Objective: To investigate the relationship between insulin resistance and hepcidin levels in pati... more Objective: To investigate the relationship between insulin resistance and hepcidin levels in patients with PCOS. Materials and methods: In this case–control study fifty- six patients with PCOS and forty – one healthy control subjects were included. Plasma levels of hepcidin, IL-6, Serum Insulin and ferritin using ELISA method, serum iron levels using a spectrophotometric method, and Insulin resistance by using HOMA were measured in the two groups of PCOS (case group) and healthy subjects (control group). The results were analyzed by student’s t-test, General Linear Model, Binary logistic and linear regression tests. Results: The mean hepcidin level was 1.97 ± 0.53 pg/ml and 2.40 ± 0.25pg/ml in the case and control groups, respectively. The t-test results showed a significant difference between the two groups (p = 0.0001). The mean of insulin level in case and control group was 30.65 ± 15.02g/dl and 14.71 ± 10.46g/dl, respectively. Background: To assess the usefulness of premature luteinization hormone (LH) surge prevention in ... more Background: To assess the usefulness of premature luteinization hormone (LH) surge prevention in an intrauterine insemination (IUI) cycle by GnRH antagonist administration. Materials and Methods: Sixty patients with unexplained or mild male infertility or minimal to mild endometriosis were enrolled in this prospective randomized controlled trial. There were twenty patients in group A (with GnRH antagonist) and 40 patients in group B (without GnRH antagonist). In all of the participants, clomiphene citrate and human menopausal gonadotropin (CC+HMG) were used for ovarian stimulation. When at least one follicle with ≥ 16 mm diameter was seen, LH surge was checked by a urinary LH kit. In patients with negative results, human chorionic gonadotropin was continued in both groups, but in group A 0.25 mg Ganirelix SQ was administered for two days, ,then in both groups human chorionic gonadotropin (HCG) was injected on the third day and IUI was done 36-40 hours later. Ongoing pregnancy was the primary outcome. Results: Baseline characters and clinical parameters were similar in both groups with the exception of ≥14 mm follicles which were higher in group A (p value= 0.003). The pregnancy rate in both groups was not significantly different, although it was higher in group B (10% in group A and 15% in group B). Conclusion: At least in CC+HMG stimulated cycles for IUI, the occurrence of premature LH surge could have a useful rule and GnRH antagonist administration could be an inappropriate intervention.

Background. Clinically, acute kidney injury (AKI) is a potentially devastating condition for whic... more Background. Clinically, acute kidney injury (AKI) is a potentially devastating condition for which no specific therapy improves efficacy of the repair process. Bone marrow mesenchymal stromal cells (BM-MSCs) are proven to be beneficial for the renal repair process after AKI in different experimental rodent models, but their efficacy in large animals and humans remains unknown. This study aims to assess the effect of autologous rhesus Macaque mulatta monkey BM-MSC transplantation in cisplatin-induced AKI. Methods. We chose a model of AKI induced by intravenous administration of 5 mg/kg cisplatin. BM-MSCs were transplanted through intra-arterial injection. The animals were followed for survival, biochemistry analysis and pathology. Results. Transplantation of 5 Â 10 6 cells/kg ameliorated renal function during the first week, as shown by significantly lower serum creatinine and urea values and higher urine creatinine and urea clearance without hyponatremia, hyperkalemia, proteinuria and polyuria up to 84 d compared with the vehicle and control groups. The superparamagnetic iron oxide nanoparticle-labeled cells were found in both the glomeruli and tubules. BM-MSCs markedly accelerated Foxp3þ T-regulatory cells in response to cisplatin-induced damage, as revealed by higher numbers of Foxp3þ cells within the tubuli of these monkeys compared with cisplatin-treated monkeys in the control and vehicle groups. Conclusions. These data demonstrate that BM-MSCs in this unique large-animal model of cisplatin-induced AKI exhibited recovery and protective properties.
