To study the cardiac and non-cardiac safety of amiodarone and measure the efficacy of amiodarone ... more To study the cardiac and non-cardiac safety of amiodarone and measure the efficacy of amiodarone in restoring and maintaining sinus rhythm. 35 patients on amiodarone therapy were examined for its safety and efficacy during a period of 6-months. The dosing schedule of amiodarone in the study population was as follows: I.V. loading doses of 150-300 mg bolus over 30 minutes, followed by 1 mg per minute for 6 hours, followed by daily oral maintenance dose of 100-400 mg; I.V. + oral loading dose of 800-2320 mg per day, to a total dose of up to 10 gm, followed by daily oral maintenance dose of 100-400 mg; Oral loading dose of 200 mg T.D.S. for 5 days followed by 200 mg B.D. for 5 days and a daily oral maintenance dose of 100-200 mg O.D. Out of 35 patients, only 2 patients developed severe ADR (hypothyroidism) and discontinued amiodarone therapy. Amiodarone could restore rhythm in 15 (83.33%) out of 18 patients with Ventricular Tachyarrhythmias (VT) and in 10 (83.33%) patients out of 12 patients with Atrial Fibrillation (AF). In other types of arrhythmias (Supraventricular Tachycardia (SVT), Ventricular Tachycardia (VT), Ventricular Premature Complexes (VPCs) & Supraventricular Premature Complexes (SVPCs), amiodarone could restore rhythm in 5 (100%) out of 5 patients. Amiodarone was found to be safe and effective in treating all types of arrhythmias.
Blood stream infections range from self-limiting infections to life-threatening sepsis causing si... more Blood stream infections range from self-limiting infections to life-threatening sepsis causing significant mortality and morbidity worldwide and require rapid and aggressive anti-microbial treatment. Rational and correct use of antibiotics requires understanding of common pathogens and drug resistance patterns in a community. A retrospective study was conducted to identify the microbial profile in the blood culture isolates and their antibiotic susceptibility patterns in a tertiary care teaching hospital. The reports of specimens submitted for blood culture during the period of January to December 2009 to the microbiology laboratory were obtained, the positive cultures were identified, and data on the microbial species and their antibiotic sensitivity patterns were collected. There were 3442 blood culture samples, of which 289 (8.39%) were identified to be culture positive (174 (60.2%) males and 115 (39.7%) females; mean age 43.9 ± 24.08 years). Of 289 positive cultures, 277 (96.19%) showed bacterial growth, 106 (37.7%) were gram-positive and 168 (59.1%) gram-negative. Candida species were isolated from 12 (4.15%) specimens. Staphylococcus aureus and E. coli were the most common bacterial organisms identified to cause blood stream infections. Staphylococcus aureus was found to be highly sensitive to vancomycin (86.61%) linezolid and ciprofloxacin (60.16%); resistant to ampicillin, erythromycin and cephalosporins. E-coli was highly sensitive to amikacin (86.8%) and piperacillin + tazobactum (77%); resistant to ampicillin and cephalosporins. Specific antibiotic utilization strategies like antibiotic restriction, combination therapy and usage according to the standard antimicrobial susceptibility testing may help to decrease or prevent the emergence of resistance and incidence of blood stream infections.

A study was conducted in 90 patients of either sex (30 patients each in streptokinase (SK), tenecteplase (TNK) and reteplase (RTP) groups), aged above 18 years, admitted with acute STEMI in the Intensive Coronary Care Unit to assess the efficacy and safety of fibrinolytics. Data on efficacy and safety parameters before and after the initiation of therapy were obtained from patient medical records and were assessed. Methods: Always Better Control (ABC) and High Moderate Low (HML) analysis were carried out for 4057 drugs purchased in the central pharmacy during the financial year of 2017-2018. The drugs were classified as A, B and C category based on their annual usage value and as Category High (H), Moderate (M) and low (L) drugs based on their unit price respectively. The knowledge of the 80 hospital pharmacists on the tools of inventory control was assessed using a structured questionnaire. Atypical antipsychotics are commonly prescribed for psychosis and metabolic Syndrome has a strong... more Atypical antipsychotics are commonly prescribed for psychosis and metabolic Syndrome has a strong relationship with atypical antipsychotic usage. A prospective study was done to assess the incidence of metabolic syndrome and its components in Psychiatric patients on atypical antipsychotics. Blood pressure, body weight, Body Mass Index and waist circumference were calculated and a 5 ml of venous blood was collected at baseline, and for 12 weeks it was follow up to determine the fasting lipid profile and blood glucose levels. Difference in values, from baseline to follow up was assessed using statistical tools. Metabolic syndrome was assessed using International Diabetes Federation criteria. Of 96 patients, 53 were male and 43 were female. Majority (40%) were in the age range of 26 to 35 years. Patients were given Olanzapine, Quetiapine, Risperidone per oral as monotherapy and Risperidone with Olanzapine or Quetiapine as combination therapy per orally. Changes observed from baseline to follow up included weight gain (89%), increase in BMI (78%), waist circumference (91%), FBS (60%), triglycerides (95%) and decreased HDL (65%) but no changes were observed in blood pressure. Olanzapine was found to cause weight gain both as single (32%) and in combined therapy (15%). Increased waist circumference was observed with all the three drugs. The study identified 5% of patients with metabolic syndrome as per criteria. Duration of antipsychotic usage had a positive correlation with alterations in the metabolic components. Gender had no statistically significant influence over the risk for metabolic syndrome. Early identification of metabolic syndrome and therapeutic intervention in Psychiatric patients on atypical antipsychotics may reduce long term morbidity.

