Patología Rev Latinoam 2012;50(3):232-236. Año tras año, los patólogos solemos acudir a conferencias y cursos de la AMP, la FMP y, en menor medida, a congresos extranjeros con la intención de actualizarnos. Las conferencias con frecuencia son excelentes y casi siempre nos ofrecen información valiosa. Desde los profesores experimentados hasta los residentes con dos o tres años de práctica suelen invertir muchas horas para mostrarnos lo mejor de su material y el día de la presentación las aulas suelen estar repletas de patólogos que buscan información novedosa. Hasta aquí las cosas marchan muy bien. Sin embargo, desde hace algunos años me he percatado, al conversar con diversos colegas, que luego de algunas semanas o meses la pérdida de la información expuesta es sorprendente. Certain neuroendocrine tumors (NET) metastasized to the liver can resemble hepatocellular carcino... more Certain neuroendocrine tumors (NET) metastasized to the liver can resemble hepatocellular carcinoma (HCC) in cytological needle biopsy. Experience concerning the histologic characteristics of metastatic NET resembling HCC in core needle hepatic biopsies has been scarce. The aim of this study is to describe the histological criteria in seven metastatic NET that resembled HCC in core needle hepatic biopsy. From a total of 285 needle biopsies with primary or metastasized hepatic neoplasms, seven cases were selected originally diagnosed as HCC or HCC vs. NET metastasized to the liver. Fourteen needle biopsies of hepatocellular carcinomas were also studied for comparative purposes. In all of these neoplasms the diagnosis of endocrine tumor was confirmed by immunohistochemical studies and the following information was recorded: age, sex, radiological alterations, primary site of the NET, and follow-up. The following histological data were also recorded: fibrotic stroma associated or not with the neoplastic cells, growth pattern, form of the cells, cellular size, mitotic figures, nucleomegaly, apparent nucleoli, chromatin in salt and pepper, plasmacytoid cells, intranuclear inclusions, and biliary pigment. In conclusion, these characteristics were common in metastasized neuroendocrine tumors: extensive stromal fibrosis, slight to moderate atypia, hyperchromatic nuclei, plasmacytoid cells, and thin delicate strands of fibrovascular tissue supporting larger acinar groups of net cells. HCC had a more infrequent fibrotic stroma, moderate to marked atypia, and in some biopsies biliary pigment, intranuclear inclusions, and clear cells.

