Nowadays, the Coronavirus pandemic has become a major concern among the people and the World Health Organization. The healthcare workers face more conflicts arising from work and family due to the number of shifts, hardship and long hours. The aim of this research is to study the work-family conflict for the nurses and doctors taking care of COVID-19 patients in Jahrom province. Materials and Methods: This cross-sectional descriptive study was carried out in 2020 on 103 nurses and doctors who were in contact with COVID-19 patients, working in hospitals associated with Jahrom medical sciences university. The tools for gathering data in this study included two questionnaires with the subjects of demographic information and the work and family conflict. Data analysis was carried out by SPSS, version 21 plus descriptive and inferential statistical tests. Results: 16.5 percent of nurses and doctors had travelled during the Coronavirus pandemic and 42.2 percent of them had taken ethics courses. The average work-family conflict was 52.75 ± 13.96. The level of workfamily conflict was lower than the average (48.26%) in most cases. Regarding different aspects of workfamily conflict, the conflict level was higher than the average (56.84 ± 21.01), while the conflict level based on behavior (45.30±22.47 and pressure (43.33±23.72) was lower than the average. The workfamily conflict among the healthcare workers who had taken the ethics course was meaningfully lower that the others (P-value = 0.049), but there was no meaningful relationship between the other demographic variables and the work-family conflict (P-value < 0.05) Conclusion: The work-study conflict for nurses and doctors taking care of COVID-19 patients in Jahrom province is lower than average. The data achieved by this study can be a basis for the beginning of further research and having educational workshops for healthcare workers. This will also help the medical field decision-makers to have a more effective planning.

Diseases and disorders are serious threats to human dignity and patients are afraid of losing their dignity. From the time of admission and hospitalization to discharge from the hospital, the patients feel they are losing their dignity in the hospital. The present study aimed to investigate status of respect for dignity of hospitalized patients from nurses' perspective in teaching hospitals in Jahrom town in 2018. Methods and Materials: This was a descriptive, cross-sectional study. The participants consisted of 170 nurses working in educational hospitals in Jahrom town in 2018. Data collection instruments were demographic questionnaire and human dignity questionnaire (from nurses' perspectives). Collected data was analyzed using descriptive and inferential statistics with SPSS v. 21. Results: Average age of the nurses participating in this study was 28 ± 5.42. The score of patient privacy was 2.27 ± 0.46. The score of patient autonomy was 2.20 ± 0.51. The score of respectful relationship with patients was 2.27 ± 0.60. Conclusion: Patient dignity and its aspects (patient privacy, respectful relationship with patients, patient autonomy and their sub-components) were reported desirable or relatively desirable.

Some scholars believe that promoting ethical climate in health care institutions will better respond to moral stress and other causes of workplace dissatisfaction. The purpose of this study was to investigate the relationship between ethical climate and tendency to stay at work in Jahrom operating room and anesthesia staff in 2018. Methods and Materials: This descriptive cross-sectional study was performed on 50 anesthesia and operating room staff of Jahrom hospitals. The data collection tool consisted of three questionnaires. The first questionnaire was about demographic information, the second was about the Olson ethical climate survey and the third was about the tendency to stay in business. Data were analyzed by SPSS software version 21 and descriptive statistics (mean, percentage and standard deviation) and inferential statistics (Spearman and Kruskal-Wallis correlation coefficient). Results: 56% of the subjects in this study were anesthesia personnel and 44% were operating room personnel. In operating room personnel, the mean score for the operating room ethical climate and all domains was above 3.5 and desirable. But in anesthesia staff, the average score for the ethical climate of the operating room and all its areas was less than 3.5 and undesirable. Of all the staff, only the median variable score of physicians' atmosphere score of 3.45 was less than 3.5. The mean score of retention level for the operating room staff was 13.63 more than the average for the anesthesia staff with an average of 13.53. The results showed that the relationship between the level of tendency to stay at work with the ethical climate of the operating room and none of its domains was significant among the personnel (p> 0.05(. Conclusion: According to the results of this study, there was no significant difference between the status of ethical climate and the tendency to persist in operating room and anesthesia staff.

