Original Research Article Introduction: Neonatal mortality is a global health problem that has be... more Original Research Article Introduction: Neonatal mortality is a global health problem that has been at the heart of World Health Organization (WHO) programs for more than 30 years. In Morocco, it represents a real scourge and remains high despite the efforts made by the Ministry of Health. We conducted this study to assess the prevalence of neonatal mortality, determine its causes and risk factors, and propose preventive measures to reduce this rate. Methods: This was a retrospective descriptive and analytical study conducted over a period of four and a half years, from January 2016 to June 2020, in the Pediatric Emergency Department of the Centre Hospitalier Universitaire Mohamed VI in Marrakech. Results: We recorded 268 deaths, representing a neonatal mortality rate of 18.3%, with a preponderance of early neonatal deaths (73.9%). The sex ratio was 1.45 in favor of males. Mean age at death was 6.5 days. 70% of pregnancies were poorly attended, medical deliveries accounted for 86% in our serie, and 90.5% of deliveries were vaginal. The majority of deaths occurred in parturients aged between 20 and 29 years (57.3%), and 28.5% of deaths occurred in primiparous women. The prematurity rate was 49.2%, and 58% of the newborns who died had a low birth weight. Prematurity was the leading cause of neonatal mortality (49.2%), followed by neonatal infection (38.5%) and perinatal asphyxia (12.3%). Conclusion: Effective management of pregnancy and the newborn in the first week of life should improve neonatal prognosis.

Neonatal tetanus is a toxi-infection caused by Clostridium tetani, with a neurological tropism, c... more Neonatal tetanus is a toxi-infection caused by Clostridium tetani, with a neurological tropism, causing significant morbidity and mortality in newborns in developing countries. The infection almost always starts in the umbilical cord. Diagnosis is clinical. Treatment is two-pronged: etiological (specific immunoglobulins, antibiotic therapy, anti-tetanus vaccination and treatment of the portal of entry) and symptomatic (sedatives and muscle relaxants, curarization). Prevention is the only effective option, and relies on vaccination of mothers during pregnancy and health education of the population. We report the case of a newborn hospitalized for neonatal tetanus and treated with antibiotic therapy combined with intrathecal serotherapy, with a good evolution.

The hydatid cyst, or hydatidosis, is a zoonotic infectious disease caused by the development of t... more The hydatid cyst, or hydatidosis, is a zoonotic infectious disease caused by the development of the larval form of the Echinococcus granulosus tapeworm in humans after accidental ingestion of embryonated eggs, which hatch into oncospheres and transform into hydatid cysts. Renal hydatid cyst is extremely rare in children. The positive diagnosis relies on epidemiological, clinical, radiological, and biological arguments. The standard treatment remains surgery. We report the case of a 5-year-old male child hospitalized for an isolated renal hydatid cyst fistulized into the urinary tract, revealed by lumbalgia and hydatiduria with a proteus mirabilis urinary infection on the cytobacteriological examination of urine. Paraclinical examinations, including ultrasound and uro-CT-scan, aided in diagnosis. Histopathological analysis confirmed the diagnosis of renal hydatidosis. Management was conservative surgical, with partial pericystectomy performed in our patient, resulting in a favorable outcome.

It represents a major public health problem as well as an economic burden for the families of affected children, health systems and the overall economies of countries. This further accentuates the role that general practitioners can play in the management of childhood epilepsy in the face of the persistent lack of neurologists and neuropediatricians in our country. Methods In order to assess the knowledge and therapeutic habits of general practitioners, we carried out a descriptive and cross-sectional study with general practitioners practicing in the two healthcare sectors: public and private, and in two settings: urban and rural, during the year 2018. The data was collected through a pre-established survey. Results In total, 155 general practitioners responded to the survey. For 85.2% of physicians, the diagnosis of childhood epilepsy was based on interrogation, physical examination, and EEG. While brain imaging would be systematic regardless of the type of epilepsy for 45.2% of doctors. Only 6 doctors (3.9%) had knowledge of the latest classifications of the "ILAE". For treatment, the majority of physicians (65.5%) adopted first-line monotherapy with valproate in leading position. Almost half of the doctors (48.4%) found that education of parents and children was always necessary. None of the GPs interviewed in our series assessed the academic impact of epilepsy. Only 32% of doctors had received continuing education on epilepsy. Conclusion The data from our study demonstrates that continuing education on the management of childhood epilepsy and the greater involvement of general practitioners were essential elements in improving care.

