Methodology: A total of 76 patients were randomly divided into two equal groups A and B. Patients in group A were given combined intra-lesional meglu- mine antimoniate and cryotherapy weekly for 06 weeks and patients in group B were given cryotherapy alone weekly for 06 weeks. At the end of 06 weeks of the treatment, direct skin smear test for Leishmania donovani bodies was performed to determine intervention efficacy. Results: Male to female ratio in A and B groups was 2.4:1. Average age was 29.16 ±10.636 years and 24.39 ±12.262 years respectively. In group A, an effica- cy of 100% was noted in all patients while in group B, an efficacy of 18.4% was noted and no response was observed in 81.6% patients. Methodology: A total of 112 patients with mild to moderate psoriasis were enrolled in this randomized controlled trial. These patients were randomly allocated into two groups (A and B) by lottery method. Betamethasone dipropionate was given to group A, while calcipotriol was given to group B. Response to treatment was assessed on the basis of 50% reduction in psoriasis area and severity index (PASI) score after 4 weeks of topical treatment. SPSS version 21 for Windows was used for data entry and analysis. Results: In group A, Betamethasosne was effective in 44 patients (78.57%), while it was not effective in 12 patients (21.43%). In group B, calcipotriol was effective in 41 patients (73.21%). while it was not effective in 15 (26.79%). The P value was 0.659 which was statistically not significant. Using data for 2011, agreement remained high when the fiveitem tool was compared with a single item (overall assessment) and 34 of the 40 previously selected vignettes remained in place (j = 0.83, 95% CI 0.73–0.93). (Data for 2012 were excluded because multiple ties precluded any meaningful comparison.) In conclusion, a five-item scoring tool to select the best clinical vignettes for presentation at academic meetings could be further simplified into a threeor even a singleitem instrument. Implementing a further simplified scoring tool might increase reviewer efficiency while preserving the tool’s discriminatory properties.

Five workshops were conducted on communication skills, including the task of breaking bad news, at three teaching hospitals of Peshawar, Pakistan. Teaching methods included interactive lectures, video presentations, role play and small group discussions. Pre- and post-workshop data was collected from all the 97 participants to assess their previous training, comfort level and perceptions regarding the subject and any improvement after attending the workshop. Of the total participants, 92 (95%) residents had not received any training in communication skills at the undergraduate level. Only 64 (66%) residents had witnessed bad news being broken by a consultant. Before the workshop, 83 (85%) residents felt either not comfortable or somewhat comfortable while breaking bad news compared to 36% post-workshop (p < 0.0000). Methods This randomized controlled trial was conducted over a period of 6 months in outpatient department of dermatology, Lady Reading Hospital, Peshawar. 101 patients were taken after nonprobability consecutive sampling in each group. Topical adapalene 0.1% gel was given to group A while topical tretinoin 0.025% gel was given to group B for once daily application. Efficacy was ascertained after the end of third month and was graded as excellent or poor response. The data were statistically analyzed using SPSS version 11. Patients in group A were subjected to combined treatment i.e., topical eflornithine to be applied twice a day and IPL session to be received by patients once a month. Patients in group B were subjected to receive IPL monthly sessions alone. Treatment was continued for 6 months in both groups. The pre-intervention and post-intervention study was conducted from January to August 2012 and comprised 7 workshops of three days each that were held at four teaching hospitals of Khyber Pakhtunkhwa province in Pakistan. Overall, there were 10 Objective Structured Clinical Examination stations and 10 written scenarios. After the pre-test, workshop was held on various aspects of professionalism which was considered 'intervention', and it was followed by a post-test similar to the pre-test at the end of day 3. Stata 12 was used for all statistical analyses. There were 136 postgraduate residents in the study. Objectives To evaluate the efficacy and safety of single dose (450mg) of fluconazole in the treat... more Objectives To evaluate the efficacy and safety of single dose (450mg) of fluconazole in the treatment of pityriasis versicolor. Patients and methods Patients suffering from pityriasis versicolor presenting to department of dermatology, Hayatabad Medical Complex, Peshawar were enrolled in the study. Diagnosis of PV was made by Wood's lamp and confirmed by examination of scales in KOH preparation. 30 patients were included in the study. Results 30 patients completed the study including 24 males and 6 females. Their age ranged from 14 years to 45 years (mean 25.3 years). Eighteen patients (60%) had complete cure and their clinical and laboratory parameters remained negative at 3 weeks and 6 weeks after completion of treatment. Twelve (40%) had no response to treatment. Treatment was welltolerated and there were no adverse affects reported with single 450mg of fluconazole. Conclusion Fluconazole 450mg as a single dose was effective in the treatment of pityriasis versicolor and well tolerated. However, long term comparative studies should be done to establish the efficacy of fluconazole as a single dose treatment in PV.

