Aims and method To investigate whether a psychiatry-specific virtual on-call training programme i... more Aims and method To investigate whether a psychiatry-specific virtual on-call training programme improved confidence of junior trainees in key areas of psychiatry practice. The programme comprised one 90 min lecture and a 2 h simulated on-call shift where participants were bleeped to complete a series of common on-call tasks, delivered via Microsoft Teams. Results Thirty-eight trainees attended the lecture, with a significant improvement in confidence in performing seclusion reviews (P = 0.001), prescribing psychiatric medications for acute presentations (P < 0.001), working in section 136 suites (places of safety) (P = 0.001) and feeling prepared for psychiatric on-call shifts (P = 0.002). Respondents reported that a virtual on-call practical session would be useful for their training (median score of 7, interquartile range 5–7.75). We present the preliminary findings from 13 papers, analysed to create a conceptual framework for a further mixed methods analysis. The studies were critically appraised for methodological quality. Result. Five key themes emerged from the preliminary analysis of 13 papers. BAME students experienced: Being 'divergent': Not feeling part of the current organisational learning milieu Lack of social capital: Difficulty in being absorbed into existing 'networks' of relationships in a manner that is 'approachable' and not 'intimidating' Continuum of discrimination: 'Indirect' impact of subtle communication processes in the learning environment undermining individual 'belief' in own performance Institutional discriminatory factors: Culture, rules, norms, and behavioural routines of educators that lead to differential outcomes for learners Lack of external support: Relative lack of interventions tackling DA. Conclusion. The key finding of this review is that British BAME undergraduate medical students experience discriminatory behaviours early in medical schools that impact on personal, educational, and professional outcomes. These factors may need to be borne in mind by postgraduate training organisations such as the Royal College of Psychiatrists as they commence the challenging task of addressing DA.

Coronavirus disease 2019 (COVID-19) is likely to have long-term mental health effects on individu... more Coronavirus disease 2019 (COVID-19) is likely to have long-term mental health effects on individuals who have recovered from COVID-19. Rightly, there is a global response for recognition and planning on how to deal with mental health problems for everyone impacted by the global pandemic. This does not just include COVID-19 patients but the general public and health care workers as well. There is also a need to understand the role of the virus itself in the pathophysiology of mental health disorders and longer-term mental health sequelae. Emerging evidence suggests that COVID-19 patients develop neurological symptoms such as headache, altered consciousness, and paraesthesia. Brain tissue oedema and partial neurodegeneration have also been observed in an autopsy. In addition, there are reports that the virus has the potential to cause nervous system damage. Abstract Acute confusional state, or delirium, is a transient disorder of brain function with cor... more Abstract Acute confusional state, or delirium, is a transient disorder of brain function with core features of impairment of consciousness with attentional deficit, other cognitive alterations, and a relatively rapid onset of the disorder with a characteristically fluctuating course. Recognition and appropriate evaluation and treatment of delirium is important to decrease the substantial morbidity and mortality associated with it. Delirium is a syndrome reflecting brain dysfunction that is almost always due to identifiable systemic or cerebral disease or to drug intoxication or withdrawal. The key to treating delirium is to identify and ameliorate any causal or contributing medical conditions. The use of psychoactive drugs should be minimized, although low-dose antipsychotics can be used to decrease psychomotor agitation if indicated. Keeping the patient in an environment that is quiet and free of unnecessary stimulation, providing extra supportive physical care, with maintenance of basic functions such as food and fluid intake, is essential.

ObjectiveData related to brain function may have the potential to improve the reliability and val... more ObjectiveData related to brain function may have the potential to improve the reliability and validity of assessments for the aetiologically and clinically heterogeneous syndrome of attention-deficit/hyperactivity disorder (ADHD). This study investigated associations between questionnaire assessments of behavioural features of adults with ADHD and an aspect of neurocognitive performance which has been reported to be impaired in adults with ADHD.MethodsFifty-nine adult patients with a DSM-IV diagnosis of ADHD, and their informants, completed questionnaires related to aspects of severity of ADHD. Associations were examined between questionnaire ratings and performance on a computer-administered task of spatial working memory (SWM).ResultsCorrelations between ratings of ADHD and SWM indicated moderate but significant correlations for patients' ratings, but not for informants' ratings.