Foreign body aspiration is a rare cause of pneumomediastinum and subcutaneous emphysema. Foreign body aspiration leading to the occurrence of pneumomediastinum in a child with asthma may go unnoticed and be wrongly attributed to asthma, which leads to delayed diagnosis as well as to life-threatening and long-term complications. Case presentation: We describe a case of a 6-year-old Moroccan boy with asthma who was admitted to our emergency department for acute dyspnea and persistent dry cough. The patient was initially treated as having acute asthma exacerbation. Owing to insufficient clinical and radiographic improvement with asthma treatment, a rigid bronchoscopy under general anesthesia was performed. A pumpkin seed was removed from the left main bronchus. Clinical and radiographic improvement was achieved after foreign body extraction. Conclusions: This case emphasizes that the possibility of foreign body aspiration should always and carefully be considered by the emergency physician when faced with a child with asthma presenting with pneumomediastinum and subcutaneous emphysema as an important differential diagnosis even in the absence of a history of foreign body aspiration.

PURPOSE The goal of this study was to analyze the epidemiological and therapeutic features and outcomes of periorbital cellulitis cases treated in the pediatric emergency department. PATIENTS AND METHODS A retrospective study including all the children aged between 1 month and 15 years treated for periorbital cellulitis in the Pediatric Emergency Department of the Mohamed VI University Teaching Hospital in Marrakech over a period of 10 years (January 1, 2010-December 31, 2019). RESULTS In all, 168 cases of periorbital cellulitis were recorded, with an increasing of the number of cases, from 2 in 2010 to 39 in 2019. The most affected age bracket was the group under 5 years of age (62.5%). The most frequent mode of entry was sinusitis (22%). Preseptal cellulitis was most common (76.7%). The main clinical signs found in orbital cellulitis were proptosis (64%) and chemosis (35.8%), versus conjunctival hyperemia (78%) in preseptal cellulitis. Ophthalmoplegia was present in two cases of orbital cellulitis. The right side was most affected (44%). An orbital CT scan was performed in all cases in our study, showing preseptal cellulitis in 129 patients (76.7%), orbital cellulitis in 14 cases (8.3%), subperiosteal abscess in 20 cases (12%) and orbital abscess in 5 cases (3%). Prior treatment with non-steroidal anti-inflammatory medication was noted in 6%. The most commonly used antibiotic was amoxicillin-clavulanic acid. Steroid treatment was prescribed in 6% of cases. Surgical treatment was indicated in 12 patients (7.1%). The mean hospital length of stay was 3 days for the preseptal cases and 8 days for the orbital cases. All patients had good outcomes with medical and/or surgical treatment. With follow-up of over one year, no complications were noted. CONCLUSION The majority of our cases had positive outcomes, highlighting the advantage of early diagnosis, adapted antibiotic treatment and multidisciplinary care, rendering surgery rarely necessary.

Serious underlying causes should be always evoked. Case Report. We describe here a 9-year-old girl who was admitted at our emergency department for vaginal bleeding and severe anemia. No history of trauma and no evidence of prior abuse were reported by the parents. Full Blood Count showed profound anemia (hemoglobin at 4 g/dl). The child was managed as a hypovolemic shock and resuscitated with intravenous fluids and urgent blood transfusion. Gynecologic examination found a live leech at the vulva and the extraction was facilitated by applying a saline solution. The child was clinically stable and discharged home the next day with ambulatory treatment. Conclusion. This case emphasizes that a through external genital exam with possible exam under anesthesia should be undertaken in all girls with unexplained vaginal bleeding and that in those living in rural areas without potable water, leech infection...

Abstract Biogas plays a vital role in many renewable and sustainable energy strategies, where CO2... more Abstract Biogas plays a vital role in many renewable and sustainable energy strategies, where CO2 removal is required to produce biomethane (i.e., biogas upgrading). New ideas and technologies have been proposed to improve the process efficiency of biogas upgrading with liquid absorbents, and ionic liquids have been identified as promising candidates. Meanwhile, there is an urgent need to effectively utilize low-quality waste heat in industry. In this study, two strategies (i.e., waste heat integration and desorption unit modification) were proposed and compared with the original high-pressure water scrubbing; in addition, the effects of the temperatures of the heat source and solvents on the energy usage and cost were further investigated. The results showed that the total cost of the process integrated with waste heat recovery decreased by 6.2% compared with the original high-pressure water scrubbing, and the higher temperature of the waste heat source corresponded to a lower cost. The comparison of different solvents further confirmed that ionic liquid-based solvents were promising, with a potential cost reduction of 24.7%; the properties of the solvent, such as the CO2 absorption capacity, selectivity, and viscosity had significant influences on the total cost, and a new and single index “comparative absorption factor” was proposed to reflect the overall impact of these properties on the cost.

Facial swelling is a common complication of dental management. The occurrence of subcutaneous emphysema, pneumothorax, and pneumomediastinum after dental procedures is rare. We present a case with subcutaneous emphysema of the upper chest, neck, chin, and pneumomediastinum after a tooth extraction and discuss the possible mechanism of subcutaneous emphysema. To prevent these complications during dental procedures, dental hand pieces that have air coolant and turbines that exhaust air in the surgical field should not be used.

