Objective: To compare the outcome of early laparoscopic cholecystectomy in acute cholecytitis due... more Objective: To compare the outcome of early laparoscopic cholecystectomy in acute cholecytitis due to cholelithiasis as compared to interval cholecystectomy in terms of conversion rate, operative time and complications.Study design: Descriptive studyPlace of study: Department of general surgery, Pakistan Atomic Energy Commission General Hospital, H-11/4 Islamabad.Duration of Study: January 2016 to June 2017.Methodology: Patients were divided into two groups. Group A had acute cholecystitis and were admitted through emergency room or surgical OPD and were operated in the same index admission on earliest possible list thus waiting time for surgery was one to four days approximately. While group B, had all elective cases, without any evidence of acute cholecystitis, mainly booked through OPD. All the patients included in both groups, were studied for initial diagnosis, duration of symptoms, duration of surgery, conversion to open cholecystectomy, per operative and post-operative complications and duration of hospital stay.Results: A total of 360 patients were operated in the hospital for cholelithiasis during the said period. Standard four ports were employed for the surgery. Group A (acute cholecystitis) had total 112 patients while group B (non-acute cholecystitis) had 248 patients. Of these 112 patients in group A, there were 77 males and 35 females. In group B, there were 112 male and 248 female patients in group B. The mean operating time for group A was 64±13 min. whereas for group B the mean duration was 60± 12 min. The average amount of blood loss during surgery for group A was 45 ml ± 33 ml and for group B was 30ml + 20ml.Conclusion: Early laparoscopic Cholecystectomy, for acute cholecystitis is cost effective, has shorter total length of hospital stay and reduces the risk of repeat cholecystitis.

Objective: To compare the frequency of post-operative infectious morbidity in patients undergoing... more Objective: To compare the frequency of post-operative infectious morbidity in patients undergoing emergency cesarean section with and without pre-operative vaginal cleansing. Methodology: A Randomized controlled trial in the department of obstetrics and gynecology, PIMS, Islamabad from 1st September 2014 to 31st January 2015 was conducted including 434 patients. Group B women received only abdominal and vulval scrubbing while Group A received additional vaginal cleansing with pyodine. Patients were followed up for development of infectious morbidity up to three weeks. Results: Mean age of 28.4 ± 4.6 years in group A and 27.6 ± 5.9 years in group B. Fever was seen in 9 (4.1%) women in group A and 16 (7.4%) in group B (p=0.149). Endometritis was noted in 3 (1.4%) women in group A and 19 (8.8%) in group B (p=0.000). Wound Infection occurred in 3 (1.4%) women in group A and 8 (3.7%) in group B (p=0.126). Conclusion: Use of antiseptic for vaginal cleaning before cesarean section decreased postpartum infectious morbidity due to endometritis.

Early surgical intervention can lead to a 100% cure. During this COVID pandemic, cancer surgeries had a setback, but our institution, be it gyanecology, radiology, or pathology, still carried them out via necessary precautions for the betterment of clinical care. Adenomyosis is the presence of ectopic endometrial tissue in the myometrium if it involves the uterus focally, it is known as adenomyoma. Radiological findings of adenomyoma can be mistaken for endometrial carcinoma. We present such a case in which a radiologically diagnosed endometrial carcinoma turned out to be adenomyoma on histopathology.

OBJECTIVE To determine the rates of lower segment Caesarean Section and their indications using m... more OBJECTIVE To determine the rates of lower segment Caesarean Section and their indications using modified Robson criteria. METHODS The retrospective study was conducted at Pakistan Institute of Medical Sciences, Islamabad, Pakistan, and comprised review of records from October 1, 2016, to September 30, 2017,related to all Caesarean Sections. The data was classified according to Robson's Ten Group Classification System. SPSS 21 was used for data analysis. RESULTS Of the 6155 deliveries, 2052(33.3%) were caesarean sections. According to the criterion used, the major contributor to overall Caesarean Section rate was Group 5 which entails previous Caesarian delivery, single, cephalic, >37 weeks. CONCLUSIONS Women with previous scar should be assessed and offered trial of labour after caesarean section, where appropriate.

