Abstract: The development of effective devices for target vessel exposure and stabilization and t... more Abstract: The development of effective devices for target vessel exposure and stabilization and the subsequent widespread diffusion of off-pump coronary artery bypass (OPCAB) as an alternative technique to conventional on-pump revascularization is discussed. Application of the OPCAB concept for patients with specific cardiac and extracardiac risks and the benefits of avoiding cardiopulmonary bypass (CPB) are explored. The consequences of incomplete revascularization and the high mortality for conversion to CPB are examined. OPCAB with assisted mini-CPB is confirmed as an emerging option which provides an excellent surgical exposure with a stable cardiac output for performing an ideal anastomosis and decreasing the inflammatory deleterious effects of the conventional CPB.

: Cardiopulmonary bypass (CPB) has revolutionized our ability to operate on the heart, great vess... more : Cardiopulmonary bypass (CPB) has revolutionized our ability to operate on the heart, great vessels, and aorta in addition to providing means of short-term support for reversible causes of cardiac and/or respiratory failure. This chapter summarizes current concepts of CPB with particular emphasis on the biomaterial developments currently taking place. Although routine application of CPB is shrinking under pressure from less invasive techniques, this may represent an opportunity to deliver a more highly evolved perfusion to the core of very sick and complex patients who may not be suitable candidates for alternative therapy. These patients will benefit from the application of new technologies currently under development, such as smaller perfusion circuits, filtration techniques, improved biomaterial surfaces, smart membranes, and biosensor technology, all aimed at making clinical perfusion a safer and more predictable procedure for the patient.

Kandidemi, yüksek ölüm oranları ile dünya çapında sık görülen bir enfeksiyondur. Pandemi günlerin... more Kandidemi, yüksek ölüm oranları ile dünya çapında sık görülen bir enfeksiyondur. Pandemi günlerinde, tüm dünyada yapılan kan bağışlarında ciddi bir azalma olmuştur. Yine 2019-2020 yılları karşılaştırıldığında bağış sayısının üçte bir oranında düştüğü görülmüştür. Türk Kızılayı verileri de pandeminin ilk günlerinde kan bağışının %90 oranında düştüğünü göstermektedir. Amerika’da yapılan bir çalışmada ise kan bağışı azalmasının yanında elektif cerrahiler ertelendiği için kan kullanımının da azaldığı görülmüştür. Bağış sırasında COVID-19’a yakalanma endişesi ve sokağa çıkma yasağı bağış sayısındaki azalmanın ana sebepleridi. Dünyada ve Türkiye’de de çözüm olarak öncelikle stoklar kullanılmış, sonrasında sosyal medya faaliyetleri ile bağışa özendirme kampanyaları yürütülerek ve daha önce bağış yapmış kişilere kısa mesaj gönderilerek kan bağışının artırılmasına çalışılmıştır. Umbrella studies have been developed to address the need for more efficient and flexible trial structures, predominantly for cancer treatments. The umbrella concept foresees data collection within a unifying trial structure, to which one or more substudies may be added at any time to address product- or therapy-specific questions. To our knowledge, the umbrella concept has not yet been used in the medical device area, but it may offer similar advantages as in other settings, particularly in areas where multiple therapies are available within one large treatment area. Hücre enerji eldesinde üstlendikleri eşsiz rolleri ile mitokondri yıllardır devam eden araştırmal... more Hücre enerji eldesinde üstlendikleri eşsiz rolleri ile mitokondri yıllardır devam eden araştırmalar ile incelenmeye devam etmektedir. Mitokondriyal transfer hasarlanmış dokuya izole edilmiş olan mitokondrinin transfer edilme süreci olarak özetlenebilir. Bu sayede kalp hasarı olan doku gibi mitokondrial fonksiyonları bozulmuş bölgede mitokondrial disfonksiyon durmunun düzeltilmesi amaçlanmaktadır. Bu konu ile ilgili özellikle kalp dokusunda birçok çalışma olmakla beraber uygulanın miyokardial iskemi, reperfüzyon hasarı gibi süreçlerde koruyucu etkinliği araştırılmaya devam etmektedir. İzole edilen mitokondrin hasarlı dokuya aktarılmasında farklı prosedürler bulunmakla birlikte, birçok çalışmada uygulama ile ilgili olumlu sonuçlar bildirlmektedir. Derlemede literatürde bu alanda yapılmış olan çalışmalar incelenmekle birlikte konuya geniş bir penceren bakılması hedeflenmiştir.

