Purpose The aim of the current study was to evaluate point of care ultrasound (POCUS) in geriatri... more Purpose The aim of the current study was to evaluate point of care ultrasound (POCUS) in geriatric patients by echoscopy using a handheld ultrasound device (HHUSD, VScan) at bedside in comparison to a high-end ultrasound system (HEUS) as the gold standard. Materials and Methods Prospective observational study with a total of 112 geriatric patients. The ultrasound examinations were independently performed by two experienced blinded examiners with a portable handheld device and a high-end ultrasound device. The findings were compared with respect to diagnostic findings and therapeutic implications. Results The main indications for the ultrasound examinations were dyspnea (44.6 %), fall (frailty) (24.1 %) and fever (21.4 %). The most frequently found diagnoses were cystic lesions 32.1 % (35/109), hepatic vein congestion 19.3 % (21/109) and ascites 13.6 % (15/110). While interns play a crucial role in running emergency services in Thailand, the POCUS usage of interns has not been well studied. METHODS: This was a retrospective observational study of the interns who worked at the Department of Emergency Medicine, a tertiary university hospital in Khon Kaen, Thailand between July 2020 and April 2021. The seven questions survey about participants’ experiences performing POCUS examinations was conceived and carried out by the research team. The primary aim of this study is to identify the frequency of POCUS examinations performed by interns in this or any other given rotation. Point of Care Ultrasound (POCUS) training has been offering invaluable guidance in the diagnostic... more Point of Care Ultrasound (POCUS) training has been offering invaluable guidance in the diagnostic process and treatment strategies, particularly in resource-limited settings. The purpose of this study was to determine participant satisfaction with the structure and scope of POCUS training in overseas elective rotation. Material and Methods: A retrospective observational analytical study was undertaken. The study focused on EM residents from Laos who participated in the international elective rotation at the Khon Kaen University during the academic years 2020-2021 and 2022-2023. The research involved an exploration of training data related to this course, encompassing participant characteristics, the structure and scope of POCUS training, the training assessments, and post completion surveys of participants. Results: The study involved 24 participants. The course was characterized by its status as an international elective program prehospital POCUS training for post graduate year 3 students. The duration of the rotation ranged from 2 to 4 weeks. Interestingly, in the academic year 2022-2023, the duration of POCUS training was notably extended to 8 hours, accounting for 4.76% of the total time. The training encompassed peer-to-peer didactic lectures, bedside teaching, and observational case studies at the Emergency Department (ED). The results of our survey, boasting a 100% response rate, indicated participants' contentment with the academic performance, the abundance of learning resources, and the knowledge gained from this rotation that would aid their future careers. The overall satisfaction with this course was highly rated, falling into the "satisfy" and "highly satisfy" categories. However, certain challenges were reported, including feelings of isolation due to the distance from relatives, concerns related to interpersonal relationships with colleagues, and a sense of being isolated. Conclusion: The participants were highly satisfied to the structure and scope of this course. The overseas elective rotation significantly enhanced the knowledge of Laos EM residents' in POCUS and underscored the crucial importance of this course in EM training.

To compare the rate and extent of absorption of two sildenafil tablet formulations (Tonafil®, T.O... more To compare the rate and extent of absorption of two sildenafil tablet formulations (Tonafil®, T.O. Chemicals (1979) Ltd., Thailand as a test formulation and Viagra®, Pfizer Pty Limited., Australia as a reference formulation) in healthy Thai male volunteers after single-dose administration under fasting condition. A randomized crossover study with a washout period of 2 weeks was conducted in 20 healthy Thai male volunteers. The volunteers received either 100 mg of the reference or test formulation. Blood samples were collected at 0, 0.16, 0.33, 0.67, 1, 1.5, 2, 2.5, 4, 6, 9 and 12 h after drug administration. The plasma sildenafil concentrations were determined using a validated high performance liquid chromatography method. The pharmacokinetic parameters of sildenafil were calculated from the observed plasma concentration-time profiles by using a non-compartmental model. The geometric means, C(max), of the reference and the test formulations were 696.42 and 734.06 ng/ml. The mean values of the AUC(0-t) and AUC(0-inf) of the test formulation were 2,073.03 and 2,237.37 ng × h/ml, while those of the reference formulation were 1,950.26 and 2,078.06 ng × h/ml. The geometric mean ratios (%) and 90% confidence intervals (CI) of the test and reference products for the log transformed C(max), AUC(0-t) and AUC(0-inf) of sildenafil were 105.40% (95.0 - 116.95%), 106.30% (99.74 - 113.28%) and 107.67% (100.83 - 116.48%). These values were within 80 to 125% of the US-FDA and the Thai-FDA criteria and therefore it can be concluded that the test formulation was bioequivalent to the reference formulation both in terms of rate and extent of absorption after single-dose administration under fasting condition.

