This book is the result of a joint effort by experts in accident investigation and accident analy... more This book is the result of a joint effort by experts in accident investigation and accident analysis from 10 countries in Europe across almost every industrial sector. The information that has been collected covers an inventory of best practices in specific areas, as well as formal rules, guidelines and scientific notions, theoretical concepts and methodological issues. The main objectives of this book are: • To describe the origin, the development, the current situation and the observed trends in Europe concerning public investigation of accidents, including comments of main objectives and organisational patterns • To identify and discuss some of the main dilemmas and conflicts between an investigation commission and the environment and that exist within a commission, together with outlining some main challenges of investigation • New entrants in the area of public safety investigations might benefit from the oversight in this book to familiarize themselves with the current situati...

Safety is frequently addressed as an emergent property of complex and dynamic systems. This contr... more Safety is frequently addressed as an emergent property of complex and dynamic systems. This contribution advocates the validity and importance of incorporating intrinsic technological hazards and systemic interrelations from a multi-actor perspective in the early phases of design and development. This perspective creates inherent properties in various system states, which may manifest themselves as emergent properties during operations. These safety properties are based on their business models, selectively focusing on primary system components such as infrastructure, vehicles or traffic management. Experiences with major aviation and railway projects highlight the potential of engineering design approaches such as multidisciplinary design optimization, value engineering and vectorial state/space modelling. Such an approach has high change potential for a specific category of high energy density complex socio-technical systems.

Historically, safety has been subjected to a fragmented approach. In the past, every department h... more Historically, safety has been subjected to a fragmented approach. In the past, every department has had its own responsibility towards safety, focusing either on working conditions, internal safety, external safety, rescue and emergency, public order or security. They each issued policy documents, which in their time were leading statements for elaboration and regulation. They also addressed safety issues with tools of various nature, often specifically developed within their domain. Due to a series of major accidents and disasters, the focus of attention is shifting from complying with quantitative risk standards towards intervention in primary operational processes, coping with systemic deficiencies and a more integrated assessment of safety in its societal context. In The Netherlands recognition of the importance of independent investigations has led to an expansion of this philosophy from the transport sector to other sectors. The philosophy now covers transport, industry, defense, natural disaster, environment and health and other major occurrences such as explosions, fires, and collapse of buildings or structures. In 2003 a multi-sector covering law will establish an independent safety board in The Netherlands. At a European level, mandatory investigation agencies are recognized as indispensable safety instruments for aviation, railways and the maritime sector, for which EU Directives are in place or being progressed [Transport accident and incident investigation in the European Union, European Transport Safety Council, ISBN 90-76024-10-3, Brussel, 2001]. Due to a series of major events, attention has been drawn to the consequences of disasters, highlighting the involvement of rescue and emergency services. They also have become subjected to investigative efforts, which in return, puts demands on investigation methodology. This paper comments on an evolutionary development in safety thinking and of safety boards, highlighting some consequences for strategic perspectives in a further development of independent accident investigation.

Such investigations enable the sector to learn by establishing the sequence of events that provides a satisfactory explanation of the accident leads to the drafting of recommendations to prevent reoccurrences. In addition, these investigations, which are publicly disseminated, encourage public confidence in the sector. Other transport sectors, including road, rail and water, have been slower in coming to enjoy the same acceptance of independent investigations; here, there is considerable variation amongst nations, and-with the exception of the International Civil Aviation Organization agreement-almost no international consistency. This article examines why aviation has had a different tradition in this regard. Reasons are found in parallel growth of aviation technology and the philosophy of investigative bodies, the inherently international aspect of commercial aviation, and the role of public and political pressure following major accidents. The safety investigative orientation of the aviation sector is gradually expanding to other transport sections and beyond that to other sectors such as fixed site production plants, health care, and management of natural disasters. With the newly installed Safety Investigation Board, the Netherlands has arguably placed itself at the head of this league table.

In the past, every department has had its own responsibility towards safety, focusing either on working conditions, internal safety, external safety, rescue and emergency, public order or security. They each issued policy documents, which in their time were leading statements for elaboration and regulation. They also addressed safety issues with tools of various nature, often specifically developed within their domain. Due to a series of major accidents and disasters, the focus of attention is shifting from complying with quantitative risk standards towards intervention in primary operational processes, coping with systemic deficiencies and a more integrated assessment of safety in its societal context. In The Netherlands recognition of the importance of independent investigations has led to an expansion of this philosophy from the transport sector to other sectors. The philosophy now covers transport, industry, defense, natural disaster, environment and health and other major occurrences such as explosions, fires, and collapse of buildings or structures. In 2003 a multi-sector covering law will establish an independent safety board in The Netherlands.