In this study, we designed a surface plasmon resonance (SPR) biosensors based on molecular imprin... more In this study, we designed a surface plasmon resonance (SPR) biosensors based on molecular imprinting to detect low amounts of thrombin directly in a short time, both in thrombin aqueous solutions and in patient serum samples. For this purpose, the thrombin imprinted and non-imprinted SPR biosensors were prepared by integrating the synthesized thrombin imprinted and non-imprinted nanoparticles on the allyl mercaptan modified gold SPR chip surface. The kinetic studies were performed with a thrombin concentration range from 0.1 to 400 pM with a detection limit of approximately 0.017 pM in thrombin aqueous solution. Morever, the limit of detection was found to be 0.033 pM in patient serum samples. When the selectivity of the thrombin imprinted SPR biosensors was compared with the competing molecules as bovine serum albumin and lysozyme, it was showed that the thrombin was 4.58 times and 3.99 times more selective than the bovine serum albumin and lysozyme molecules, respectively. The shelf life and reusability of the designed SPR biosensor showed that there was a 12.93% decrease in the performance according to the kinetic analyzes that performed after 8 months. In addition, thrombin detection from the patient serum samples was performed using both SPR biosensor and the enzyme-linked immunosorbent assay methods, and were calculated recoveries.

Özet Amaç Çalışmamız yeni kuşak fotokatalizörlerden olan apatit kaplı demir titanat uygulamasının... more Özet Amaç Çalışmamız yeni kuşak fotokatalizörlerden olan apatit kaplı demir titanat uygulamasının, olası anti-mikrobiyal etkinliğinin araştırılmasını; in vitro ve hastane içi uygulamalarda test edilmesini amaçlamaktadır. Gereç ve Yöntemler 30 adet standart steril petri kutusu buhar fazında 20 ppm apatit kaplı demir titanat uygulamasına tabi tutulduktan sonra floresan ışık altında 4 gün tutuldu. 10 kutuya 0,5 McFarland (1.5X108 CFU/mL-CFU=koloni oluşturucu birim) Pseudomonas aeruginosa, 10 kutuya 0,5 McFarland Acinetobacter baumannii ekimi yapıldı. 10 kutu ise kontrol olarak saklandı. Örnekler uygulama sonrası bakteriyel sağkalım oranı (CFUX100/CFU) yönünden değerlendirildi. İkinci aşamada hastanede belli mekânlarda yüzeylere özel uygulama kitiyle buhar fazında 0.012 L/m2 uygulama yapıldı. Fotokatalizör uygulamasından hemen önce ve 1 ay sonra havadaki parçacık miktarı lumimetre ile ölçülerek karşılaştırıldı. Bulgular Bakteriyel sağkalım oranları Pseudomonas aeruginosa için fotokatalizör kaplı yüzeylerde kontrol grubuna oranla 2. günden sonra anlamlı olarak azalmış bulundu (p<0.001) (%60±8 / %95±9). Fark 4. güne kadar artarak devam etti (3. gün: %35±5 / %90±9; 4. gün: %22±5 / %85±8). Acinetobacter baumannii uygulanan yüzeylerde de bakteriyel sağkalım 2. günden sonra anlamlı olarak düşük çıktı (%55±7 / %87±8) (p<0.01). Fark 4. güne kadar artarak devam etti (3. gün: %40±5 / %80±8; 4. gün: %15±5 / %78±7). Fotokatalizör kaplı yüzeylerde havadaki parçacık miktarında, ameliyathanede %97.15, yoğun bakımda %95.61, doktor odasında %98.30, serviste %94.13 ve hastane mutfağında %97.04 azalma belirlendi. Sonuç Fotokatalizör kaplamanın öncü değerlendirme çalışmalarından biri olan araştırmamız sonucunda, gerek laboratuar gerekse klinik ortamlarda bakterisit ve bakteriostatik etkinliği ortaya konulmuş ve hastane sterilizasyonunda ucuz ve güvenli bir alternatif olabileceği düşünülmüştür.

