Research paper thumbnail of Mental disorders pattern in staff of a military unit in Iran: the role of metabolic syndrome on latent class membership

BMC Psychiatry, Oct 19, 2021

This study aimed to identify the subgroups of staff based on mental disorders and assess the independent role of metabolic syndrome (MetS) on the membership of participants in each latent class. Methods: This cross-sectional study was conducted among 694 staff of a military unit in Tehran in 2017. All staff of this military unit was invited to participate in this study. The collected data included demographic characteristics, anthropometric measures, blood pressure, biochemical parameters, and mental disorders. We performed latent class analysis using a procedure for latent class analysis (PROC LCA) in SAS to identify class membership of mental disorders using Symptom Checklist-90. Results: Three latent classes were identified as healthy (92.7%), mild (4.9%), and severe (2.4%) mental disorders. Having higher age significantly decreased the odds of belonging to the mild class (adjusted OR (aOR = 0.21; 95% confidence interval (CI): 0.05-0.83) compared to the healthy class. Also, obesity decreased the odds of membership in mild class (aOR = 0.10, 95% CI: 0.01-0.92) compared to healthy class. On the other hand, being female increased the odds of being in severe class (aOR = 9.76; 95% CI: 1.35-70.65) class in comparison to healthy class. Conclusion: This study revealed that 7.3% of staff fell under mild and severe classes. Considering educational workshops in the workplace about mental disorders could be effective in enhancing staff's knowledge of these disorders. Also, treatment of comorbid mental disorders may help reduce their prevalence and comorbidity.

Accordingly, the present study intends to evaluate the effectiveness of brief cognitive-behavioral therapy on reducing suicidal ideation of a group of soldiers. Methods: In a quasi-experimental study based on a pre-and post-test design and control group, 24 soldiers were first selected using purposive sampling and then randomly divided into two experimental and control groups of 12 people. Brief cognitive-behavioral therapy was performed in the form of 10 sessions on a weekly basis for the experimental group. Data collection tools were Beck Scale for Suicidal Ideation (BSSI). The collected data were analyzed using univariate analysis of covariance methods in SPSS (V21.0) software. Results: Data analysis showed that brief cognitive-behavioral therapy was significantly effective in reducing soldiers' suicidal ideation after the intervention (P <0.05). Conclusion: Based on the research findings, it can be concluded that brief cognitive-behavioral therapy is effective in reducing soldiers' suicidal ideation. Therefore, the intervention used in the present study can be used as an appropriate method to reduce suicidal ideation, especially in soldiers and militaries, in psychological centers.

OBJECTIVE In response to the increasing rates of suicide in military personnel throughout the wor... more OBJECTIVE In response to the increasing rates of suicide in military personnel throughout the world, there is an increasing focus on the development and implement of interventions aimed at preventing suicide among this group. Therefore, the goal of the present systematic review was to examine the effectiveness of interventions focused on preventing suicidal ideation and behavior in military personnel. METHOD Search for articles was conducted in PsycINFO, PubMed, Web of Science, EMBASE, Google Scholar, Cochrane Central Register of Controlled Trials (CENTRAL), IranMedex, Scientific Information Database (SID), and MagIran. International databases were searched from June 2008 to May 2019, and Iranian databases were searched from their inception to May 2019. RCT and non-RCT studies focused on the effectiveness of preventive interventions for suicide in military personnel were included in the analysis. Narrative synthesis of results was the main strategy for data analysis. RESULTS According to the inclusion and exclusion criteria, 18 articles were selected. Interventions were divided into four categories according to their nature: interventions based on psychotherapy, interventions based on crisis management, interventions based on pharmacotherapy, and community-based interventions. In addition, most studies had moderate methodological quality. CONCLUSION Most of the preventive interventions were effective in reducing suicidal ideation and behavior in military personnel; however, in some cases, there were serious challenges in terms of effectiveness. Interventions based on cognitive-behavioral approaches constitute a significant portion of the interventions. Overall, RCTs and non-RCTs, especially community-based studies, need to use more rigorous examinations in order to gain research and clinical support. HIGHLIGHTS Interventions based on psychotherapy and community-based interventions were the most commonly used interventions, respectively. The majority of interventions based on psychotherapy came from cognitive-behavioral approaches. There are few evidence-based studies on prevention of suicide in military personnel.

Results Three latent classes were identified as healthy (92.7%), mild (4.9%), and severe (2.4%) mental disorders. Having higher age significantly decreased the odds of belonging to the mild clas...
