Head and neck region is involved in 10% of cases, mostly biphasic in histology. Submandibular gland is an uncommon anatomic location for primary synovial sarcoma. Herein, we present a unique case of primary submandibular gland synovial sarcoma morphologically mimicking a solitary fibrous tumor. Moreover, the neoplastic cells reveal an aberrant p63 nuclear expression. The immunoprofile and the molecular studies confirmed TLE1 expression and SYT (18-11.2) rearrangement, respectively. To the best of our knowledge, the present case is the first with a p63 expression and the second reported case of primary submandibular gland synovial sarcoma in English literature.

Tremendous effort is needed to improve the quality of care to decrease the burden of this health problem
on national hospitals. To be able to establish a new trauma system, we need to have a better insight into
the current level of care and practice. This study aims to shed light on trauma patients presenting to the
largest emergency department (ED) in Makkah, Saudi Arabia, and determine the length of stay (LOS) for
all trauma patients.
Methodology: During the 31‑day study period, 1984 trauma visits in Alnoor Specialist Hospital in Makkah
have been analyzed. The LOS of all the patients presented to ED is calculated and then compared to the
LOS of all nontrauma patients.
Results: The analysis showed a significant increase in LOS of trauma patients. Several factors have been
identified as the major causes of prolonged stay. A priceless insight of the current care provided to trauma
patients is described in this study. Further efforts should be focused on the need of advanced trauma care
to improve patient’s care.

P... more Background/Aim: Colorectal carcinoma (CRC) carries a high incidence of morbidity and mortality. Prognosis is related to nodal metastasis and stage. Clusterin is a widely distributed glycoprotein with not yet fully understood functions. Clusterin may be overexpressed in some tumours or under expressed in other tumours. The aim behind this study is to examine the relation of clusterin cytoplasmic immunostaining to tumour characteristics, disease relapse, and survival in CRC. Materials and Methods: Paraffin blocks of 133 CRCs were retrieved from the Department of Pathology, King Abdulaziz University, Jeddah, Saudi Arabia. Immunostaining was done using antibody to clusterin. Staining expression in 10% of malignant cells was used as a cutoff to determine low immunostaining and high immunostaining. Statistical tests were used to evaluate the association of clusterin immunostaining with clinicopathological parameters. Results: Immunohistochemical results showed clusterin low immunostaining in CRC and nodal metastases. No association was found between clusterin immunostaining and tumour grade, age, tumour invasiveness, distant metastases, vascular invasion, nodal metastases, relapse, and survival. Conclusion: Our study showed low clusterin immunostaining in CRC with lack of association with prognostic indicators in CRC. These results raise the controversy of understanding the role of clusterin in CRC. Further molecular studies are required to explore more about possible mechanisms of clusterin association with tumorigenicity, apoptosis, tumour growth progression, local and vascular invasion, and metastasis of CRC.

Rhabdomyosarcoma is considered the most common soft tissue sarcoma arising in patients younger th... more Rhabdomyosarcoma is considered the most common soft tissue sarcoma arising in patients younger than 15 years old, accounting for 5%-10% of childhood solid tumors. Sarcoma of the kidney represents 1% of all primary renal malignancies. Primary renal rhabdomyosarcoma is a very rare entity with limited number of cases reported in the literature. In this paper we present two cases of primary renal rhabdomyosarcoma in pediatric patients. The two tumors involved the renal parenchyma and occurred in 2-year-old girl and 6-year-old boy, respectively. Histopathology examination and immunohistochemistry studies confirm the diagnosis of embryonal rhabdo-myosarcoma with pleomorphic component, and pleomorphic rhabdomyosarcoma, respectively. Both cases are treated with chemotherapy and show a good response with no evidence of recurrence or metastasis. The aim of this paper is to expand the differential diagnosis of primary mesenchymal kidney tumors in pediatric age group.

Primary BLs are rare ovarian oncology neoplasms. Only few cases are reported in English literature, most of them are presented in pediatric and young adult age groups. Rare cases are reported in adult females. Although BL is a rare primary ovarian neoplasm, it is very important to establish an accurate diagnosis as early as possible for therapeutic purposes.

