This chapter proposes to address these questions through the prism of four teachers in the field ... more This chapter proposes to address these questions through the prism of four teachers in the field of global health education. The students they teach and interact with, as well as their needs as active citizens in a globalised world, will provide the backdrop for further reflection. The personal journeys of the authors between countries, disciplines, professions, learning and teaching will thus be conceptualised as experiences continuously being reconstructed and deconstructed in the classroom through processes of being, belonging and becoming. It is argued that these processes can be used as resources to create inclusive and equitable quality education.

En la practica medica de los Naporuna en la Amazonia ecuatoriana se puede observar el uso de recu... more En la practica medica de los Naporuna en la Amazonia ecuatoriana se puede observar el uso de recursos medicos tanto de la medicina "tradicional” como de la medicina "moderna”. En casos de enfermedad, se suele acudir tanto al curandero como al medico y usar tanto plantas medicinales como medicamentos "occidentales”. El modelo convencional de "pluralismo medico” no ha podido servir para la explicacion de los hallazgos de un estudio empirico medico-antropologico entre los Naporuna. Partiendo de la supuesta oposicion entre mal aire y Malaria, el presente articulo describe el pluralismo medico entre los Naporuna en base a un gran numero de casos concretos. La revision de varias cuestiones teoricas conduce hacia a una vision mas amplia del pluralismo medico, mas alla de la percepcion erronea de sus elementos como "sistemas medicos” bien definidos.

Syndemics investigates synergistic, often deleterious interactions among comorbid health conditions, especially under circumstances of structural and political adversity. Health and human rights research draws on international law to argue that all people deserve access not only to health care, but also to the underlying determinants of good health. Taking the urgent matter of migrant health as an empirical focus, we juxtapose the fields of syndemics and health and human rights, identify their complementarities, and advocate for a combined approach. Health care for migrants is part of every day medical practice in Germany. Since summer 2004, the... more Health care for migrants is part of every day medical practice in Germany. Since summer 2004, the Medical School at Gießen University offers the first teaching program on "medicine and cultural diversity" in Germany, which includes strictly medical topics (like epidemiology) as well as cultural and social issues. This article presents the teaching goals of this interdisciplinary course, with emphasis on the cultural and social elements of the curriculum. The methodology and first results of the evaluation of the course are presented, followed by a discussion of the importance of teaching "migrant health" for medical education in general. "Intercultural competence" is concluded to be a key qualification for human and individualized health care, independent of the migration background of a patient.

Intercultural perspectives in health are thus developed in very different places and situations. Both the criteria for defining the target groups as well as the ways the health problems of these groups are looked at, are shaped by the particular local context. However, some challenges are common to almost all situations where the topic of ethnic diversity and health is considered. And in contrast to the rather novel term, the issues at stake in "intercultural heath" are not new at all. Unaccompanied refugee minors (URM) represent one of the most vulnerable refugee groups due to the... more Unaccompanied refugee minors (URM) represent one of the most vulnerable refugee groups due to their young age, developmental status, and insufficient coping strategies. Clinical observations indicate that the frequency of mental health problems varies between different URM subgroups. In the present research project, clinical interviews as a source of qualitative data were combined with quantitative psychometric information in a mixed-method approach in order to study the patterns of mental health problems in 561 URM from four different language groups (Arabic, Farsi, Somali, and Tigrinya) immediately after arrival in the host country (Germany). Qualitative analysis obtained as differentiating categories "language, countries of origin, age, and gender"; quantitatively, the Refugee Health Screener (RHS-15) was applied. According to the positive screening results, the highest number of mental complaints was returned by children and adolescents speaking Farsi (65.9%) and Somali (65.8%). They were followed by URM speaking Arabic (49.4%) and Tigrinya (43.3%). The results were influenced not only by origin, but also by age (with higher burden among older Farsi-speaking URM) and gender (with higher burden among male URM). Although the prevalences in URM subgroups differ, the observed high rates of positive screening results in our sample of URM from Germany substantiate the need for early detection of mental complaints and appropriate mental health care for at least every second URM.