<jats:p>Background: Hypertriglyceridemia may contribute to hardening or thickening of the a... more <jats:p>Background: Hypertriglyceridemia may contribute to hardening or thickening of the artery walls which increases the risk of stroke and heart disease. Therefore, the normal level of serum triglyceride can prevent both cardiovascular and cerebrovascular diseases. Gamma-oryzanol, a phytosterol compound presents in rice bran oil, has been shown to exhibit lipid-lowering effects in both animals and human. Objective: To evaluate the effect of gamma-oryzanol-enriched-rice bran oil from Thai brown rice, prepared by innovation technology, on plasma triglyceride (TG) level in patients with hypertriglyceridemia. Materials and Methods: Double-blind randomized clinical trial was carried out in 54 subjects with hypertriglyceridemia. They were divided into two groups (27 patients per group) and treated with RBOh (gamma-oryzanol-enriched rice bran oil, 20,000 ppm of gamma-oryzanol), and RBOn (cooking oil, gamma-oryzanol 5,000 ppm). The treatments were performed for 8 weeks. Physical examinations and laboratory tests were performed at weeks 0 (as baseline), 4th and 8th of the study. Lifestyle modifications were conducted in both groups. Results: Significant reduction of plasma TG levels at the 4th and 8th weeks of the treatment were found in RBOh-treated group with p-value = 0.024 and 0.006, respectively. In RBOn-treated group, significant reduction of plasma TG and increase in HDL levels and was observed at the 4th week, but not the 8th week of the treatment, with p-value = 0.001 and 0.020, respectively. TG/HDL cholesterol ratio was significantly reduce at the 4th and 8th week in RBOh-treated group (p-value = 0.011 and 0.048, respectively) and at the 4th week in RBOn-treated group (p-value = 0.003). No serious side effects were found throughout the study period in any group of treatment. Conclusion: Rice bran oil containing gamma oryzanol could reduce the TG level in hypertriglyceridemic patients. The RBOh, gamma oryzanol-enriched rice bran oil, showed prominent effects in reducing serum TG level and TG/HDL ratio. Therefore, RBOh intakes might be able to help prevent the occurrence of cerebrovascular and cardiovascular disease in the patients with hypertriglyceridemia. To compare the bioavailability of two meloxicam tablet formulations (MEL-OD, Zydus Cadila Healthc... more To compare the bioavailability of two meloxicam tablet formulations (MEL-OD, Zydus Cadila Healthcare Limited, India, as a test formulation and Mobic, Boehringer Ingelheim International GmbH, Germany, as a reference formulation) in healthy Thai male volunteers under fasting condition. A randomized, 2-treatment, 2-period, 2-sequence, single dose, crossover with a washout period of 2 weeks, was conducted in 26 healthy Thai male volunteers. Blood samples were collected 0, 1, 2, 3, 3.5, 4, 4.5, 5, 5.5, 6, 7, 8, 10, 12, 24, 36, 48, 72 and 96 h post dose. Plasma concentrations of meloxicam were determined using a validated HPLC method. The pharmacokinetic parameters of meloxicam were determined using a non-compartmental model. The mean Cmax was 1,027.32 +/- 251.91 and 1,151.89 +/- 282.58 ng/ml while the mean AUC0-t was 34,024.31 +/- 11,811.68 and 35,137.66 +/- 11,970.47 ng x h/ml for the test and reference formulation, respectively. In addition, the mean AUC0-infinity for test formulation was 37,241.44 +/- 14,888.85 ng x h/ml and for the reference formulation was 39,541.04 +/- 16,624.64 ng x h/ml. The median tmax for the test and reference formulation was 4.50 (range 2.00 - 12.00) and 4.50 (range 3.00 - 10.00), respectively. The geometric means (90% confidence intervals) of the ratio for the log-transformed pharmacokinetic parameters, Cmax, AUC0-t and AUC0-inf were 0.8919 (82.58 - 96.32%), 0.9697 (89.46 - 105.10%) and 0.9525 (87.68 - 103.47%), respectively. It can be concluded that two meloxicam tablet formulations are bioequivalent both in term of rate and extent of absorption after single-dose administration under fasting condition.

