BACKGROUND Temporomandibular joint disorders are characterised by disturbance of mouth opening, T... more BACKGROUND Temporomandibular joint disorders are characterised by disturbance of mouth opening, TMJ pain and noise (clicking). The involvement of dislocation and deformation of the disc and the formation of adhesive lesions in the temporomandibular joint cavity are characteristic of this disease. With the development of the small endoscope, arthroscopy is now used for both examination and treatment of temporomandibular joint disorders. The objective of this study was primarily aimed at investigating the efficacy of arthroscopic lysis and lavage and examination of the temporomandibular joint in patients having pain and clicking sound in temporomandibular joint. MATERIALS AND METHODS In this pilot, non-randomised study patients with symptoms of TMJ disease were included. Preoperative evaluation including complete history with physical examination of patients along with relevant radiograph and/or CT scan/MRI was done. Routine investigations were done. The treatment plan was carried out through Arthroscopic examination and lysis and lavage of temporomandibular joint. Patients were operated either under general anaesthesia (nasotracheal intubations) or local anaesthesia. Proper medication, physical therapy and diet were advised to patients. Outcomes were measured using pain reduction in preauricular region, reduction in clicking/grating joint sound, maximal interincisal opening, deviation of mandible and disability improvement. Patients were followed at post-operative day-1, week-1, week-2, week-4, week-8 and week-12. RESULTS Out of a total of six patients, arthroscopic lysis and lavage reduced pain and increasing mandibular range of motion in approximately 83% of patients. Patients aged between 20-30 years were the predominant age group presenting with complaints of pain clicking sound in temporomandibular joint. Females (66.6%) were most commonly affected with pain clicking sound in temporomandibular joint. CONCLUSION Arthroscopic lysis and lavage is efficacious treatment modality for patients suffering and from pain clicking sound in temporomandibular joint. However, studies with larger sample size are necessary to corroborate the findings of the present study for their wider use in clinical practice.

To rebuild a structurally and functionally satisfactory neocondyle is a challenging problem. Aim of this study is to re-assess the coronoid as a graft for condyle reconstruction. Twenty patients of TMJ ankylosis without involvement of the coronoid process in ankylotic mass, coming in age group elder than 14 years were selected for the study over a period 3 years from 2011 to 2014. Clinical examination, radiographs, and photographs were used postoperatively to evaluate the grafts and TMJ function. After osteoarthrectomy coronoid process was detached and fixed with a 4 hole L-shaped titanium miniplate to form neo condyle. No donor site morbidity was observed as reported with other autogenous grafts. Aims: Oral verrucous lesions, typically presenting as slowly enlarging, grey or white, warty, exo... more Aims: Oral verrucous lesions, typically presenting as slowly enlarging, grey or white, warty, exophytic overgrowths on the buccal mucosa or gingiva may be verrucous carcinoma (VC), verrucous hyperplasia (VH), proliferative verrucous leukoplakia (PVL), or may show the conventional invasive pattern of squamous cell carcinoma (SCC). This study was undertaken with the aim to report the demography of different types of oral verrucous lesions and evaluate their longterm outcome. Material & methods: This clinical study comprised 15 patients with verrucous lesions of the oral cavity who underwent biopsy for confirmation of diagnosis. Results: Among these, 12 (80%) were male and only three (20%) were female. The age distribution ranged from 32 to 74 years with a median age of 45 years at the time of diagnosis. The most common site of VC within the oral cavity was the buccal mucosa (46.7%). 5 were proven cases of VC, 3 PVL and 7 VH. All cases of VH, PVL were managed by surgical excision, while VC was managed by surgical excision with superficial ostectomy followed by two cycles of chemotherapy additionally, if micro-invasion was observed on histopathological examination. The 2-year overall survival and tumour control rate was 93.4%. Recurrence was not seen in any patient till the time of analysis at a follow up of 3e5 years. Conclusion: Accurate diagnosis of the verrucous lesions is important and should be differentiated from grade I squamous cell carcinoma. Management should be based upon the nature, behaviour and malignant potential of the lesion.

Aim The present study is to compare the effectiveness of iliac crest graft and medpor implant, fo... more Aim The present study is to compare the effectiveness of iliac crest graft and medpor implant, for repairing traumatic orbital floor defects. Materials and methods A total of 20 patients were included in the study. Autogenous iliac crest graft and medpor implant was used in 10 patients of the each group. Patients were evaluated for the presence or absence of diplopia, enophthalmos, infraorbital nerve paresthesia, and ocular motility disorders. Surgical indications for orbital exploration included entrapment of orbital tissues, large orbital defect (greater than 50% of the orbital floor or more than 8 mm), or orbital floor defects with involvement of other zygomaticofrontal complex fractures. Results All patients were successfully treated by restoration of the orbital wall continuity. Follow-up was done at 1-12 weeks. One patient had postoperative infection. There was no graft extrusion. Conclusions Both the groups showed satisfactory results, but group II was better than group I, as there was no donor site morbidity. Porous polyethylene (Medpor) is a biocompatible and high-density polyethylene implant. It is well tolerated by surrounding tissue, and its porous structure is rapidly infiltrated by host tissue. It is a highly stable and somewhat flexible porous alloplast that has rapid tissue in growth into its pores.