Study Design: Cross-sectional study. Place and Duration of Study: Department of Obstetrics and Gynaecology, ATH, Abbottabad from 1st June 2020 to 30th November 2020. Methodology: One hundred and thirty pregnant women with previous one scar were included in the study. All participants were managed as per department protocol and frequency of cesarean section was noted. Results: The age range was from 18 to 35 years with mean age of 29.361±1.95 years, mean gestational age 38.592±1.26 weeks, mean parity 1.923±0.91 and mean weight was 70.776±4.20 kg. Cesarean section was seen in 25.4% patients. Conclusion: Prior scar women are at higher risk of caesarean section unless the type of delivery method. Aim: To compare the efficacy of 90% Trichloroacetic acid and 88% phenol in Matrixectomy (extracti... more Aim: To compare the efficacy of 90% Trichloroacetic acid and 88% phenol in Matrixectomy (extraction of partial nail) for the management of ingrown toenails. Methods: This was a single center prospective randomized clinical study, conducted in the General surgery department of Pakistan Institute of Medical Sciences Hospital, Islamabad, from July 2018 to July 2019. Out of all study participants 50% patients treated with 88% Phenol and the remaining 50% treated with 90% Trichloroacetic acid. Postoperatively patients were followed for 48 hours, 1, 4, 12 weekly up to 3 months. Efficacy was defined in term of pain wound bleeding, allergic reaction, wound infection and recurrence. Data was collected on self-designed Proforma. Results: Total 100 patients of ingrown toenails were selected. Average age of patients was 26.31±8.74 years. Study Design: Cross Sectional study. Setting: OPD of MCH, PIMS. Period: July 2019 to December 2019. Material & Methods: A total of 80 married women were enrolled, 40 women who had induced miscarriage (group A) and 40 from spontaneous miscarriage (group B). Data is obtained on predesigned questionnaire. Primary outcome was acceptance of contraception (LARC) after induced versus spontaneous miscarriage. Results: About 51% of women were in the age group of 30–35 years and out of total 80 women, 67 (83%) women had parity of three or above (27 % had parity of three). Out of 66 women who used contraception, 35 (53%) had induced miscarriage (group A) and 31 (46%) had spontaneous miscarriage (group B). Aim: To compare the efficacy of 90% Trichloroacetic acid and 88% phenol in Matrixectomy (extracti... more Aim: To compare the efficacy of 90% Trichloroacetic acid and 88% phenol in Matrixectomy (extraction of partial nail) for the management of ingrown toenails. Methods: This was a single center prospective randomized clinical study, conducted in the General surgery department of Pakistan Institute of Medical Sciences Hospital, Islamabad, from July 2018 to July 2019. Out of all study participants 50% patients treated with 88% Phenol and the remaining 50% treated with 90% Trichloroacetic acid. Postoperatively patients were followed for 48 hours, 1, 4, 12 weekly up to 3 months. Efficacy was defined in term of pain wound bleeding, allergic reaction, wound infection and recurrence. Data was collected on self-designed Proforma. Results: Total 100 patients of ingrown toenails were selected. Average age of patients was 26.31±8.74 years. METHODS The retrospective study was conducted at Pakistan Institute of Medical Sciences, Islamabad, Pakistan, and comprised review of records from October 1, 2016, to September 30, 2017,related to all Caesarean Sections. The data was classified according to Robson's Ten Group Classification System. SPSS 21 was used for data analysis. RESULTS Of the 6155 deliveries, 2052(33.3%) were caesarean sections. According to the criterion used, the major contributor to overall Caesarean Section rate was Group 5 which entails previous Caesarian delivery, single, cephalic, >37 weeks. CONCLUSIONS Women with previous scar should be assessed and offered trial of labour after caesarean section, where appropriate.

Study Design: Cross Sectional study. Setting: OPD of MCH, PIMS. Period: July 2019 to December 2019. Material & Methods: A total of 80 married women were enrolled, 40 women who had induced miscarriage (group A) and 40 from spontaneous miscarriage (group B). Data is obtained on predesigned questionnaire. Primary outcome was acceptance of contraception (LARC) after induced versus spontaneous miscarriage. Results: About 51% of women were in the age group of 30–35 years and out of total 80 women, 67 (83%) women had parity of three or above (27 % had parity of three). Out of 66 women who used contraception, 35 (53%) had induced miscarriage (group A) and 31 (46%) had spontaneous miscarriage (group B). Patients with other chronic or pre-existing diseases like known hyperlipidemias and metabolic syndrome were excluded. All enrolled women underwent detailed medical history and examination. Blood pressure and proteinuria by dipstick method was checked by a staff nurse. Serum triglyceride levels were done by PIMS laboratory. p-value ≤ 0.05 was taken as significant.
