks can be reconstructed with a wide range of surgical techniques. The supraclavicular flap is easy to harvest without the need for free tissue transfer. It provides a relatively large flap for neck resurfacing with tissue very similar to that of the neck. Material and Methods: This study was carried out in the Plastic Surgery Department of GMC Srinagar between November 2015 and October 2019. The aim of this study is to present supraclavicular artery island fl ap as an alternative choice in patients who need a free fl ap like radial forearm or ALT fl ap for reconstruction of oral mucosal defects, but are not fi t for such microsurgical procedures. Material and methods: This study was carried out in the Plastic Surgery Department of GMC Srinagar. It involved 12 patients, in whom supraclavicular artery island fl ap was done to reconstruct oral lining defects from January 2018 to December 2020. Results: The study involved 12 patients (9 males and 3 females). Mean age was 26.5 years (range 12-57 years). The cause of oral mucosal defect was post tumour resection in eight, fi re arm injury in three, and corrosive injury in one patient. The following complications occurred: partial distal necrosis in 1 case, and donor site graft loss in 1case. All complications were managed conservatively. Conclusion: This fl ap is thin, malleable, and is easily and rapidly harvested with minimal donor site morbidity. It can be used for single stage reconstruction of many types of head and neck defects.

Soft tissue management around the lower third of the leg and foot presents a considerable challenge to the reconstructive plastic surgeon. The options in this region are limited. A durable flap is the preferred option for coverage of such defects. This study was conducted over a period of 10 years to evaluate the efficacy of distally based Sural flap in coverage of the lower third of leg, ankle and foot defects. Material and Methods: A retrospective study was conducted in the department of Burns, Plastic, and Reconstructive surgery GMC Srinagar. The patients who underwent treatment from March-2008 to Feb-2018 were the subjects of study. Distally based sural fasciocutaneous flap was used for coverage in all cases and its survival, successful coverage of the defect and donor site morbidity studied. Results: Out of 100 flaps, 80 showed complete survival (80%). Partial flap loss (up to one third but less than half of the flap) was found in 3 patients (3%), marginal flap necrosis in 4 patients (4%), venous congestion developed in 5 patients (5%) and complete loss in 4 patients (4%) with extensive heel injury. Conclusions: The distally based sural flap is a versatile and reliable flap for the coverage of soft tissue defects of the distal lower extremity. The procedure is done as a single stage; the dissection is easy with short operating time and minimal morbidity.

A reconstructive surgeon has to be ready to face new challenges every day. Negative pressure wound therapy has revolutionized the management of complex wounds. We are presenting our experience with this wound care modality in the past 11 years.Methods: It was a prospective study conducted from January 2006 to December 2016 on patients having wounds of varied etiologies, who consented to participate in this study. Custom made low cost NPWT was used till definitive wound closure.Results: A total of 568 patients consented to participate in the study during these 11 years. No major complications were seen. Most of these were males (60.73%) in their 3rd and 4th decade. Trauma was the leading cause of wounds in 38.14%, followed by diabetic foot wounds in 21.5%. To study the possible role of proinflammatory interleukin 6 -174 G&am... more To study the possible role of proinflammatory interleukin 6 -174 G>C (rs 1800795) and -634 C>G (rs 1800796) polymorphism in the pathogenesis of non-small cell lung cancer (NSCLC). A total of 190 NSCLC patients and 200 healthy controls were evaluated for polymorphic analysis of -174 G/C and -634 C/G by PCR-RFLP followed by DNA sequencing. A significant association was observed in the genotypic and allelic distribution of IL-6 -174 G/C in the NSCLC group as compared to control group [OR = 2.7 (1.77-4.11), p < 0.0001]. Smokers with the -174C allele were found to be significantly associated with NSCLC (p = 0.01), while 634C/G SNP showed an inverse relation [OR-0.4, p < 0.0001]. The present investigation revealed a significant association of the IL6 -174 G/C gene promoter polymorphism with NSCLC, and thus, the IL-6 -174G/C genotype can be considered as one of the biological markers in the etiology of NSCLC.

A new class of compounds targeting cyclooxygenase 2 (COX-2) together with other different clinica... more A new class of compounds targeting cyclooxygenase 2 (COX-2) together with other different clinically used therapeutic strategies has recently shown a promise for the chemoprevention of several solid tumors including lung cancer. The aim was to study the possible role of COX-2-8473 T/C NP and its expression in the pathogenesis of non-small cell lung cancer. One hundred ninety non-small cell lung cancer (NSCLC) patients and 200 healthy age-, sex-, and smoking-matched controls were used for polymorphic analysis, and 48 histopathologically confirmed NSCLC patients were analyzed for COX-2 messenger RNA (mRNA) and protein expression. Our results showed that the frequencies of variant genotypes 8473 CT/CC were significantly less common in the cases (30.0 %) than in the controls (36 %), suggesting that the 8473C variant allele is related with lower susceptibility in NSCLC (OR=0.79, 95 % CI 0.54-1.4). However, the frequency of COX-2-8473 TC and CC genotypes were significantly associated with age in NSCLC (P= 0.02). Quantitative real-time expression analysis showed a significant increase in the COX-2 mRNA in tumor tissues as compared to their adjacent normal tissues [delta cycle threshold (ΔCT) = 9.25 ± 4.67 vs 5.63 ± 3.85, P = 0.0001]. Multivariate logistic regression analyses revealed that the COX-2 expression was associated significantly with age (P=0.044). Also, an increasing trend was observed in stages I and II and in female patients compared to stages III and IV and male patients, respectively, but no statistical significance was observed. However, COX-2 mRNA expression shown no association with the-8473C variant allele. Our findings indicate that the COX-2 T8473C polymorphism may contribute to NSCLC cancer susceptibility in the Kashmiri population, while our expression analysis revealed a significant increase of COX-2 in tumor tissues as compared to their adjacent normal tissues, suggesting that it could become an important therapeutic marker in NSCLC in the future.

The presence of inflammatory cells and higher levels of pro-inflammatory cytokines in the tumor microenvironment and their surrounding tissues is gaining much importance in research. Materials and Methods: One hundred ninety NSCLC cases and 200 age, smoking and sex matched controls were evaluated for association of IL-8-251 (rs4073) and IL-8-845 (rs2227532) in our population. Restriction fragment length polymorphism (RFLP) was used followed by direct sequencing for the detection of SNPs. Results: The IL-8-845 polymorphism was not found in our population. No significant association was observed between the IL-8-251 AT genotypes and IL-8-25 AA genotypes and NSCLC (p=0.05) in our population. The IL-8-251 A allele was also non-significant (p=0.05) in NSCLC patients. Conclusions: In conclusion, this report reveals lack of association between IL-8-251 A/T polymorphism and NSCLC in our Kashmir Valley population.

The presence of inflammatory cells and higher levels of pro-inflammatory cytokines in the tumor microenvironment and their surrounding tissues is gaining much importance in research. Materials and Methods: One hundred ninety NSCLC cases and 200 age, smoking and sex matched controls were evaluated for association of IL-8-251 (rs4073) and IL-8-845 (rs2227532) in our population. Restriction fragment length polymorphism (RFLP) was used followed by direct sequencing for the detection of SNPs. Results: The IL-8-845 polymorphism was not found in our population. No significant association was observed between the IL-8-251 AT genotypes and IL-8-25 AA genotypes and NSCLC (p=0.05) in our population. The IL-8-251 A allele was also non-significant (p=0.05) in NSCLC patients. Conclusions: In conclusion, this report reveals lack of association between IL-8-251 A/T polymorphism and NSCLC in our Kashmir Valley population.
