Corona virus (COVID-19) is an epidemy of respiratory disease caused by a novel corona virus and i... more Corona virus (COVID-19) is an epidemy of respiratory disease caused by a novel corona virus and it was declared by the World Health Organization (WHO) to be a global health pandemic emergency. Due to the neuropsychiatric manifestation of Covid-19 that have been associated with psychotic disorders, in this paper we choose to present a case from "Elena Doamna" Psychiatric Hospital from Galati, Romania and to correlate it with other cases from literature in a mini review presentation. In our paper, we present the case of a patient of 44-year-old men, with no past psychiatric history whose behavior included psycho-motor agitation, perception and thinking disorders, disorganized behaviour, attempted suicide by stabbing. The last perspectives from the two years of pandemic together with psychiatric disease linked the virus infections with psychosis to the main concern that Covid-19 could determine psychiatric disorders. There were also presented same literature studies of patients with no personal pathological history in the psychiatric field which developed psychiatric disorders after COVID-19 infection. SARS-CoV-2 has a psychological impact on the mental health status of the worldwide and, especially when it is associated with psychotic symptoms and can affect the quality-of-life. In some cases, the virus affected the brain and as a result, the psychosis symptoms could be an emerging phenomenon associated with the corona virus. Based on the DSM V and ICD-10 criteria, the diagnosis was of acute psychiatric disorders with symptoms of schizophrenia (F23.1). The case report and review reliefs that there is a causal link between the SARS CoV-2 infection and mental disorders, which is currently being investigated.

Interstitial lung diseases (ILD) represent a group of disorders that are generally thought to share a common pattern of physiologic abnormality, characterized by a restrictive ventilatory defect and reduced lung diffusing capacity (DLCO). Objective of the study. To find distinctive features of the pulmonary function tests results in different types of ILD. Material and Methods. 40 consecutive patients with ILD, admitted to IFP “Chiril Draganiuc”, Chisinau, during January 2019-February 2020, were included in a retrospective study. We divided the cohort in 5 subgroups: IPF patients – 8 cases, NSIP – 7, Hypersensitivity pneumonitis (HP) – 9, Sarcoidosis patients – 10, and 6 histiocytosis (Hx) cases. Background: Status epilepticus (SE) is a major medical emergency and requires not only an emergen... more Background: Status epilepticus (SE) is a major medical emergency and requires not only an emergency symptomatic treatment with antiepileptic drugs (AED) but also a rapid identification and treatment of the underlying cause. This narrative review summarizes the most important advances in SE classification and treatment. Data sources included being PubMed / Medline, and tracking references of the relevant studies, reviews and books. SE is now defined as "a condition resulting either from the failure of the mechanisms responsible for seizure termination or from the initiation of mechanisms, which lead to abnormally prolonged seizures (after time point t1). It is a condition, which can have long-term consequences (after time point t2), including neuronal death, neuronal injury, and alteration of neuronal networks, depending on the type and duration of seizures. " The most effective acute treatments for early SE are the intravenous benzodiazepines (lorazepam, diazepam, and clonazepam) and intramuscular midazolam. In children, oral or intranasal midazolam are useful alternatives. The intravenous antiepileptic drugs (phenytoin, valproate, levetiracetam, phenobarbital and lacosamide) are administered in confirmed SE. Treatment options in refractory SE are intravenous anesthetics; ketamine, magnesium, steroids and other drugs are used in super-refractory SE , showing variable results and outcomes. Conclusions: Over time, major progress has been made in defining, classifying, and understanding of SE mechanisms. Despite this, the first-line drug management is ineffective in up to 40% of patients with SE. The super-refractory SE treatment is still unknown and no evidence-based data have been found yet. Thus, SE treatment strategies vary substantially from one institution to another due to the lack of data supporting a specific treatment plan.