Background Vitiligo has a considerable impact on Quality of Life (QOL), in a society, like ours w... more Background Vitiligo has a considerable impact on Quality of Life (QOL), in a society, like ours with pigmented skin. Dermatology Life Quality Index (DLQI) is a simple and practical tool, which can be used to study the effect of any disease on QOL. Objectives To assess the impact of vitiligo on QOL in our patients using DLQI and to corelate DLQI score with age, sex, marital status and sites of involvement. Patients and methods One hundred patients (52 females and 48 males) of vitiligo were enrolled in the study. Using a ten item DLQI, translated into Urdu, patients were asked to indicate how their lives had been affected over the preceding week. Results The mean DLQI score was 9.56 ranging from 0 to 29.The difference between mean DLQI score for males (8.17) and females (10.85) was statistically significant. Young females (n=40) had mean DLQI score of 11.2. Unmarried females (n=16) had greater impairment of QOL (mean DLQI score = 8.28). Patients with involvement of exposed sites of body scored (mean DLQI score= 11.03) higher than patients with disease on covered parts of body. Conclusions The impact of vitiligo on QOL is greater on young and unmarried females with involvement of exposed parts of body.

OBJECTIVE To compare the efficacy of intense pulse light therapy (IPL) and triple combination cre... more OBJECTIVE To compare the efficacy of intense pulse light therapy (IPL) and triple combination cream (TCC) versus intense pulse light therapy and triple combination cream alone in epidermal melasma treatment, downgrading MASI score to more than 10. STUDY DESIGN Randomized controlled trial. PLACE AND DURATION OF STUDY Dermatology Department, Lady Reading Hospital, Peshawar, from August 2014 to January 2015. METHODOLOGY Patients of 18-45 years were included in the study with Fitzpatrick skin type II-V. Sample of 96 patients was divided in to three groups of 32 each, through consecutive (non-probability) sampling method. Detailed history was taken, Woods Lamp Examination done, and melasma area and severity index (MASI) score was calculated. TCC had to be applied daily at night for two months by group A patients while group B was consigned for IPL therapy fortnightly, and those in group C were given both for two months. Efficacy was compared by recalculating MASI score at treatment end as well as at follow-up after 4 weeks, using Chi-square test with significance at p < 0.05. RESULTS Male and female patients were 10 (31.2%) and 22 (68.8%) in group A, 7 (21.9%) and 25 (78.1%) in group B, while in group C were 12 (37.5%) and 20 (62.5%). The average age was 28.70 +8.70 years. MASI score reduction was achieved in 22 (68.8%) patients in group A; whereas, in 20 (62.5%) and 30(93.8%) patients in group B and C, respectively. Efficacy-wise distribution was significant (p=0.009). CONCLUSION Intense pulse light therapy and triple combination cream are more efficacious in epidermal melasma treatment than intense pulse light therapy and triple combination cream alone.

