Extravasation of cytotoxics is a dreaded complication of the intravenous route medication, especi... more Extravasation of cytotoxics is a dreaded complication of the intravenous route medication, especially in chemotherapy. The use of saline washout is insufficiently documented in the literature. Here, we report the excellent results of using saline washout in the management of these injuries. Three patients presenting extravasation of anthracycline, calcium gluconate, and radiologic contrast were treated in our department by saline washout. The technique was a use of saline to flush the extravasation area after liposuction of the body site injured. All patients were treated in an outpatient setting. No case of skin necrosis nor other complications related to the extravasation or to the surgical technique have been reported during the follow-up. Patients with extravasation should be referred to the plastic department at early stage. Saline washout is reliable technique to prevent skin and soft tissue necrosis caused by extravasation of cytotoxics.

Anthracycline extravasation remains a feared serious complication of chemotherapy. At late presen... more Anthracycline extravasation remains a feared serious complication of chemotherapy. At late presentation, deep ulceration and extensive soft tissue damage are seen. Hand extravasation of anthracycline may lead to tendon and nerves destruction with functional and economical impairments. We report a case of Epirubicin extravasation seen at day 25 in a 46-year-old woman treated for breast cancer. A groin flap failed due to the persistence of anthracyclin in the wound. A split thickness skin graft was done after all the tendons were removed. The chemotherapy was interrupted for two months. Wide serial debridements are needed to achieve the removal of all molecules of anthracycline that are observed when granulating tissue is observed permanently in the wound.

Les sequelles de brulure de la main sont frequentes et diverses, esthetiques et fonctionnelles, s... more Les sequelles de brulure de la main sont frequentes et diverses, esthetiques et fonctionnelles, simples ou complexes. Nous rapportons notre experience de cinq ans de traitement des sequelles de brulure de la main dans le centre national des brules et de chirurgie plastique du CHU Ibn-Rochd de Casablanca. Notre etude a concerne 65 patients âges de 2 a 70 ans dont 45 hommes et 20 femmes, chez qui nous avons realise 143 interventions chirurgicales sur 105 mains. Les sequelles siegent au niveau du dos de la main dans 57 cas, suivi des commissures dans 43 cas. Les differents moyens de reconstruction ont ete utilises avec notamment la greffe cutanee dans 58 cas et les lambeaux locaux dans 32 cas. Nous avons evalue le degre de satisfaction des patients des differents moyens utilises. Concernant la greffe cutanee, 94% sont satisfaits des resultats obtenus au niveau de la face dorsale, 90% de l'utilisation des lambeaux locaux au niveau de la premiere commissure et 95% de l'utilisatio...
But : le cancer des levres est une pathologie frequente, representant pres de 1/5 des cancers des... more But : le cancer des levres est une pathologie frequente, representant pres de 1/5 des cancers des voies aerodigestives superieures. il a pour but de presenter a travers cette serie, les differentes techniques chirurgicales du traitement du cancer des levres ainsi que les differents moyens de reparation. Materiel et methode : notre travail est une etude retrospective concernant 100 cas de cancer des levres traites de Janvier 1999 a decembre 2002. Resultats : l'âge moyen est de 59 ans, le sex ratio homme/femme est de 7,3. Parmi nos patients 85 % etaient tabagiques et 22 % etaient alcooliques, avec une association tabac-alcool dans 20 %. Dans notre serie, 71 % des cas etaient classes TI-T2, et 29 % etaient classes T3-T4. Le siege tumoral a ete la levre inferieure dans 82 % des cas, et superieure dans 10 % des cas. Les Auteurs presentent, a travers deux cas cliniques, une variete technique pour la levee du lambeau sural, permettant d'obtenir des palettes cutanees de grande surface avec une bonne securite vasculaire. C'est un lambeau fasciocutane remontant jusqu'a un centimetre du creux poplite et incluant, lors de la levee, l'aponevrose, le nerf sural, la petite veine saphene et le nerf sural lateral.