<jats:p>Background: Airway management is an important aspect of care in emergency medical s... more <jats:p>Background: Airway management is an important aspect of care in emergency medical services (EMS) used to prevent obstruction of the respiratory track and inadequate oxygenation. The laryngeal mask airway (LMA) is an easy-to-use supraglottic airway device developed in 1988 that has a high success rate in both adults and children. However, there have yet been no studies regarding LMA use in EMS in Thailand. Objective: To determine the success rate of LMA in EMS and factors associated with successful LMA insertion. Materials and Methods: This was a cross-sectional study consisting of Srinagarind Hospital EMS patients over 18 years of age who underwent LMA airway management between March 2016 and March 2020. Data were collected from the Srinagarind Hospital EMS operation database and hospital information database system. Results: A total of 154 patients were enrolled with a mean age of 43.1+8.5 years, 89 (57.8%) of whom were male. Non trauma patients accounted for 72.7% of all cases. The average oxygen saturation before and after LMA use were 71.4+12.3% and 94.4+3.2%, respectively (p = 0.022). Students in their first and third year of residency training were able to successfully perform LMA airway management on the first attempt in 83.3% and 98.4% of cases, respectively (p = 0.025). Conclusion: Airway management using the LMA had a high success rate on the first attempt, required less time for insertion than other comparable devices, and resulted in a high level of oxygen saturation. Bhd., Malaysia, as a test product and Risperdal 1 mg Quicklet, Janssen Ortho LLC, Gurabo, Puerto Rico, as a reference product, in healthy male volunteers under fasting condition. A randomized, 2-treatment, 2-period, 2-sequence, single dose, crossover with a washout period of 2 weeks, was conducted in 24 healthy Thai male volunteers. Blood samples were collected at 0, 0.25, 0.5, 0.75, 1, 1.25, 1.5, 1.75, 2, 2.5, 3, 3.5, 4, 5, 6, 8, 10, 12, 24, 36, 48, 72 and 96 h following drug administration. Plasma concentrations of risperidone and 9-hydroxyrisperidone were determined using a validated LC-MS-MS method. The pharmacokinetic parameters of risperidone and 9-hydroxyrisperidone were determined using a non-compartmental model. The geometric means ratios (%) and 90% confidence interval (CI) of the test and reference products for the log-transformed pharmacokinetic parameters, Cmax, AUC0-t and AUC0-inf of risperidone were 104.49 % (92.79% - 117.66%), 100.96 % (92.15% - 110.61 %) and 97.99 % (90.72% - 105.85%). The 90% CI of geometric means ratios of the test and reference products for the log-transformed pharmacokinetic parameters, Cmax, AUC0-t and AUC0-inf of 9-hydroxyrisperidone were 97.00%, 96.97% and 97.49%. The 90% CI for the geometric means ratios (test/reference) of the log-trasformed Cmax, AUC0-t and AUC0-inf of risperidone and its major active metabolite were within the bioequivalence acceptance criteria of 80% - 125% of the US-FDA.

The starting point was essentially based upon clinical and/or haematological indications and it is emphasized that conclusions should be drawn in close dialogue with the clinician and experts in cytogenetics and histopathology. Reports made in the laboratory should be based upon an integration of cytomorphological, immunophenotypical and molecular data. These guidelines are not intended to be used as universal 'diagnostic pathways', but should be useful in developing local diagnostic pathways. It is well understood that this consensus, being valid anno 2009, may rapidly change with new technologies being introduced and new targets discovered.