We aimed to assess the need for interceptive therapy of dentomaxillary anomalies on a group of 147 children, 69 of age 6 years old and 78 of age 9 years old, which called for an orthodontic or pedodontic treatment during 2014- 2015 in 4 private offices in Craiova. We observed a high prevalence of caries in temporary and young permanent dentition (52.98% for age 6 years old and 37.17% for age 9 years old) and of early loss of temporary teeth (17.39% for age 6 years old and 23.07% for age 9 years old). For this narrative review, PubMed database and Infomedica library were searched by using “drug-resistance in epilepsy” and “treatment of drug-resistant epilepsy” as key words. The following filters were applied: “Clinical Trial”, “Meta-analysis”, “Multicenter Study”, and “Randomized Controlled Trial”, covering the period of 01.01.2005–06.01.2021.Several hypotheses have been proposed, i.e., pharmacokinetic, intrinsic severity, gene, target, transporter, and neural network hypotheses. Many controlled trials showed different results in terms of seizure control after combined methods of therapies. The evolution of research in the field of psychotherapy is not definitive, but always flexible wh... more The evolution of research in the field of psychotherapy is not definitive, but always flexible when new therapeutic and/or pharmacological procedures are established. Aim: To show evidence for the importance of psychotherapy in the treatment of depression. Case description: As a method of therapy that is used successfully, there is cognitive behavioural therapy (CBT). The reported case study confirms by the results obtained and the objectives achieved, that CBT prevents much better recurrences in psychiatric disorders. Conclusion: Our case confirms that psychotherapy increases the quality of the patient's life in the fight against depression, modulation of one's own affect (narcissistic disorder), time management, development of communication and acceptance skills. It decreased anxiety and cardiovascular symptoms. The role of psychotherapy in the treatment of mental illness is beneficial and effective and facilitates compliance with antidepressant and anxiolytic treatment.

În faţa tendinţei relativ frecvente a multor persoane-cadre medicale sau nude a administra băutur... more În faţa tendinţei relativ frecvente a multor persoane-cadre medicale sau nude a administra băuturi carbogazoase de tipul Coca-Cola în cazul unor simptome gastrice de tip greaţă sau vărsături la pacienţii pediatrici sau adulţi, am încercat să cercetăm dacă există un fundament ştiinţific la baza acestei practici. Acidul fosforic prezent în această băutură este regăsit în produse care se pot elibera fără prescripţie medicală (OTC), recomandate în stări de greaţă şi vărsături. Pe de altă parte, literatura medicală citează studii în care s-a apelat la acest tip de băuturi în tratarea unor bezoari gastrici sau intestinali, fecaloame sau chiar în unele situaţii de reflux gastric sau gastropareză. Unul dintre studiile analizate relevă efectul antibacterian al acestui tip de băutură pe bacterii precum Bacillus cereus, Enterococcus faecalis, Escherichia coli, Escherichia coli O:157 H:7, Salmonella enteritidis, Yersinia enterocolitica. Aşteptăm efectuarea mai multor studii privind eficacitatea terapeutică antiemetică şi antimicrobiană a acestei băuturi şi punerea în balanţă a efectelor pozitive versus cele negative în utilizarea sa la copii.

Facing us the relatively frequent tendency of many people-medical professionals or not-to adminis... more Facing us the relatively frequent tendency of many people-medical professionals or not-to administer Coca-Colatype carbonated beverages in the case of nausea-type gastric symptoms or vomiting in pediatric or adult patients, we have tried to investigate whether there is a scientific basis for this practice. The phosphoric acid present in this beverage is found in over the counter products (OTC), recommended in the case of nausea and vomiting. On the other hand, the medical literature cites studies in which this type of beverage has been used in the treatment of gastric or intestinal bezoars, fecalomas or even in some cases of gastric reflux or gastroparesis. One of the studies analyzed reveals the antibacterial effect of this type of beverage, on bacteria such as Bacillus cereus, Enterococcus faecalis, Escherichia coli, Escherichia coli O:157 H:7, Salmonella enteritidis, Yersinia enterocolitica. We expect more studies to be carried out on the anti-emetic and antimicrobial therapeutic efficacy of this drink and to balance the positive versus negative effects in its use in children.