Aspects of the design of a minimized circuit program are outlined, including good planning, cross-discipline work and co-operation among surgeon, anesthesiologist, nurse and perfusionists. Mini-CPB systems are described – these are modular and portable extracorporeal circuits that can be used anywhere including during patient transport.

Patient blood management (PBM) is a patient-centered, evidence-based, multidisciplinary approach ... more Patient blood management (PBM) is a patient-centered, evidence-based, multidisciplinary approach aimed at optimizing hemoglobin concentration, ensuring the continuity of hemostasis and minimizing blood loss in patients undergoing surgery. The aims of this study were: (1) to explore the cost-effectiveness of comprehensive anemia management, the first pillar of PBM, in non-cardiac and cardiac surgery from the Turkish Social Security Institution's (SSI's) perspective; and (2) to explore the potential budget impact of PBM for coronary artery bypass grafting (CABG) and hip and knee arthroplasty to the SSI. Methods: Cost-effectiveness and budget impact models were developed based on the avoided postoperative adverse events following implementation of the first pillar of PBM for non-cardiac and cardiac surgical patients. The probabilities of adverse events (sepsis with and without pneumonia, renal failure, myocardial infarction and stroke) were taken from a recent meta-analysis and the costs of treating these adverse events to the SSI were estimated through expert views and the use of SSI guidelines. Results: The PBM arm dominated the control arm for both non-cardiac and cardiac surgeries in terms of cost-effectiveness in the simulated cohort of patients and was associated with improved outcomes and lower costs (1768 and 1244 avoided adverse events, and incremental cost reductions for non-cardiac and cardiac surgery of 7504 Turkish lira [TRY] and 6102 TRY, respectively). The budget impact analysis showed that PBM is a potential cost-saving option for the SSI, with savings of up to 196,937,705 TRY (€12,841,697) for hip and knee arthroplasty and 24,642,504 TRY (€1,606,861) for CABG surgery. Conclusion: PBM is a cost-effective option with a potential of cost-saving for cardiac and non-cardiac surgery in Turkey.

ECLS and Ventricular Assist Devices have become essential tools in the care of critically ill patients with respiratory failure, post-cardiotomy failure, and viral infection. During this pandemic, there have been many advancements and rapid growth towards options of long term mechanical cardiopulmonary support, especially in the adult patient population. ECLS cases reported to the International Extracorporeal Life Support Organization registry, showing >73,000 patients. With the growth of this patient population and the changing "ECLS specialist-Patient" family relationship, it is essential to explore handling the grief of the patients. Individuals dealing with pain demonstrate a variety of reactions as they adjust to a life without a loved one. This review examines the efficacy of theories related to ECLS specialists, assisting a client, and families grieving the loss of a significant person in their life.

The use of prosthetic valves carries a high incidence of complications including thromboembolism,... more The use of prosthetic valves carries a high incidence of complications including thromboembolism, hemolysis, infection, impaired hemodynamic function and mechanical failure. Unstented homograft valves provide a good quality of life, particularly in terms of a reasonable freedom from these complications. This paper presents the first simultaneous replacement of the mitral and aortic valves using fresh unstented pulmonary and aortic homografts collected from the same donor. We believe that this technique will be especially useful in developing countries facing financial difficulties and problems with patient education.

