Pseudo-Ludwig angina is a swelling that develops generally due to a cause other than an infection... more Pseudo-Ludwig angina is a swelling that develops generally due to a cause other than an infection and can lead to fatal conditions such as severe respiratory obstruction. Due to anticoagulant use is rarely reported in the literature. This study presented a patient whose INR value was within the normal range and who developed sublingual swelling after dental treatment (intraoral trauma). A 68-year-old female patient was admitted to the emergency department complaining of increasing intraoral swelling on examination with a sublingual 3x5 cm hematoma. Warfarin was stopped, and she was monitored for possible intubation risk in the service. The patient did not develop respiratory distress and was discharged 4 days later. The INR range within which intraoral interventions can be performed safely is still unclear. The dentist should keep in mind that such complications may develop and be prepared for bleeding complications before treatment using warfarin patients.

Çalışmaya, 1998 yılı içinde Ankara Yüksek Đhtisas Hastanesi Kardiyoloji Kliniğine, KAH öntanısı ile yatırılan 121'i erkek ( yaş ortalaması = 54,8 ±11) ve 44'ü kadın (yaş ortalaması = 55,9 ±11), ardışık 165 hasta alındı. Leptin düzeyleri erkeklerde 7,45 ± 7,1 mgr/L, kadınlarda 19,95±16,4 mgr/L olarak saptandı (p< 0.001). Daha önce leptin ile yapılan çalışmalarda, kadınlardaki leptin düzeyi yüksekliği nedeninin, vücut yağ oranlarının erkeklerden daha fazla olmasından kaynaklanabileceği belirtilmiştir. Ancak, kadınlardaki farklı olan vücut kitle indeksi ve vücut yağ yüzdesi düzeltilmesi yapıldıktan sonra da kadınlardaki leptin düzeyi erkeklere göre 1.5-2 kat daha yüksek saptandı (p < 0.05). Hastaların yapılan koroner angiografilerinde erkeklerin 94'ünde (%77,7), kadınların 22'sinde (%52,4) KAH saptandı. Vücut kitle indeksi ile vücut yağ yüzdesi (r=0.77, p<0.001), bel-kalça oranı (r=0,34, p< 0.001), Trigliserid düzeyi (r=0,21,p< 0.05) ve leptin (r=0,52, p<0.001) arasında pozitif ilişki saptandı. Vücut yağ yüzdesi ile, trigliserid düzeyi (r=0,18, p< 0.05), BKO (r=0,32, p< 0.001) ve leptin (r=0,45, p<0.001) arasında pozitif ilişki mevcuttu. BKO ile leptin (r=0,22, p<0.05) ve fibrinojen (r=0,20, p< 0.05) arasında pozitif ilişki saptandı. Leptin ve fibrinojen arasında ise negatif ilişki saptandı (r=-0,23, p< 0.05). Leptin ile koroner arter hastalığı ve lipid parametreleri arasında ilişki saptanmadı. Ancak sigara içenlerde içmeyenlere göre leptin seviyesi ve vücut yağ yüzdesi daha düşük saptandı (p<0.01). Sonuç olarak; enerji metabolizmasının düzenleyicisi ve obezite gen ürünü olan leptin ile KAH ve lipid parametreleri arasındaki herhangi bir ilişki tespit edilemezken, leptin ile sigara ve fibrinojen arasında negatif ilişki , obezite göstergesi olan vücut yağ yüzdesi ve vücut kitle indeksi ile pozitif ilişki saptandı.

The aim of this study was to investigate whether an association exists between mitral annular calcification and carotid atheroma. In addition, the characteristics of carotid atheromas were compared between patients with and without mitral annular calcifica tion. The authors found that there was a significant association between the presence of mitral annular calcification and carotid atheroma. Mitral annular calcification in the elderly may be a form of atherosclerosis.

In this study, the effect of valve replacement on diastolic parameters was evaluated preop erativ... more In this study, the effect of valve replacement on diastolic parameters was evaluated preop eratively and postoperatively at 3, 6, and 12 months by comparing diastolic parameters in patients after aortic valve replacement with freestyle stentless porcine xenografts for aortic stenosis. Depending on deceleration time (DT) and isovolumetric relaxation time (IVRT) with preoperative echocardiographic assessment, patients were divided into two groups: restrictive physiology (DT ≤ 150 msec and IVRT < 100 msec, 20 patients), and nonrestrictive physiology (DT > 150 msec and IVRT ≥ 100 msec, 27 patients). Although left ventricular mass index signif icantly decreased in both groups, improvement in DT, IVRT, and ejection fraction occurred only in patients with restrictive physiology. As a result, the patients with restrictive diastolic characteristics had more benefit than the patients with nonrestrictive physiology after aortic valve replacement.

As well as the severity of stenosis, embologenic properties of plaques are also responsible for ischemic poten tials of atherosclerotic plaques in carotid arteries. In this study, the authors aimed to define the characteristics of plaque morphology in patients with and without aortic valvular calcification (AVC) and to show the association between AVC and carotid plaque characteristics. Carotid plaque morphology in 182 consecutive patients with AVC was compared with plaque charac teristics in 170 patients without AVC. Risk factors for atherosclerosis, age, and gender were similar in patients with and without AVC.
