Endoscopic sinus surgery being an approach, there is no proper classification system to address t... more Endoscopic sinus surgery being an approach, there is no proper classification system to address the extent of surgery required for Sinonasal pathologies. The current study attempts to propose and evaluate the role and efficacy of a classification system for endoscopic sinus surgery in addressing various Sinonasal and skull base pathologies. Materials and methods: It's a prospective study that included 50 patients. We classified ESS into six types based on the extent of surgery. After thorough evaluation, depending on the extent of the disease, all 50 patients were planned and treated according to the classification. Patients were followed for one year.

The olfactory fossa is prone to injury during FESS and the risk of injury depends on the depth of the olfactory fossa. Aim: To assess the depth of the olfactory fossa by multidetector Computed Tomography (CT) and to study the incidence and distribution of asymmetry of olfactory fossa depth. Materials and Methods: A cross-sectional, observational study was conducted at Department of Radiology, Dr Pinnamaneni Siddhartha Institute of Medical Sciences and Research Foundation, Gannavaram, Andhra Pradesh, India from November 2019 to October 2019. The sample size was 500 patients. All patients above the age of 18 years who were referred to the department of radiology for CT paranasal sinuses were included. Acquired Tracheo-esophageal fistula (TEF) is a challenging and complicated condition. The larynge... more Acquired Tracheo-esophageal fistula (TEF) is a challenging and complicated condition. The laryngeal protection is lost in acquired TEF cases due to the established connection between the esophagus and the airways leading to aspiration, pneumonia, and acute respiratory distress syndrome. Malignancy contributes to about 80% of acquired TEF. Nonmalignant causes for TEF include prolonged ventilation, trauma (iatrogenic, penetrating, or blunt injury), foreign bodies, corrosive burns, and granulomatous infections. With the advancements in critical care, the incidence of TEF post-ventilation is on the rise in recent decades. We would like to share our experience managing ten cases of nonmalignant acquired cervical TEF by the lateral cervical approach at our institute. Apart from the isolated TEF cases, one patient with concomitant tracheal stenosis was repaired simultaneously with good postoperative results. TEF was identified in two cases following removal of T-tube and solid stent respectively and was repaired successfully with lateral cervical approach with strap muscle flap interposition.

Aim: To assess various preoperative and peroperative clinical findings in endonasal dacryocystorh... more Aim: To assess various preoperative and peroperative clinical findings in endonasal dacryocystorhinostomy (DCR) as prognostic indicators of the surgical outcome. Materials and methods: A prospective study of 25 cases of chronic dacryocystitis with epiphora. All the patients were divided into two groups based on the presence or absence of medial canthal swelling with regurgitation of mucopus on finger pressure over medial canthus. Other factors included duration of epiphora, associated nasal problems, condition of the bone, sac wall, and lumen of the sac during surgery. Results: Out of 14 patients with medial canthal swelling (group I), all were successful on the table with free flow of saline, but one patient developed symptoms on 6 months follow-up and that was treated as failure. Out of 11 patients without medial canthal swelling, five were successful with free flow of saline on the table and six were unsuccessful because of tight or absent flow with regurgitation. Conclusion: Presence of medial canthal swelling and regurgitation of mucopus on digital pressure over medial canthus is a good prognostic indicator for a favorable surgical outcome. In patients without medial canthal swelling, we need to rely on syringing or dacryocystogram. Early intervention gives good results.

The study was undertaken to compare the micro ear anatomy of three commonly available animal mode... more The study was undertaken to compare the micro ear anatomy of three commonly available animal models which are expected to have similar anatomy to human and to find out suitable model among them as an alternative for human cadaver temporal bone dissection. This is an observational study of comparison of micro ear anatomy of the three animal models with human. Decapitated heads of cattle, pig and sheep were collected from slaughter houses, soft tissues along with brain were removed and preserved in commercially available formalin preservative. CT scan was taken for the three specimens and 3D reconstructions were done. Each specimen was subjected to micro dissection and the anatomical features were studied and compared with human. Among the three animal models sheep is found to be an ideal model for a beginner because of ease of exposure of bone, very thin cortical bone, and no cellularity, good exposure of all the middle ear structures and similar interrelations of middle ear structures with human. Pig may not be an ideal model because of abundant fatty soft tissues, thick periosteum very narrow space occupied by the middle ear, difficulty in accessing cellularity small fragile ossicular chain and overhanging facial nerve.

Evaluation of adenoid enlargement is usually done clinically, but its reliability in young children is questionable. Although various diagnostic procedures have been proposed to diagnose adenoid hypertrophy, there is poor concordance on the ideal method by clinicians. The purpose of this study is to corroborate a clinical grading that could guide the clinicians in accurate diagnosis of adenoid hypertrophy and to warrant the cases needing adenoidectomy and to calibrate this clinical grading with endoscopic and x-ray findings.A cross-sectional study was conducted in a tertiary health care among 60 children aged between 3-14 years who presented with signs and symptoms of chronic adenoid hypertrophy are evaluated with clinical grading, endoscopy, lateral neck x-ray, and findings were documented. Purpose: The inhalation of nasal snuff (powdered tobacco) is a common addiction in the Indian sub... more Purpose: The inhalation of nasal snuff (powdered tobacco) is a common addiction in the Indian subcontinent. In the western world, there is a resurgence of interest in nasal snuff because it does have the morbidity associated with smoked tobacco. Very few studies have reported the long-term effects of snuff on nasal mucosa. The objective of the present study was to investigate the effect of long-term use of snuff on the nasal mucosa. Materials and methods: We conducted a retrospective study on 29 snuff users. We investigated the reasons for initiation of this particular form of addiction along with the clinical signs and symptoms of long-term snuff usage. At the time of the study, all patients complained of one or more nasal symptoms. Nasal obstruction and nasal discharge taken together were reported by 62.5% of patients.
