PRESENTACIÓN DE UN CASO Y REVISIÓN DE LA LITERATURA Presentamos un caso de un histiocitoma fibroso maligno (HFM) de cordón espermático. Un varón de 80 años fue admitido en nuestro hospital con una masa escrotal izquierda, relacionada con un traumatismo genital hacia varios meses. Ante la sospecha de un tumor testicular, se practicó una orquiectomia radical izquierda. El examen histológico de la masa tumoral, reveló un Histiocitoma fibroso maligno. El tumor estaba firmemente adherido al cordón espermático. El HFM es un tumor de alta malignidad derivado del tejido conectivo, el cual puede ocasionalmente, afectar al tracto genital masculino. No hay acuerdo respecto a un tratamiento principal y el pronóstico en general, es pobre. Palabras clave: Histiocitoma fibroso maligno. Cordón espermático. Tumor paratesticular.

Background: Kangaroo Mother Care (KMC) is dened as skin-to-skin contact between a mother and her... more Background: Kangaroo Mother Care (KMC) is dened as skin-to-skin contact between a mother and her newborn baby derived from practical similarities to marsupial care giving, proximately exclusive breastfeeding and early discharge from hospital. This concept was proposed as an alternative to conventional methods of care for low birth weight (LBW) infants, and in replication to quandaries of earnest overcrowding in Neonatal Intensive Care Units (NICUs). KMC essentially utilizes the mother as a natural incubator Aim and Objectives: The aim was to assess the feasibility, acceptability and the effectiveness of KMC in LBW infants. It avoids agitation routinely experienced in busy ward. Material and Methods: A pilot open-labeled quasi-randomised clinical trial was conducted in Level III NICU of a teaching institution. 60 newborn infants <2500 g, meeting inclusion criteria were alternatively randomised into two groups: Kangaroo Mother Care (KMC) and Conventional Methods of Care (CMC). Kangaroo mother care was practiced with minimum total period of eight hours a day intermittently for the intervention group while the controls remained in incubators or cots. Weight, head circumference, length, morbidity episodes, hospital stay, feeding patterns were monitored for all infants till postmenstrual age of 42 weeks in preterm babies or till a weight of 2500 g is achieved in term SGA babies. Results: The pilot study conrmed that trial processes were efcient, the intervention was acceptable (to mothers and nurses) and that the outcome measures were appropriate; KMC babies achieved signicantly better growth at the end of the study (For preterm babies, weight, length and head circumference gain were signicantly higher in the KMC group (weight 19.28±2.9g/day, length 0.99±0.56cm/week and head circumference 0.72±0.07 cm/week) than in the CMC group (P <0.001). A signicantly higher number of babies in the CMC group suffered from hypothermia, hypoglycemia, and sepsis. Conclusion: Kangaroo mother care improves growth and reduces morbidities in low birth weight infants. It is simple, acceptable to mothers and can be continued at home.

Systemic lupus erythematosus (SLE) is a chronic autoimmune disease characterized by involvement o... more Systemic lupus erythematosus (SLE) is a chronic autoimmune disease characterized by involvement of different organ systems with varied manifestations, and the presence of circulating autoantibodies directed against self-antigens occurs in both children and adults, commonly affecting females of reproductive age. Childhood-onset SLE (cSLE) has more severe disease and more widespread organ involvement as compared to adults. There are two opinions regarding age to define pediatric-onset lupus, which is either before age 16 or onset before age 18. Here, we report a case of SLE in a 14-year-old male child due to its rare occurrence in the pediatric age group, especially in males presenting as tuberculosis mimic with complaints of not gaining weight and fever.

Apert syndrome is a congenital acrocephalosyndactyly syndrome. It is mainly presented by craniosy... more Apert syndrome is a congenital acrocephalosyndactyly syndrome. It is mainly presented by craniosynostosis, syndactyly of the hands and feet and dysmorphic facial features. The condition has an autosomal dominant inheritance assigned to mutations in the Fibroblast Growth Factor Receptors (FGFR-2) gene. The reported case is a 9 months old boy with clinical suspicion of Apert syndrome as he had turricephaly, stubby hands with short fingers, omphalocele measuring 10 × 10 cm, bilateral undescended testes, Congenital Talipes Equinovarus (CTEV) and polydactyl and syndactyl of great and second toes. He was developmentally normal. The omphalocele was surgically repaired. Early diagnosis and intervention helps rehabilitation of patients with Apert syndrome and help them to lead a better life.