Objectives: The present study aimed to analyze the prognostic value of preoperative serum vitamin... more Objectives: The present study aimed to analyze the prognostic value of preoperative serum vitamin D level in patients who underwent coronary artery bypass graft (CABG) surgery. Materials and Methods: The data of 360 adult patients who underwent isolated CABG surgery were retrospectively reviewed. We reached the data of preoperative serum vitamin D [25-hydroxyvitamin D (25-OHD)] values of 305 patients. The patient population was divided into two groups based on preoperative serum 25-OHD levels with a normal range of 25-75 nmol/L (group I: patients with preoperative serum 25-OHD level <25 nmol/L and group II: patients with preoperative serum 25-OHD level ≥25 nmol/L). The effect of preoperative 25-OHD level on operative mortality (mortality which occurred during the first 30 days after the operation) was determined using regression analysis and the results were expressed as Odds ratio (OR) with a 95% confidence interval (CI). A p value <0.05 was considered statistically significant. Results: In the present study, operative mortality was 3.93% (n=12). One hundred and fifty seven patients (51.5%) had serum 25-OHD levels <25 nmol/L. The mean serum 25-OHD levels were significantly lower in females than in males (p<0.001). On logistic regression analysis, preoperative serum 25-OHD level was found to be independently associated with operative mortality (OR: 0.201, 95% CI: 0.043-0.935; p=0.041). Conclusion: The presence of vitamin D deficiency seems to be an independent predictor of operative mortality after cardiac surgery in this retrospective study; however, prospective randomized trials are warranted to clarify the effect of preoperative vitamin D supplementation on postoperative outcomes in cardiac surgical patients.

Pandemi günlerinde, tüm dünyada yapılan kan bağışlarında ciddi bir azalma olmuştur. Yine 2019-2020 yılları karşılaştırıldığında bağış sayısının üçte bir oranında düştüğü görülmüştür. Türk Kızılayı verileri de pandeminin ilk günlerinde kan bağışının %90 oranında düştüğünü göstermektedir. Amerika’da yapılan bir çalışmada ise kan bağışı azalmasının yanında elektif cerrahiler ertelendiği için kan kullanımının da azaldığı görülmüştür. Bağış sırasında COVID-19’a yakalanma endişesi ve sokağa çıkma yasağı bağış sayısındaki azalmanın ana sebepleridi. Mitokondriyal transfer hasarlanmış dokuya izole edilmiş olan mitokondrinin transfer edilme süreci olarak özetlenebilir. Bu sayede kalp hasarı olan doku gibi mitokondrial fonksiyonları bozulmuş bölgede mitokondrial disfonksiyon durmunun düzeltilmesi amaçlanmaktadır. Bu konu ile ilgili özellikle kalp dokusunda birçok çalışma olmakla beraber uygulanın miyokardial iskemi, reperfüzyon hasarı gibi süreçlerde koruyucu etkinliği araştırılmaya devam etmektedir. İzole edilen mitokondrin hasarlı dokuya aktarılmasında farklı prosedürler bulunmakla birlikte, birçok çalışmada uygulama ile ilgili olumlu sonuçlar bildirlmektedir. In this study, we designed a surface plasmon resonance (SPR) biosensors based on molecular imprin... more In this study, we designed a surface plasmon resonance (SPR) biosensors based on molecular imprinting to detect low amounts of thrombin directly in a short time, both in thrombin aqueous solutions and in patient serum samples. For this purpose, the thrombin imprinted and non-imprinted SPR biosensors were prepared by integrating the synthesized thrombin imprinted and non-imprinted nanoparticles on the allyl mercaptan modified gold SPR chip surface. The kinetic studies were performed with a thrombin concentration range from 0.1 to 400 pM with a detection limit of approximately 0.017 pM in thrombin aqueous solution. Morever, the limit of detection was found to be 0.033 pM in patient serum samples. When the selectivity of the thrombin imprinted SPR biosensors was compared with the competing molecules as bovine serum albumin and lysozyme, it was showed that the thrombin was 4.58 times and 3.99 times more selective than the bovine serum albumin and lysozyme molecules, respectively. The shelf life and reusability of the designed SPR biosensor showed that there was a 12.93% decrease in the performance according to the kinetic analyzes that performed after 8 months. In addition, thrombin detection from the patient serum samples was performed using both SPR biosensor and the enzyme-linked immunosorbent assay methods, and were calculated recoveries.