Odontogenic myxoma (OM) is a rare benign but locally aggressive tumour of the jaws. It is usually... more Odontogenic myxoma (OM) is a rare benign but locally aggressive tumour of the jaws. It is usually seen in the second to third decade of life. Women are more frequently affected than men and it has more predilections for the mandible rather than the maxilla. OM presents as an asymptomatic swelling in most of the cases. Owing to the non-capsulated and aggressive nature of OM, a high rate of recurrence has been reported. Here we present a case of OM in a 52-year-old woman managed by segmental mandibulectomy. Sign of recurrence was seen after 18 months of follow-up.

[1] Failure to primary therapy is often caused by unresectable lesions, recurrence of regional lymph node metastasis and a high incidence of second primary tumors caused by field cancerization and multistep carcinogenesis. [2,3] The identification of new diagnostic and prognostic factors and setting of novel therapeutic targets is thus necessary to improve the 5-year survival rates in oral squamous cell carcinoma (OSCC). This is a case report of a large inflammatory dentigerous cyst in a 10-year-old female patient treated conservatively by marsupialization method saving all teeth (mandibular permanent left canine, first and second premolars) in relation to the cyst.

The term oro-antral fistula is understood to mean of fistular canal covered with epithelia which ... more The term oro-antral fistula is understood to mean of fistular canal covered with epithelia which may or may not be filled with granulation tissue or polyposis of the sinus mucous membrane. With the presence of a fistula the sinus is permanently open, which enables the passage of microbial flora of the oral cavity into the maxillary sinus and the occurrence of inflammation with all possible consequences. Every now and then various researchers have proposed innumerable techniques to treat this defect. Starting from simple tissue flaps to autogenous grafts to alloplastic materials, an array of procedures have been evaluated in literature but the most promising technique still needs to be evaluated. Consequently, after reviewing an array of such procedures, our present study focussed on a new technique for the closer of oro-antral fistulas using autogenous auricular cartilage graft supported by buccal advancement flap. A total of 20 patients of oro-antral fistula were included in the study and after excising the fistular tract a double layer closure was done by placing auricular cartilage over the defect followed by buccal mucoperiosteal flap. The graft was harvested using posterior auricular approach. Assessment of patients was done at the end of 1 week, 3 weeks, 6 weeks, and 3 months. We found that the autogenous auricular cartilage graft is an effective sealing material in oro-antral fistula closure. We recommend this technique for the defect size ≤10 mm(2) in which future dental implant placement is sought as it allows easy sinus lifting procedure.

Mandibular trauma are a frequent injury of maxillofacial skeleton due to the mandible&amp... more Mandibular trauma are a frequent injury of maxillofacial skeleton due to the mandible's prominence and relative lack of support. Maxillofacial injuries can be complex and can involve the skin and soft-tissues as well as bones resulting in fractures. Although, several cases of severe crush injury of the lower jaw have already been presented but total avulsion of mandible has rarely been reported. As with any facial fracture, consideration must be given for the need of emergency treatment to secure the airway or to obtain hemostasis if necessary before initiating definitive treatment. We presents a case report of completely avulsed mandible along with soft-tissue of midface region in pediatric patient following trauma, which was attempted to manage or repaired by successful microvascular surgical technique. Unfortunately, we lost the patient in next postoperative day due to cardiac complication. Avulsion of the lower jaw may further complicate the obstructive nature of the upper airway. Multi-specialty involvement in their management may be needed for management of such type of avulsion injury.

Treatment of inflammatory type of dentigerous cyst in children should be done with the aim of saving developing permanent teeth which should not be sacrificed as far as possible. This is a case report of a large inflammatory dentigerous cyst in a 10-year-old female patient treated conservatively by marsupialization method saving all teeth (mandibular permanent left canine, first and second premolars) in relation to the cyst.

With the presence of a fistula the sinus is permanently open, which enables the passage of microbial flora of the oral cavity into the maxillary sinus and the occurrence of inflammation with all possible consequences. Every now and then various researchers have proposed innumerable techniques to treat this defect. Starting from simple tissue flaps to autogenous grafts to alloplastic materials, an array of procedures have been evaluated in literature but the most promising technique still needs to be evaluated. Consequently, after reviewing an array of such procedures, our present study focussed on a new technique for the closer of oro-antral fistulas using autogenous auricular cartilage graft supported by buccal advancement flap. A total of 20 patients of oro-antral fistula were included in the study and after excising the fistular tract a double layer closure was done by placing auricular cartilage over the defect followed by buccal mucoperiosteal flap. The graft was harvested using posterior auricular approach. Assessment of patients was done at the end of 1 week, 3 weeks, 6 weeks, and 3 months. We found that the autogenous auricular cartilage graft is an effective sealing material in oro-antral fistula closure. We recommend this technique for the defect size ≤10 mm(2) in which future dental implant placement is sought as it allows easy sinus lifting procedure.

Maxillofacial injuries can be complex and can involve the skin and soft-tissues as well as bones resulting in fractures. Although, several cases of severe crush injury of the lower jaw have already been presented but total avulsion of mandible has rarely been reported. As with any facial fracture, consideration must be given for the need of emergency treatment to secure the airway or to obtain hemostasis if necessary before initiating definitive treatment. We presents a case report of completely avulsed mandible along with soft-tissue of midface region in pediatric patient following trauma, which was attempted to manage or repaired by successful microvascular surgical technique. Unfortunately, we lost the patient in next postoperative day due to cardiac complication. Avulsion of the lower jaw may further complicate the obstructive nature of the upper airway. Multi-specialty involvement in their management may be needed for management of such type of avulsion injury.
