Lateral table tilt or a pelvic wedge are commonly used to reduce inferior vena cava compression during obstetric anesthesia in the supine position but limited studies have evaluated the role of intrauterine resuscitation by maneuvers. We compared the effects of table tilt versus pelvic tilt position on intrauterine resuscitation for caesarian section under spinal anesthesia. Materials and methods: This double blind randomized clinical trial conducted on 150 women undergoing cesarean section. Patients were randomly allocated to pelvic tilt (using a wedge under right buttock) or table tilt (15 degree left tilt of surgical table). Spinal anesthesia was performed, Sensory and motor block were evaluated every minute and time of block onset was recorded. Mean arterial blood pressure, heart rate and arterial oxygen saturation were measured every 5 minutes till the end of the operation. Fetal Apgar scores were recorded at 1 st and 5 th minutes after delivery. Data were analyzed using SPSS software. Results: MAP and HR were significantly higher in table tilt group in comparison with pelvic tilt (p< 0/05). No significant difference was detected between the two groups in terms of sensory and motor block (p>0/05). Apgar scores of neonates were almost the same and no statistically significant differences between the two groups in 1 st and 5 th minutes Apgar score was noted(p>0/05). Conclusions: Table tilt of 15 degrees during cesarean delivery with spinal anesthesia was effective in preventing maternal hypotension and bradycardia and is favorable in intrauterine resuscitation.

In this chapter, we report p+-n+-i-n+ (n -type) and n+-p+-i-p+ (p -type) SiGe/Si hetero double ga... more In this chapter, we report p+-n+-i-n+ (n -type) and n+-p+-i-p+ (p -type) SiGe/Si hetero double gate tunnel field-effect transistor (TFET) (H-DGTFET) for low power circuit applications. To achieve the optimum performance of the above devices, the Silicon (Si) and Germanium (Ge) composition of 30% and 70% (SiO3 Ge0.7), respectively, considered in source region, and a heavily doped (HD) layer placed in the channel near the source-channel junction are employed. Due to lower tunnel resistance offered by SiGe, the technology computer aided design (TCAD) device simulations of both the configurations show superior results in terms of DC and analog/radio frequency (RF) parameters as compared to the conventional TFETs. However, linearity of n-type device is analyzed in terms of VIP2, VIP3, and PldB. Furthermore, the circuit-level performance assessment is done by implementing complementary primary digital circuits (such as an inverter, NAND, and NOR logics) using lookup table-based Verilog-A model of the H-DGTFET. Comparison table shows impressive results in terms of digital performance parameters such as static noise margin (SNM), noise margin high (NMH ), noise margin low (NML ), high-to -low delay (τphl ), low-to-high delay (τphl,), and propagation delay (τp) as compared to the recently reported work

Abstract The research and practice of information systems have progressed by leaps and bounds due... more Abstract The research and practice of information systems have progressed by leaps and bounds due to the emergence of big data analytics and machine learning. The paper undertakes a bibliometric study to analyze the contributions of major authors, universities/organizations, and countries in terms of productivity, citations, and bibliographic coupling. A sample of 2160 articles from the Scopus for the period 2006–2020 is the basis of the study. The publications are grouped into five clusters, of which Cluster 1 is consistently dominant in the information systems publication landscape. Cluster 2 includes published studies on the Internet of Things, security, and cloud computing, which have also been widely researched. Cluster 3, the third-largest cluster, has attempted to investigate social media analytics. Cluster 4 aims to look into the impact of classification and predictive, which is found to have sustained research interest. Topics with scant coverage in terms of papers are primarily in Cluster 5, indicating saturation in the area and the need for conducting inter-disciplinary studies. The results of our study provide valuable insights for potential contributors and global audiences in terms of emerging topics for research.

In this paper, for the first time, the impact of drain doping profile on device electrostatics an... more In this paper, for the first time, the impact of drain doping profile on device electrostatics and circuit performance of novel InAs/Si Hetero-junction Double Gate Tunnel Field Effect Transistor (H-DGTFET) has been investigated. A highly doped layer placed near the source and channel junction decreases the width of the depletion region, thus, improving the ONcurrent and circuit performance. For this purpose, the effects of drain doping profile on the analog/RF performance of H-DGTFET is studied in terms of transconductance (g m), parasitic capacitances, cutoff frequency (f T) and gain bandwidth (GBW) product. The value of f T is increased by 13.84% and the GBW is improved by 144.7% for GD profile in Drain with CL ¼ 0.05 when compared to UD profile. Further, the impact of drain doping profile on the circuit performance has been investigated by implementing digital and analog/RF circuits most widely used for nanoelectronic applications. For this, Verilog-A model has been developed for InAs/Si H-DGTFET. The circuit level performance assessment is carried out by implementing inverter, common source amplifier, inverter amplifier and pseudo differential amplifier by using Complementary TFET (CTFET) technology in a Verilog-A environment.