Özet Amaç Çalışmamız yeni kuşak fotokatalizörlerden olan apatit kaplı demir titanat uygulamasının... more Özet Amaç Çalışmamız yeni kuşak fotokatalizörlerden olan apatit kaplı demir titanat uygulamasının, olası anti-mikrobiyal etkinliğinin araştırılmasını; in vitro ve hastane içi uygulamalarda test edilmesini amaçlamaktadır. Gereç ve Yöntemler 30 adet standart steril petri kutusu buhar fazında 20 ppm apatit kaplı demir titanat uygulamasına tabi tutulduktan sonra floresan ışık altında 4 gün tutuldu. 10 kutuya 0,5 McFarland (1.5X108 CFU/mL-CFU=koloni oluşturucu birim) Pseudomonas aeruginosa, 10 kutuya 0,5 McFarland Acinetobacter baumannii ekimi yapıldı. 10 kutu ise kontrol olarak saklandı. Örnekler uygulama sonrası bakteriyel sağkalım oranı (CFUX100/CFU) yönünden değerlendirildi. İkinci aşamada hastanede belli mekânlarda yüzeylere özel uygulama kitiyle buhar fazında 0.012 L/m2 uygulama yapıldı. Fotokatalizör uygulamasından hemen önce ve 1 ay sonra havadaki parçacık miktarı lumimetre ile ölçülerek karşılaştırıldı. Bulgular Bakteriyel sağkalım oranları Pseudomonas aeruginosa için fotokatalizör kaplı yüzeylerde kontrol grubuna oranla 2. günden sonra anlamlı olarak azalmış bulundu (p<0.001) (%60±8 / %95±9). Fark 4. güne kadar artarak devam etti (3. gün: %35±5 / %90±9; 4. gün: %22±5 / %85±8). Acinetobacter baumannii uygulanan yüzeylerde de bakteriyel sağkalım 2. günden sonra anlamlı olarak düşük çıktı (%55±7 / %87±8) (p<0.01). Fark 4. güne kadar artarak devam etti (3. gün: %40±5 / %80±8; 4. gün: %15±5 / %78±7). Fotokatalizör kaplı yüzeylerde havadaki parçacık miktarında, ameliyathanede %97.15, yoğun bakımda %95.61, doktor odasında %98.30, serviste %94.13 ve hastane mutfağında %97.04 azalma belirlendi. Sonuç Fotokatalizör kaplamanın öncü değerlendirme çalışmalarından biri olan araştırmamız sonucunda, gerek laboratuar gerekse klinik ortamlarda bakterisit ve bakteriostatik etkinliği ortaya konulmuş ve hastane sterilizasyonunda ucuz ve güvenli bir alternatif olabileceği düşünülmüştür.