The aim of the study was to investigate the incidence and factors linked with nonalcoholic fatty ... more The aim of the study was to investigate the incidence and factors linked with nonalcoholic fatty liver disease (NAFLD) among type-2 diabetes mellitus (T2DM) patients in Warangal, Telangana, India. It was an observational, cohort crosssectional study; a target sample of 100 T2DM patients was recruited from a single study site of Warangal. Detailed medical history was obtained and laboratory examinations and ultrasound imaging were carried out to evaluate the NAFLD. The overall incidence of NAFLD was 80%. The risk of NAFLD was significantly high in subjects with uncontrolled blood glucose levels, abnormal waist circumference, increased aspartate aminotransferase (AST), and elevated triglyceride levels (p < 0.05). In our study subjects, the consumption of alcohol is the predominant risk factor (Odds ratio: 6.652; 95% confident interval: 1.86-23.74 and p = 0.00*) significantly causing NAFLD among the study subjects. Grade-1 and 2 fatty livers are typically associated with elevated AST levels and a high incidence was observed in men.

Malnutrition can be estimated using a fully quantitative scoring system Subjective Global Assessment-Dialysis Malnutrition Score which is simple, reliable and dynamic. The primary objective of the study was to assess the severity of malnutrition in patients with end stage renal disease and undergoing hemodialysis in a tertiary care teaching hospital in Chennai, using Subjective Global Assessment-Dialysis Malnutrition Score and correlate it with standard indicators of malnutrition like anthropometric and biochemical parameters of the study population by Pearson's correlation. Anthropometric assessment included height, body weight, triceps skin fold thickness, mid arm circumference, mid arm muscle circumference % and biochemical parameters included serum albumin, transferrin, ferritin, total protein, total cholesterol, blood urea nitrogen and creatinine. Based on the scores, of the 66 patients, 91% were moderately malnourished. There was a significant negative correlation between modified Subjective Global Assessment-Dialysis Malnutrition Score and anthropometric measures such as triceps skin fold thickness, mid arm circumference, mid arm muscle circumference; biochemical markers such as albumin, transferrin and ferritin. The data obtained from this study confirm that a high degree of malnutrition was prevalent in patients on hemodialysis, as shown by anthropometric assessment, biochemical markers of malnutrition and Subjective Global Assessment-Dialysis Malnutrition Score. Nutritional status as determined by Subjective Global Assessment-Dialysis Malnutrition Score is a useful and reliable index for identifying patients at risk for malnutrition and it correlates well with anthropometric and biochemical assessment. may be integrated in regular assessment of malnutrition in patients on maintenance hemodialysis.