Bisphosphonates have been used for many years worldwide for the treatment of osteoporosis, Paget'... more Bisphosphonates have been used for many years worldwide for the treatment of osteoporosis, Paget's disease, but especially for the treatment of various bone density disorders such as tumour formations, bone metastases associated with osteolysis and hypercalcemia. The effects of bisphosphonates on the bones are due to their action on osteoclasts by inhibiting them. Although the existence of the risk of osteonecrosis of the jaw in patients undergoing bisphosphonate treatment is known, this pathology has been considered very rare for a long period and many times the risk of its occurrence has been ignored. Since the use of bisphosphonates is becoming more prevalent and the duration of such treatment is steadily increasing, the incidence of osteonecroses associated with this treatment is also increasing. OBJECTIVE. The objective of this study is to highlight the fact that the incidence of cases of bisphosphonate-related osteonecrosis of the jaw has been increasing in recent years. METHODS. We have carried out a search and analysis of all cases with osteonecrosis as main diagnosis in the Oral Maxillo-Facial Surgery Clinic during the period from January 2009 and December 2018. RESULTS. We found 84 patients with osteonecrosis of the jaw following bisphosphonate treatment. Of these, 37 were men and 47 women with an average age of 67 and 59 respectively. There were 26 cases (31%) with incidence in the jaw, 52 cases (62%) with incidence in the mandible, and 6 patients (7%) manifested bilateral osteonecrosis of the jaw. Of these, 20% were on the left side, 36% on the right side and 44% on the median line. CONCLUSIONS. The incidence of bisphosphonate-related osteonecrosis of the jaw is steadily increasing due to the more frequent use of a class of bisphosphonates with a strong impact on the oro-facial bone system. Therapeutic intervention, supportive psychotherapy and CBT, have as main objective cognitive restructuring, identifying and addresing psychological causes that cause, precede and trigger eating disorders - anorexia nervosa. Aim: To identify the role of psychotherapy and the entire interdisciplinary team in the treatment of anorexia nervosa. Case description: We present a case report of a 15-yearold girl diagnosed with anorexia nervosa, where I applied CBT. Background: Status epilepticus (SE) is a major medical emergency and requires not only an emergen... more Background: Status epilepticus (SE) is a major medical emergency and requires not only an emergency symptomatic treatment with antiepileptic drugs (AED) but also a rapid identification and treatment of the underlying cause. This narrative review summarizes the most important advances in SE classification and treatment. Data sources included being PubMed / Medline, and tracking references of the relevant studies, reviews and books. SE is now defined as "a condition resulting either from the failure of the mechanisms responsible for seizure termination or from the initiation of mechanisms, which lead to abnormally prolonged seizures (after time point t1). It is a condition, which can have long-term consequences (after time point t2), including neuronal death, neuronal injury, and alteration of neuronal networks, depending on the type and duration of seizures. " The most effective acute treatments for early SE are the intravenous benzodiazepines (lorazepam, diazepam, and clonazepam) and intramuscular midazolam. In children, oral or intranasal midazolam are useful alternatives. The intravenous antiepileptic drugs (phenytoin, valproate, levetiracetam, phenobarbital and lacosamide) are administered in confirmed SE. Treatment options in refractory SE are intravenous anesthetics; ketamine, magnesium, steroids and other drugs are used in super-refractory SE , showing variable results and outcomes. Conclusions: Over time, major progress has been made in defining, classifying, and understanding of SE mechanisms. Despite this, the first-line drug management is ineffective in up to 40% of patients with SE. The super-refractory SE treatment is still unknown and no evidence-based data have been found yet. Thus, SE treatment strategies vary substantially from one institution to another due to the lack of data supporting a specific treatment plan.

We aimed to assess the need for interceptive therapy of dentomaxillary anomalies on a group of 147 children, 69 of age 6 years old and 78 of age 9 years old, which called for an orthodontic or pedodontic treatment during 2014- 2015 in 4 private offices in Craiova. We observed a high prevalence of caries in temporary and young permanent dentition (52.98% for age 6 years old and 37.17% for age 9 years old) and of early loss of temporary teeth (17.39% for age 6 years old and 23.07% for age 9 years old). For this narrative review, PubMed database and Infomedica library were searched by using “drug-resistance in epilepsy” and “treatment of drug-resistant epilepsy” as key words. The following filters were applied: “Clinical Trial”, “Meta-analysis”, “Multicenter Study”, and “Randomized Controlled Trial”, covering the period of 01.01.2005–06.01.2021.Several hypotheses have been proposed, i.e., pharmacokinetic, intrinsic severity, gene, target, transporter, and neural network hypotheses. Many controlled trials showed different results in terms of seizure control after combined methods of therapies. The evolution of research in the field of psychotherapy is not definitive, but always flexible wh... more The evolution of research in the field of psychotherapy is not definitive, but always flexible when new therapeutic and/or pharmacological procedures are established. Aim: To show evidence for the importance of psychotherapy in the treatment of depression. Case description: As a method of therapy that is used successfully, there is cognitive behavioural therapy (CBT). The reported case study confirms by the results obtained and the objectives achieved, that CBT prevents much better recurrences in psychiatric disorders. Conclusion: Our case confirms that psychotherapy increases the quality of the patient's life in the fight against depression, modulation of one's own affect (narcissistic disorder), time management, development of communication and acceptance skills. It decreased anxiety and cardiovascular symptoms. The role of psychotherapy in the treatment of mental illness is beneficial and effective and facilitates compliance with antidepressant and anxiolytic treatment.