The proposed structure does not required impurity doping for formation of the drain and the source regions. In this concern, source and drain regions are formed by selecting appropriate work-function of metal electrode. The source and drain regions are not formed by conventional ways of ion implantation or diffusion. Hence, proposed structure is immune greatly to the process variation, issues of doping control and random dopant fluctuations which are serious problems in ultrathin silicon devices. For further improvement in ON state current and analogue/RF figures of merit dual work function of single gate material is considered. The electrical characteristics of the proposed device with the D-VTFET are simulated and compared.

Gingival myiasis is a rare disease in the humans associated with poor oral hygiene, suppurative oral lesions, alcoholism, and senility, among other conditions. We present a case of gingival myiasis in the maxillary anterior region on the palatal surface in a 21-year-old mentally challenged male with moderate periodontitis and neurologic deficit. The diagnosis was made on the presence of larvae in the lesion. Treatment done was a manual removal of the larvae, one by one, with the help of the clinical forceps, surgical debridement of the oral wound, and subsequent management of the periodontal disease.

Traumatic rupture of the right dome of the diaphragm is an uncommon clinical entity. The diagnosi... more Traumatic rupture of the right dome of the diaphragm is an uncommon clinical entity. The diagnosis is difficult due to lack of specificity in clinical signs and chest film findings. Clinical incidence of rupture of the right dome of the diaphragm due to blunt trauma is much less common (10%) compared to the left (90%) and usually associated with more grievous injuries with very high pre hospital mortality thus accounting for rare clinical diagnosis. We report a case of 26 year old male who had the blunt trauma chest referred to us after 7 days of injury with complaints of shortness of breath and vomiting. On investigations the patient was diagnosed as a case of the ruptured right dome of the diaphragm with intrathoracic herniation of the stomach. The patient also had fracture pelvis. Surgical exploration was done through right 6th intercostal space which revealed intrathoracic herniation of the stomach, liver and transverse colon, which were healthy. Contents reduced into the abdomen and diaphragm was repaired. Post operative chest X-ray suggested complete expansion of the right lung with no residual herniation of abdominal contents. Post operative recovery was uneventful with the patient discharged on 14th post operative day.

Studies have clearly shown benefits in terms of survival, improvement in dyspnea, exercise tolerance, and other associated conditions like pulmonary hypertension that arise due to the disease. However, guidelines regarding prescription of oxygen and to make choice of the delivery devices are not explicit. Furthermore, there still prevails a large unawareness and confusion among physicians to properly prescribe and advise patients about the use of oxygen for home therapy customized as per individual needs. The availability of several new oxygen delivery and conservation devices and techniques over the past few decades has provided a wide spectrum of options to be used in combinations or alone. This article retrospectively tries to review the studies, trials, and researches published so far in this field to give a broad idea based on consistent scientific evidence to help physicians frame their set of guidelines for prescribing long-term oxygen therapy.

Objectives: Trauma has emerged as a major public health problem in developing as well as develope... more Objectives: Trauma has emerged as a major public health problem in developing as well as developed countries, and vascular trauma is an important component of this problem. The objective of the present study was to analyze the causes of injury, surgical approaches, outcome, and complications with extremity vascular trauma at a tertiary level referral center in North India. Materials and Methods: Retrospective analyses of 3948 patients operated for peripheral vascular injuries between November 2003 and November 2013 was done. Diagnosis was made by physical examination and hand held Doppler alone or in combination with computed tomography CT angiography. Primary vascular repair was carried out where possible; if not possible the interposition vein graft was placed. Patients with isolated venous trauma and patients with obviously unsalvageable lower extremity injury requiring primary amputation were excluded from the study. Results: Of the 3948 patients, 3347 were males (84.78%) and 601 of them were females (15.22%), and their aged ranging from 5 years to 80 years. Mean duration of the presentation was 11 h after the injury. The most common etiological reason was road traffic accidents; nearly 67% of the patients were in penetrating trauma group and 58% among all blunt traumas. Incidence of concomitant orthopedic injuries was very 73.6%. The most common injured artery was brachial artery (36.4%), followed by popliteal artery (30.4%) and femoral artery (21.7%). Surgical procedures performed were primary repair in 58.3%, whereas interposition vein grafts were placed in 41.4% cases. Of all, 78% had viable and functional limbs and 13% had viable, but non-functional limbs and rate of amputation after repair was 9%. Conclusions: Early diagnosis and treatment during vascular injuries has a prognostic value for saving the extremity and life of the patient. Vascular injuries require immediate surgical intervention, regardless of localization. Patients who suffer vascular injuries to the extremities should be transferred to vascular surgery centers as soon as possible. Priorities must be established in the management of associated injuries, and delayed repair must be avoided when ischemic changes are present.