A randomized, 2-treatment, 2-period, 2-sequence, single-dose, crossover study with a washout period of 3 weeks, was conducted in 24 healthy Thai male volunteers. Blood samples were collected at 0, 1, 2, 3, 4, 5, 6, 7, 8, 10, 12, 24, 36, 48, 72, 96 and 120 hours following drug administration. Plasma concentrations of sertraline were determined using validated LC-MS/MS method. Noncompartmental pharmacokinetics and statistical analyses were performed using SAS software for Windows, release 9.1 (SAS Institute Inc., Cary, NC, USA). The ratio of least square means and the 90% confidence intervals (CI) of the log-transformed data were 0.9950 (0.9111-1.0866) for Cmax, 1.0153 (0.9576-1.0764) for AUC(0-t) and 1.0110 (0.9510-1.0747) for AUC(0-infinity). In addition, the median tmax values for the test and reference formulations were similar (5.00 h). The 90% CI for Cmax, AUC(0-t) and AUC(0-infinity) were within the 0.8-1.25 interval of the US-FDA. However, POCUS examination in elderly populations has not been well studied. AIM: This was a retrospective observational study of elderly patients at the Department of Emergency Medicine who had received POCUS examination at a tertiary university hospital, Thailand. METHODS: The study was conducted throughout January 2020–December 2020. Patients’ characteristics and ultrasonography findings were recorded. RESULTS: A total of 191 elderly patients were evaluated. Median patient age was 75.65 years; 56.02% of patients were female. Chief complaints where POCUS scans were applied were respiratory (36.65%) and cardiovascular system-related (21.99%). Therefore, the normal level of serum triglyceride can prevent both cardiovascular and cerebrovascular diseases. Gamma-oryzanol, a phytosterol compound presents in rice bran oil, has been shown to exhibit lipid-lowering effects in both animals and human. Objective: To evaluate the effect of gamma-oryzanol-enriched-rice bran oil from Thai brown rice, prepared by innovation technology, on plasma triglyceride (TG) level in patients with hypertriglyceridemia. Materials and Methods: Double-blind randomized clinical trial was carried out in 54 subjects with hypertriglyceridemia. They were divided into two groups (27 patients per group) and treated with RBOh (gamma-oryzanol-enriched rice bran oil, 20,000 ppm of gamma-oryzanol), and RBOn (cooking oil, gamma-oryzanol 5,000 ppm). The treatments were performed for 8 weeks. Physical examinations and laboratory tests were performed at weeks 0 (as baseline), 4th and 8th of the study. Lifestyle modifications were conducted in both groups. Results: Significant reduction of plasma TG levels at the 4th and 8th weeks of the treatment were found in RBOh-treated group with p-value = 0.024 and 0.006, respectively. In RBOn-treated group, significant reduction of plasma TG and increase in HDL levels and was observed at the 4th week, but not the 8th week of the treatment, with p-value = 0.001 and 0.020, respectively. TG/HDL cholesterol ratio was significantly reduce at the 4th and 8th week in RBOh-treated group (p-value = 0.011 and 0.048, respectively) and at the 4th week in RBOn-treated group (p-value = 0.003). No serious side effects were found throughout the study period in any group of treatment. Conclusion: Rice bran oil containing gamma oryzanol could reduce the TG level in hypertriglyceridemic patients. The RBOh, gamma oryzanol-enriched rice bran oil, showed prominent effects in reducing serum TG level and TG/HDL ratio. Therefore, RBOh intakes might be able to help prevent the occurrence of cerebrovascular and cardiovascular disease in the patients with hypertriglyceridemia. To compare the bioavailability of two meloxicam tablet formulations (MEL-OD, Zydus Cadila Healthc... more To compare the bioavailability of two meloxicam tablet formulations (MEL-OD, Zydus Cadila Healthcare Limited, India, as a test formulation and Mobic, Boehringer Ingelheim International GmbH, Germany, as a reference formulation) in healthy Thai male volunteers under fasting condition. A randomized, 2-treatment, 2-period, 2-sequence, single dose, crossover with a washout period of 2 weeks, was conducted in 26 healthy Thai male volunteers. Blood samples were collected 0, 1, 2, 3, 3.5, 4, 4.5, 5, 5.5, 6, 7, 8, 10, 12, 24, 36, 48, 72 and 96 h post dose. Plasma concentrations of meloxicam were determined using a validated HPLC method. The pharmacokinetic parameters of meloxicam were determined using a non-compartmental model. The mean Cmax was 1,027.32 +/- 251.91 and 1,151.89 +/- 282.58 ng/ml while the mean AUC0-t was 34,024.31 +/- 11,811.68 and 35,137.66 +/- 11,970.47 ng x h/ml for the test and reference formulation, respectively. In addition, the mean AUC0-infinity for test formulation was 37,241.44 +/- 14,888.85 ng x h/ml and for the reference formulation was 39,541.04 +/- 16,624.64 ng x h/ml. The median tmax for the test and reference formulation was 4.50 (range 2.00 - 12.00) and 4.50 (range 3.00 - 10.00), respectively. The geometric means (90% confidence intervals) of the ratio for the log-transformed pharmacokinetic parameters, Cmax, AUC0-t and AUC0-inf were 0.8919 (82.58 - 96.32%), 0.9697 (89.46 - 105.10%) and 0.9525 (87.68 - 103.47%), respectively. It can be concluded that two meloxicam tablet formulations are bioequivalent both in term of rate and extent of absorption after single-dose administration under fasting condition.