Gastrointestinal lymphoma, uncommon in the West, is far more prevalent in developing countries wh... more Gastrointestinal lymphoma, uncommon in the West, is far more prevalent in developing countries where it falls into two groups: ‘Western’-type lymphomas, similar to those seen in developed countries, and the so-called Mediterranean-type lymphoma. It is now accepted that Mediterranean lymphoma represents, in the majority if not in all cases, the late stage of α heavy chain disease (α-HCD). This disease is characterized by abnormal secretion of an immunoglobulin fragment; α-HCD and Mediterranean lymphoma constitute two ends of a spectrum of pathology now classified as immunoproliferative small intestinal disease (IPSID). IPSID is associated predominantly with poor socioeconomic conditions; patients present with progressive malabsorption in the second and third decades of life. Diagnosis is established by small bowel biopsy, with or without high serum levels of the α heavy chain protein. To explore the diagnostic concordancy of surgery pathologists in deceptive prostatic carcinoma bi... more To explore the diagnostic concordancy of surgery pathologists in deceptive prostatic carcinoma biopsies (DPCs) purposely selected as of difficult interpretation. Ten experienced pathologists belonging to five tertiary care centers in Mexico City participated. A total of 25 slides (16 DPCs and 9 benign biopsies) were marked and circulated among the participants. They were asked to classify each slide in one of five categories ranging from carcinoma to benign hyperplasia using the criteria of Epstein. The 16 DPCs had been confirmed by immunohistochemistry studies and by review of two expert uropathologists from the MD Anderson Medical Center in Houston TX. A weighted kappa was use to evaluate the concordancy of the pathologists. The concordancy with the experts opinion ranged from substantial in one pathologist (KW = 0.77) to bad in another (KW = -0.07) and the mean kappa was 0.32. The similarity between some carcinomas and many benign glandular proliferations has been mentione... more The similarity between some carcinomas and many benign glandular proliferations has been mentioned in the literature for decades. The description of the main histologic features of pseudohyperplastic carcinoma has been very useful in avoiding errors of interpretation, particularly false-negative results. In recent years, we have found some histologic variants of this neoplasm that have not been mentioned previously. In order to classify the different histologic growth patterns and comment on their differential diagnosis, we reviewed the architectural and cytologic features of 34 cases of pseudohyperplastic adenocarcinoma in 2 radical prostatectomies, 4 transurethral resections, and 28 needle biopsies. Growth patterns most commonly observed included nodular, complex, and mixed (nodular and complex) patterns. Other less frequent histologic varieties included adenosis-like pattern, prostatic intraepithelial neoplasia-like pattern, pseudohyperplastic adenocarcinoma with xanthomatous features, and limited pseudohyperplastic adenocarcinoma. Frequent changes in neoplastic glands included papillary infoldings, large/cystic glands, and branching. Criteria associated with malignancy include nuclear enlargement (92%), apparent nucleoli (85%), pink amorphous secretions (78%), and transition to small acinar carcinoma (70%). However, in some biopsies, nuclear atypia was little apparent. Fifteen of the 34 cases were misdiagnosed as benign and 5 as other malignant neoplasms, and included the following diagnoses: hyperplastic nodules (11), prostatic adenosis (2), diffuse adenosis of the peripheral zone (1), benign cystic glands (1), and less frequently other malignant tumors including xanthomatous carcinoma (2), low-grade prostatic adenocarcinoma (2), and atrophic carcinoma (1). It is important to recognize the different growth patterns of this neoplasm in order to avoid an underdiagnosis of malignancy.

To define the PSA levels (Prostatic Specific Antigen) in our patients with benign or malignant pr... more To define the PSA levels (Prostatic Specific Antigen) in our patients with benign or malignant prostatic biopsy. 100 patients with clinical suspicion of prostatic carcinoma (high levels of PSA and/or abnormal findings at digito-rectal examination) and who underwent a prostatic needle biopsy were reviewed. There were 66 benign and 34 carcinomas. The median PSA was 11.2 ng/mL in the benign cases and 45.6 in cancer. In cases with small increases in PSA (49.9 ng/mL), 94% were benign; there was still a majority of benign cases (63%) in patients with a PSA of 10-29.9 ng/mL. Only in those with levels of 30+, malignancy was a majority (81%). There was a considerable overlap of PSA levels in our benign and malignant patients, and only values of 30+ ng/mL were highly suggestive of carcinoma. A patient with a metaplastic breast carcinoma as a second primary carcinoma after a renal pelvis ... more A patient with a metaplastic breast carcinoma as a second primary carcinoma after a renal pelvis neoplasm, an association not described before, is reported. Her first admission in October 1983 was due to a silent hematuria secondary to a mass in the right kidney. The histopathologic diagnosis was a grade II transitional cell carcinoma of the renal pelvis without distant or regional lymph nodes metastasis. Four years later, a right upper and outer quadrant breast mass was detected. Malignant disease was diagnosed by needle biopsy, and a simple mastectomy (Madden type) was performed. The histologic evaluation showed a metaplastic carcinoma with a pseudosarcomatous appearance: fifteen axillary lymph nodes showed only lymphoreticular hyperplasia. After surgery, the patient received locoregional radiotherapy. At present, eight years and five months after the first malignant neoplasia and four years and four months after the second malignancy, the patient is asymptomatic and free of disea...

A 69-year-old man was admitted because of a 12 year history of dysphagia and cough. Direct laryngo-pharyngoscopy revealed a polypoid, yellow, smooth, submucosal tumor with well defined borders. At CT scan the tumor showed a hypodense pattern and measured 3 cm. Microscopically the neoplasm was composed mainly by typical lipomatous tissue, bundles of collagen and pleomorphic multinucleated cells, some with floret-like appearance. Lipoblasts and mitoses were not found. Six months after resection there was no evidence of recurrence.
