Abdominal paracentesis aspirated fresh blood. Ultrasound and CT scan abdomen showed loculated haemoperitoneum. A definitive diagnosis could not be made andan exploratory laparotomy was undertaken which revealed a large cystic lymphangioma of greater omentum with acute massive spontaneous haemorrhage. It was excised in toto along with the involved omentum leading to excellent recovery. Abdominal cystic lymphangioma first presenting as a spontaneous, life threatening haemorrhage has to our knowledge, not been reported before. It may have to be included in the differential diagnosis of acute haemoperitoneum.

This condition is the most common cause of persistently direct (conjugated) hyper bilirubinemia in the first 03 month of life. Kasai portoenterostomy & liver transplantation battle to become todays leading therapy. The main focus of attention however remains that the results after portoenterostomy are decided by the promptness of the initial workup & referral to surgery. More then 80% of babies with NBA have satisfactory bile flow after hepatic portoenterostomy if the procedure is done before 8th week of life.

Objective: (1) To develop a hypothesis about the relative effect of these 2 therapies on risk adjusted outcomes through 18 months in premature infants. (2) To obtain data that would be useful in designing and conducting a successful trial of this hypothesis. Design: A prospective, observational study.
