Aim: We aim to determine the preoperative anxiety levels of patients with open-heart surgery and ... more Aim: We aim to determine the preoperative anxiety levels of patients with open-heart surgery and examine the relationship between anxiety levels and postoperative complications. Material and Methods: In our prospective study, 200 patients who will undergo open-heart surgery preoperative anxiety levels were measured using the Hamilton anxiety scale (HAM-A). Anxiety levels and the development of complications were investigated. Results: Preoperative anxiety was more common in females (p=0.001). The length of stay in the intensive care unit (p=0.006) and the hospital stay (p=0.005) were found to be longer in patients with high anxiety. It was observed that high preoperative anxiety severity increased the risk of developing postoperative complications (p=0.000024). Conclusion: We can say that preoperative anxiety, which is not considered very important, is seen more frequently than expected for patients who will undergo open-heart surgery. In our study, it was observed that high anxiety levels increased the risk of postoperative complications.
60-year-old male patient presented to the emergency clinic complaining of nausea, vomiting and ab... more 60-year-old male patient presented to the emergency clinic complaining of nausea, vomiting and abdominal pain with no remarkable medical history except hypertension and active smoking. The computed tomography (CT) revealed a Stanford Btype aortic dissection starting from the thoracic aorta to the iliac arteries and polycystic horseshoe kidneys with multiple cysts in the liver. Since polycystic horseshoe kidney and Stanford type B dissection of the thoracic aorta is rare, and no study showing coexistence in the literature was found. This is the first case report that reveals the association of these clinical findings and CT images in literature.

There are several reports on the application of variable degrees of vacuum pressure to hardshell ... more There are several reports on the application of variable degrees of vacuum pressure to hardshell venous reservoirs. The aim of the current study was to compare the hemolytic effects of vacuum-assisted venous drainage (VAVD) at two different vacuum levels with the classical gravity siphon method. A prospective, equally randomized (1: 1: 1), parallel group study was performed in elective coronary artery bypass grafting (CABG) operations. (n = 162) were divided into three groups: gravity siphon (group 1, n = 55), VAVD at -40 mmHg (group 2, n = 55) and VAVD at -80 mmHg (group 3, n = 52). Hemolysis tests were performed at 2, 24 and 48 h following the operations. There were no deaths in this study. Plasma-free hemoglobin (PfHb) levels showed a difference at 2 h (p < 0.001) compared to 24 h (p = 0.02) between the groups. Haptoglobin (Hp) levels also revealed hemolysis in groups 2 and 3 at all sampling times. Constant negative suction at -80 mmHg during elective coronary bypass operations caused more hemolysis. We do not recommend a constant suction of -80 mmHg for VAVD.

Hemorrhage is one of the major complications of cardiac surgery when using cardiopulmonary bypass... more Hemorrhage is one of the major complications of cardiac surgery when using cardiopulmonary bypass (CPB). Despite all kinds of medical and surgical treatment, post bypass bleeding is still a serious problem. Hypothermia, acidosis, hypovolemia, and hypoperfusion lead to disruption of the coagulation process in severe trauma as well as in CPB. Hyperfibrinolysis (HF) is a life-threatening bleeding disorder in major trauma patients. The incidence of HF has been estimated in the range of 15% to 20%. The gold standard for the detection of HF is thrombelastography or thromboelastometry. Blood products and tranexamic acid are used in the treatment of hyperfibrinolysis. The agents that can be used as an alternative to tranexamic acid in HF treatment are aprotinin and epsilon aminocaproic acid. Early diagnosis and treatment of HF are life-saving. For this purpose, we wanted to share this rare pathology.

Background/aim: Evaluate the risk factors associated with pseudoaneurysms' development after the ... more Background/aim: Evaluate the risk factors associated with pseudoaneurysms' development after the percutaneous interventional procedures performed by cardiology, interventional radiology (IR), and the other clinics. Materials and methods: We retrospectively analyzed the ultrasound scans in the hospital database and picture archiving system (PACS) and enrolled a total of 132 patients during the period from October 2015 and December 2019. We evaluated the maximum diameter and volume of the pseudoaneurysm with the patient and procedure-related factors with univariate analysis. Results: We found that the patients with hypertension and without peripheric artery disease (PAD) had greater sac diameter (p = 0.010 and p = 0.016) and increased sac volume (p = 0.029 and p = 0.007). However, the sac volume increased in patients with diabetes than those without (p = 0.003). Both the increased maximum diameter and the volume of the pseudoaneurysm sac were in the patients in whom the procedure was applied in the common femoral artery (CFA) and with the venous intervention (p < 0.010 and p < 0.016; p = 0.004 and p = 0.001, respectively). We found that platelet count correlated negatively with the sac's maximum diameter and the volume (r =-0.383, p < 0.001 and r =-0.486, p < 0.001, respectively) duration of intervention correlated positively with the sac's maximum diameter and the volume (r = 0.205, p = 0.019 and r = 0.320, p < 0.001). Conclusion: Our study reveals that prolonged procedure duration, simultaneous arterial and venous accesses, peripheral artery disease, thrombocytopenia, and puncture site are the aggressive risk factors of pseudoaneurysms size after angiographic procedures.

Coronary artery perforations and dissections, associated with cardiac tamponade or acute vessel c... more Coronary artery perforations and dissections, associated with cardiac tamponade or acute vessel closure, are life-threatening complications of percutaneous coronary intervention. In some cases, subepicardial hematomas could occur and compress the vessel. A 59-year-old woman was admitted to our hospital with chest pain and was diagnosed with non–ST-elevation myocardial infarction. Coronary angiography showed the total occlusion of the diagonal artery. During the intervention, left main coronary artery dissection and intramural hematoma occurred as coronary complications. The left main coronary artery was stented; however, the extension of the hematoma through the ostium of the left anterior descending artery caused further complications. The patient underwent an urgent coronary artery bypass graft surgery and was discharged on the seventh postoperative day.

Methods: Between January 2017 and January 2020, a total of 14 patients (12 males, 2 females; mean age: 66.6±8.4 years; range, 54 to 77 years) who underwent elective ascending aortic surgery without genetic disease or family history in the etiology were included in the study. Aortic tissues taken from the patients with ascending aortic aneurysms and cadavers without any aortic pathology were compared histopathologically and biomechanically.