Both at home and as part of international student exchanges, medical students are confronted with different ways of thinking and acting in relation to health and disease. Despite an increasing number of courses on cultural competence and global health at German-speaking medical schools, systematic approaches are lacking on how to integrate this topic into medical curricula. This paper is based on a structured consensus-building process by a multidisciplinary committee composed of faculty and students. In response to this, the 2nd Global Consultation on Migration and Health- held in Colombo, Sri Lanka in February 2017 - facilitated discussions concerning the role of research in supporting evidence-informed health responses that engage with migration. This study tries to pave the way for the implementation of an adequate instrument which at the same time assesses these needs of suffering people whilst responding to the need for mental health assessments specifically designed for refugees. This was done by testing the implementation of a screening method (Refugee Health Screener RHS-15) for trauma related mental health problems in refugees. Two refugee samples in Germany (differing in arrival time: 126 applicants for asylum residing in the initial reception center and 116 living in long term communal accommodations) were assessed with the culturally sensitive Refugee Health Screener (RHS-15) to detect the incidence of mental health problems amongst them. El artículo se titula “Nuevas tareas médicas en la nueva Rusia”, y recoge las experiencias de Kuczynski durante de su primera estadía en Rusia como profesor invitado en el Instituto Médico de Omsk (Siberia) y de la siguiente expedición médico-geográfica a la estepa kirguiz entre 1923 y 1924. Refugees and asylums seekers can present as a highly vulnerable group with an increased risk for ... more Refugees and asylums seekers can present as a highly vulnerable group with an increased risk for the development of mental disorders. We developed and established a concept of clinical psychiatric care for refugees on a general psychiatric ward that systematically takes into account the social, cultural and legal dimensions relevant for mental health of refugees. This concept presents a framework for treatment, which not only offers security and orientation for the patients but also for the treatment team. The present treatment guide should provide structured working in apparently hopeless situations, which due to language difficulties, trans-cultural features and serious diseases at least in the short term seem to be unchangeable. Due to the implementation of the treatment concept, from the perspective of the team, there is a noticeable relief and significant improvement concerning the interaction with refugees on the ward. We have experienced that handling patients according to this treatment concept has mutually influenced both, the treatment outcome of refugees as well as the clinical setting. It became possible not only to integrate refugees on a common psychiatric ward but opens the way for reciprocal exchange between treatment team, refugees and other patients in terms of acculturation. We expect that in the future the number of asylum seekers will remain high because of wars across the globe. Therefore, it can be assumed that there will be a need for differentiated and flexible treatment concepts for the inpatient treatment of refugees also in the future.

We developed and established a concept of clinical psychiatric care for refugees on a general psychiatric ward that systematically takes into account the social, cultural and legal dimensions relevant for mental health of refugees. This concept presents a framework for treatment, which not only offers security and orientation for the patients but also for the treatment team. The present treatment guide should provide structured working in apparently hopeless situations, which due to language difficulties, trans-cultural features and serious diseases at least in the short term seem to be unchangeable. Due to the implementation of the treatment concept, from the perspective of the team, there is a noticeable relief and significant improvement concerning the interaction with refugees on the ward. We have experienced that handling patients according to this treatment concept has mutually influenced both, the treatment outcome of refugees as well as the clinical setting. It became possible not only to integrate refugees on a common psychiatric ward but opens the way for reciprocal exchange between treatment team, refugees and other patients in terms of acculturation. We expect that in the future the number of asylum seekers will remain high because of wars across the globe. Therefore, it can be assumed that there will be a need for differentiated and flexible treatment concepts for the inpatient treatment of refugees also in the future.

Syndemics investigates synergistic, often deleterious interactions among comorbid health conditions, especially under circumstances of structural and political adversity. Health and human rights research draws on international law to argue that all people deserve access not only to health care, but also to the underlying determinants of good health.
