Aspergillus spp is a fungus known to cause acquired and nosocomial infections in human beings. Isolated Ovarian Aspergillosis as well fungal infections of ovary are very rare. A. fumigatus, the most commonly infecting species. Anti-fungal prophylaxis should be given to high-risk patients. Posaconazole is often given as prophylaxis in immunocompromised patients. Case Report: 50 year P2L2 postmenopausal woman, came with pain in lower abdomen since 6 months. History of loss of weight (10kgs) in last 1 year. Medical and surgical history was not contributory.On per vaginal examination uterus was atrophic retroverted with restricted mobility. A vague non tender cystic to firm mass felt in left adnexa which was not separate from the uterus and right fornix was free. Blood investigation including CA-125 was within normal range. Background: Contraception is the deliberate use of artificial methods or other techniques to prev... more Background: Contraception is the deliberate use of artificial methods or other techniques to prevent pregnancy as a consequence of sexual intercourse. Depot medroxy progesterone (DMPA) injection 150 mg intramuscular was used for contraception in this study, once every 3 months±14 days. The objective was to establish safety and effectiveness of DMPA in lactating and non-lactating mothers, observe side effects related to usage of DMPA and also establish its effectiveness as contraception method. Methods: This is retrospective study performed at Family Planning Association of India's Kutumb Sudhar Kendra. Mumbai Central. Result: Patients who had received first dose at 6 weeks of postpartum period and those who received between 6 weeks to one year showed lactation in 84.16% and non-lactation in 15.84%. 28.71% lactating patients had taken injection at 6 weeks of post-partum period and 71.29% had taken between 6 weeks to 1 year. Lactating patients who had taken injection at 6 weeks and continued DMPA for more than 4 injections were 48.27%. 36.84% non-lactating patients had continued for more than 4 injections. This signifies that acceptance and continuation rate of DMPA is more in lactating patients than in non-lactating patients. Conclusions: DMPA is an effective method of contraception in lactating mothers; proper counseling will increase awareness and acceptance of DMPA in lactating mothers.

Background: To study indications of second stage caesarean section. To study intraoperative and p... more Background: To study indications of second stage caesarean section. To study intraoperative and postoperative complications. To review maternal and perinatal outcome.Methods: This retrospective study was conducted at a tertiary teaching Institute. All second stage caesarean sections performed between a period 2008 to 2012 were analysed in terms of duration of 2nd stage of labour, instrumentation attempted prior to caesarean section, indication of caesarean-section, Intraoperative complications, maternal and perinatal outcome.Results: During the study period 2760, caesarean sections were performed out of them 61 were 2nd stage caesarean section. Out of 61 Caeserean 14 were given prior instrumental trial, followed by 2nd stage caesarean section. Intraoperative complications were higher in terms of blood loss, primary PPH, extension of uterine incision, in one case bladder injury was noticed. Patwardhan method was used in 23% cases for delivery deeply engaged head. Atonic PPH was seen ...

Swyer syndrome was first described by Jim Swyer in 1955. It is a form of "Pure Gonadal Dysgenesis... more Swyer syndrome was first described by Jim Swyer in 1955. It is a form of "Pure Gonadal Dysgenesis". The affected female has 46XY karyotype. A 17 year old unmarried girl came with complaints of primary amenorrhea, non development of breast. On examination she has normal built. Examination of secondary sexual characters revealed no breast development, pubic and axillary hairs were sparse. Female type of genitalia with vaginal opening. Serum FSH was 117.5 mIU/ml. Thyroid and Prolactin was in normal range. Karyotype showed genotype of 46XY. Diagnostic laparoscopy showed streak gonads, small uterus, and normal fallopian tubes. Diagnosis of Swyer syndrome was made. Patient was started on hormonal replacement therapy (HRT). In Swyer syndrome there is a mutation of SRY gene leads failure of development of testis. Mullerian duct development takes place by default. These patients can have normal sexual intercourse and can become pregnant by donor oocyte. They need to be on HRT. The risk of gonadoblastoma and dysgerminoma are very high in streak gonads so bilateral gonadectomy is advised.