Providing resources to meet all health and medical needs in the health sector, even in wealthy co... more Providing resources to meet all health and medical needs in the health sector, even in wealthy countries, does not occur completely. Hence, the issue of resource allocation and prioritization has always been one of the management challenges in health systems. Patient introduction: The patient was a 63-year-old man taken to the hospital because of a reduced level of consciousness. In the ambulance, the patient had a heart attack and received electric shock due to ventricular fibrillation (VF). Immediately after being admitted to the hospital, he was hospitalized in the intensive care unit (ICU). After two days, the patient's consciousness was still low and his pupils showed a slight reaction to light. A few hours later, the patient suffered a seizure and the CT scan results indicated hypoxic brain damage. Despite medical and special care efforts, he has remained in the same situation in the ICU and no improvement has been found. It is 22 days that he has been in the hospital. Recently, an 18-year-old patient has been taken to the hospital due to a stroke in a car accident. However, there is no blank capacity of ICU in other wards of the hospital and we face a shortage of ICU beds. Conclusion: Through the establishment of the hospital ethics committee, review of indications, and involvement of the 62-year-old patient's fellows in decision-making, one bed can be added to one of the wards and the necessary ICU services can be provided to the patient with a higher emergency status.

Many drugs have tried to prevent or reduce shivering, but the ideal drug has not been finalized yet. The purpose of this study was to determine the effect of Dexmedetomidine with bupivacaine in spinal anesthesia to prevent shivering in orthopedic fracture surgery of femur and tibia. Materials and Methods: This randomized clinical trial study was performed on 60 patients 18-50 years old with ASA class 1 and 2 who were candidates for tibia and femur orthopedic fracture surgery in 2018. Patients were randomly divided into two groups; Intervention group: 12.5 mg bupivacaine (2.5 cc) and 5 μg Dexmedetomidine (0.5 ml) and control group: 12.5 mg bupivacaine and 0.5 ml normal. Systolic, diastolic, and mean arterial blood pressure and blood O2% saturation were recorded before spinal anesthesia, after anesthesia and then every 15 minutes until surgery and recovery time. At 2, 4, 8, 12 and 24 hours postoperatively shivering was evaluated. Data were analyzed using descriptive statistics indices (mean, percentage and standard deviation) and inferential statistical tests (chi-square, t-test, and Mann-Whitney u) using SPSS 21 software. Results: The mean age of patients in the control group was 36.6 ±14.7 years and the intervention group was 32.7± 8.6 years. There was no significant difference in the frequency of shivering between the control and intervention groups at 2 (4.3% vs. 7.1%) and 4 (4.5% vs. 24.1%). Although, intervention subjects had no any shivering state at hours of 8, 12 and 24, but there was no significantly different compared to control group (7.1%, 3.6% and 3.6%, respectively). Conclusion: The results of study showed that addition of 5 μg Dexmedetomidine to bupivacaine in spinal anesthesia reduces the incidence of postoperative shivering, but this difference was not statistically significant. Thus, in this subject it seems bee need to studies with whole and large sample of participants.

The disease is diagnosed based on the presence of a characteristic subcutaneous mass, elevated peripheral blood eosinophilia, IgA E levels, and biopsy, which indicates abnormal proliferation of lymph follicles and vascular endothelium with eosinophilic infiltration. Case Report: The patient is a 35-year-old woman who suffered from first degree localized burns in a large area of the body with warm water 2 months before the diagnosis and then the symptoms of Kimura disease appeared in order and after performing several tests and sampling of the disease Proved. Conclusion: It seems that in our patient, a large superficial burn acted as a skin shock and caused the symptoms of Kimura disease, so that the patient showed all the symptoms of this disease within 2 months, including skin symptoms (Dryness, burning, thickening, severe itching and excessive wrinkles), skin rashes on the back of the neck and then swelling in the nose and back of the eyes, and finally an almond-sized mass on the right side of the back Is the neck. The patient has heartburn, loss of appetite and weight loss. His illness has also responded to steroid treatment.