La foudre etait alors associee a la colere des dieux ou a la notion de châtiment. La fulguration correspond a un transfert d’energie entre un cumulonimbus de charge negative et un objet de charge positive se trouvant au niveau du sol. Les lesions determinees sont a la fois thermiques et electrothermiques. Bien que l’arret cardiorespiratoire soit une cause bien documentee de deces, la plupart des cas rapportes dans la litterature decrivent un eventail tres disparate des sequelles qui surviennent suite a cet accident. Its objectives are to present the experience of the plastic and reconstructive surgery unit of the universitary hospital of Casablanca. The authors present a retrospective study about 100 cases of lips cancer treated between January 1999 and December 2002. The average age is 59 years. The sex-ratio is 7.3. Eighty-five percent of our patients were smokers and 22% were alcoholics, the association alcoholic and smoker was found in 20%. In our study 71% were classified T1-T2 and 29% as T3-T4. The tumoral location was the lower lip in 82% and the upper lip in 10%. The histological study showed a squamous cell carcinoma type in 85% and a basal-cell carcinoma type in 14% of cases. After the tumour's excision, the reparation was done by cross-lip flap in 25%, the naso-lateral flap in 16% and the Gillies flap in 8%. This retrospective study was performed in order to analyse the epidemiology and aetiology of 543 ... more This retrospective study was performed in order to analyse the epidemiology and aetiology of 543 cases of paediatric burns and to determine the elements that might have contributed to their prevention, which remains the treatment of choice for this pathology. The paediatric hospitalization rate for burns was 45.7% (mean age, 4.25 yr), the highest proportion of patients (42.5%) being in the 1-5 yr age group. The male/female ratio in all age groups was 1.7/1. The environment where burn injury most commonly occurred was the home (85.1%), and with accidental causes, accounting for 96.5% of the causes (mainly liquids, 69.3%). The mean percentage of burned body surface area was 21% with a total burned body surface area of ≥ 20% in 52.3% of cases. The upper limbs (79.1%) were the most affected body site; and the children were referred from other hospitals in more than 6 h in 65.5% of cases. The mortality rate was 13.2%

The characteristic features of respiratory disorders in burn patients are presented in a review o... more The characteristic features of respiratory disorders in burn patients are presented in a review of the literature. Respiratory disorders in burn patients are related to a direct mechanism, when the bronchial tree is injured by the transfer of thermal energy due to the burn, and/or by an indirect mechanism, when the lung's respiratory and ventilatory function is impaired by localized, regional, or general burns-related phenomena. These respiratory disorders are aggravated by the inhalation of gases contained in smoke. Diagnosis of these disorders is clinical and paraclinical, and has to be performed early and continuously. The patients require various types of therapy, e.g. oxygen therapy, clearing of the airways, improvement of ventilatory mechanics and respiratory function, antibiotic therapy, or surgical correction of sequelae. This taking in charge inevitably has to be multidisciplinary.

We reported a case of thoracic reconstruction by an association of flaps, free fibular and pedicled of the latismus dorsi, after a resection of a shrewd tumour of the thoracic partition. This reconstruction has permitted the contribution of a musculocutaneous cover of good quality and the contribution of a skeletal support satisfying at a time. The association of flaps can be considered like one of the solutions for the reconstruction of the big losses of substance of full thickness of the thoracic partition requiring a restoration of the thoracic skeleton.

The author report twenty one cases of tissue reconstruction defects secondary to necrotizing cell... more The author report twenty one cases of tissue reconstruction defects secondary to necrotizing cellulitis of perineum and external genital organs, treated at the burns and plastic surgery unit of CHU Ibn Rochd of Casablanca, between 1992 and 1994. Depending on the extent of the defect, the techniques used were gracilis musculocutaneous flap in 8 cases, thigh fasciocutaneous flap in 7 cases, groin flap in 4 cases, simple skin sutures and skin grafts in one case, respectively. The morphological results were satisfactory. The mean hospital stay after reconstruction was 15 days.

The patients' average age was 22 yr and the female/male sex ratio was 1.4:1. The major location was the cephalic portion (75.5% of cases). One hundred and thirty-six prostheses were placed, of which 36 were prematurely removed, mostly because of exposure (14% of cases). The nal result was excellent in 50% of cases, the best results being in the scalp.

L’examen clinique trouait une masse vulvaire indolore de consistance molle, esurant environ 20 centimètres de grand axe, dévelopée aux dépens de la grande lèvre gauche (Fig. 1). ’échographie montrait une lésion homogène, à composante raisseuse lobulée faisant évoquer un lipome. Une inciion nympho-hyménale était réalisée. L’exérèse permettait ’ablation d’une masse jaunâtre encapsulée, qui mesuait 19 × 8 centimètres et qui pesait 260 grammes (Fig. 2). ’examen anatomopathologique montrait un aspect graiseux encapsulé, lobulé, myxoïde, fait de tissu adipeux ature, sans remaniements hémorragiques ou nécrotiques, t sans composante sarcomateuse. Les suites opératoires taient simples. Avec un recul de 18 mois, aucune récidive ’était notée.
