Background: Benign nerve sheath tumours present as isolated soft tissue masses and are classified... more Background: Benign nerve sheath tumours present as isolated soft tissue masses and are classified as solitary neurofibroma and schwannoma. They cause virtually identical symptoms and signs, and there are no clinically distinct pathognomonic features. Failure to appreciate the possibility and the nature of these tumours at the time of surgery may lead to a catastrophic loss of function for the patient. Aim of the study is to analyse peripheral nerve sheath tumours in reference to; Hospital based incidence, age and sex distribution peripheral nerve involved most preoperative diagnostic tool efficacy histopathological variations and surgical outcome. Materials and Methods: the study was carried out at SKIMS Srinagar from January 2005 to October 2014.Study included 49 patients, with an average age of 38 years (range 7-65 years). The diagnostic tools used were FNAC, NCV, USG, & MRI for location & provisional diagnosis, surgical excision was performed and final diagnosis achieved by HPE. Results: The incidence of isolated peripheral nerve sheath tumors in our study was 4.9 per year. In case of schwannomas upper limbs were involved more frequently than lower limbs (2:1), while neurofibromas were more evenly distributed (1.16:1). Presenting symptoms were mass, pain, weakness, dysesthesia an average follow-up of 6 months after surgery pain syndromes had complete resolution in 66% vs 44%, paraesthesia 80% vs 50% and motor strength restoration to normal in 45.5% vs 6.25% patients in schwannomas and neurofibromas respectively. Conclusion: Meticulous dissection with Intracapsular tumour removal under magnification can achieve complete tumour removal without neurological loss or recurrence.

Background: To assess the treatment outcome and overall efficacy of the novel technique of a tran... more Background: To assess the treatment outcome and overall efficacy of the novel technique of a transvaginal subfascial synthetic sling (TVSS) in comparison to the standard trans-obturator tape (TOT-O) mid-urethral sling for female stress urinary incontinence (SUI). Materials and methods: The study included 206 female SUI patients managed at our institution between March 2015 and December 2019. The patients were randomly distributed into 2 comparable groups (Group A as TOT-O and Group B as TVSS) with respect to age, trouble due to SUI assessed with respect to degree of incontinence (number of episodes and diapers used per day), and body mass index (≤40 kg/m2), with 100 patients in the TOT-O group as group A and 106 patients in TVSS as group B. Preoperative variables related to the number of incontinent episodes and diapers usage were equal in both groups. The procedure was done under spinal anesthesia and results were assessed in terms of improvement in SUI as the primary outcome, any sexual dysfunction, complications, and overall satisfaction as secondary outcomes. Results: Symptomatic improvement after the procedure was seen in all patients with complete resolution of symptoms in 91 patients (91.%) in the TOT-O group (group A) versus 96 patients (90.56%) in the TVSS (group B). Postoperative complications included urinary retention in 6% versus 5.6%, increased day time urinary frequency in 8% versus 6.6%, urge incontinence in 4% versus 2.8%, and groin/thigh pain in 12% versus 0.9%, respectively. Mesh incision was done in 1 (1%) versus none (0%), and local mesh excision for mesh erosion in 2 (2%) versus 1 (0.9%) at 3 months after the procedure in the TOT-O group and the TVSS group, respectively. The p value and Chi-Square test with respect to the clinical profile and satisfaction with respect to complete resolution of symptoms was calculated using Open EPI software which were insignificant. Sexual function (SF) was assessed using the Brief Index of SF for Women questionnaire. All the patients were satisfied with respect to SF at 6 months of follow-up. Conclusions: The TVSS as a novel technique for female SUI is less invasive, simpler to learn, with less postoperative pain with resolution of SUI, rapid recovery, and good personal satisfaction compared to the standard TOT-O in procedure.

Materials and methods: A prospective study of carried out from 2016 to 2019. A total of 15 patients were included in the study with a lesion (angiomyolipoma) more than 10 cm in size, suitable for nephron sparing surgery. The workup of the patients included history, baseline blood investigations, ultrasonography, and CT urography including angiographic films. All the patients were taken up for selective of angioembolization of the feeding vessels of the AML carried out by the interventional radiologist followed by nephron sparing surgery in the same sitting. The short term outcomes studied were warm ischemia time, average blood loss, and length of post-operative hospital stay. A total of 119 patients with benign prostatic hyperplasia were included in the study carried out at our center between March 2016 and December 2018. The patients were divided into two groups, with 56 (47.06%) patients in the ejaculation-preserving transurethral resection of the prostate group and 63 (52.94%) patients in the ejaculation-preserving holmium laser enucleation of the prostate group.