Patients above 18 years of age, on cancer chemotherapy were included. ADRs experienced by patients were identified and categorized using National Cancer Institute Common Terminology Criteria version 3.0 Questionnaire. The causality assessment of suspected ADRs was done using WHO and Naranjo's scales, severity assessment of ADRs using Hartwig and Siegel scale and preventability of ADRs using Schumock and Thornton scale. Methods: Always Better Control (ABC) and High Moderate Low (HML) analysis were carried out for 4057 drugs purchased in the central pharmacy during the financial year of 2017-2018. The drugs were classified as A, B and C category based on their annual usage value and as Category High (H), Moderate (M) and low (L) drugs based on their unit price respectively. The knowledge of the 80 hospital pharmacists on the tools of inventory control was assessed using a structured questionnaire. A prospective study was done to assess the incidence of metabolic syndrome and its components in Psychiatric patients on atypical antipsychotics. Blood pressure, body weight, Body Mass Index and waist circumference were calculated and a 5 ml of venous blood was collected at baseline, and for 12 weeks it was follow up to determine the fasting lipid profile and blood glucose levels. Difference in values, from baseline to follow up was assessed using statistical tools. Metabolic syndrome was assessed using International Diabetes Federation criteria. Of 96 patients, 53 were male and 43 were female. Majority (40%) were in the age range of 26 to 35 years. Patients were given Olanzapine, Quetiapine, Risperidone per oral as monotherapy and Risperidone with Olanzapine or Quetiapine as combination therapy per orally. Changes observed from baseline to follow up included weight gain (89%), increase in BMI (78%), waist circumference (91%), FBS (60%), triglycerides (95%) and decreased HDL (65%) but no changes were observed in blood pressure. Olanzapine was found to cause weight gain both as single (32%) and in combined therapy (15%). Increased waist circumference was observed with all the three drugs. The study identified 5% of patients with metabolic syndrome as per criteria. Duration of antipsychotic usage had a positive correlation with alterations in the metabolic components. Gender had no statistically significant influence over the risk for metabolic syndrome. Early identification of metabolic syndrome and therapeutic intervention in Psychiatric patients on atypical antipsychotics may reduce long term morbidity.

The aim of the study was to investigate the incidence and factors linked with nonalcoholic fatty ... more The aim of the study was to investigate the incidence and factors linked with nonalcoholic fatty liver disease (NAFLD) among type-2 diabetes mellitus (T2DM) patients in Warangal, Telangana, India. It was an observational, cohort crosssectional study; a target sample of 100 T2DM patients was recruited from a single study site of Warangal. Detailed medical history was obtained and laboratory examinations and ultrasound imaging were carried out to evaluate the NAFLD. The overall incidence of NAFLD was 80%. The risk of NAFLD was significantly high in subjects with uncontrolled blood glucose levels, abnormal waist circumference, increased aspartate aminotransferase (AST), and elevated triglyceride levels (p < 0.05). In our study subjects, the consumption of alcohol is the predominant risk factor (Odds ratio: 6.652; 95% confident interval: 1.86-23.74 and p = 0.00*) significantly causing NAFLD among the study subjects. Grade-1 and 2 fatty livers are typically associated with elevated AST levels and a high incidence was observed in men.

Malnutrition can be estimated using a fully quantitative scoring system Subjective Global Assessment-Dialysis Malnutrition Score which is simple, reliable and dynamic. The primary objective of the study was to assess the severity of malnutrition in patients with end stage renal disease and undergoing hemodialysis in a tertiary care teaching hospital in Chennai, using Subjective Global Assessment-Dialysis Malnutrition Score and correlate it with standard indicators of malnutrition like anthropometric and biochemical parameters of the study population by Pearson's correlation. Anthropometric assessment included height, body weight, triceps skin fold thickness, mid arm circumference, mid arm muscle circumference % and biochemical parameters included serum albumin, transferrin, ferritin, total protein, total cholesterol, blood urea nitrogen and creatinine. Based on the scores, of the 66 patients, 91% were moderately malnourished. There was a significant negative correlation between modified Subjective Global Assessment-Dialysis Malnutrition Score and anthropometric measures such as triceps skin fold thickness, mid arm circumference, mid arm muscle circumference; biochemical markers such as albumin, transferrin and ferritin. The data obtained from this study confirm that a high degree of malnutrition was prevalent in patients on hemodialysis, as shown by anthropometric assessment, biochemical markers of malnutrition and Subjective Global Assessment-Dialysis Malnutrition Score. Nutritional status as determined by Subjective Global Assessment-Dialysis Malnutrition Score is a useful and reliable index for identifying patients at risk for malnutrition and it correlates well with anthropometric and biochemical assessment. may be integrated in regular assessment of malnutrition in patients on maintenance hemodialysis.

Patients above 18 years of age, on cancer chemotherapy were included. ADRs experienced by patients were identified and categorized using National Cancer Institute Common Terminology Criteria version 3.0 Questionnaire. The causality assessment of suspected ADRs was done using WHO and Naranjo's scales, severity assessment of ADRs using Hartwig and Siegel scale and preventability of ADRs using Schumock and Thornton scale. Results: The study was conducted in 109 patients (46 males, 63 females; mean age 53.75±12.75 years). Majority of the patients had breast cancer.
