BackgroundBehçet's disease (BD) is characterized by recurrent oral and/or genital aphthous ul... more BackgroundBehçet's disease (BD) is characterized by recurrent oral and/or genital aphthous ulcers accompanied by cutaneous, ocular, articular, gastrointestinal and central nervous system lesions. Diagnosing BD can be a clinical challenge in patients presenting with a limited number of organ manifestations, especially with single major organ involvement. We reported the first controlled doppler ultrasound study showing increased common femoral vein (CFV) thickness in BD (1). We recently also showed that increased CFV thickness is a distinctive feature of BD, rarely present in other inflammatory or vascular diseases such as ankylosing spondylitis, systemic vasculitides, venous insufficiency, and non-inflammatory DVT with a specificity higher than 80% for the cut-off value of ≥ 0.5 mm. Objectives This study aimed to investigate in mononuclear cell culture supernatants, in vitro cytokine responses to S. Sanguis in patients with BD compared to psoriasis (PS) and healthy controls (HC). Methods Twelve patients with BD (4F/8M, mean age: 33.6 years), 7 with PS (4F/3M, mean age: 38.2 years) and 10 HC (7F/3M, mean age: 27.5 years) were included in the study. IL-4, IL-10, and IFN-γ responses were measured in peripheral blood mononuclear cell culture supernatants by ELISA after 48 h stimulations with S. Sanguis (10 μg/ml) and PHA (20 μg/ml). Peripheral Blood Mononuclear Cells (PBMCs) were isolated from 20 ml of venous blood of all participants. It causes decreased self‐esteem and life‐satisfaction, increased depression and suicidality, and difficulty in coping with the illness. The primary aim of this study was to investigate the internalized stigma state of psoriatic patients and to identify the factors influencing internalized stigma. The secondary aim was to identify the correlation of internalized stigma with quality of life and perceived health status. This multicentre, cross‐sectional study comprised 1485 patients. Patients with psoriasis are at an increased risk of developing liver disease due to various facto... more Patients with psoriasis are at an increased risk of developing liver disease due to various factors. The existing data regarding the treatment of psoriasis patients with associated liver cirrhosis is limited. We report four patients of psoriasis with liver cirrhosis who were treated with TNF-alpha inhibitors for a mean duration of 35.4 months. Two patients were treated with etanercept, one with adalimumab and one was treated with both infliximab and etanercept. Three patients tolerated the treatment well without any deterioration of liver disease whereas one died of progressive liver disease. Although large-scale, controlled studies are needed, this case series provides insights regarding the long-term safety of TNF-alpha inhibitors in patients with psoriasis and liver cirrhosis.

Mycosis fungoides (MF) simulates a variety of dermatologic disorders histopathologically and clin... more Mycosis fungoides (MF) simulates a variety of dermatologic disorders histopathologically and clinically, well deserving the designation of a great mimicker. Interstitial MF is a rare, but well-recognized histopathological variant resembling the interstitial form of granuloma annulare or the inflammatory phase of morphea. From a clinical standpoint, MF can have a wide array of manifestations, including an anecdotal presentation with lesions clinically suggestive of lichen sclerosus (LS). We herein report a 25-year-old man with a history of patch-stage MF who later developed widespread LS-like lesions histopathologically consistent with interstitial MF. In some biopsies, additional features resembling LS were discerned. We think that our case might represent a unique variant of interstitial MF presenting with LS-like lesions. The diagnostic challenge arising from this uncommon presentation is discussed together with review of the literature.

The aim of this study was to develop a patient-derived disease activity index in Beh-çet's disease (BD). One hundred and seventy-seven BD patients (M/F: 69/108) were included in the study. Data were collected by clinical examination and a questionnaire regarding a Mucocutaneous Index (MI) with genital ulcer activity (GI), erythema nodosum activity (EI) and the composite index (CI) for oral ulcers, as subscales of it. Self-reported treatment evaluation was carried out as criterion validity. Patients whose symptoms completely disappeared or decreased significantly, were categorised as 'improved group', others were classified as 'non-improved group'. Among the study group, 79.7% of the patients (n=141) were active, whereas 20.3% were inactive (n=36). Scores of CI, GI, EI and MI score were 0±0 in inactive ones, whereas scores were 5.65±2.36 for CI, 0.81±2.34 for GI, 0.91±2.3...

The single nucleotide polymorphism (SNP) of TIRAP (Serine 180 leucine, S180L) that is shown to be... more The single nucleotide polymorphism (SNP) of TIRAP (Serine 180 leucine, S180L) that is shown to be associated with Behçet's disease (BD) in a European-derived cohort, but not in Middle Eastern patients is investigated in two other populations. Two cohorts of BD patients and controls from Turkey (n=797) and Italy (n=633) were genotyped by sequence specific primer-polymerase chain reaction (SSP-PCR) or TaqMan q-PCR assays. Genotype and allele frequencies in TIRAP S180L (rs8177374) were not different between BD patients and controls in either ethnicity. Furthermore, a meta-analysis between the Turkish and the Italian BD cohorts did not reveal an association between this non-synonymous SNP in TIRAP and BD (meta-analysis OR=0.94, meta-analysis p=0.61, Q statistic heterogeneity p=0.11). TIRAP S180L gene polymorphism, which was previously shown to be associated with BD in a Caucasian population, has been replicated in either Turkish or Italian population in our study.