It is considered a common cause of childhood morbidity and mortality. We present a unique and unusual case of inhalation of a piece of tissue in a 10-month-old infant admitted to the Pediatric Emergency Department of the Mohamed VI Teaching University Hospital in Marrakech in 2021. The patient was initially seen with acute dyspnea, persistent dry cough, and signs of respiratory distress. The auscultation revealed decreased vesicular murmurs in the right lung field. Chest radiography did not reveal the presence of the foreign body, justifying the use of rigid bronchoscopy for diagnostic and therapeutic purposes, and revealing the presence of a tissue in the bronchial area. PURPOSE The goal of this study was to analyze the epidemiological and therapeutic features and outcomes of periorbital cellulitis cases treated in the pediatric emergency department. PATIENTS AND METHODS A retrospective study including all the children aged between 1 month and 15 years treated for periorbital cellulitis in the Pediatric Emergency Department of the Mohamed VI University Teaching Hospital in Marrakech over a period of 10 years (January 1, 2010-December 31, 2019). RESULTS In all, 168 cases of periorbital cellulitis were recorded, with an increasing of the number of cases, from 2 in 2010 to 39 in 2019. The most affected age bracket was the group under 5 years of age (62.5%). The most frequent mode of entry was sinusitis (22%). Preseptal cellulitis was most common (76.7%). The main clinical signs found in orbital cellulitis were proptosis (64%) and chemosis (35.8%), versus conjunctival hyperemia (78%) in preseptal cellulitis. Ophthalmoplegia was present in two cases of orbital cellulitis. The right side was most affected (44%). An orbital CT scan was performed in all cases in our study, showing preseptal cellulitis in 129 patients (76.7%), orbital cellulitis in 14 cases (8.3%), subperiosteal abscess in 20 cases (12%) and orbital abscess in 5 cases (3%). Prior treatment with non-steroidal anti-inflammatory medication was noted in 6%. The most commonly used antibiotic was amoxicillin-clavulanic acid. Steroid treatment was prescribed in 6% of cases. Surgical treatment was indicated in 12 patients (7.1%). The mean hospital length of stay was 3 days for the preseptal cases and 8 days for the orbital cases. All patients had good outcomes with medical and/or surgical treatment. With follow-up of over one year, no complications were noted. CONCLUSION The majority of our cases had positive outcomes, highlighting the advantage of early diagnosis, adapted antibiotic treatment and multidisciplinary care, rendering surgery rarely necessary.

Serious underlying causes should be always evoked. Case Report. We describe here a 9-year-old girl who was admitted at our emergency department for vaginal bleeding and severe anemia. No history of trauma and no evidence of prior abuse were reported by the parents. Full Blood Count showed profound anemia (hemoglobin at 4 g/dl). The child was managed as a hypovolemic shock and resuscitated with intravenous fluids and urgent blood transfusion. Gynecologic examination found a live leech at the vulva and the extraction was facilitated by applying a saline solution. The child was clinically stable and discharged home the next day with ambulatory treatment. Conclusion. This case emphasizes that a through external genital exam with possible exam under anesthesia should be undertaken in all girls with unexplained vaginal bleeding and that in those living in rural areas without potable water, leech infection...

New ideas and technologies have been proposed to improve the process efficiency of biogas upgrading with liquid absorbents, and ionic liquids have been identified as promising candidates. Meanwhile, there is an urgent need to effectively utilize low-quality waste heat in industry. In this study, two strategies (i.e., waste heat integration and desorption unit modification) were proposed and compared with the original high-pressure water scrubbing; in addition, the effects of the temperatures of the heat source and solvents on the energy usage and cost were further investigated. The results showed that the total cost of the process integrated with waste heat recovery decreased by 6.2% compared with the original high-pressure water scrubbing, and the higher temperature of the waste heat source corresponded to a lower cost. The comparison of different solvents further confirmed that ionic liquid-based solvents were promising, with a potential cost reduction of 24.7%; the properties of the solvent, such as the CO2 absorption capacity, selectivity, and viscosity had significant influences on the total cost, and a new and single index “comparative absorption factor” was proposed to reflect the overall impact of these properties on the cost.

Facial swelling is a common complication of dental management. The occurrence of subcutaneous emphysema, pneumothorax, and pneumomediastinum after dental procedures is rare. We present a case with subcutaneous emphysema of the upper chest, neck, chin, and pneumomediastinum after a tooth extraction and discuss the possible mechanism of subcutaneous emphysema. To prevent these complications during dental procedures, dental hand pieces that have air coolant and turbines that exhaust air in the surgical field should not be used.

It is considered a common cause of childhood morbidity and mortality. We present a unique and unusual case of inhalation of a piece of tissue in a 10-month-old infant admitted to the Pediatric Emergency Department of the Mohamed VI Teaching University Hospital in Marrakech in 2021. The patient was initially seen with acute dyspnea, persistent dry cough, and signs of respiratory distress. The auscultation revealed decreased vesicular murmurs in the right lung field. Chest radiography did not reveal the presence of the foreign body, justifying the use of rigid bronchoscopy for diagnostic and therapeutic purposes, and revealing the presence of a tissue in the bronchial area.
