Materials and Methods: Prescription data for 150 patients (80 women and 70 men; mean age 33.7 6 12.7 years) treated successfully with CAT (F22 Aligners, Sweden & Martina, Due Carrare, Italy) and requiring only a single, minimal finishing phase were acquired retrospectively. The inclusion criteria were Class I dental malocclusion with only minimal crowding (3 mm), 12-20 aligner steps per arch, no use of auxiliaries or interarch elastics, and rotations 258 for round-shaped teeth. The prescribed and corrective movements to be achieved in the main and finishing treatment phases, respectively, were quantified by the dedicated clear aligner setup software. The magnitudes of inclination (buccal-lingual crown tipping), angulation (mesial-distal crown tipping), rotation, intrusion, and extrusion were extracted and analyzed by tooth type, maxilla and mandible, and both arches. Descriptive statistics, that is, mean, standard deviation, and percentage, were calculated for each movement investigated. Classification and regression trees (CART) were generated using the model-based recursive partitioning approach, and the corrective movements were correlated with respect to both the amount of the movements prescribed and the tooth type. Statistical significance was set at 5%. Results: Inclination and rotation required the greatest correction, whereas angulation, intrusion, and extrusion required only minimal correction. Expressed as a percentage of prescribed movement, mean corrective movements were 20.5% for inclination, 14.5% angulation, 28.4% rotation, 11.7% extrusion, and 22% intrusion. According to CART, all corrective movements except extrusion depended on both tooth type and the magnitude of prescribed movement. Conclusions: To achieve more efficient CAT, approximately 20% overcorrection should be added to the initial planning phase when planning challenging movements such as inclination and rotation.

Five different thermoplastic aligner materials were selected, whose elastic properties varied: F22 Evoflex, F22 Aligner, Durasoft, Erkoloc-Pro and Duran. The static properties of these materials—in particular, stiffness, stress–strain curve and yield stress—were measured with a three-point bending test. For all the tests that were performed, a minimum of three samples per material were tested. The yield load, yield strength, deformation and particularly the stiffness of each material were found to be similar in the single-layer samples, while the double-layer samples showed far lower stiffness values and were similar one to another. Due to her adult skeletal age, a bone-borne appliance was applied in order to obtain both purely skeletal rapid maxillary expansion and bilateral molar distalization. After accurate matching between the pretreatment cone-beam computed tomography scan and digital models, 4 self-drilling miniscrews were inserted palatally using a computer-aided design and computer-aided manufacturing (CAD/CAM) surgical template to guide their correct and safe placement, and a bone-borne appliance was fitted. After this first phase, the hybrid clear aligner approach was used to obtain alignment, levelling, and arch coordination, with the use of a partial lingual fixed appliance on the maxillary incisors.

This case report demonstrates how the use of skeletal anchorage and appropriate orthodontic biome... more This case report demonstrates how the use of skeletal anchorage and appropriate orthodontic biomechanics can be used to resolve even complex cases, reducing unwanted dental movements and allowing the use of a partial vestibular appliance. It describes the complete mesialization of the left upper third molar and space closure following extraction of tooth UR7, due to vertical fracture one year after previous endodontic therapy for caries. This therapeutic choice was conditioned by the need not to alter the good pre-existing interarch relationships, and above all by the patient's request to be treated conservatively, and therefore not to undergo prosthetic implant rehabilitation. The use of orthodontic miniscrews and the careful application of orthodontic biomechanics, based on both direct and indirect anchorage, enabled the treatment objectives to be achieved in a reasonable period of time with reduced treatment costs.

Objective: To investigate the effectiveness of Suresmile® lingual therapy on torque, tip, and rot... more Objective: To investigate the effectiveness of Suresmile® lingual therapy on torque, tip, and rotations measures through digital evaluation of planning and post-treatment digital models. Material and Methods: A sample of 12 Caucasian adult patients (4 men; mean age 30.6 years ± 3.9 and 8 women; mean age 31.4 years ± 4.5) treated with the Suresmile® lingual orthodontic technique was retrospectively selected, regardless of the type of malocclusion. Digital planning was performed with Suresmile® software, while lingual therapy was accomplished with interactive self-ligating lingual brackets and customized Suresmile® arches. First, digital models of planning and post-treatment digital models were compared using VAM software (and the discrepancies were analyzed through MANOVA and four multivariate. Then, Tukey and Bonferroni's post-hoc tests are performed. Results: The accuracy average values are 60.11 ± 27.67% for torque, 53.52 ± 27.37% for tip and 59.19 ± 26.42% for rotation, while for inaccuracy values are 2.72° ± 2.23° for torque, 2.98° ± 2.16° for tip and 3.58° ± 3.29° for rotation. No significant differences have been recorded evaluating different sectors of both arches. Conclusion: This retrospective preliminary study highlight how overcorrections, especially in the Suresmile lingual technique, should be performed during orthodontic planning. Moreover, the study gets bases for further, more structured future studies that should involve larger and more homogeneous samples.