În faţa tendinţei relativ frecvente a multor persoane-cadre medicale sau nude a administra băutur... more În faţa tendinţei relativ frecvente a multor persoane-cadre medicale sau nude a administra băuturi carbogazoase de tipul Coca-Cola în cazul unor simptome gastrice de tip greaţă sau vărsături la pacienţii pediatrici sau adulţi, am încercat să cercetăm dacă există un fundament ştiinţific la baza acestei practici. Acidul fosforic prezent în această băutură este regăsit în produse care se pot elibera fără prescripţie medicală (OTC), recomandate în stări de greaţă şi vărsături. Pe de altă parte, literatura medicală citează studii în care s-a apelat la acest tip de băuturi în tratarea unor bezoari gastrici sau intestinali, fecaloame sau chiar în unele situaţii de reflux gastric sau gastropareză. Unul dintre studiile analizate relevă efectul antibacterian al acestui tip de băutură pe bacterii precum Bacillus cereus, Enterococcus faecalis, Escherichia coli, Escherichia coli O:157 H:7, Salmonella enteritidis, Yersinia enterocolitica. Aşteptăm efectuarea mai multor studii privind eficacitatea terapeutică antiemetică şi antimicrobiană a acestei băuturi şi punerea în balanţă a efectelor pozitive versus cele negative în utilizarea sa la copii.

Facing us the relatively frequent tendency of many people-medical professionals or not-to adminis... more Facing us the relatively frequent tendency of many people-medical professionals or not-to administer Coca-Colatype carbonated beverages in the case of nausea-type gastric symptoms or vomiting in pediatric or adult patients, we have tried to investigate whether there is a scientific basis for this practice. The phosphoric acid present in this beverage is found in over the counter products (OTC), recommended in the case of nausea and vomiting. On the other hand, the medical literature cites studies in which this type of beverage has been used in the treatment of gastric or intestinal bezoars, fecalomas or even in some cases of gastric reflux or gastroparesis. One of the studies analyzed reveals the antibacterial effect of this type of beverage, on bacteria such as Bacillus cereus, Enterococcus faecalis, Escherichia coli, Escherichia coli O:157 H:7, Salmonella enteritidis, Yersinia enterocolitica. We expect more studies to be carried out on the anti-emetic and antimicrobial therapeutic efficacy of this drink and to balance the positive versus negative effects in its use in children.

Bisphosphonates have been used for many years worldwide for the treatment of osteoporosis, Paget'... more Bisphosphonates have been used for many years worldwide for the treatment of osteoporosis, Paget's disease, but especially for the treatment of various bone density disorders such as tumour formations, bone metastases associated with osteolysis and hypercalcemia. The effects of bisphosphonates on the bones are due to their action on osteoclasts by inhibiting them. Although the existence of the risk of osteonecrosis of the jaw in patients undergoing bisphosphonate treatment is known, this pathology has been considered very rare for a long period and many times the risk of its occurrence has been ignored. Since the use of bisphosphonates is becoming more prevalent and the duration of such treatment is steadily increasing, the incidence of osteonecroses associated with this treatment is also increasing. OBJECTIVE. The objective of this study is to highlight the fact that the incidence of cases of bisphosphonate-related osteonecrosis of the jaw has been increasing in recent years. METHODS. We have carried out a search and analysis of all cases with osteonecrosis as main diagnosis in the Oral Maxillo-Facial Surgery Clinic during the period from January 2009 and December 2018. RESULTS. We found 84 patients with osteonecrosis of the jaw following bisphosphonate treatment. Of these, 37 were men and 47 women with an average age of 67 and 59 respectively. There were 26 cases (31%) with incidence in the jaw, 52 cases (62%) with incidence in the mandible, and 6 patients (7%) manifested bilateral osteonecrosis of the jaw. Of these, 20% were on the left side, 36% on the right side and 44% on the median line. CONCLUSIONS. The incidence of bisphosphonate-related osteonecrosis of the jaw is steadily increasing due to the more frequent use of a class of bisphosphonates with a strong impact on the oro-facial bone system.
