Rectus sheath hematomas are uncommon and are being caused by the injuries of the epigastric vesse... more Rectus sheath hematomas are uncommon and are being caused by the injuries of the epigastric vessels and the rectus muscle. They are usually located infraumblically and often misdiagnosed as inflammatory diseases or tumours of the abdomen. We present a 70-year-old woman who had complaints of severe abdominal pain on the left side, nausea and vomiting after heavy lifting. Rectus sheath hematoma was detected by abdominal ultrasonography and computerized tomography. The patient recovered uneventfully after bed rest, intravenous fluid replacement, blood transfusion and analgesic treatment. Computerized tomography must be chosen for definitive diagnosis. Ideal treatment is conservative.

The causative factors of incisional hernia are related to the condition of the patient, the main disease, surgical techniques and postoperative complications. The subcutaneous herniation of the left hepatic lobe through the abdominal wall is a rare condition. In this case report, we discuss an incarcerated incisional hernia associated with medial segment of the left hepatic lobe.

Laparoscopic cholecystectomy (LC) using an electrosurgery energy source was successfully performe... more Laparoscopic cholecystectomy (LC) using an electrosurgery energy source was successfully performed in 59 (95%) out of 62 selected patients. The procedures were performed by different surgical teams at Trakya University, Medical Fakulty, in the department of General Surgery and the Karl-Franzens-University School of Medicine, in the department of General Surgery. Cholangiography was routine at Karl Franzens University and selective at Trakya University. Laparoscopic intraoperative cholangiography (IOC) was performed in 48 (81.3%) patients, and open IOC was performed in 3 patients. Two patients had common duct stones; one of which was unsuspected preoperatively. These cases underwent endoscopic retrograde cholangiopancreatography (ERCP) and endoscopic papillotomy (EP). One patient had a choledocal tumor, unsuspected preoperatively. Anatomical anomalies were not identified. Cholangiography could not be performed in one case in which there was no suspected pathology. The exact reason for the development of inguinal hernia has not been completely determined. Howev... more The exact reason for the development of inguinal hernia has not been completely determined. However, it is known that the fascia transversalis (FT) is one of the structures preventing development of hernias. In the etiology of the inguinal hernia, disorders in collagen metabolism have been proposed, and the role of metalloproteinases in remodeling the collagen has recently been of great importance. We could not encounter any study where the role of metalloproteinase inhibitors was evaluated in inguinal hernia. We obtained samples of FT from patients with direct and indirect hernia and used an immunohistochemical method to determine tissue inhibitor of metalloproteinase-2 (TIMP-2) expression. In the study group, samples of FT were taken during the operation from 45 patients, of which 35 were indirect and 10 were direct inguinal hernias. In the control group, samples of FT from various abdominal incisions were also taken from 45 patients with no hernia and operated upon for another pathology. TIMP-2 scores of a direct inguinal hernia were significantly less than those of the control group. However, no difference has been found between the TIMP-2 scores of an indirect inguinal hernia and those of the control group. Decreased TIMP-2 scores in patients with a direct inguinal hernia, compared with both the indirect inguinal hernia group and the control group, explain the reason for the increase in matrix metalloproteinase-2 (MMP-2) that has been proposed in some studies. Therefore, it can be expressed that a decreased activity of TIMP-2 plays a role in inguinal hernia development.

Trauma is one of the major cause of death in the young population. The patients treated due to tr... more Trauma is one of the major cause of death in the young population. The patients treated due to trauma were evaluated retrospectively during the last nine years. 212 cases with trauma were hospitalised in our clinic, between 1.7.1990-11.4.1999. 190 (89.6%) of them were operated and 22 (10.4%) of them were observed selectively. 36 (17%) of the cases were female and 176 (83%) of them were male. The mean age was 32.4 (16-81). 93 (43%) of them were hospitalized following traffic accidents; 56 (26.4%) of them stab wounds, 33 (15.6%) of them gunshots and 30 (14.1%) of them blunt trauma. The diagnosis was done through the diagnostic peritoneal lavage 95.1% of the traffic accident cases. Cases of patients with inguinoscrotal hernia containing the urinary bladder, which also termed as... more Cases of patients with inguinoscrotal hernia containing the urinary bladder, which also termed as "scrotal cystocele", are very rare. These patients usually present with frequent episodes of urinary tract infection, difficulty in walking, pollakiuria and difficulty in initiating micturition because of incarceration of the urinary bladder into the scrotum. The condition is often diagnosed incidentally, or during the course of surgical repair of inguinal hernia. We present a case of a large scrotal cystocele with preoperative findings and imaging.

