Background Osteopoikilosis, also referred to as disseminated condensing osteopathy, spotted bone ... more Background Osteopoikilosis, also referred to as disseminated condensing osteopathy, spotted bone disease, or osteopecilia, is a rare bone disorder. The case presented here showcases multiple disc lesions in the spine, extensive multifocal skin lesions, and positive test results for dermatomyositis and multifocal enthesopathy, accompanied by neurological symptoms. This manifestation represents a novel variant of the disease. Case presentation Our patient is a 46-year-old mosque Kurdish servant presenting with complaints of pain in the right leg, lower back, right hand, and neck. Additionally, the patient has been experiencing redness in the right buttock and ipsilateral thigh, as well as gradually expanding and stiffening skin lesions on the left shin for the past 3 weeks. Painful neck movements and a positive Lasegue test were also observed in the right leg. The patient reports pain in the right buttock accompanied by a substantial erythematous area with induration measuring 8 × 15 cm, as well as an erythematous and maculopapular lesion measuring 6 × 18 cm on the left shin. Conclusions Our patient is a 46-year-old man presenting with complaints of skin lesions and pain in the lower back, pelvis, neck, and limbs. The X-ray reveals shoulder, pelvis, knee, and ankle involvement, while spinal involvement is observed in the neck and lumbar region. Furthermore, the bone scan indicates extensive enthesopathy in various regions, a unique manifestation not previously reported in similar cases.

Methicillin-resistant Staphylococcus aureus (MRSA) is one of the most signifuicant pathogens in I... more Methicillin-resistant Staphylococcus aureus (MRSA) is one of the most signifuicant pathogens in Iran; it is one of the WHO-declared microbial resistance emergencies; and also one of the most important challenges facing the prevalence of resistance. The aim of this study was to detect MRSA using Broth Micro Dilution method and meta-analysis in Iran from 2007 to 2016. Methods: Persian databases (including Magiran, Irandoc, and SID) and International databases (including pubmed, science direct, and scopus) were searched during this period (2007-2016), such that the high heterogeneity (50% < I2) in this study was analyzed using the DerSimonian Laird method. Data were categorized into subgroups based on year of study and province. Due to the high validity of the diagnosis of organisms and quantitative results, antimicrobial susceptibility test (AST) was used to detect MRSA. Data analysis was performed using statsdirect software. Results: Based on the available data in medical databases, 678 articles were selected. Background: In some studies, the level of adenosine deaminase (ADA) in sputum and effusion liquid... more Background: In some studies, the level of adenosine deaminase (ADA) in sputum and effusion liquids was used for the diagnosis of tuberculosis (TB). But it is not always possible to access these materials. The goal of this study is to assess the diagnostic value of serum ADA levels in pulmonary TB patients. Materials and methods: In this study, 40 sputum smear-positive TB patients who were hospitalized and 40 non-TB patients who referred for surgeries were selected. A serum sample was collected and serum ADA level was measured by ADA kit. Results: The average (SD) of serum ADA in TB and non-TB patients were 20.88 (±5.97) and 10.69 (±2.98) U/L, respectively (P value < 0.05). The best cut-off point was 14 U/L. The calculated area under the receiver operating characteristic (ROC) curve was 0.955 (95% CI, 0.914-0.995); sensitivity was 92.7% (95% CI, 84.7-100) and specificity was 88.1% (95% CI, 78.3-97.8) (P < 0.001). Conclusion: Serum ADA level may be proposed as a proper index for TB diagnosis.

Therefore, the aim of this study was to investigate the incidence and geographical distribution of anthrax using the Geographic Information System (GIS) and predict its incidence in Iran in 2021. Methods: This study is descriptive analytical study. Information on anthrax was obtained from the Center for Communicable Diseases Control during 2010-2015. In the next step, ArcGIS 9.3 was used to prepare geographic maps of the disease incidence and frequency. Therefore, using the Raster Calculator tool, the disease prediction map was drawn. Results: The highest incidence of anthrax during 2010-2015 was observed in the provinces of Kurdistan, North Khorasan, and Chaharmahal and Bakhtiari, respectively. This study evaluated the epidemiology of suspected cases of pandemic influenza A (H1N1) virus inf... more This study evaluated the epidemiology of suspected cases of pandemic influenza A (H1N1) virus infection in 2009-2010 in Kurdistan province, a frontier province of the Islamic Republic of Iran. A questionnaire covering demographic characteristics, clinical presentation and outcome, and history of exposure and travel was completed by patients attending health centres and hospitals in the province. Nasal and throat swabs were analysed by RT-PCR. A total of 1059 suspected cases were assessed; H1N1 influenza A was confirmed in 157 (14.8%). The highest proportion of confirmed cases was 30.0%, among children aged < 1 year. In multivariate analysis, previous contact with symptomatic influenza patients (OR = 2.17) and hospitalization (OR = 3.88) were the only significant risk factors for confirmed H1N1 infection. Age, sex, residency, presenting symptoms and history of national or international travel were not significant. Influenza A (H1N1) virus has spread in Islamic Republic of Iran; probably transmitted by travellers to Kurdistan.

