The aim was: correlation between frequency of papillary edema in intra-cranial tumours and to ver... more The aim was: correlation between frequency of papillary edema in intra-cranial tumours and to verify Romanian medical legislation for ophthalmoscopy. This paper is a retrospective study for a period of 2 years (2005-2006). We included in study 446 patients treated in Hospital "Sf Treime" Iaşi, Romania for expansive intra-cranial tumours. Ocular signs and symptoms (especially papillary edema) were observed for these patients. Mean age was 50.76+/- 18.30 years and in 46.87% of cases we found papillary edema in different stages. papillary edema depends after localization and size of intra-cranial tumours, histological aspects and its aggressivity.

High performance of a hospital follows aims: reaction and needs of the community, integrity of he... more High performance of a hospital follows aims: reaction and needs of the community, integrity of health system, availability of hospital services for all population without difference of social, culture, religion or economic status. This paper reviews some experience in developing countries for understanding hospital performance and methods for measuring the quality of hospital management. All these methods are studied by manager and the staff of Hospital "Sf. Spiridon" Iaşi, Romania for a strong medical and economic management.

The retinal circulation represents a unique window for the direct, non invazive in vivo status of... more The retinal circulation represents a unique window for the direct, non invazive in vivo status of the systemic mycrocirculation, but it can also offer scientific support for theories related strictly to ocular diseases, such as glaucoma (vascular theory). The interaction between intraocular pressure, retinal vessels and cerebrospinal fluid pressure located at the retrolaminar part of the optic nerve has been of great interest for both ophthalmologists or neuroscientists, both clinicians and researchers. The retinal vascular bed has structural and functional similarities with other vascular teritories such as Central Nervous System (CNS), kidneys and heart, so as for the ophthalmologist any vascular change in the retina can signalize a systemic disease (diabetes mellitus, stroke, arterial hypertension) that is more or less symptomatic for the patient. The purpose of this meta-analysis was to reviview recent literature data related to the connections between different hemodynamic structures and their impact on the retinal blood flow.

The progressive death of retinal ganglion cells and optical neural loss suggests the following hypothesis: primary destructive event (ischemia, elevated intraocular pressure and genetic risk factors) and secondary degeneration (apoptosis and excitotoxicity). The concept of neuroprotection is a therapeutical paradigm for preventing and delaying neuronal cell death and maintaining the function of neurons. Other additional concept--neurorescue is used to prevent neuronal death by interrupting the biological cascade of secondary events. Most substances that can be found (glutamate, nitric oxide, a.o) in excess in extra cellular spaces induce damage of retinal ganglion cells, while others (memantine, gingko biloba, nicergoline, flavonoids, ascorbic acid) are used to restore these cells and their axons, i.e., neuroreparation and neuroregeneration in various degrees. This paper is an up-date of numerous studies which demonstrate that open-angle glaucoma is considered to be a neurodegenerative disease connected with alterations of the cardiovascular, central nervous, immune, endocrine, psychological systems and of sleep, requiring the introduction of glaucoma in general medical context.

The aim of study is to examine the occurrence of battered women seen in Clinica I Oftalmologie Ia... more The aim of study is to examine the occurrence of battered women seen in Clinica I Oftalmologie Iaşi. It is a retrospective study for a 5 year period for documentation of injuries to women as a result of "domestic" violence. The patient range in age was from 15 to 90 years. Injuries ranged including laceration and haemophthalmos in most of the cases. In conclusion, the ophthalmologist must see "domestic" violence in these injuries.

In 10-25% of cases, thyroid ophthalmopathy (or dysthyroid ophthalmopathy) occurs in the absence of both clinical and biochemical evidence of thyroid dysfunction. In patients with Graves' disease, ocular modification may precede, coincide with or follow of Graves' disease. Dysthyroid ophthalmopathy has in 85% of cases bilateral proptosis more than a half in unilateral proptosis.

Primary open-angle glaucoma (POAG) is a progressive optic nerve disease characterized by neurodeg... more Primary open-angle glaucoma (POAG) is a progressive optic nerve disease characterized by neurodegeneration of retinal ganglionic cells and their axons. Local inflammatory responses are implicated in the pathology of glaucoma. Despite the large number of studies conducted to identify non-genetic risk factors, only race, age, intraocular pressure, and corneal thickness were consistently identified. Of these, only intraocular pressure is a modifiable factor and also the only means we have at our fingertips to influence the development of glaucomatous disease. Tooth loss or periodontal disease is associated with systemic endothelial dysfunction, which has been implicated in primary open-angle glaucoma. The main hypothesis behind this correlation is that infectious and inflammatory processes located in the teeth can produce factors that will be released into systemic blood circulation. At the eye level, these factors are responsible for triggering the inflammatory response, neurodegeneration and, in some cases, open-angle glaucoma. Mounting evidence suggests that neuroinflammation is a key process in glaucoma, while the precise roles are yet to be discovered. In this article we reviewed the literature on neuroinflammation in glaucoma triggered by oral cavity microbiota and infiltration of peripheral immune cells. In conclusion, if an association between dental health, the oral microbiome and POAG were established, this would have potentially significant implications for the understanding and management of glaucoma and possibly other neurodegenerative diseases and we will be able to work together in order to prevent both glaucoma and dental diseases.

Metastases from intracranial meningiomas are rare, and among them, meningiomas with hepatic disse... more Metastases from intracranial meningiomas are rare, and among them, meningiomas with hepatic dissemination are extremely rare. Therefore, there are currently no guidelines for staging and treatment of metastatic disease in meningioma, a disease that is a challenge for both the clinician and the pathologist. Our literature review revealed 24 cases of liver metastases originating from intracranial meningiomas. We used them to analyze the pathological patterns of dissemination and to assess the different management strategies available, the most efficient and beneficial being surgery and chemotherapy, especially in the case of meningiomas with hepatic and/or systemic dissemination.

The characteristic granulomatous inflammation of the vessel wall of large and medium-sized arteries is present. To maintain high standards of management, ophthalmologists need to be aware of the clinical available diagnostic tests and treatment strategies. Corticosteroids remain the treatment of choice.

Ophthalmopediatrics and ocular oncologists have sought effective alternative methods for treating retinoblastoma. The management of retinoblastoma has been changed significant past few years. Recently, advances in understanding of retinoblastoma have led to trials of new treatment modalities aimed at decreasing morbidity and continuing excellent survival. This review tries to present the new and modern aspects in retinoblastoma therapy.

Purpose Evaluation of influence of medium factors in children with retinoblastoma and quality of ... more Purpose Evaluation of influence of medium factors in children with retinoblastoma and quality of life after treatment.Methods Retrospective study for 84 eyes treated for retinoblastoma (Rb) in Ophthalmological Department, Hospital “Prof. Dr. Nicolae Oblu” Iasi. Clinical parameters studied: age of diagnosis, adjuvant factors (hypoxia, pregnancy hemorrhages, congenital rubella, ionizing radiation, endocrinological disease of mother) and hereditary factors. Statistical evaluation: program SPSS 16.0, test Mann‐Whitney, Kruskal‐Wallis were used.Results Between 1953 and 2010 in Ophthalmological Clinic were treated 702 eyes tumors and 77 were diagnosticated with Rb. Unilateral cases were 70 cases and 7 cases bilateral Rb. Hypoxia were at 24,7% Rb versus 4,8% children without Rb (cwRb) – risk factor 2,609. Hemorrhages during pregnancy were 27% in mother with children with Rb versus mothers with cwRb – risk factor 1,735. Mother inflammations were registrated at 39% children with Rb versus 17...