This strong interest has driven the establishment of professional medical societies with the sole purpose of furthering the understanding of hernias and hernia repair. In the more than 100 years of development, industry has played a major role in advancing the technology to perfect the performance of hernia repair with the hope of establishing the "best" technique and its associated technology. However, with the development of newer prosthetics and approaches to hernia repair, many surgeons do not fully understand the properties of the available prosthetics. The goal of this review is to highlight the different types of meshes in an effort to clarify to surgeons what types of materials are available to them and how to select an appropriate one for a given case.

Gallbladder cancers are recognised common tumours of the gastrointestinal system. Histologically,... more Gallbladder cancers are recognised common tumours of the gastrointestinal system. Histologically, almost all of them are adenocarcinomas. Previously, only two cases of signet-ring cell carcinoma of the gallbladder have been published, and little is known about the disease. Here, we present the case of a 76-year-old man with signet cell carcinoma of the gallbladder. Despite radical surgery, followed by 5-fluorouracil infusion chemotherapy, our patient died within 3 months. Advanced information may help to improve the prognosis of this tumour.

In the more than 100 years of development, industry has played a major role in advancing the technology to perfect the performance of hernia repair with the hope of establishing the "best" technique and its associated technology. However, with the development of newer prosthetics and approaches to hernia repair, many surgeons do not fully understand the properties of the available prosthetics. The goal of this review is to highlight the different types of meshes in an effort to clarify to surgeons what types of materials are available to them and how to select an appropriate one for a given case.

Rectus sheath hematomas are uncommon and are being caused by the injuries of the epigastric vesse... more Rectus sheath hematomas are uncommon and are being caused by the injuries of the epigastric vessels and the rectus muscle. They are usually located infraumblically and often misdiagnosed as inflammatory diseases or tumours of the abdomen. We present a 70-year-old woman who had complaints of severe abdominal pain on the left side, nausea and vomiting after heavy lifting. Rectus sheath hematoma was detected by abdominal ultrasonography and computerized tomography. The patient recovered uneventfully after bed rest, intravenous fluid replacement, blood transfusion and analgesic treatment. Computerized tomography must be chosen for definitive diagnosis. Ideal treatment is conservative.

The causative factors of incisional hernia are related to the condition of the patient, the main disease, surgical techniques and postoperative complications. The subcutaneous herniation of the left hepatic lobe through the abdominal wall is a rare condition. In this case report, we discuss an incarcerated incisional hernia associated with medial segment of the left hepatic lobe.

The procedures were performed by different surgical teams at Trakya University, Medical Fakulty, in the department of General Surgery and the Karl-Franzens-University School of Medicine, in the department of General Surgery. Cholangiography was routine at Karl Franzens University and selective at Trakya University. Laparoscopic intraoperative cholangiography (IOC) was performed in 48 (81.3%) patients, and open IOC was performed in 3 patients. Two patients had common duct stones; one of which was unsuspected preoperatively. These cases underwent endoscopic retrograde cholangiopancreatography (ERCP) and endoscopic papillotomy (EP). One patient had a choledocal tumor, unsuspected preoperatively. Anatomical anomalies were not identified. Cholangiography could not be performed in one case in which there was no suspected pathology. Histologically, almost all of them are adenocarcinomas. Previously, only two cases of signet-ring cell carcinoma of the gallbladder have been published, and little is known about the disease. Here, we present the case of a 76-year-old man with signet cell carcinoma of the gallbladder. Despite radical surgery, followed by 5-fluorouracil infusion chemotherapy, our patient died within 3 months. Advanced information may help to improve the prognosis of this tumour.

However, it is known that the fascia transversalis (FT) is one of the structures preventing development of hernias. In the etiology of the inguinal hernia, disorders in collagen metabolism have been proposed, and the role of metalloproteinases in remodeling the collagen has recently been of great importance. We could not encounter any study where the role of metalloproteinase inhibitors was evaluated in inguinal hernia. We obtained samples of FT from patients with direct and indirect hernia and used an immunohistochemical method to determine tissue inhibitor of metalloproteinase-2 (TIMP-2) expression. In the study group, samples of FT were taken during the operation from 45 patients, of which 35 were indirect and 10 were direct inguinal hernias. In the control group, samples of FT from various abdominal incisions were also taken from 45 patients with no hernia and operated upon for another pathology. TIMP-2 scores of a direct inguinal hernia were significantly less than those of the control group. However, no difference has been found between the TIMP-2 scores of an indirect inguinal hernia and those of the control group. Decreased TIMP-2 scores in patients with a direct inguinal hernia, compared with both the indirect inguinal hernia group and the control group, explain the reason for the increase in matrix metalloproteinase-2 (MMP-2) that has been proposed in some studies. Therefore, it can be expressed that a decreased activity of TIMP-2 plays a role in inguinal hernia development.