This study compared the effects of ondansetron and dexamethasone on nausea and vomiting under spinal anesthesia. METHODS One hundred and twenty patients of 15 to 35 years old with ASA class I and II were enrolled. Before administering either ondansetron or dexamethasone, blood pressure and heart rate of the patients were recorded. The patients received 70 mg of 5% lidocaine for spinal anesthesia. Patients who received 6 mg of ondansetron were considered as group A, while group B received 8 mg of dexamethasone. The level of nausea and vomiting, blood pressure, heart rate and respiratory rate of each patient was measured at 1, 5, 10, 15 and 30 minutes after spinal anesthesia and during recovery (every 5 minutes). Therefore, these factors should be considered together. The purpose of this study was to determine the relationship between ethical distress and willingness to leave their work among nurses working in Jahrom University of Medical Sciences in 2016. Materials & Methods: This descriptive cross-sectional study was performed on nurses working in hospitals affiliated to Jahrom University of Medical Sciences in 2016. All nurses working in hospitals affiliated to Jahrom University of Medical Sciences were enrolled. Data collection tools included three demographic information questionnaires, Corley's ethical distress questionnaire and nurses' willingness to remain in the Nursing profession. Data were analyzed using SPSS software version 21 and descriptive and inferential statistical tests. Results: The mean age of the participants was 285.4 years. Nurses' willingness to leave the service was 1.04±1.03 and ethical distress severity score was 0.72.±2.44%. Spearman test results showed that there was no significant relationship between tendency to quit work and moral distress in severity (p-value> 0.337) and repetition (p-value>0.444). Mann-Whitney test showed that there was no significant relationship between gender variables with variables of desire to quit, severity of moral distress and repetition of moral distress (p-value>0.05). The results of Kruskal-Wallis test showed that there was no significant relationship between educational level, service sector and type of employment with variables of tendency to quit, severity of moral distress and repetition of moral distress (p-value> 0.05). Conclusion: The results of the present study showed that there was no significant relationship between mean moral distress in two dimensions of severity and repetition with the intention to quit nurses. Although the ethical distress reported in this study is moderate, this lack of relevance cannot be taken into account, but sometimes nurses have to be reluctant to quit due to lack of work and difficult economic

This study compared the analgesic effects of paracetamol and magnesium sulfate. METHODS Sixty patients with American Society of Anesthesiologists (ASA) class I or II patients who were candidates for surgery of the lower limbs were randomly divided into three equal groups who were age and gender matched. Group 1 received paracetamol, and group 2, the magnesium sulfate during surgery and group 3 as the control. Pain intensities were measured and recorded using the Visual Analog Scale before surgery, in the recovery room, and 6, 12, and 18 hours after surgery. RESULTS Pain intensities (7.10, 5.80, and 4.10) were higher in the control group; 6, 12, and 18 hours after surgery compared to the paracetamol (6.45, 4.15, 2.50) and the magnesium groups (7.25, 4.55, and 2.05), but the difference was not statistically significant. With the emergence of the Novel Coronavirus (2019-nCoV), researchers worldwide have started detec... more With the emergence of the Novel Coronavirus (2019-nCoV), researchers worldwide have started detecting the probable pathogenesis of the disease. The renin-angiotensin system (RAS) and angiotensin-converting enzymes have received a good deal of attention as possible pathways involved in 2019-nCoV pathogenesis. As the experiments seeking to find potential medications acting on these pathways are being conducted in the early phases, having an ecological worldview on the relationship between the prevalence of COVID-19 disease and the genetic differences in the genes involved in the RAS system could be valuable for the field. In this regard, we conducted a meta-analysis study of the prevalence of ACE (I/D) genotype in countries most affected by the COVID-19. In the meta-analysis, 48,758 healthy subjects from 30 different countries were evaluated in 116 studies, using the Comprehensive Meta-analysis software. The I/D allele frequency ratio was pooled by a random-effect model. The COVID-19 prevalence data of death and recovery rates were evaluated as the latitudes for the meta-regression analysis. Our results demonstrated that with the increase of the I/D allele frequency ratio, the recovery rate significantly increased (point estimate: 0.48, CI 95%: 0.05-0.91, p = 0.027). However, there was no significant difference in the case of death rate (point estimate: 1.74, CI 95%: 4.5-1.04, p = 0.22). This ecological perspective coupled with many limitations does not provide a direct clinical relevance between the COVID-19 and RAS system, but it shows potential pathophysiological associations. Our results raise concerns about ethnic and genetic differences that could affect the effectiveness of the currently investigated RAS-associated medications in different regions.

