Introduction: In modern medical education, with its transformational changes, teacher-centered le... more Introduction: In modern medical education, with its transformational changes, teacher-centered learning is transformed into student-centered learning. This shift has escalated so fast with momentum, and its value in teaching and learning has been evaluated in many ways. In the current scenario, problem-based learning (PBL) is a well-recognized effective method of teaching and learning. The credit goes to McMaster University in Hamilton, Ontario, Canada, for the establishment of PBL. In Malaysia, many medical schools have applied this method in their curriculum, one of which is University Malaysia, Sarawak (UNIMAS), which has also adopted PBL in its undergraduate curriculum since 1996. Purpose: The aim of this study is to determine the students’ acceptance of PBL and its positive and negative impact on their learning. Methodology: It was a cross-sectional study conducted to determine the acceptance of the students of PBL. The study population is selected using convenience sampling of 140 out of 148 pre-clinical year-2 students who were exposed to the PBL method, the focus group discussion (FGD) was conducted based on pre-framed questions to know the impact of PBL on the students’ learning. Results: Results showed that the respondents had the satisfaction and accepted PBL. Sixty students from 6 PBL groups were involved in FGD. Feedback from FGD revealed their difficulties with the conduction of 1st session of PBL, for instance, problems in searching resources, new learning environments and peers from different regions. There were positive responses spelled by the subjects that the PBL has improved their communication skills, critical thinking, and self-esteem. Conclusion: Overall the PBL has proved beneficial evidenced a positive impact on the learning process of medical students.

The association of ovarian teratoma and anti-N-Methyl-Daspartate receptor (anti-NMDAR) is one of ... more The association of ovarian teratoma and anti-N-Methyl-Daspartate receptor (anti-NMDAR) is one of the most common autoimmune encephalitis syndromes and it is a serious and potentially fatal pathology that occurs in young women. This case report describes of a pediatric patient with anti-NMDAR encephalitis. A-12-year-old girl presented with abnormal behavior for one week came to Emergency Department of Sarawak General Hospital, Malaysia. She had psychotic spectrum symptoms including suicidal tendency. She was diagnosed with anti-NMDAR encephalitis as positive antibody was seen in her cerebrospinal fluid. She was treated with Injection Immunoglobulin. She turned out to have teratoma which was successfully removed later. Her progress was remarkable after the surgery with the Immunoglobulin. A multi-disciplinary team involving a psychiatrist, neurologist and gynaecologist liaised with intensivist to successfully manage the case and achieve the good outcome.

To be precise, it is a congenital absence of the skull, scalp, and forebrain (cerebral hemispheres). It was categorized under neural tube defects (NTD). As a whole, NTD is the second most common fatal anomaly after cardiac defects. The neural tube defects are a group of central nervous system ailments, which result from the failure of normal primary neurulation, an embryologic process completes in humans by almost day 26-28 the post-conception. In this case, the normal closure of the anterior neuropore fails, which results in anencephaly. The case report demonstrates the clinical experience that would broaden our knowledge of this uncommon but distressing and dilemmatic event. We highlighted the ultrasound features to recognize the condition by first and second-trimester scans and the role of folic acid in the prevention of further occurrence. The diagnosis was confirmed anencephaly with formal ultrasound from the radiologist, who reported no definite brain parenchyma noted, the head was small in size, and the patient was informed about the condition and its prognosis. The reported case was found to be rare and clinicians emphasized the contribution of professional guidance for its management since the case is critical and the ethical concerns need careful handling to avoid adverse psychosocial outcomes such as grief, guilt, anxiety, and loss of self-confidence.

