We analyzed the loss of heterozygosity (LOH) and microsatellite instability (MSI) of hMSH2, hMLH1, and p53 genes in 55 patients with intrahepatic cholangiocarcinoma by using polymerase chain reaction based microsatellite markers D2S119, D3S1611, and TP53, respectively and determined the association between microsatellite alterations and patient survival. A total of 27 (49.1%) out of 55 cases exhibited microsatellite alterations in one locus or more. Of 55 samples, 11 (20%) demonstrated MSI at D2S119 and four (7%) showed MSI at D3S1611. LOH was shown in seven out of 36 (19%) informative cases for D3S1611 and 16 out of 50 (32%) for TP53. Microsatellite alterations at loci studied were significantly associated with poor survival (P ¼ 0:0098). This study suggests that genetic alterations of DNA mismatch repair genes and tumor suppressor gene p53 may be involved in cholangiocarcinogenesis and these alterations may be of value as prognostic indicators for liver fluke related cholangiocarcinoma.

The genetic mechanisms involved in the development of CCA are not well understood and only a few cytogenetic studies have been published. In this study, genomic instability in 30 Thai cases of intrahepatic cholangiocarcinoma (ICC) was assessed using an arbitrarily primed- polymerase chain reaction (AP-PCR) method. Genetic alterations were analyzed as banding pattern changes between tumors and corresponding normal DNA. The abnormal band present at the highest frequency (23/30 cases, 77%) appeared with the AO16 primer. Statistical analysis also showed that DNA alteration from this primer was significantly associated with the moderately to poorly differentiated histological type (P=0.038). Kaplan-Meier survival curves showed borderline significance for this DNA aberration (P=0.06 by the log-rank test). This DNA ...

To our knowledge, no direct evidence concerning fibroblasts in the genesis of cholangiocarcinoma (CCA) has previously been presented. This study aims to assess the value of activated fibroblasts with high alpha-smooth muscle actin (alpha-SMA) expression as an indicator for survival in CCA patients. The immunohistochemistry results indicated a high expression of alpha-SMA in CCA fibroblasts which had a statistically significant correlation with larger tumor size (P=0.009) and shorter survival time (P=0.013). The effect of CCA-associated fibroblasts (Cfs) on non-tumorigenic biliary epithelial cells (H-69) and CCA cell lines was investigated in vitro and compared to the effect of non-tumorigenic liver fibroblasts (Lfs). The increased proliferation effect of Cfs having high alpha-SMA on H-69 and 4 CCA cell lines compared to Lfs that expressed low alpha-SMA was observed. Cel...

The prevalence of CCA is markedly variable with the highest incidence in the northeast Thailand, followed by other parts of Southeast Asia and China. Currently, there is still no reliable biomarker for diagnosis or treatment. NADPH-quinone oxidoreductase 1 (NQO1) is a xenobiotic metabolizing enzyme detoxifying chemical stressors and antioxidants, thereby providing cytoprotection in normal tissues. However, NQO1 is over-expressed in some cancers, suggesting roles in carcinogenesis and tumor progression. In this study, we examined NQO1 activity in surgical specimens from CCA patients and found much higher values than in the adjacent normal tissues. Immunohistochemical analysis revealed strong staining in tumor epithelial elements, whereas the non-tumor bile ducts and liver parenchyma were weakly stained. NQO1 mRNA expression in tumor tissues was widely varied among 43 patients. A significant associatio...

Diagnosis of this cancer is practically based on clinical and utrasonography. However, several markers can be used in conjunction with the standard procedure to increase diagnosis accuracy including biliary cytology as described in carcinoma of other pancreatobiliary diseases. On the basis of cancer cells can be exfoliated into the bile and they can be detected by routine Papanicolaou staining. In endemic areas of cholangiocarcinoma and opisthorchiasis, there is no report about biliary cytology and percentage of positive malignant cells presented in the bile, here we examined biliary cytology of the gallbladder and/or hepatic bile to clarify its usefulness potential for using in diagnosis. Objective : To study biliary cytology and examine Opist...