Tubercular aneurysmal dilatation of aorta with dissection leading to aortic regurgitation is a rare but important complication of tuberculosis. With worldwide resurgence of tuberculosis due to increasing incidence of drug-resistant tuberculosis and its association with acquired immunodeficiency syndrome, the tubercular aneurysm has become a real clinical entity. Although tubercular aortitis is fairly common, tuberculous mycotic aneurysm of the aorta is rare, with involvement of the aortic root being exceedingly uncommon. Here we describe a case of 18 year old male presenting with severe breathlessness and was found to have dissecting aortic root aneurysm with aortic regurgitation with active pulmonary tuberculosis and spinal deformity in the form of kyphoscoliosis. He underwent a Bentall procedure, and excised aortic root tissue showed epithelioid cell granulomas with panarteritis.

BackgroundThe Government of India has been issuing notifications regarding packaging and labellin... more BackgroundThe Government of India has been issuing notifications regarding packaging and labelling rules for tobacco products since 2003 under the ‘Cigarettes and Other Tobacco Products Act’ (COTPA) guidelines.MethodsThe present cross-sectional study was carried out during November 2016 to March 2017 to assess the extent of compliance with the COTPA prescribed packaging and labelling rules for packages of tobacco products marketed in 11 slum areas of Bhubaneswar, India. From 81 retail outlets, 42 different brands of tobacco products were purchased which consisted of 23 in smoking form (17 brands of cigarette, 6 brands of bidi) and 19 in smokeless form (9 brands of gutkha, 4 brands of khaini and 6 brands of betel quid with tobacco).ResultsIn most of the product packages, particularly in smokeless tobacco, the health warnings were not in compliance with the COTPA specifications. In this paper, for the first time, the impact of drain doping profile on device electrostatics an... more In this paper, for the first time, the impact of drain doping profile on device electrostatics and circuit performance of novel InAs/Si Hetero-junction Double Gate Tunnel Field Effect Transistor (H-DGTFET) has been investigated. A highly doped layer placed near the source and channel junction decreases the width of the depletion region, thus, improving the ONcurrent and circuit performance. For this purpose, the effects of drain doping profile on the analog/RF performance of H-DGTFET is studied in terms of transconductance (g m), parasitic capacitances, cutoff frequency (f T) and gain bandwidth (GBW) product. The value of f T is increased by 13.84% and the GBW is improved by 144.7% for GD profile in Drain with CL ¼ 0.05 when compared to UD profile. Further, the impact of drain doping profile on the circuit performance has been investigated by implementing digital and analog/RF circuits most widely used for nanoelectronic applications. For this, Verilog-A model has been developed for InAs/Si H-DGTFET. The circuit level performance assessment is carried out by implementing inverter, common source amplifier, inverter amplifier and pseudo differential amplifier by using Complementary TFET (CTFET) technology in a Verilog-A environment.

The proposed structure does not required impurity doping for formation of the drain and the source regions. In this concern, source and drain regions are formed by selecting appropriate work-function of metal electrode. The source and drain regions are not formed by conventional ways of ion implantation or diffusion. Hence, proposed structure is immune greatly to the process variation, issues of doping control and random dopant fluctuations which are serious problems in ultrathin silicon devices. For further improvement in ON state current and analogue/RF figures of merit dual work function of single gate material is considered. The electrical characteristics of the proposed device with the D-VTFET are simulated and compared.

Gingival myiasis is a rare disease in the humans associated with poor oral hygiene, suppurative oral lesions, alcoholism, and senility, among other conditions. We present a case of gingival myiasis in the maxillary anterior region on the palatal surface in a 21-year-old mentally challenged male with moderate periodontitis and neurologic deficit. The diagnosis was made on the presence of larvae in the lesion. Treatment done was a manual removal of the larvae, one by one, with the help of the clinical forceps, surgical debridement of the oral wound, and subsequent management of the periodontal disease.