The pre-clinical performances have some predictive value in the clinical discipline. Basic Medical Science (BMS) taught in pre-clinical years also supported the development of clinical reasoning skills and critical analysis of medical intervention. This study explored the preference, attitude and perception toward BMS subjects among the pre-clinical medical students. Materials and methods: An institution-based cross-sectional study was conducted among 189 pre-clinical students in a public university in East Malaysia by using the nine-scale statements questionnaires by West and co-workers. The association of ovarian teratoma and anti-N-methyl-D-aspartate receptor (NMDAR) encephalitis i... more The association of ovarian teratoma and anti-N-methyl-D-aspartate receptor (NMDAR) encephalitis is a serious and potentially fatal pathology that occurs in young women and that is under-recognized. Our objectives were to analyze prevalence and outcome of this association, and increase awareness over this pathology. MEDLINE and SCOPUS for all studies published prior to November 30, 2013 including the search terms: "encephalitis" and "teratoma" were considered. All articles (119) reporting one or more cases of anti-NMDAR encephalitis and confirmed ovarian teratoma (174 cases) were included. No language restrictions were applied. Suspicious cases with no evidence of ovarian teratoma (n = 40) and another type of encephalitis also associated to ovarian teratoma (n = 20) were also considered for comparison and discussion. Data of publication and case report, surgery and outcome were collected. The distribution of published cases is heterogeneous among different countries and continents, probably in relation with level of development and health care. The mean patient age is 24 years and in the majority of cases (74%), a mature teratoma was identified, sometimes microscopically following ovarian removal or at autopsy. The clinical presentation featured psychiatric symptoms and behavioural changes, with a median delay for surgery of 28 days. Twelve women died (7%), most frequently from encephalitis-related complications. In conclusion, the association ovarian teratoma and anti-NMDAR encephalitis is relatively unknown or not reported in many countries and among gynecologists. Heightened recognition of behavioral changes, diagnosis through transvaginal ultrasound and subsequent tumor removal in addition to diagnostic confirmation through the presence of anti-NMDAR antibodies must be emphasized.

One of the subtypes of influenza A, the H1N1 strain, also known as swine flu, is especially of high risk for development of complications in pregnant women. The influenza A virus infection is difficult to diagnose clinically because its presenting symptoms are similar to those of the common cold but are more severe, last longer, and can be potentially life-threatening. This case also presented with common cold symptoms but her condition worsened later. Fortunately, obstetric health providers were vigilant enough to address the developing infection and its related complications. It was the cooperative effort of multidisciplinary team care which resulted in a favourable outcome in both mother and baby.

Most of the big masses are originated from the uterus or ovarian origin and it is vital for the patient to find out the most possible diagnosis. With the improvement of modern technology, the ultrasound plays an important role to get the diagnosis, however, it has some limitations like difficulty to visualize the whole mass. Other investigations like CT and especially, MRI will help to reach the appropriate diagnosis and as a result, aid in making an efficient plan for the respective diagnosis with least morbidity. It is a great challenge for the clinician but those who have adequate knowledge, competency in surgical skill with well-learnt experience will not consider it as a big deal.

Atherogenic index of plasma (AIP) was found to be one of the strongest markers in predicting the cardiovascular disease (CVD) risk. This study was to determine the AIP and its relationship with other CVD risk factors. Materials and Methods. This cross-sectional study was done among 349 staff of a public university in Sarawak. Data were collected using questionnaire, blood sampling, and anthropometric and blood pressure measurement. Data were analyzed using IBM SPSS version 20. Results. A total of 349 respondents participated with majority females (66.8%), aged 38.5 ± 7.82 years. Nearly 80% of the respondents were overweight and obese, 87.1% with high and very high body fat, and 46.9% with abnormal visceral fat. For AIP category, 8.9% were found to be in intermediate and 16.4% were at high risk. Elevated lipid profile showed that total cholesterol (TC) is 15.5%, low density lipoprotein (LDL) is 16.1%, and triglyceride (TG) is 10.6%. AIP was significantly correlated with b...

Most of the big masses are originated from the uterus or ovarian origin and it is vital for the patient to find out the most possible diagnosis. With the improvement of modern technology, the ultrasound plays an important role to get the diagnosis, however, it has some limitations like difficulty to visualize the whole mass. Other investigations like CT and especially, MRI will help to reach the appropriate diagnosis and as a result, aid in making an efficient plan for the respective diagnosis with least morbidity. It is a great challenge for the clinician but those who have adequate knowledge, competency in surgical skill with well-learnt experience will not consider it as a big deal.

Atherogenic index of plasma (AIP) was found to be one of the strongest markers in predicting the cardiovascular disease (CVD) risk. This study was to determine the AIP and its relationship with other CVD risk factors. Materials and Methods. This cross-sectional study was done among 349 staff of a public university in Sarawak. Data were collected using questionnaire, blood sampling, and anthropometric and blood pressure measurement. Data were analyzed using IBM SPSS version 20. Results. A total of 349 respondents participated with majority females (66.8%), aged 38.5 ± 7.82 years. Nearly 80% of the respondents were overweight and obese, 87.1% with high and very high body fat, and 46.9% with abnormal visceral fat. For AIP category, 8.9% were found to be in intermediate and 16.4% were at high risk. Elevated lipid profile showed that total cholesterol (TC) is 15.5%, low density lipoprotein (LDL) is 16.1%, and triglyceride (TG) is 10.6%. AIP was significantly correlated with b...
