Diffuse large B cell lymphoma (DLBCL) is the histological subtype of non-Hodgkin’s lymphoma, repr... more Diffuse large B cell lymphoma (DLBCL) is the histological subtype of non-Hodgkin’s lymphoma, representing approximately 30%. The most common primary extranodal sites of DLBCL are the gastrointestinal (GI) tract, the head and neck, and the skin/soft tissue. We report a case of DLBCL with cutaneous involvement presenting with skin nodules and GI manifestations such as obstructive jaundice and upper GI bleeding. Malignant cystic pancreatic tumor occupying the head and body with invasion to lower end of common bile duct and periampullary region causing biliary obstruction and mesenteric lymphadenopathy were found in abdominal computed tomography and endoscopic ultrasonography. There was also a large gastric ulcer (Forrest IIa) at the greater curvature of body of the stomach. Histopathological results of the skin and stomach were consistent with diffuse large B cell lymphoma; gastric biopsy being negative for leucocyte common antigen. We hypothesized that EUS accuracy could be influenced by the location, histology and ethnicity and evaluated those factors in EUS performed in the community Methods: This is a retrospective analysis of all patients diagnosed with gastric and GEJ cancers who had EUS followed by surgery without neoadjuvant chemotherapy between 1/2006 and 12/2013. EUS and surgical T and N staging were scored for accuracy, understaging or upstaging by EUS. The reported accuracy varies from 40% to 90%. We aim to investigate the staging performance of EUS at our institution and hypothesize that EUS accuracy can be influenced by tumor location, histology, and patient ethnicity. Methods: We performed a retrospective analysis of patients diagnosed with gastric and GEJ cancer between January 2006 and June 2015 who had EUS proceeded directly by surgery. T and N staging by EUS were analyzed for their sensitivity, specificity, and positive (PPV) and negative predictive values using surgery as the gold standard. The following variables were evaluated: tumor histology (intestinal or diffuse type), location (GEJ, cardia, body, or antrum), and patient ethnicity (Asian versus non-Asian).