We presented a case of a urinary bladder paraganglioma in a 26-year-old male who presented with a painless gross hematuria and hypertensive crisis occurring during his trans-urethral resection. Our diagnosis was based upon patient' s history, abdominal-pelvic computed tomography scan, which revealed a left sided bladder wall mass 2.26X 4.4 X 3.28 cm that was obstructing the left distal ureter and causing signifi cant left severe hydroureter and hydronephrosis. Preoperative preparation was done with beta blocker and alpha blocker two weeks prior to surgery. The patient underwent an open surgical partial cystectomy, left nephroureterectomy and, complete external and internal iliac lymphadenectomy. Gross pathological examination of the mass revealed a 3x4.5x4 cm growth originating from the wall of urinary bladder. Microscopic examination revealed nests invaded into muscularis propria of the urinary bladder, with no evidence of distant metastasis. Immunohistochemical studies showed positive tumor cells staining for chromogranin and synaptophysin while S100 highlighted the surrounding sustentacular cells. During the fi rst 3-month post-operative follow-up period, the patient' s blood pressure was stabilized and the symptoms gradually subsided. Urinary bladder paragangliomas should be considered among the diff erential diagnosis of bladder tumors..

Mastalgia (defined as self-reported breast pain, unilateral or bilateral for a period of not less... more Mastalgia (defined as self-reported breast pain, unilateral or bilateral for a period of not less than two months) is one of the symptoms that might affect young ladies who seek medical advice. Method and Results: Pretested questionnaire was used to collect data. 418 ladies aged (18-45 years) were included in the study, 340 had unilateral or bilateral breast pain for a period of not less than two months, either localized (54%) or generalized. 19.7% had the pain for more than 10 times per year. (77%) of those ladies were used to drink coffee once daily, (13%) of them were using contraceptive pills, and 49 patients were smokers and all of them had breast pain. (60.8%) were obese with significant P value (0.003). Conclusion: Mastalgia is common among the Saudi population. Providing a good healthcare and preventing the risk factors will reduce the number of ladies visiting the outpatient department (OPD). The management should start with lifestyle changes, education and reassurance.

Gastrointestinal stromal tumors (GISTs) are the most common mesenchymal tumors of the digestive s... more Gastrointestinal stromal tumors (GISTs) are the most common mesenchymal tumors of the digestive system, although they account for only 0.1-3% of all gastrointestinal (GI) malignancies. They can arise anywhere along the GI tract with gastric predominance. Concurrent occurrence of GIST and gastroesophageal junction (GEJ) neoplasm is rare. We report a 55-year-old gentleman presenting with a polyp at the GEJ and a synchronous, large, and pedunculated gastric mass at the greater curvature. Those were treated with a wedge resection of the gastric pedunculated mass with negative margins along with transgastric submucosal resection of the GEJ polyp. Pathological examination confirmed synchronous invasive GEJ adenocarcinoma and a high-grade gastric GIST.

we report an interesting case of a benign epithelioid schwannoma found in one thigh of an otherwise healthy 30-
year-old man. This tumor was benign, as is indicated by its long pre-surgery clinical history, encapsulation, bland
cellular morphology, absence of necrosis or atypical mitotic figures, and recurrence-free survival. This entity
should be kept in the differential diagnosis to avoid misdiagnosing it as a malignant soft tissue tumor.

Hemangiomas of the central nervous system are very rare lesions. There are only a few cases of intracranial capillary hemangioma (ICH) arising in adults reported in the literature. We present a case of 59-year-old female with intermittent recurrent headache localized in the frontal area. Magnetic resonance imaging revealed left frontal extra-axial mass with peripheral enhancement. The patient underwent complete surgical resection of the tumor. Histopathology examination of the lesion revealed well defined vascular lesion composed of closely packed plump endothelial cells lining slit-like vascular channels containing scattered red blood cells. No evidence of infiltrative brain parenchyma was seen. Ki-67 proliferative index was low, less than 2%. The final diagnosis was confirmed to be ICH by histopathology and immunohistochemistry studies. The patient has remained healthy and free of disease 39 months since her initial surgery. ICH is a benign vascular lesion which rarely occurs in the central nervous system, particularly in the intracranial region. It can mimic malignant lesions on radiologic studies. Histopathology examination is the gold standard for diagnosis. If total resection is achieved, prognosis is generally good with no evidence of recurrence.