India being the most populous country in South East Asia, shares the major burden of LBW ABSTRACT Background: The low birth weight (LBW) infant remains at much higher risk of mortality than the infant at normal weight at birth. In the neonatal period when most infant deaths occur, the proportion of low birth weight infants is the major determinant of the magnitude of mortality rates. Mortality and morbidity among low birth weight babies are a major public health problem in our country. It is important to identify risk factors associated with LBW babies. Methods: Prospective observational study was conducted on 222 neonates with low birth weight admitted in Neonatal intensive care unit (NICU) of Acharya Vinoba Bhave Rural Hospital, Jawaharlal Nehru Medical College, Sawangi Meghe, Wardha from September 2015 to August 2017. Maternal risk factors and neonatal morbidities were recorded. The study was designed to assess the risk factors of LBW babies and their short-term outcome. Results: 222 low birth weight babies were studied, among which 36(16.22%) were weighed less than 1500 grams and 186 (83.78%) were 1500-2500 grams. The most common maternal risk factors which were associated with very low birth weight were low socioeconomic status, occupations with more strenuous activity, low maternal educational status, poor antenatal care, poor pregnancy weight gain and chronic illnesses. Anemia was present in 99 (44.595%) mothers. Among other risk factors during pregnancy oligohydramnios was present in 43 (19.369%) mothers, followed by preeclampsia in 42 (18.918%) mothers and pyrexia in 31 (13.963%) mothers. 166 (74.76%) babies are small for gestational age (SGA) babies. Overall mortality rate was 40.54%. Conclusions: Most of LBW babies are SGA babies. Low socioeconomic status, low maternal educational status and poor antenatal care were the important risk factors. Morbidity and mortality of low birth weight babies could be reduced considerably by proper health education, improved antenatal care, prompt identification of high risk pregnancies, proper referral, better nursing care and management.

The aim of this work was to study the commonest causes of respiratory distress in NICU, analysis of each cause, and to deter-mine the strategic plan needed to improve the outcome of these cases.Patients and Methods: A prospective study was conducted through the period from January 2003 to October 2004. Data were collected from all patients admitted in the unit during this period.Results: Total number of, 855(17.27%) newborn were admitted to NICU, 140 of them developed respiratory distress comprising 16.37% of all NICU admissions and with an incidence of 2.83%. It is frequently detected incidentally on surgery for other diseases or autopsy. We encountered incidentally detected subserosal nodule in proximal jejunum during exploratory laparotomy for other cause which was resected and on histopathology confirmed to be HP. Histologically, most of the tumours are located in the submucosa, rarely in the muscularis propria, and only seldom in the subserosal. This case is of great interest because of the subserosal location of the tumour.

The most common clinical symptom is strangulation combined with mechanical intestinal obstruction. Abdominal CT scan can precisely confirm the diagnosis and early surgical intervention is essential for better outcome.

It typically manifests by painless subcutaneous nodules in head or neck region and often accompanied by regional lymphadenopathy, salivary gland involvement along with hyper eosinophilia and elevated serum IgE. Most cases have been described predominantly in Chinese and Japanese people. Kimura's disease has been confused with Angiolymphoid Hyperplasia with Eosinophilia (ALHE), from which it probably should be distinguished as a separate entity. The course is usually benign except for the potential cosmetic disfigurement. The diagnosis may be suggested by a fine needle aspirate, but is established by a biopsy. A case of Kimura's disease in a 42 year male with recurrent left submandibular swelling is presented together with a brief review of the literature.

The aim of this work was to study the commonest causes of respiratory distress in NICU, analysis of each cause, and to deter-mine the strategic plan needed to improve the outcome of these cases.Patients and Methods: A prospective study was conducted through the period from January 2003 to October 2004. Data were collected from all patients admitted in the unit during this period.Results: Total number of, 855(17.27%) newborn were admitted to NICU, 140 of them developed respiratory distress comprising 16.37% of all NICU admissions and with an incidence of 2.83%. The commonest causes of respiratory distress in our study were transient tachypnea of newborn (TTN) 57 (40.7%) respiratory distress syndrome (RDS) 24 cases (17.2%), birth asphyxia 16 cases (11.4%) and Meconium aspiration syndrome (MAS) 13 cases (9.3%). It is frequently detected incidentally on surgery for other diseases or autopsy. We encountered incidentally detected subserosal nodule in proximal jejunum during exploratory laparotomy for other cause which was resected and on histopathology confirmed to be HP. Histologically, most of the tumours are located in the submucosa, rarely in the muscularis propria, and only seldom in the subserosal. This case is of great interest because of the subserosal location of the tumour.

It generally occurs in elderly emaciated patients with accompanying diseases. Because it is difficult to diagnose before surgery, the morbidity and mortality rates for obturator hernia are high. The most common clinical symptom is strangulation combined with mechanical intestinal obstruction. Abdominal CT scan can precisely confirm the diagnosis and early surgical intervention is essential for better outcome.

It typically manifests by painless subcutaneous nodules in head or neck region and often accompanied by regional lymphadenopathy, salivary gland involvement along with hyper eosinophilia and elevated serum IgE. Most cases have been described predominantly in Chinese and Japanese people. Kimura's disease has been confused with Angiolymphoid Hyperplasia with Eosinophilia (ALHE), from which it probably should be distinguished as a separate entity. The course is usually benign except for the potential cosmetic disfigurement. The diagnosis may be suggested by a fine needle aspirate, but is established by a biopsy. A case of Kimura's disease in a 42 year male with recurrent left submandibular swelling is presented together with a brief review of the literature.