We aimed to further evaluate CpG-island hypermethylation in CCA at candidate loci, which may have potential as diagnostic or prognostic biomarkers. We analysed methylation of 26 CpG-islands in 102 liver fluke related-CCA and 29 adjacent normal samples using methylation-specific PCR (MSP). Methylation of interest loci was confirmed using pyrosequencing and/or combined bisulfite restriction analysis, and protein expression by immunohistochemistry. A number of CpG-islands (OPCML, SFRP1, HIC1, PTEN and DcR1) showed frequency of hypermethylation in >28% of CCA, but not adjacent normal tissues. The results showed that 91% of CCA were methylated in at least one CpG-island. The OPCML was the most frequently methylated locus (72.5%) and was more frequently methylated in less differentiated CCA. Patients with methylated DcR1 had significantly longer ove...

Srinagarind Hospital performed this procedure for a long time but it has never been studied about its results. We aim to evaluate the results. Methods: This descriptive study collected data of patients who were diagnosed as hypopharyngeal cancer and underwent total laryngectomy which the pharyngcal defect couldn’t be repaired by primary suture. We included patients from January 1 st , 1997 to October 31 st , 2009. The main data consisted of incidence of postoperative pharyngeal leakage, date of start oral feeding and length of hospital stay. Results: There were 26 cases included. Twenty three patients were reconstructed by gastric pull-up and 4 cases (17.4%) had postoperative pharyngeal leakage. The other 3 cases were reconstructed by pectoralis major myocutaneous flap (PMMF) and 2 cases (66.6%) had postoperative pharyngeal leakage. ...

A 12-monthreport from our blood bank demonstrated an inappropriate use of preoperative blood order with the crossmatch to transfusion (C/T) ratio of 2.08.Objective : The aim of this study was to assess the cost effectiveness of routine preoperative blood order for elective laparoscopic cholecystectomy. The results will be evidence to implement to ourhospital blood order guideline. Methods: All elective laparoscopic cholecystectomy cases performed in our institution between January and December 2011 were retrospectively reviewed. With the use of electronic databases of our hospital, open cholecystectomy and cholecystectomy secondary to other procedure were excluded. Preoperative blood order and blood transfusion intraoperative andwithin 24 hours postoperatively were identified from medical and anesthetic records. Crossmatch to transfusion (C/T) ratio and blood usage parameters (transfusion probability; %T...

MATERIAL AND METHOD The SF-36 questionnaires were sent by mail to 93 living donors who underwent nephrectomy between Jan 1, 1990 and Dec 31, 2008. The first part collected demographic data and the donor/recipient relationship, the second surveyed QoL, and the third asked about decision-making, donation-related stress and feedback. RESULTS Forty-nine questionnaires were returned completed (30 women; 19 men: mean age 44.2 +/- 9.5 (range, 28-65) years). Thirty-one participants (61%) were siblings of the recipients. The QoL scores were not significantly different from the general Thai population; albeit nominally higher for mental health and social function. Upon reflection, only 5 (10%) had second thoughts. CONCLUSION Donor nephrectomy did not affect QoL; thus,...

Simultaneously there has been an apparent change in the presentation of the disease and indication for surgery. The aim of this study was to examine the clinical presentation and pathology of parathyroid glands for primary hyperparathyroidism. Methods : This retrospective descriptive study was carried out over 14 years, from 1994 to 2008, at Srinagarind Hospital, Faculty of Medicine, KhonKaen University. Information on the clinical presentation, parathyroid hormone level, serum calcium level and pathological report of parathyroid glands was obtained from retrospective parathyroid surgical database. Results : Out of 40 patients diagnosed with primary hyperparathyroidism at Srinagarind Hospital, there were 27 female, mean age 47.22(13 – 73) years and 13 male patients. Bone fracture was the most common clinical presentation found in 19 patients, the second was renal ca...