The stomach is the most common site (60%) followed by the small bowel (30%-35%) particularly jejunum and ileum, colorectum (5%) and rarely affect esophagus and appendix. Most gastrointestinal stromal tumors arise sporadically, however, less commonly they develop in association with various clinical syndromes like Carney triad, Carney-Stratakis syndrome, familial gastrointestinal stromal tumor syndrome and neurofibromatosis type1 (NF1). We report a 65-year-old male patient presented with gastric mass (7.5 × 4.5 × 3.5 cm) arising from the posterior gastric wall. Glomus tumors of the kidney are rare tumors and only a few cases have been reported in the medical literature. An extensive search revealed a very limited number of primary renal glomus tumors. Although most of these cases were benign in nature, including a case with uncertain diagnosis of malignant potential, two were malignant. We present a unique case of a 57-year-old male patient with an incidentally discovered 2 cm left renal mass. Histopathology examination and immunohistochemical studies confirm the diagnosis of glomangioma (a form of glomus tumor). The patient was followed for one year after partial nephrectomy and showed a benign course without any evidence of local recurrence or metastasis. Emergency Department (ED) in Alnoor Hospital is considered the pulsating unit in the hospital by ... more Emergency Department (ED) in Alnoor Hospital is considered the pulsating unit in the hospital by facing a daily challenge through a huge exposure to number of patients round between 500 -700 per day in average. With this busy service in ED, our study emerges to measure the quality of provided services to patients in term of measuring the total length of stay time (LOS) in ED and its influencing factors. This is a prospective study aiming to estimate the average time patients spend in ED of Alnoor Hospital during the month of January (2013). In addition, it inspects factors influencing the LOS. The questionnaire which conducted and filled by emergency team over all patients was consisted of the following data: arrival time to ED, initial time of assessment by nurse, initial time of assessment by doctor, time of arrival to specific area, consultation time, arrival time of consulted specialty, time of laboratory investigation, time of radiological investigation, time of final disposition and time of physical disposition. For the 7604 patient visits analyzed, mean ED LOS was 3.02 hour (SD = 5.03 hour). About half of the patients spent less than 59 minutes (44%), 32.6% spent 1 to 3:59 hour, 15.2% spent 4 to 7:59 hour, and 8.2% of the patients spent more than 8 hours. A priceless such study will offer an opportunity to evaluate the recent ED performance and assist to adapt future optimization strategies to improve the quality of services provided to the patient.

The stomach is the most common site (60%) followed by the small bowel (30%-35%) particularly jejunum and ileum, colorectum (5%) and rarely affect esophagus and appendix. Most gastrointestinal stromal tumors arise sporadically, however, less commonly they develop in association with various clinical syndromes like Carney triad, Carney-Stratakis syndrome, familial gastrointestinal stromal tumor syndrome and neurofibromatosis type1 (NF1). We report a 65-year-old male patient presented with gastric mass (7.5 × 4.5 × 3.5 cm) arising from the posterior gastric wall. Glomus tumors of the kidney are rare tumors and only a few cases have been reported in the medical literature. An extensive search revealed a very limited number of primary renal glomus tumors. Although most of these cases were benign in nature, including a case with uncertain diagnosis of malignant potential, two were malignant. We present a unique case of a 57-year-old male patient with an incidentally discovered 2 cm left renal mass. Histopathology examination and immunohistochemical studies confirm the diagnosis of glomangioma (a form of glomus tumor). The patient was followed for one year after partial nephrectomy and showed a benign course without any evidence of local recurrence or metastasis. Emergency Department (ED) in Alnoor Hospital is considered the pulsating unit in the hospital by ... more Emergency Department (ED) in Alnoor Hospital is considered the pulsating unit in the hospital by facing a daily challenge through a huge exposure to number of patients round between 500 -700 per day in average. With this busy service in ED, our study emerges to measure the quality of provided services to patients in term of measuring the total length of stay time (LOS) in ED and its influencing factors. This is a prospective study aiming to estimate the average time patients spend in ED of Alnoor Hospital during the month of January (2013). In addition, it inspects factors influencing the LOS. The questionnaire which conducted and filled by emergency team over all patients was consisted of the following data: arrival time to ED, initial time of assessment by nurse, initial time of assessment by doctor, time of arrival to specific area, consultation time, arrival time of consulted specialty, time of laboratory investigation, time of radiological investigation, time of final disposition and time of physical disposition. For the 7604 patient visits analyzed, mean ED LOS was 3.02 hour (SD = 5.03 hour). About half of the patients spent less than 59 minutes (44%), 32.6% spent 1 to 3:59 hour, 15.2% spent 4 to 7:59 hour, and 8.2% of the patients spent more than 8 hours. A priceless such study will offer an opportunity to evaluate the recent ED performance and assist to adapt future optimization strategies to improve the quality of services provided to the patient.
