The objectives of this cross sectional study are to determine the mean bone mineral density (BMD) and to identify osteopenia and osteoporosis in patients of CKD on maintenance hemodialysis. Methods: Twenty three male and 18 female patients with age between 18 and 50 years were enrolled in this study. The BMD of the lumbar vertebral spine (LV) and the neck of femur (FN) were measured in all patients. Data were analyzed using SPSS version 20.0 software and the level of significance was considered as P <0.05. Results: The mean BMD in the LV (L2-L4) was 1.18 ± 0.19 gm/cm2 in male and 1.04 ± 0.13 gm/cm2 in female patients (P =0.011). Original Research Article Objective: In this study our main goal is to assess antiproteinuric eff... more Original Research Article Objective: In this study our main goal is to assess antiproteinuric effects of Doxycycline in patients with diabetic nephropathy. Method: This is a prospective interventional study and conducted at the Department of Nephrology in DMCH.The study included 60 clinically proven adult patients of DN. All patients were on optimal doses of angiotensin-converting enzyme inhibitors (ACEIs) or angiotensin receptor blockers (ARBs) for 2 months before the study. The patients were divided into two groups named control (group I, n = 30) and intervention group (group II, n = 30). Control group patients were maintained on optimal doses of ACEIs or ARBs, whereas intervention group patients received Doxycycline (100 mg/day) for a period of 3 months in addition to ACEIs or ARBs. Data were collected at month 0 and at month 1 at month 3. Results: The study result revealed that before intervention, the mean basal levels of 24 hours proteinuria was 2.2 ± 1.3g/day for Group I and 2.7 ± 1.42 g/day for Group II. P value is not significant in both group at baseline (p=0.2). Adequate glycemic control was achieved with insulin, oral hypoglycemic agents or both in all the patients. It reduced to 2.0 ± 1.2g/day for Group I and 2.5± 1.3g/day for Group II, at the end of 1 month. At the end of 3 months, a significant decline of proteinuria was observed in both the groups. In Group I it had a mean of 1.95 ± 1.2g/day, whereas it was 1.25± 0.78g/day in Group II. A statistically significant difference existed between the control and intervention groups (p < 0.05), at 3 months. Conclusion: Doxycycline has significantly reduces proteinuria in diabetic nephropathy patients.

The aim of this study was to determine whether donor age is related to families' decision-making regarding consent for organ donation. METHODS We obtained the data of 2451 organ donors with brain death (men 1645, women 806; mean age, 46.5 ± 16.2 years) registered with the Korean Network for Organ Sharing for the period between December 2012 and December 2017. The duration between the admission of the patient and the first brain death assessment was determined. RESULTS The mean duration from admission to the first brain death examination was significantly longer in the 0 to 30 age group (16.23 ± 6.01 days) compared with the 31 to 83 age group (6.7 ± 1.07 days) (P < .001). There was a strong negative correlation (r = 0.795, P = .010) between age and the mean duration from admission to the first brain death examination. CONCLUSION Because the family members of younger potential organ donors needed more time to provide consent, the first brain death examination and, therefore, the donation process was delayed in cases of young donors.

Methods:This analytic, descriptive, cross-sectional study was performed over 191 patients of maintenance haemodialysis in 2014. Information related to the patients including age, gender, residence, pruritus was extracted from questionnaires. Serum levels of intact parathormone were measured & data were analyzed. Results: 68% of the patients had pruritus. The Mean ± SD of serum parathormone was 53.25±7.96 pg/ml in patients with pruritus and 81.91±9.34 pg/ml in patients without pruritus. Disturbance in calcium and phosphate metabolism are observed in these patients. One of the most common cutaneous manifestations in patients on hemodialysis is pruritus. The aim of this study is to evaluate the association between pruritus with serum calcium and phosphate levels in these patients. Methods: This analytic, descriptive, cross-sectional study was performed in 2014 over 191 patients of maintenance haemodialysis. Information related to the patients age, gender, residence, pruritus, duration and frequency of dialysis, was extracted from questionnaires. Serum levels of calcium, phosphate and albumin were measured & data were analyzed. Results: 68% of the patients had pruritus, of whom 48.2% had serum calcium levels below the normal range. Medical Management of Brain-Dead Organ Donors With improving healthcare services, the demand for ... more Medical Management of Brain-Dead Organ Donors With improving healthcare services, the demand for organ transplants has been increasing daily worldwide. Deceased organ donors serve as a good alternative option to meet this demand. The first step in this process is identifying potential organ donors. Specifically, braindead patients require aggressive and intensive care from the declaration of brain death until organ retrieval. Currently, there are no specific protocols in place for this, and there are notable variations in the management strategies implemented across different transplant centers. Some transplant centers follow their own treatment protocols, whereas other countries, such as Bangladesh, do not have any protocols for potential organ donor care. In this review, we discuss how to identify brain-dead donors and describe the physiological changes that occur following brain death. We then summarize the management of brain-dead organ donors and, on the basis of a review of the literature, we propose recommendations for a treatment protocol to be developed in the future.

BACKGROUND This study assesses the level of knowledge and attitudes toward organ donation among d... more BACKGROUND This study assesses the level of knowledge and attitudes toward organ donation among doctors, nurses, medical students, patients, and relatives of patients at Dhaka Medical College Hospital in Dhaka, Bangladesh. METHODS A cross sectional study was conducted at Dhaka Medical College Hospital among doctors, nurses, medical students, patients, and attendants (100 participants in each group). Participants completed a 20-item self-administered questionnaire that assessed levels of knowledge and attitudes regarding organ donation. RESULTS Among the 500 participants, 85% had heard about organ donation, but only 46% of doctors, 33% of nurses, and 41% of medical students could report the names of all the organs that can be donated. One-fourth (25.4%) had agreed to donate their organs (23% of doctors, 17% of nurses, 28% of medical students, 29% of patients, and 30% of attendants), and 26.0% did not agree to donate (17% of doctors, 19% of nurses, 16% of medical students, 42% of patients, and 36% of relatives). Less than half (43.8%) of participants reported that they would donate organs to help someone when they die. Among the entire sample, 16% reported that their family would be upset if they donated organs, and 12.6% reported that it was against their religion. CONCLUSION Although medical professionals have a better understanding of organ donation than nonmedical individuals, their attitudes toward organ donation are similar to that of the general population. A strategic approach should be taken to develop and launch public campaigns targeting both medical professionals and the general population to educate and raise awareness about organ donation.