Some scholars believe that promoting ethical climate in health care institutions will better respond to moral stress and other causes of workplace dissatisfaction. The purpose of this study was to investigate the relationship between ethical climate and tendency to stay at work in Jahrom operating room and anesthesia staff in 2018. Methods and Materials: This descriptive cross-sectional study was performed on 50 anesthesia and operating room staff of Jahrom hospitals. The data collection tool consisted of three questionnaires. The first questionnaire was about demographic information, the second was about the Olson ethical climate survey and the third was about the tendency to stay in business. Data were analyzed by SPSS software version 21 and descriptive statistics (mean, percentage and standard deviation) and inferential statistics (Spearman and Kruskal-Wallis correlation coefficient). Results: 56% of the subjects in this study were anesthesia personnel and 44% were operating room personnel. In operating room personnel, the mean score for the operating room ethical climate and all domains was above 3.5 and desirable. But in anesthesia staff, the average score for the ethical climate of the operating room and all its areas was less than 3.5 and undesirable. Of all the staff, only the median variable score of physicians' atmosphere score of 3.45 was less than 3.5. The mean score of retention level for the operating room staff was 13.63 more than the average for the anesthesia staff with an average of 13.53. The results showed that the relationship between the level of tendency to stay at work with the ethical climate of the operating room and none of its domains was significant among the personnel (p> 0.05(. Conclusion: According to the results of this study, there was no significant difference between the status of ethical climate and the tendency to persist in operating room and anesthesia staff.

Hence, the issue of resource allocation and prioritization has always been one of the management challenges in health systems. Patient introduction: The patient was a 63-year-old man taken to the hospital because of a reduced level of consciousness. In the ambulance, the patient had a heart attack and received electric shock due to ventricular fibrillation (VF). Immediately after being admitted to the hospital, he was hospitalized in the intensive care unit (ICU). After two days, the patient's consciousness was still low and his pupils showed a slight reaction to light. A few hours later, the patient suffered a seizure and the CT scan results indicated hypoxic brain damage. Despite medical and special care efforts, he has remained in the same situation in the ICU and no improvement has been found. It is 22 days that he has been in the hospital. Recently, an 18-year-old patient has been taken to the hospital due to a stroke in a car accident. However, there is no blank capacity of ICU in other wards of the hospital and we face a shortage of ICU beds. Conclusion: Through the establishment of the hospital ethics committee, review of indications, and involvement of the 62-year-old patient's fellows in decision-making, one bed can be added to one of the wards and the necessary ICU services can be provided to the patient with a higher emergency status.

Many drugs have tried to prevent or reduce shivering, but the ideal drug has not been finalized yet. The purpose of this study was to determine the effect of Dexmedetomidine with bupivacaine in spinal anesthesia to prevent shivering in orthopedic fracture surgery of femur and tibia. Materials and Methods: This randomized clinical trial study was performed on 60 patients 18-50 years old with ASA class 1 and 2 who were candidates for tibia and femur orthopedic fracture surgery in 2018. Patients were randomly divided into two groups; Intervention group: 12.5 mg bupivacaine (2.5 cc) and 5 μg Dexmedetomidine (0.5 ml) and control group: 12.5 mg bupivacaine and 0.5 ml normal. Systolic, diastolic, and mean arterial blood pressure and blood O2% saturation were recorded before spinal anesthesia, after anesthesia and then every 15 minutes until surgery and recovery time. At 2, 4, 8, 12 and 24 hours postoperatively shivering was evaluated. Data were analyzed using descriptive statistics indices (mean, percentage and standard deviation) and inferential statistical tests (chi-square, t-test, and Mann-Whitney u) using SPSS 21 software. Results: The mean age of patients in the control group was 36.6 ±14.7 years and the intervention group was 32.7± 8.6 years. There was no significant difference in the frequency of shivering between the control and intervention groups at 2 (4.3% vs. 7.1%) and 4 (4.5% vs. 24.1%). Although, intervention subjects had no any shivering state at hours of 8, 12 and 24, but there was no significantly different compared to control group (7.1%, 3.6% and 3.6%, respectively). Conclusion: The results of study showed that addition of 5 μg Dexmedetomidine to bupivacaine in spinal anesthesia reduces the incidence of postoperative shivering, but this difference was not statistically significant. Thus, in this subject it seems bee need to studies with whole and large sample of participants.

