Gastrointestinal stromal tumours (GIST) are rare mesenchymal neoplasms that can develop in the en... more Gastrointestinal stromal tumours (GIST) are rare mesenchymal neoplasms that can develop in the entire length of the gastrointestinal tract or sometimes from the omentum and mesentery. In most cases, they arise from the stomach, less often the jejunum or ileum. We report the case of a 53-year-old-female patient, without significant medical history, consulting for chronic epigastric pain, with no other associated signs. She had underwent an abdominal ultrasound completed with a Contrast-enhanced computed tomography (CT) of the abdomen showing a large tissular mass developed at the expense of the cephalic portion of the pancreas, with central calcifications and heterogeneous enhancement after contrast injection. Pancreaticoduodenectomy, partial colectomy and antrectomy has been performed and histological examination with immunohistochemical study had concluded the diagnosis of pancreatic primary GIST.

The use of propensity score computing matching is an interesting and original approach, which reduces bias significantly, but does not eliminate it. Radical surgery was performed in highly selected patients in the initial series (17•4 per cent). In essence, conservative surgery should consist of: decompression-evacuation, 'sterilization', management of cyst-bile duct communications, management of the remaining cavity, and drainage 1 , but it is unclear from the paper whether this strategy was followed exactly in all patients. Selection of and experience with a technique affect the success of each stage and reflect upon the endpoints in assessing the efficacy of conservative surgery. Safe decompression and cleansing of a cyst cavity reduce the incidence of primary recurrence. The type of protoscolicides and the exposure time to these agents determine the efficacy of 'sterilization' of the cavity; in this context, hydrogen peroxide may not be the optimal choice. The detection and management of cyst-bile duct communications determine the incidence of fistulas and affect length of hospital stay. Cholangiography through the communication orifice may help define the level of the cyst-bile duct communication and any debris contained in it. More than 80 per cent of such communications can be managed by simple suture, with no need for external drainage. The three techniques employed in the present study for obliteration of the cavity (omentoplasty, capitonnage, drainage) have different success and complication rates. In radical surgery, cystectomy is done by dissection along the cyst surface, reducing postoperative bile leakage and bleeding rates, and this manages exogenous vesiculation 2. It is prudent to define recurrence rate as recurrence at either the primary cyst site or other liver sites. On-site early recurrence reflects operative failure and is extremely rare with radical surgery. Liver cysts grow at a rate of 10-40 mm per year, so it is important to define recurrent cyst size and time elapsed to exclude overlooked cysts. Most studies demonstrate a difference between radical and conservative surgery in recurrence and complication rates and hospital stay, especially at hepatopancreatobiliary centres. Surgery is the standard, but not the only, option. The results of the present study, although interesting, will not affect how surgery for hydatid disease is done worldwide. Management of hepatic hydatid cysts should be image-guided, stage-specific, based on cyst characteristics, and dependent on available expertise and equipment 3 .

Clip migration following laparoscopic cholecystectomy (LC) is a rare and late complication of LC.... more Clip migration following laparoscopic cholecystectomy (LC) is a rare and late complication of LC. The first case of surgical clip migration after LC was reported in 1992, and since then less than 100 cases have been reported in the literature. We report the case of cholangitis secondary to a surgical clip migration in an 83 years old male patient, 8 years after LC. Contrast-enhanced computed tomography of the abdomen (CT) showed intra and extrahepatic ducts dilatation secondary to a hyperdense object located in the distal common bile duct (CBD). It was removed successfully from the CBD by endoscopic retrograde cholangiopancreatography after sphincterotomy. At the last follow-up of one year after her admission, the patient is symptom-free with normal liver enzyme and abdominal CT. Surgical clip migration into CBD, should be included in the differential diagnosis while treating patients with the past surgical history of LC. Early diagnosis and treatment of this complication can avoid serious complications.

The coexistence of lobular panniculitis, polyarthritis, and intraosseous fat necrosis in patients... more The coexistence of lobular panniculitis, polyarthritis, and intraosseous fat necrosis in patients with acute pancreatitis is commonly referred to as pancreatitis, panniculitis, and polyarthritis (PPP) syndrome. It is a rare condition associated with serious complications and high mortality. A 70-year-old female was admitted with severe acute necrotizing pancreatitis due to gallstone disease. Laboratory findings indicated an intense systemic inflammatory response syndrome (SIRS). The patient rapidly progressed toward persistent organ failure. During her hospital stay, she also developed panniculitis and polyarthritis, related to severe acute pancreatitis. Eventually, the patient expired despite medical therapy.

The emergence of smart antenna technology gave the advantage to reduce interference inherent in d... more The emergence of smart antenna technology gave the advantage to reduce interference inherent in dense multipath propagation channels to improve coverage and increase system capacity, so, various localization systems have been proposed based on the Impulse Radio Ultra Wide band (IR-UWB) signals using direction of arrival estimation. In order to ensure high precision in indoor environment, DOA estimation techniques have been developed. In this paper, performance of many high resolution methods for Impulse Radio Ultra Wide band (IR-UWB) systems in frequency domain such as ROOT-MUSIC, estimation of signal parameters via rotational invariance technique (ESPRIT) and Matrix PENCIL are evaluated, as well as these techniques are compared also with the performance corresponding to the Cramer- Rae lower bound (CRB). Synthesis results and relevant performance comparisons are presented.

