We compared the efficacy of remifentanil and the combination of alfentanil and sodium nitroprussi... more We compared the efficacy of remifentanil and the combination of alfentanil and sodium nitroprusside on controlled hypotension and bleeding at surgical site. Thirty ASA I-II patients undergoing tympanoplasty were randomly assigned to remifentanil and alfentanil-sodium nitroprusside groups, equal in number. Anesthesia was induced with remifentanil or alfentanil combined with propofol and rocuronium, and maintained with remifentanil or alfentanil-sodium nitroprusside infusions combined with propofol. All patients were ventilated with a mixture of 33% O2 and 66% N2O. Invasive mean arterial blood pressure, heart rate, peripheral oxygen saturation, the amount of bleeding at surgical site, and blood gas values were recorded. Controlled hypotension was achieved at a target mean arterial pressure of 60+/-5 mmHg in both groups. General anesthesia also has some disadvantages, especially with respect to airway control in patients with ankylosing spondylitis. We present herein a gravida with ankylosing spondylitis who had a cesarean delivery performed under spinal anesthesia. A 30-yr-old gravida at 38 weeks gestation with a 9 yr history of ankylosing spondylitis was admitted to our hospital in labor. She was scheduled for an elective repeat cesarean delivery. Spinal anesthesia was induced using a 22-gauge Quincke spinal needle with 1.8 mL of 0.5% heavy bupivacaine + 0.2 mL (10 microg) of fentanyl at the L3-4 interspace in the left lateral position by the median approach. Most pathologic signs can be attributed to compression effect on intraabdominal organs, vascular structures and ascites made by tumor. After removal of such huge pelvic masses, serious clinical hypotension and inferior vena cava (IVC) syndrome can be seen related to the aspiration of high levels of fluid or resection of bulky mass. Here, we present four cases of bulky ovarian masses by means of gynecologic surgery, anaesthesia and pathology. Case: 54, 35, 39, 42 years old patients having 20 kg and 40x20cm; 14 kg and 40x40x40 cm, 17 kg and 45x50 cm and 53x50 cm adnexal masses respectively were operated. Histopathologies were found each to be the first two borderline mucinous carcinomas, third one was benign endometrioid tumor and the fourth one was mucinous cystadenoma complexed with teratom structures. Objective: In our study we aimed to compare the maternal and neonatal effects of desflurane and s... more Objective: In our study we aimed to compare the maternal and neonatal effects of desflurane and sevoflurane in women undergoing elective caesarean section. Methods: After the approval from Ethics Committee of our Hospital, 40 healthy parturients undergoing elective caserean section were allocated randomly into two groups after their informed consent was taken. Desflurane 3% in Group D or sevoflurane 1% in Group S were used in a 50% nitrous oxide and oxygen mixture for maintenance of anaesthesia. Systolic, diastolic, mean blood pressure, heart rate values and peripheral oxygen saturation were recorded as baseline and followed up with 5 min intervals peroperatively. Methods: Sixty patients in ASA I-II physical status undergoing abdominal hysterectomy operation were randomly assigned to two groups as: planned to apply rocuronium 0.6 mg.kg-1 to extubate with clinical observation (Group R); atracurium 0.5 mg.kg-1 to extubate with clinical observation (Group A). At the end of the operation all patients were antagonized with neostigmine. TOF values at extubation and at 1., 3., 5., 10. minutes after extubation, operation and anaesthesia times, total fentanyl, neuromuscular blocker agent and neostigmine doses were recorded. Still it can cause cardiovascular changes like hypotantion and bradycardia. So unilateral anaesthesia is prefered in high risk patients for less hemodynamic instability. We aimed to share our unilateral spinal anaesthesia experience in an ASA IV E patient with ejection fraction 28%.