The diagnosis is difficult due to lack of specificity in clinical signs and chest film findings. Clinical incidence of rupture of the right dome of the diaphragm due to blunt trauma is much less common (10%) compared to the left (90%) and usually associated with more grievous injuries with very high pre hospital mortality thus accounting for rare clinical diagnosis. We report a case of 26 year old male who had the blunt trauma chest referred to us after 7 days of injury with complaints of shortness of breath and vomiting. On investigations the patient was diagnosed as a case of the ruptured right dome of the diaphragm with intrathoracic herniation of the stomach. The patient also had fracture pelvis. Surgical exploration was done through right 6th intercostal space which revealed intrathoracic herniation of the stomach, liver and transverse colon, which were healthy. Contents reduced into the abdomen and diaphragm was repaired. Post operative chest X-ray suggested complete expansion of the right lung with no residual herniation of abdominal contents. Post operative recovery was uneventful with the patient discharged on 14th post operative day.

Studies have clearly shown benefits in terms of survival, improvement in dyspnea, exercise tolerance, and other associated conditions like pulmonary hypertension that arise due to the disease. However, guidelines regarding prescription of oxygen and to make choice of the delivery devices are not explicit. Furthermore, there still prevails a large unawareness and confusion among physicians to properly prescribe and advise patients about the use of oxygen for home therapy customized as per individual needs. The availability of several new oxygen delivery and conservation devices and techniques over the past few decades has provided a wide spectrum of options to be used in combinations or alone. This article retrospectively tries to review the studies, trials, and researches published so far in this field to give a broad idea based on consistent scientific evidence to help physicians frame their set of guidelines for prescribing long-term oxygen therapy.

Objectives: Trauma has emerged as a major public health problem in developing as well as develope... more Objectives: Trauma has emerged as a major public health problem in developing as well as developed countries, and vascular trauma is an important component of this problem. The objective of the present study was to analyze the causes of injury, surgical approaches, outcome, and complications with extremity vascular trauma at a tertiary level referral center in North India. Materials and Methods: Retrospective analyses of 3948 patients operated for peripheral vascular injuries between November 2003 and November 2013 was done. Diagnosis was made by physical examination and hand held Doppler alone or in combination with computed tomography CT angiography. Primary vascular repair was carried out where possible; if not possible the interposition vein graft was placed. Patients with isolated venous trauma and patients with obviously unsalvageable lower extremity injury requiring primary amputation were excluded from the study. Results: Of the 3948 patients, 3347 were males (84.78%) and 601 of them were females (15.22%), and their aged ranging from 5 years to 80 years. Mean duration of the presentation was 11 h after the injury. The most common etiological reason was road traffic accidents; nearly 67% of the patients were in penetrating trauma group and 58% among all blunt traumas. Incidence of concomitant orthopedic injuries was very 73.6%. The most common injured artery was brachial artery (36.4%), followed by popliteal artery (30.4%) and femoral artery (21.7%). Surgical procedures performed were primary repair in 58.3%, whereas interposition vein grafts were placed in 41.4% cases. Of all, 78% had viable and functional limbs and 13% had viable, but non-functional limbs and rate of amputation after repair was 9%. Conclusions: Early diagnosis and treatment during vascular injuries has a prognostic value for saving the extremity and life of the patient. Vascular injuries require immediate surgical intervention, regardless of localization. Patients who suffer vascular injuries to the extremities should be transferred to vascular surgery centers as soon as possible. Priorities must be established in the management of associated injuries, and delayed repair must be avoided when ischemic changes are present.

Tubercular aneurysmal dilatation of aorta with dissection leading to aortic regurgitation is a rare but important complication of tuberculosis. With worldwide resurgence of tuberculosis due to increasing incidence of drug-resistant tuberculosis and its association with acquired immunodeficiency syndrome, the tubercular aneurysm has become a real clinical entity. Although tubercular aortitis is fairly common, tuberculous mycotic aneurysm of the aorta is rare, with involvement of the aortic root being exceedingly uncommon. Here we describe a case of 18 year old male presenting with severe breathlessness and was found to have dissecting aortic root aneurysm with aortic regurgitation with active pulmonary tuberculosis and spinal deformity in the form of kyphoscoliosis. He underwent a Bentall procedure, and excised aortic root tissue showed epithelioid cell granulomas with panarteritis.