A total of 80consecutive patients who presented with a relapse over a two year period (2013-2015) were included inthe study. Methodology A total of 78 patients were included in the study, and were divided into two groups, having 39 patients each. Patients in group A were subjected to combined treatment i.e., topical eflornithine to be applied twice a day and IPL session to be received by patients once a month. Patients in group B were subjected to receive IPL monthly sessions alone. Treatment was continued for 6 months in both groups. After completion of treatment, average number of terminal hair and percentage hair reduction were calculated to see the efficacy. The pre-intervention and post-intervention study was conducted from January to August 2012 and comprised 7 workshops of three days each that were held at four teaching hospitals of Khyber Pakhtunkhwa province in Pakistan. Overall, there were 10 Objective Structured Clinical Examination stations and 10 written scenarios. After the pre-test, workshop was held on various aspects of professionalism which was considered 'intervention', and it was followed by a post-test similar to the pre-test at the end of day 3. Stata 12 was used for all statistical analyses. There were 136 postgraduate residents in the study. Patients and methods Patients suffering from pityriasis versicolor presenting to department of dermatology, Hayatabad Medical Complex, Peshawar were enrolled in the study. Diagnosis of PV was made by Wood's lamp and confirmed by examination of scales in KOH preparation. 30 patients were included in the study. Results 30 patients completed the study including 24 males and 6 females. Their age ranged from 14 years to 45 years (mean 25.3 years). Eighteen patients (60%) had complete cure and their clinical and laboratory parameters remained negative at 3 weeks and 6 weeks after completion of treatment. Twelve (40%) had no response to treatment. Treatment was welltolerated and there were no adverse affects reported with single 450mg of fluconazole. Conclusion Fluconazole 450mg as a single dose was effective in the treatment of pityriasis versicolor and well tolerated. However, long term comparative studies should be done to establish the efficacy of fluconazole as a single dose treatment in PV.

Dermatology Life Quality Index (DLQI) is a simple and practical tool, which can be used to study the effect of any disease on QOL. Objectives To assess the impact of vitiligo on QOL in our patients using DLQI and to corelate DLQI score with age, sex, marital status and sites of involvement. Patients and methods One hundred patients (52 females and 48 males) of vitiligo were enrolled in the study. Using a ten item DLQI, translated into Urdu, patients were asked to indicate how their lives had been affected over the preceding week. Results The mean DLQI score was 9.56 ranging from 0 to 29.The difference between mean DLQI score for males (8.17) and females (10.85) was statistically significant. Young females (n=40) had mean DLQI score of 11.2. Unmarried females (n=16) had greater impairment of QOL (mean DLQI score = 8.28). Patients with involvement of exposed sites of body scored (mean DLQI score= 11.03) higher than patients with disease on covered parts of body. Conclusions The impact of vitiligo on QOL is greater on young and unmarried females with involvement of exposed parts of body.

STUDY DESIGN Randomized controlled trial. PLACE AND DURATION OF STUDY Dermatology Department, Lady Reading Hospital, Peshawar, from August 2014 to January 2015. METHODOLOGY Patients of 18-45 years were included in the study with Fitzpatrick skin type II-V. Sample of 96 patients was divided in to three groups of 32 each, through consecutive (non-probability) sampling method. Detailed history was taken, Woods Lamp Examination done, and melasma area and severity index (MASI) score was calculated. TCC had to be applied daily at night for two months by group A patients while group B was consigned for IPL therapy fortnightly, and those in group C were given both for two months. Efficacy was compared by recalculating MASI score at treatment end as well as at follow-up after 4 weeks, using Chi-square test with significance at p < 0.05. RESULTS Male and female patients were 10 (31.2%) and 22 (68.8%) in group A, 7 (21.9%) and 25 (78.1%) in group B, while in group C were 12 (37.5%) and 20 (62.5%). The average age was 28.70 +8.70 years. MASI score reduction was achieved in 22 (68.8%) patients in group A; whereas, in 20 (62.5%) and 30(93.8%) patients in group B and C, respectively. Efficacy-wise distribution was significant (p=0.009). CONCLUSION Intense pulse light therapy and triple combination cream are more efficacious in epidermal melasma treatment than intense pulse light therapy and triple combination cream alone.