<jats:p>Background: Airway management is an important aspect of care in emergency medical s... more <jats:p>Background: Airway management is an important aspect of care in emergency medical services (EMS) used to prevent obstruction of the respiratory track and inadequate oxygenation. The laryngeal mask airway (LMA) is an easy-to-use supraglottic airway device developed in 1988 that has a high success rate in both adults and children. However, there have yet been no studies regarding LMA use in EMS in Thailand. Objective: To determine the success rate of LMA in EMS and factors associated with successful LMA insertion. Materials and Methods: This was a cross-sectional study consisting of Srinagarind Hospital EMS patients over 18 years of age who underwent LMA airway management between March 2016 and March 2020. Data were collected from the Srinagarind Hospital EMS operation database and hospital information database system. Results: A total of 154 patients were enrolled with a mean age of 43.1+8.5 years, 89 (57.8%) of whom were male. Non trauma patients accounted for 72.7% of all cases. The average oxygen saturation before and after LMA use were 71.4+12.3% and 94.4+3.2%, respectively (p = 0.022). Students in their first and third year of residency training were able to successfully perform LMA airway management on the first attempt in 83.3% and 98.4% of cases, respectively (p = 0.025). Conclusion: Airway management using the LMA had a high success rate on the first attempt, required less time for insertion than other comparable devices, and resulted in a high level of oxygen saturation. A randomized, 2-treatment, 2-period, 2-sequence, single dose, crossover with a washout period of 2 weeks, was conducted in 24 healthy Thai male volunteers. Blood samples were collected at 0, 0.25, 0.5, 0.75, 1, 1.25, 1.5, 1.75, 2, 2.5, 3, 3.5, 4, 5, 6, 8, 10, 12, 24, 36, 48, 72 and 96 h following drug administration. Plasma concentrations of risperidone and 9-hydroxyrisperidone were determined using a validated LC-MS-MS method. The pharmacokinetic parameters of risperidone and 9-hydroxyrisperidone were determined using a non-compartmental model. The geometric means ratios (%) and 90% confidence interval (CI) of the test and reference products for the log-transformed pharmacokinetic parameters, Cmax, AUC0-t and AUC0-inf of risperidone were 104.49 % (92.79% - 117.66%), 100.96 % (92.15% - 110.61 %) and 97.99 % (90.72% - 105.85%). The 90% CI of geometric means ratios of the test and reference products for the log-transformed pharmacokinetic parameters, Cmax, AUC0-t and AUC0-inf of 9-hydroxyrisperidone were 97.00%, 96.97% and 97.49%. The 90% CI for the geometric means ratios (test/reference) of the log-trasformed Cmax, AUC0-t and AUC0-inf of risperidone and its major active metabolite were within the bioequivalence acceptance criteria of 80% - 125% of the US-FDA.

The starting point was essentially based upon clinical and/or haematological indications and it is emphasized that conclusions should be drawn in close dialogue with the clinician and experts in cytogenetics and histopathology. Reports made in the laboratory should be based upon an integration of cytomorphological, immunophenotypical and molecular data. These guidelines are not intended to be used as universal 'diagnostic pathways', but should be useful in developing local diagnostic pathways. It is well understood that this consensus, being valid anno 2009, may rapidly change with new technologies being introduced and new targets discovered.

A randomized, 2-treatment, 2-period, 2-sequence, single-dose, crossover study with a washout period of 3 weeks, was conducted in 24 healthy Thai male volunteers. Blood samples were collected at 0, 1, 2, 3, 4, 5, 6, 7, 8, 10, 12, 24, 36, 48, 72, 96 and 120 hours following drug administration. Plasma concentrations of sertraline were determined using validated LC-MS/MS method. Noncompartmental pharmacokinetics and statistical analyses were performed using SAS software for Windows, release 9.1 (SAS Institute Inc., Cary, NC, USA). The ratio of least square means and the 90% confidence intervals (CI) of the log-transformed data were 0.9950 (0.9111-1.0866) for Cmax, 1.0153 (0.9576-1.0764) for AUC(0-t) and 1.0110 (0.9510-1.0747) for AUC(0-infinity). In addition, the median tmax values for the test and reference formulations were similar (5.00 h). The 90% CI for Cmax, AUC(0-t) and AUC(0-infinity) were within the 0.8-1.25 interval of the US-FDA. The test formulation (Serlift, Ranbaxy Laboratories Ltd., Gurgaon, Haryana, India) is bioequivalent to the reference formulation (Zoloft, Pfizer Australia Pty Ltd., West Ryde, New South Wales, Australia) both in terms of rate and extent of absorption after single-dose administration under fasting condition.

However, POCUS examination in elderly populations has not been well studied. AIM: This was a retrospective observational study of elderly patients at the Department of Emergency Medicine who had received POCUS examination at a tertiary university hospital, Thailand. METHODS: The study was conducted throughout January 2020–December 2020. Patients’ characteristics and ultrasonography findings were recorded. RESULTS: A total of 191 elderly patients were evaluated. Median patient age was 75.65 years; 56.02% of patients were female. Chief complaints where POCUS scans were applied were respiratory (36.65%) and cardiovascular system-related (21.99%).