Trichobezoar is an underdiagnosed entity that has to be considered in children and adolescents, e... more Trichobezoar is an underdiagnosed entity that has to be considered in children and adolescents, especially females, suffering from trichotillomania and trichophagia. Late diagnosis of trichobezoars showed that they cause gastrointestinal bleeding or perforation. A 17-year-old girl patient was admitted with abdominal pain and gastrointestinal bleeding. On abdominal examination, a well-defined mass in the epigastrium and the left upper quadrant was identified. Upper gastrointestinal (GI) endoscopy identified an enormous trichobezoar, which was later removed by surgery. Misdiagnosed gastric bezoars may cause life-threatening complications. Early detection for trichobezoar requires effective screening of trichotillomania. Psychiatric counseling is important to prevent bezoar recurrence.

A giant choledochal cyst is defined as a cyst with a maximum diameter of ≥ 10 cm. It is an unusual entity and rarely revealed in adulthood. We describe the case of a giant, infected CC presenting as acute pancreatitis with results of abdominal ultrasound and computed tomography consistent with a pancreatic pseudocyst. The diagnosis of CC was made on magnetic resonance cholangiopancreatography (MRCP) findings. We proceeded with cholecystectomy and the complete resection of the diverticulum after its dissection. The defect in the common bile duct was closed transversally over a T-tube. At the last follow-up two years after his admission, the patient is symptom-free with normal liver enzymes To the best of our knowledge, this is the first case reported of giant CC complicated with both infection and pancreatitis Categories: General Surgery

Huit malades ont eu une résection-anastomose : 5 avaient été traités par prostatectomie et se son... more Huit malades ont eu une résection-anastomose : 5 avaient été traités par prostatectomie et se sont tous compliqués (4 fistules anastomotiques) alors que ceux ayant bénéficié d'une RT prostatique n'ont pas présenté de complication (n = 3). Trois malades ont eu une intervention de Hartman (prostatectomie = 2), 4 une amputation abdomino-périnéale (prostatectomie = 3), et 2 une tumorectomie après radiothérapie pré-opératoire (prostatectomie = 2). Aucune complication n'est survenue chez ces 9 patients. Conclusion.-Cette étude ne retrouve pas la morbidité élevée liée aux antécédents de radiothérapie mais a mis en évidence un taux élevé de fistule en présence d'un antécédent de prostatectomie. La réalisation d'une anastomose basse doit être discutée dans cette situation.

Celiacomesenteric trunk (CMT) refers to the common origin of celiac trunk and superior mesenteric... more Celiacomesenteric trunk (CMT) refers to the common origin of celiac trunk and superior mesenteric artery which is a very rare anatomical variation. CMT is incidentally diagnosed during angiography or abdominal computed tomography scanning. The diagnosis of CMT may inform surgical practice and prevent damage during invasive radiologic procedures, lowering thus the rate of iatrogenic errors. Complete common mesentery is in its turn a rarer congenital anomaly that arises from an abnormal rotation of primitive small intestine during embryonic development. We report a case of a 48-year-old man, suffering from chronic abdominal pain, and postprandial discomfort. The patient underwent an abdominal contrast-enhanced computed tomography that detected a CMT associated with common complete mesentery. L'intervention en urgence a trouvé une hernie paraduodénale gauche obstructive à l'origine d'un volvulus, avec nécrose des anses grêles incarcérées pour un patient, traité par résection anastomose en un temps et un grêle souffrant mais viable pour le deuxième malade, traité par la réduction du grêle incarcéré et l'obturation de l'orifice herniaire. Nous discutons à l'occasion de ces observations les particularités diagnostiques et thérapeutiques de cette affection rare.

Ligation of the cystic duct is usually performed with surgical clips, which have the potential to migrate into the common bile duct with time. This paper describes a case of cholangitis secondary to clip migration in a 42-year-old male patient 9 years after the initial laparoscopic cholecystectomy. Magnetic resonance cholangiopancreaography imaging revealed a surgical clip lodged in dilated common bile duct. The patient was managed successfully by endoscopic retrograde cholangiopancreatography.