OBJECTIVE This study aims to evaluate success factors implicated in clinical orthodontic miniscre... more OBJECTIVE This study aims to evaluate success factors implicated in clinical orthodontic miniscrew stability after their interradicular placement in maxilla. MATERIALS AND METHODS Six hundred seventy-six miniscrews were inserted in maxillary interradicular sites in a sample of 276 patients (109 males and 167 females; mean age 19 ± 1.7 years) and immediately loaded. Percentage failure rate was recorded, and the influence of the following factors was investigated: structural (miniscrew length, diameter and body shape), operative (side of insertion site, pilot hole drilling or not) and biological (maximal insertion torque [MIT] and type of gingiva). A chi-square test with Monte Carlo correction was performed to detect the influence of these variables on the failure rate of orthodontic miniscrews. Then both multivariate logistic regression and post hoc analysis were performed, followed by classification and regression tree (CART) analysis. RESULTS The average success rate was 88%. The principal factors implicated in the failure rate were miniscrew length, MIT values and type of gingiva. Specifically, 8 mm miniscrew length, alveolar mucosa and 5-10 Ncm MIT values were linked to higher failure rates. According to CART, the main variable influencing failure is miniscrew length (≤ 8 mm for higher failure rates). For others, MIT values of 5-10 Ncm are linked to higher failure rates (p < 0.05). CONCLUSION Orthodontic miniscrews inserted in the maxilla display good success rates. However, clinicians should be discouraged from using miniscrews of length ≤ 8 mm and MIT values < 10 Ncm, even with longer miniscrews. CLINICAL RELEVANCE Information about factors related to failure rate of miniscrews placed at posterior maxillary interradicular sites is given.

Objective: To exploit 3D measurement technology to determine any correlation between neonatal pal... more Objective: To exploit 3D measurement technology to determine any correlation between neonatal palate morphology and occlusal outcomes at five years in patients receiving surgery for unilateral cleft lip and palate (CLP). Material and Methods: Twenty-nine neonatal and 5-year models treated by the same surgeon using the same protocol for CLP correction were scanned using a high-resolution structured-light scanner and stored in stl format. Dedicated software was used to make linear and surface area measurements on the neonatal models, and each digitized 5-year model was assigned a Five-Year-Old (5YO) index score on three separate occasions by the same investigator. Results: Minimum, maximum, mean, standard deviation and standard error were calculated for each variable considered, and the Pearson coefficient was used to identify any correlations between neonatal variables and 5YO scores. Linear regression analysis showed that the only variable to approach significance was the posterior width of the cleft, which showed an R 2 equal to 0.111, indicating that it accounts for 11% of the variability of the 5YO index. There was no other appreciable correlation between linear measurements, surface areas, or their interrelationships. Conclusion: There is no correlation between neonatal morphological characteristics and occlusal outcomes at 5 years in CLP patients treated via the surgical protocol considered.

Objective: To evaluate the accuracy of Virtual Surgical Planning (VSP) comparing VSPs and postope... more Objective: To evaluate the accuracy of Virtual Surgical Planning (VSP) comparing VSPs and postoperative CBCT scans in patients undergoing bimaxillary orthognathic surgery of severe Skeletal Class III malocclusion. Material and Methods: Twenty-three patients (9 males and 14 females, mean age 24.1 ± 7.0 years) were selected and submitted to bimaxillary orthognathic surgery. Pre-operative VSPs and postoperative CBCTs were compared using both linear (taking into account four skeletal and six dental landmarks, each one described by the respective coordinates) and angular measures (seven planes in total). The threshold discrepancies for post-operative clinical acceptable results were set at ≤2 mm for liner and ≤4° for angular discrepancies. The mean difference values and its 95% confidence interval were identified, comparing which planned and which obtained in absolute value. Results: There were significant statistical differences for all absolute linear measures investigated, although only two overcome the linear threshold value of 2mm in both X and Y-linear dimensions. Linear deviations in Z-linear dimension do not reach statistical significance. All 12 angular measures reach the statistical significance, although none overcome the threshold angular value of 4°. Angular deviation for roll register the higher accuracy in contrast to pitch and yaw. Conclusion: Virtual surgical planning is a reliable planning method to be used in orthognathic surgery field; as a matter of fact, although some discrepancies between the planned on the obtained are evident, most of them meet the tolerability range.

This therapeutic choice was conditioned by the need not to alter the good pre-existing interarch relationships, and above all by the patient's request to be treated conservatively, and therefore not to undergo prosthetic implant rehabilitation. The use of orthodontic miniscrews and the careful application of orthodontic biomechanics, based on both direct and indirect anchorage, enabled the treatment objectives to be achieved in a reasonable period of time with reduced treatment costs.

