BACKGROUND In the recent years, the number of studies exclusively examining epidemiologic pattern... more BACKGROUND In the recent years, the number of studies exclusively examining epidemiologic patterns of Upper Gastrointestinal Bleeding (UGIB) has been quite limited. However, most epidemiologic studies have shown a decrease in the incidence of all causes of upper gastrointestinal bleeding. Although, the incidences of peptic ulcers have remained unchanged. Gastrointestinal bleeding is a very common emergency accounting for 7-8% of acute medical admissions. UGIB is 4-5 times more common than the lower GI haemorrhage. Acute erosive gastritis is the most common cause followed by oesophageal varices, peptic ulcer and reflux oesophagitis. Upper GI bleed is more common in men than women (ratio 3:2) and the frequency increases with age. Hence, the present study was designed to study the aetiological profile of patients presenting with upper gastrointestinal bleeding. MATERIALS AND METHODS This one year cross-sectional study was conducted from January 2013 to December 2013. Sample size of 50 was considered. Patients aged 18 years and above presenting with upper gastrointestinal bleeding and who are fit for endoscopy were selected. Endoscopy was performed in all patients within 24 hrs. of admission and data was plotted in terms of rates, ratios and percentages and continuous data was expressed as mean ± standard deviation. RESULTS In the present study, most of the patients reported past history of liver disease and intake of NSAIDs and aspirins. In the present study, on clinical examination, most of the patients had pallor followed by tenderness. In the present study, the commonest diagnosis was cirrhosis of liver due to alcohol-induced with portal hypertension. CONCLUSION Upper GI endoscopy revealed varices as most common cause of upper gastrointestinal bleeding.

This study was aimed at assessing the value of preoperative forward ejection fraction (FEF) and c... more This study was aimed at assessing the value of preoperative forward ejection fraction (FEF) and corrected forward ejection fraction (CFEF) as an index of postoperative myocardial function. Utilizing 34 patients with mitral regurgitation, usefulness of the preoperative FEF and CFEF was evaluated with special reference to incidence of postoperative low output syndrome (LOS) and improvement in the New York Heart Association (NYHA) classification, comparing other indices of left ventricular function such as cardiac index (CI), stroke index (SI), left ventricular end-diastolic volume index (LVEDVI) and left ventricular end-systolic volume index (LVESVI). There was a significant correlation between FEF and CI, SI, LVEDVI or LVESVI, and between CFEF and CI, SI, LVEDVI or LVESVI. The FEF and CFEF, however, were better indicators for predicting the occurrence of postoperative LOS than EF. Postoperative improvements in the NYHA classification were parallel to that of the FEF and CFEF changes, but did not coincide with that of the EF. Based on these findings, it is concluded that FEF and CFEF are more useful indicators than EF for evaluation of left ventricular myocardial performance in patients with mitral regurgitation. forward ejection fraction ; corrected forward ejection fraction

Chemotherapy induced LV Dysfunction hinders the applicability of these agents. Anthracycline chemotherapy plays a major role in many cancer treatments, especially breast cancer. Cardiotoxicity is a major concern in this group as it can have early onset or late onset, clinically apparent or subclinical. Echocardiography is the non-invasive technique of choice in the imaging of patients before, during and after undergoing chemotherapy. Speckle tracking echocardiography gives an objective and qualitative assessment of LV function. This study was done to assess the risk factors associated with chemotherapy induced LV Dysfunction along with the feasibility of 2D speckle tracking strain imaging. Methods: 54 consecutive patients above the age of 18 years diagnosed to have breast cancer from November 2015 to October 2016 were included in this study. The patients were further divided into LV Dysfunction group and Non LV Dysfunction group at the end of the follow up. The baseline clinical, echocardiographic parameters and follow up parameters were compared in the 2 groups. Results: Anthracycline induced LV Dysfunction was found in 14.8% of the study population. Advanced age, low BMI, Diabetes, advanced cancer stage, number of chemotherapy cycles were all found to be associated with increased risk of developing chemotherapy induced LV dysfunction. 2D strain imaging was helpful in early detection of LV Dysfunction in 25% of the LV Dysfunction group. Among the patients who had LV Dysfunction at the end of the chemotherapy regimen, 75% had persistent LV Dysfunction during follow-up where as 25% recovered their LV Function. Conclusion: Chemotherapy induced LV Dysfunction remains a major public health concern and it is not uncommon. The development of LV Dysfunction in this group of patients should be carefully monitored by 2D strain imaging as it can help in detection of early onset of LV Dysfunction.