Patient blood management (PBM) is a patient-centered, evidence-based, multidisciplinary approach ... more Patient blood management (PBM) is a patient-centered, evidence-based, multidisciplinary approach aimed at optimizing hemoglobin concentration, ensuring the continuity of hemostasis and minimizing blood loss in patients undergoing surgery. The aims of this study were: (1) to explore the cost-effectiveness of comprehensive anemia management, the first pillar of PBM, in non-cardiac and cardiac surgery from the Turkish Social Security Institution's (SSI's) perspective; and (2) to explore the potential budget impact of PBM for coronary artery bypass grafting (CABG) and hip and knee arthroplasty to the SSI. Methods: Cost-effectiveness and budget impact models were developed based on the avoided postoperative adverse events following implementation of the first pillar of PBM for non-cardiac and cardiac surgical patients. The probabilities of adverse events (sepsis with and without pneumonia, renal failure, myocardial infarction and stroke) were taken from a recent meta-analysis and the costs of treating these adverse events to the SSI were estimated through expert views and the use of SSI guidelines. Results: The PBM arm dominated the control arm for both non-cardiac and cardiac surgeries in terms of cost-effectiveness in the simulated cohort of patients and was associated with improved outcomes and lower costs (1768 and 1244 avoided adverse events, and incremental cost reductions for non-cardiac and cardiac surgery of 7504 Turkish lira [TRY] and 6102 TRY, respectively). The budget impact analysis showed that PBM is a potential cost-saving option for the SSI, with savings of up to 196,937,705 TRY (€12,841,697) for hip and knee arthroplasty and 24,642,504 TRY (€1,606,861) for CABG surgery. Conclusion: PBM is a cost-effective option with a potential of cost-saving for cardiac and non-cardiac surgery in Turkey.

ECLS and Ventricular Assist Devices have become essential tools in the care of critically ill patients with respiratory failure, post-cardiotomy failure, and viral infection. During this pandemic, there have been many advancements and rapid growth towards options of long term mechanical cardiopulmonary support, especially in the adult patient population. ECLS cases reported to the International Extracorporeal Life Support Organization registry, showing >73,000 patients. With the growth of this patient population and the changing "ECLS specialist-Patient" family relationship, it is essential to explore handling the grief of the patients. Individuals dealing with pain demonstrate a variety of reactions as they adjust to a life without a loved one. This review examines the efficacy of theories related to ECLS specialists, assisting a client, and families grieving the loss of a significant person in their life.

Unstented homograft valves provide a good quality of life, particularly in terms of a reasonable freedom from these complications. This paper presents the first simultaneous replacement of the mitral and aortic valves using fresh unstented pulmonary and aortic homografts collected from the same donor. We believe that this technique will be especially useful in developing countries facing financial difficulties and problems with patient education.

Objectives: The present study aimed to analyze the prognostic value of preoperative serum vitamin... more Objectives: The present study aimed to analyze the prognostic value of preoperative serum vitamin D level in patients who underwent coronary artery bypass graft (CABG) surgery. Materials and Methods: The data of 360 adult patients who underwent isolated CABG surgery were retrospectively reviewed. We reached the data of preoperative serum vitamin D [25-hydroxyvitamin D (25-OHD)] values of 305 patients. The patient population was divided into two groups based on preoperative serum 25-OHD levels with a normal range of 25-75 nmol/L (group I: patients with preoperative serum 25-OHD level <25 nmol/L and group II: patients with preoperative serum 25-OHD level ≥25 nmol/L). The effect of preoperative 25-OHD level on operative mortality (mortality which occurred during the first 30 days after the operation) was determined using regression analysis and the results were expressed as Odds ratio (OR) with a 95% confidence interval (CI). A p value <0.05 was considered statistically significant. Results: In the present study, operative mortality was 3.93% (n=12). One hundred and fifty seven patients (51.5%) had serum 25-OHD levels <25 nmol/L. The mean serum 25-OHD levels were significantly lower in females than in males (p<0.001). On logistic regression analysis, preoperative serum 25-OHD level was found to be independently associated with operative mortality (OR: 0.201, 95% CI: 0.043-0.935; p=0.041). Conclusion: The presence of vitamin D deficiency seems to be an independent predictor of operative mortality after cardiac surgery in this retrospective study; however, prospective randomized trials are warranted to clarify the effect of preoperative vitamin D supplementation on postoperative outcomes in cardiac surgical patients.