Before lumbar puncture, intracranial mass lesions that increase intracranial pressure, must be ruled out, so brain CT-scan may be considered . In this study we evaluated the accuracy of physical examination for intracranial mass in patients with meningismus. Materials and Methods: 100 patients who had symptoms and signs of acute meningitis were evaluated and then brain scan was performed. After this, clinical manifestations were compared with CT results and sensitivity, specificity, positive and negative predictive values and accuracy were analyzed. Results: 73% of patients were men and 27% women. Most of patients were between 22 to 31 years old. 34% of patients had meningitis, 22% had SOL, 69% had one neurological symptom or sign at least. The purpose of this study was to investigate the prevalence of HIV and risky behavior among seasonal migrant workers in Kurdistan Province. Materials and Methods: This cross-sectional study included 600 seasonal migrant workers in Kurdistan Province from 2018 to 2019. Data about high-risk behavior were collected by using a standard questionnaire. A rapid HIV test was used to detect HIV in the study participants. Using Stata software V14, data were analyzed by chi-square, Mann-Whitney U test, and logistic regression modeling. Methods: Blood samples were collected from 30 subjects with positive pathological tests and 62 patients with negative pathological tests as a control group. The enzymatic activity of total ADA and its isoenzymes was determined. Results: tADA and ADA2 isoenzyme activity was significantly higher in cancerous patients compared to benign controls in serum and BAL fluid. We aimed to investigate different clinical features and CT scan findings of patients with COVID-19 based on disease severity to have a better understanding of this disease. For this purpose, 90 patients with coronavirus were examined retrospectively, which were divided into three categories based on the severity of the disease: mild/moderate, severe, and very severe. It has been shown that mean age and duration of hospitalization of patients increased with increasing the severity of disease. The most common clinical symptoms were shortness of breath, cough, and fever. Kikuchi-Fujimoto disease is an uncommon, benign, autoimmune condition characterized by lymphadeno... more Kikuchi-Fujimoto disease is an uncommon, benign, autoimmune condition characterized by lymphadenopathy, fever, and neutropenia. It is a self-contained condition of unidentified etiology. A 38-year-old female of Iranian Kurdish origin referred to us with multiple neck swellings, fever, and generalized arthropathy. There was no weight loss and no history of tuberculosis, medication intake, or allergy. Clinical examination revealed bilateral large, mobile, and tendercervical lymphadenopathy. Cardiovascular, respiratory, and neurological examination was normal. The patient had leucopenia and thrombocytopenia. Kikuchi-Fujimoto disease must be considered in the differential diagnosis of a female patient with fever and cervical lymphadenopathy.

Materials and Methods: 100 patients who had symptoms and signs of acute meningitis were evaluated and then brain scan was performed. After this, clinical manifestations were compared with CT results and sensitivity, specificity, positive and negative predictive values and accuracy were analyzed. The purpose of this study was to investigate the prevalence of HIV and risky behavior among seasonal migrant workers in Kurdistan Province. Materials and Methods: This cross-sectional study included 600 seasonal migrant workers in Kurdistan Province from 2018 to 2019. Data about high-risk behavior were collected by using a standard questionnaire. A rapid HIV test was used to detect HIV in the study participants. Methods: Blood samples were collected from 30 subjects with positive pathological tests and 62 patients with negative pathological tests as a control group. The enzymatic activity of total ADA and its isoenzymes was determined. Results: tADA and ADA2 isoenzyme activity was significantly higher in cancerous patients compared to benign controls in serum and BAL fluid. Using a cut-off level of respectively 35.22 U/L and 31.80 U/L for BAL total ADA and ADA2, sensitivity and specificity were 100% and 81% for total ADA and 95% and 98% for ADA2. The results of laboratory examinations show positive Wright and Coombs Wright in blood and C.S.F. For this purpose, 90 patients with coronavirus were examined retrospectively, which were divided into three categories based on the severity of the disease: mild/moderate, severe, and very severe. It has been shown that mean age and duration of hospitalization of patients increased with increasing the severity of disease. The most common clinical symptoms were shortness of breath, cough, and fever. It is a self-contained condition of unidentified etiology. A 38-year-old female of Iranian Kurdish origin referred to us with multiple neck swellings, fever, and generalized arthropathy. There was no weight loss and no history of tuberculosis, medication intake, or allergy. Clinical examination revealed bilateral large, mobile, and tendercervical lymphadenopathy. Cardiovascular, respiratory, and neurological examination was normal. The patient had leucopenia and thrombocytopenia. Kikuchi-Fujimoto disease must be considered in the differential diagnosis of a female patient with fever and cervical lymphadenopathy.
