Introduction: Optimal timing of cord clamping has long been a controversial issue. Recently many studies have shown that delayed cord clamping leads to remarkable decreases in risks of morbidity such as intraventricular haemorrhage, necrotising enterocolitis, hypoperfusion, need for transfusion and optimal haemoglobin haematocrit levels in preterm infants. Conclusion: Delayed clamping of the umbilical cord will ensure more positive outcomes in preterm neonates.

Background: Although the effects of cardiac troponin I (cTnI) have been documented in infants, th... more Background: Although the effects of cardiac troponin I (cTnI) have been documented in infants, the associations among the value of maternal and cord blood total homocysteine (Hcy) levels, cord blood cTnI and the score for neonatal acute physiology and perinatal extension II (SNAPPE-II) values in infants have not been documented. The aim of this study was to determine the value of maternal total Hcy (mtHcy) and cord blood total Hcy (ctHcy) levels, cTnI and SNAPPE-II as predictive factors of morbidity in newborns. Methods: Maternal and cord blood samples were routinely collected for analysis from all prospective participating volunteers. In this case-control study, both hospitalized (n = 71) and non-hospitalized (n = 148) newborns were identified and followed until discharge. Results: Regression analysis revealed that pre-eclampsia, gestational age, mtHcy and SNAPPE-II values were significantly and independently associated with morbidity. Cord blood pH, ctHcy levels, SNAPPE-II values, and pre-eclampsia were associated with mtHcy levels. A similar association was found among cTnI, cord blood pH, mtHcy levels and ctHcy levels. The specificity and sensitivity values of mtHcy in predicting newborn morbidity were 62% and 78%, respectively, while the specificity and sensitivity values of SNAPPE-II in predicting newborn morbidity were 96% and 97%, respectively. Conclusions: Elevated mtHcy levels were associated with cTnI, SNAPPE-II values, cord blood gas, and neonatal morbidities. These results suggest that SNAPPE-II may be an early predictor of morbidity after delivery and that elevated mtHcy levels may be an early prenatal biomarker of morbidity in newborns.

Aim: To determine the value of cord blood cardiac troponin I levels (cTnI) as an early prognostic... more Aim: To determine the value of cord blood cardiac troponin I levels (cTnI) as an early prognostic factor in critically ill newborns, and to compare cord cTnI levels with the prognostic value of the score for neonatal acute physiology (SNAP). Methods: Cord arterial samples were collected routinely for blood gas analysis, and cord venous samples for cTnI and cardiac-specific creatine kinase assay. The study group (ns109) comprised critically ill newborns who required mechanical ventilation. The control group (ns96) comprised newborns who were either completely healthy (ns48) or were followed in a level I neonatal care unit due to moderate-severity problems. Results: The critically ill newborns had significantly higher cTnI levels than control babies (median wmin-maxx 1.4 w0-13x vs. 0 w0-1.8x ng/mL, respectively; P-0.001). In critically ill newborns, non-survivors had significantly higher cTnI levels than survivors (median wmin-maxx 6.6 w1.3-13.0x vs. 1.3 w0-8.0x ng/mL, respectively; P-0.001). Receiver-operator curve analysis revealed that, compared with SNAP, cTnI was a more sensitive predictor of mortality in critically ill newborns (area under curves0.96; 95% CIs0.90-1.02). Conclusion: Significantly elevated cord cTnI may be a valuable predictor of mortality in critically ill newborns.

The position of the patient should be added to cuff length and width, use of inappropriate stethescopes, and other known technical factors that influence the indirect measurement of blood pressure. As previously noted, 2 however, Londe et al have continued to use the fifth Korotkoff sound to indicate the diastolic pressure. It might be of interest to repeat the study using the fourth Korotkoff sound, as recommended by the American Heart Association, to see whether measurement of that diastolic component is also affected.

What this study adds? Maternal vitamin D deficiency was found to be the most common cause of late neonatal hypocalcemia in our study. Due to the immaturity of regulating factors of parathormone and calcium, serum iPTH levels may not reach expected levels and serum phosphorus levels may remain high in vitamin D deficient neonates, posing a diagnostic dilemma by mimicking primary hypoparathyroidism.

Two days later, an acute, perforated, duodenal ulcer was detected in the patient. Surgery was performed, and the patient made an uncomplicated recovery. Peptic ulceration is underdiagnosed in children and this leads to delay in diagnosis and appropriate management. Peptic ulceration may occur during severe illness or viral infections, but perforation is rare.

In this paper, we report a 10-year-old boy with primary enuresis nocturna and attention deficit hyperactivity disorder who developed two episodes of acrocyanosis, the first of which was after imipramine treatment and the second after desipramine usage. We also review the possible pathogenesis of acrocyanosis. We suggest that physicians should be aware of this rare side effect of tricyclic antidepressants.

Due to the immaturity of regulating factors of parathormone and calcium, serum iPTH levels may not reach expected levels and serum phosphorus levels may remain high in vitamin D deficient neonates, posing a diagnostic dilemma by mimicking primary hypoparathyroidism.

Tek dozluk ve üç günlük tedavi grupları ile on günlük tedavi grupları karşılaştırıldıklarında, aralarındaki fark­ lar, mikroorganizmanın eliminasyonu ve rekürrensler yönünden önemli bulundu (p < 0.05).

Seventy-eight pregnant women and 65 infants who were followed up in various health centers were included in the sudy. 25-hydroxyvitamin-D (25-OHvitD), calcium (Ca), phosphorus (P) and alkaline phosphatase levels were measured in blood samples drawn from pregnant women in the last trimester. Two days later, an acute, perforated, duodenal ulcer was detected in the patient. Surgery was performed, and the patient made an uncomplicated recovery. Peptic ulceration is underdiagnosed in children and this leads to delay in diagnosis and appropriate management. Peptic ulceration may occur during severe illness or viral infections, but perforation is rare.

In this paper, we report a 10-year-old boy with primary enuresis nocturna and attention deficit hyperactivity disorder who developed two episodes of acrocyanosis, the first of which was after imipramine treatment and the second after desipramine usage. We also review the possible pathogenesis of acrocyanosis. We suggest that physicians should be aware of this rare side effect of tricyclic antidepressants.

Materials and Methods Newborns whose serum bilirubin levels were ⩾256 mmol (15 mg/dl) were enrolled. Sixty-five newborns, 37 neonates and 53 healthy neonates were included in the jaundice with unknown aetiologies group, with known aetiologies group and control group, respectively. Genomic DNA was isolated from blood samples with the use of a DNA isolation kit (Maxim Biotech Inc, San Fransisco, California, USA).