Surgery remains the basic treatment, and can be divided into radical and conservative approaches. The purpose of this study was to compare the results of radical and conservative surgery in the treatment of liver hydatid cysts. Methods Data from all patients with liver hydatid cyst treated in a hepatobiliary surgical unit, between January 1990 and December 2010, were retrieved from a retrospective database. To minimize selection bias, propensity score matching was performed, based on 17 variables representing patient characteristics and operative risk factors. The primary outcome measure was hydatid cyst recurrence. Results One hundred and seventy patients were matched successfully, representing 85 pairs who had either a radical or a conservative approach to surgery. But Determiner les facteurs epidemiologiques, clinicobiologiques, endoscopiques et therapeutiques... more But Determiner les facteurs epidemiologiques, clinicobiologiques, endoscopiques et therapeutiques associes au risque de colectomie chez des patients presentant une CAG corticoresistante. Materiel et methodes Nous avons mene une etude retrospective sur 10 ans colligeant les patients hospitalises pour une RCH compliquee de CAG corticoresistante. Le diagnostic de CAG etait retenu selon les criteres d’Oxford modifies. Resultats Durant la periode de l’etude, 75 patients ont ete colliges. L’âge moyen au moment du diagnostic de la RCH etait 45 ans. Il s’agit de 30 hommes et 45 femmes suivis sur une periode moyenne de 34,66 mois [6–120 mois]. Les antecedents familiaux de MICI ont ete notes chez 6 patients. La CAG etait inaugurale chez 24 patients (42,1 %) et emaillant l’evolution de la maladie chez 33 patients (57,9 %), survenant apres un delai moyen de 8 ans par rapport a la de decouverte de RCH. Le recours a la colectomie etait necessaire chez 26 patients (45,61 %). L’indication de la colectomie etait : un echec du traitement par la ciclosporine chez 18 patients et suite a une recidive d’une poussee severe sous thiopurines chez 8 malades avec un delai moyen de 9,6 mois [2–22 mois]. Les facteurs associes a un risque accru de colectomie au cours d’une CAG corticoresistante etaient : l’antecedent d’une CAG (p = 0,039), un traitement anterieur par les thiopurines (p = 0,043) ainsi qu’un taux d’albumine inferieur a 30 g/L (p = 0,026). CMT is incidentally diagnosed during angiography or abdominal computed tomography scanning. The diagnosis of CMT may inform surgical practice and prevent damage during invasive radiologic procedures, lowering thus the rate of iatrogenic errors. Complete common mesentery is in its turn a rarer congenital anomaly that arises from an abnormal rotation of primitive small intestine during embryonic development. We report a case of a 48-year-old man, suffering from chronic abdominal pain, and postprandial discomfort. The patient underwent an abdominal contrast-enhanced computed tomography that detected a CMT associated with common complete mesentery. L'intervention en urgence a trouvé une hernie paraduodénale gauche obstructive à l'origine d'un volvulus, avec nécrose des anses grêles incarcérées pour un patient, traité par résection anastomose en un temps et un grêle souffrant mais viable pour le deuxième malade, traité par la réduction du grêle incarcéré et l'obturation de l'orifice herniaire. Nous discutons à l'occasion de ces observations les particularités diagnostiques et thérapeutiques de cette affection rare.

Ligation of the cystic duct is usually performed with surgical clips, which have the potential to migrate into the common bile duct with time. This paper describes a case of cholangitis secondary to clip migration in a 42-year-old male patient 9 years after the initial laparoscopic cholecystectomy. Magnetic resonance cholangiopancreaography imaging revealed a surgical clip lodged in dilated common bile duct. The patient was managed successfully by endoscopic retrograde cholangiopancreatography.

Surgery remains the basic treatment, and can be divided into radical and conservative approaches. The purpose of this study was to compare the results of radical and conservative surgery in the treatment of liver hydatid cysts. Methods Data from all patients with liver hydatid cyst treated in a hepatobiliary surgical unit, between January 1990 and December 2010, were retrieved from a retrospective database. To minimize selection bias, propensity score matching was performed, based on 17 variables representing patient characteristics and operative risk factors. The primary outcome measure was hydatid cyst recurrence. Results One hundred and seventy patients were matched successfully, representing 85 pairs who had either a radical or a conservative approach to surgery. Materiel et methodes Nous avons mene une etude retrospective sur 10 ans colligeant les patients hospitalises pour une RCH compliquee de CAG corticoresistante. Le diagnostic de CAG etait retenu selon les criteres d’Oxford modifies. Resultats Durant la periode de l’etude, 75 patients ont ete colliges. L’âge moyen au moment du diagnostic de la RCH etait 45 ans. Il s’agit de 30 hommes et 45 femmes suivis sur une periode moyenne de 34,66 mois [6–120 mois]. Les antecedents familiaux de MICI ont ete notes chez 6 patients. La CAG etait inaugurale chez 24 patients (42,1 %) et emaillant l’evolution de la maladie chez 33 patients (57,9 %), survenant apres un delai moyen de 8 ans par rapport a la de decouverte de RCH. Le recours a la colectomie etait necessaire chez 26 patients (45,61 %). L’indication de la colectomie etait : un echec du traitement par la ciclosporine chez 18 patients et suite a une recidive d’une poussee severe sous thiopurines chez 8 malades avec un delai moyen de 9,6 mois [2–22 mois]. Les facteurs associes a un risque accru de colectomie au cours d’une CAG corticoresistante etaient : l’antecedent d’une CAG (p = 0,039), un traitement anterieur par les thiopurines (p = 0,043) ainsi qu’un taux d’albumine inferieur a 30 g/L (p = 0,026).