The disease is diagnosed based on the presence of a characteristic subcutaneous mass, elevated peripheral blood eosinophilia, IgA E levels, and biopsy, which indicates abnormal proliferation of lymph follicles and vascular endothelium with eosinophilic infiltration. Case Report: The patient is a 35-year-old woman who suffered from first degree localized burns in a large area of the body with warm water 2 months before the diagnosis and then the symptoms of Kimura disease appeared in order and after performing several tests and sampling of the disease Proved. Conclusion: It seems that in our patient, a large superficial burn acted as a skin shock and caused the symptoms of Kimura disease, so that the patient showed all the symptoms of this disease within 2 months, including skin symptoms (Dryness, burning, thickening, severe itching and excessive wrinkles), skin rashes on the back of the neck and then swelling in the nose and back of the eyes, and finally an almond-sized mass on the right side of the back Is the neck. The patient has heartburn, loss of appetite and weight loss. His illness has also responded to steroid treatment.

This study compared the effects of ondansetron and dexamethasone on nausea and vomiting under spinal anesthesia. METHODS One hundred and twenty patients of 15 to 35 years old with ASA class I and II were enrolled. Before administering either ondansetron or dexamethasone, blood pressure and heart rate of the patients were recorded. The patients received 70 mg of 5% lidocaine for spinal anesthesia. Patients who received 6 mg of ondansetron were considered as group A, while group B received 8 mg of dexamethasone. Therefore, these factors should be considered together. The purpose of this study was to determine the relationship between ethical distress and willingness to leave their work among nurses working in Jahrom University of Medical Sciences in 2016. Materials & Methods: This descriptive cross-sectional study was performed on nurses working in hospitals affiliated to Jahrom University of Medical Sciences in 2016. All nurses working in hospitals affiliated to Jahrom University of Medical Sciences were enrolled. Data collection tools included three demographic information questionnaires, Corley's ethical distress questionnaire and nurses' willingness to remain in the Nursing profession. Data were analyzed using SPSS software version 21 and descriptive and inferential statistical tests. Results: The mean age of the participants was 285.4 years. Nurses' willingness to leave the service was 1.04±1.03 and ethical distress severity score was 0.72.±2.44%. Spearman test results showed that there was no significant relationship between tendency to quit work and moral distress in severity (p-value> 0.337) and repetition (p-value>0.444). Mann-Whitney test showed that there was no significant relationship between gender variables with variables of desire to quit, severity of moral distress and repetition of moral distress (p-value>0.05). The results of Kruskal-Wallis test showed that there was no significant relationship between educational level, service sector and type of employment with variables of tendency to quit, severity of moral distress and repetition of moral distress (p-value> 0.05). Conclusion: The results of the present study showed that there was no significant relationship between mean moral distress in two dimensions of severity and repetition with the intention to quit nurses. The renin-angiotensin system (RAS) and angiotensin-converting enzymes have received a good deal of attention as possible pathways involved in 2019-nCoV pathogenesis. As the experiments seeking to find potential medications acting on these pathways are being conducted in the early phases, having an ecological worldview on the relationship between the prevalence of COVID-19 disease and the genetic differences in the genes involved in the RAS system could be valuable for the field. In this regard, we conducted a meta-analysis study of the prevalence of ACE (I/D) genotype in countries most affected by the COVID-19. In the meta-analysis, 48,758 healthy subjects from 30 different countries were evaluated in 116 studies, using the Comprehensive Meta-analysis software. The I/D allele frequency ratio was pooled by a random-effect model. The COVID-19 prevalence data of death and recovery rates were evaluated as the latitudes for the meta-regression analysis. Our results demonstrated that with the increase of the I/D allele frequency ratio, the recovery rate significantly increased (point estimate: 0.48, CI 95%: 0.05-0.91, p = 0.027). However, there was no significant difference in the case of death rate (point estimate: 1.74, CI 95%: 4.5-1.04, p = 0.22). This ecological perspective coupled with many limitations does not provide a direct clinical relevance between the COVID-19 and RAS system, but it shows potential pathophysiological associations. Our results raise concerns about ethnic and genetic differences that could affect the effectiveness of the currently investigated RAS-associated medications in different regions.