Material and Methods: A sample of 12 Caucasian adult patients (4 men; mean age 30.6 years ± 3.9 and 8 women; mean age 31.4 years ± 4.5) treated with the Suresmile® lingual orthodontic technique was retrospectively selected, regardless of the type of malocclusion. Digital planning was performed with Suresmile® software, while lingual therapy was accomplished with interactive self-ligating lingual brackets and customized Suresmile® arches. First, digital models of planning and post-treatment digital models were compared using VAM software (and the discrepancies were analyzed through MANOVA and four multivariate. Then, Tukey and Bonferroni's post-hoc tests are performed. Results: The accuracy average values are 60.11 ± 27.67% for torque, 53.52 ± 27.37% for tip and 59.19 ± 26.42% for rotation, while for inaccuracy values are 2.72° ± 2.23° for torque, 2.98° ± 2.16° for tip and 3.58° ± 3.29° for rotation. No significant differences have been recorded evaluating different sectors of both arches. Conclusion: This retrospective preliminary study highlight how overcorrections, especially in the Suresmile lingual technique, should be performed during orthodontic planning. Moreover, the study gets bases for further, more structured future studies that should involve larger and more homogeneous samples.

OBJECTIVE This study aims to evaluate success factors implicated in clinical orthodontic miniscre... more OBJECTIVE This study aims to evaluate success factors implicated in clinical orthodontic miniscrew stability after their interradicular placement in maxilla. MATERIALS AND METHODS Six hundred seventy-six miniscrews were inserted in maxillary interradicular sites in a sample of 276 patients (109 males and 167 females; mean age 19 ± 1.7 years) and immediately loaded. Percentage failure rate was recorded, and the influence of the following factors was investigated: structural (miniscrew length, diameter and body shape), operative (side of insertion site, pilot hole drilling or not) and biological (maximal insertion torque [MIT] and type of gingiva). A chi-square test with Monte Carlo correction was performed to detect the influence of these variables on the failure rate of orthodontic miniscrews. Then both multivariate logistic regression and post hoc analysis were performed, followed by classification and regression tree (CART) analysis. RESULTS The average success rate was 88%. The principal factors implicated in the failure rate were miniscrew length, MIT values and type of gingiva. Specifically, 8 mm miniscrew length, alveolar mucosa and 5-10 Ncm MIT values were linked to higher failure rates. According to CART, the main variable influencing failure is miniscrew length (≤ 8 mm for higher failure rates). For others, MIT values of 5-10 Ncm are linked to higher failure rates (p < 0.05). CONCLUSION Orthodontic miniscrews inserted in the maxilla display good success rates. However, clinicians should be discouraged from using miniscrews of length ≤ 8 mm and MIT values < 10 Ncm, even with longer miniscrews. CLINICAL RELEVANCE Information about factors related to failure rate of miniscrews placed at posterior maxillary interradicular sites is given.

Objective: To exploit 3D measurement technology to determine any correlation between neonatal pal... more Objective: To exploit 3D measurement technology to determine any correlation between neonatal palate morphology and occlusal outcomes at five years in patients receiving surgery for unilateral cleft lip and palate (CLP). Material and Methods: Twenty-nine neonatal and 5-year models treated by the same surgeon using the same protocol for CLP correction were scanned using a high-resolution structured-light scanner and stored in stl format. Dedicated software was used to make linear and surface area measurements on the neonatal models, and each digitized 5-year model was assigned a Five-Year-Old (5YO) index score on three separate occasions by the same investigator. Results: Minimum, maximum, mean, standard deviation and standard error were calculated for each variable considered, and the Pearson coefficient was used to identify any correlations between neonatal variables and 5YO scores. Linear regression analysis showed that the only variable to approach significance was the posterior width of the cleft, which showed an R 2 equal to 0.111, indicating that it accounts for 11% of the variability of the 5YO index. There was no other appreciable correlation between linear measurements, surface areas, or their interrelationships. Conclusion: There is no correlation between neonatal morphological characteristics and occlusal outcomes at 5 years in CLP patients treated via the surgical protocol considered.

Objective: To evaluate the accuracy of Virtual Surgical Planning (VSP) comparing VSPs and postope... more Objective: To evaluate the accuracy of Virtual Surgical Planning (VSP) comparing VSPs and postoperative CBCT scans in patients undergoing bimaxillary orthognathic surgery of severe Skeletal Class III malocclusion. Material and Methods: Twenty-three patients (9 males and 14 females, mean age 24.1 ± 7.0 years) were selected and submitted to bimaxillary orthognathic surgery. Pre-operative VSPs and postoperative CBCTs were compared using both linear (taking into account four skeletal and six dental landmarks, each one described by the respective coordinates) and angular measures (seven planes in total). The threshold discrepancies for post-operative clinical acceptable results were set at ≤2 mm for liner and ≤4° for angular discrepancies. The mean difference values and its 95% confidence interval were identified, comparing which planned and which obtained in absolute value. Results: There were significant statistical differences for all absolute linear measures investigated, although only two overcome the linear threshold value of 2mm in both X and Y-linear dimensions. Linear deviations in Z-linear dimension do not reach statistical significance. All 12 angular measures reach the statistical significance, although none overcome the threshold angular value of 4°. Angular deviation for roll register the higher accuracy in contrast to pitch and yaw. Conclusion: Virtual surgical planning is a reliable planning method to be used in orthognathic surgery field; as a matter of fact, although some discrepancies between the planned on the obtained are evident, most of them meet the tolerability range.
