Evidences imply that distinctive variants of multicyclic CHOP chemotherapy may improve the treatment outcome in intermediate grade NHL. The objective of this study was to scrutinize the spectrum of toxicities following six cycles of CHOP chemotherapy regimen in subjects with intermediate grade NHL and to assess the variation of the severity of these toxicities with each cycle of chemotherapy and grade them according to WHO guidelines. This prospective study was conducted on de novo intermediate grade NHL patients with performance status 0 to 3, aged >20<60 years, receiving CHOP chemotherapy regimen. Out of 30 patients who satisfied the inclusion criteria, only 25 patients completed 6 cycles of chemotherapy and were evaluated for various toxicities. The rising prevalence of the pandemic of obesity urges the need for newer and effective drugs in ... more The rising prevalence of the pandemic of obesity urges the need for newer and effective drugs in its management. The pharmacotherapy of obesity is burdened by limitations in the number of drugs, inefficacy and side effects of existing drugs and continuous withdrawal of marketed drugs. This article gives an overview of factors involved in the regulation of energy homeostasis and the occurrence of obesity by a presumable shift in the controlling pathways. There are various putative targets which are implicated in the pathogenesis of obesity and these are the potential targets of drugs used in the treatment. A better understanding of Pathophysiology of obesity and the current status of pharmacotherapy can open up new vistas in this promising and needful area of research.

Background: Pharmacovigilance (PV)/drug safety and rational prescribing are relevant to both prac... more Background: Pharmacovigilance (PV)/drug safety and rational prescribing are relevant to both practitioners and policy-makers but the competency of professionals is still insufficient. Aim and Objective: The purpose of the study was to evaluate the effectiveness of an interactive structured teaching, learning module on PV and rational prescription in enhancing the competency of medical undergraduates. The secondary objective was to assess the perceptions of students and faculty about the module. Materials and Methods: After ethical approval, sixty-four 2nd year M.B.B.S students in a tertiary care teaching hospital participated in an intervention study using a structured teaching-learning module. This interactive module was made according to the WHO good prescription guidelines. Students were enrolled by non-probability convenience sampling. After orientation about the study, informed consent was taken; and the students were then divided into groups. The control group was taught in conventional way and the interventional group with the module. Outcome of both groups was assessed by posttests conducted by validated questionnaire. The faculty and students feedback were collected using pre-validated questionnaire based on Likert scale. The mean scores and level of significance were assessed using independent sample t-test and P < 0.05 was considered as statistically significant. Results: The findings clearly indicate that teaching module was very effective. There was retention of the topic by the students evident from the scores in assessments. Faculty feedback showed increased satisfaction in teaching and increased understanding among students. Conclusions: The undergraduate medical students, knowledge of what drugs they have to prescribe to a patient does not necessarily translate to a rational prescription unless we give proper training to them. This module provides learners an opportunity to develop and practice skills that directly improve self-directed learning.

Under-reporting of Adverse Drug Reactions is a serious issue hampering the dynamics of Pharmacovigilance programme. Pharmacovigilance is a shared responsibility of all the stake holders. This study was mainly aimed to assess the Knowledge, Attitude and Perceptions of Health care Professionals towards Pharmacovigilance and estimate factors contributing to under reporting of Adverse Drug Reactions. This was a cross sectional observational questionnaire based study done in a tertiary care hospital in South India. Healthcare professionals who responded (71.4%) includes prescribers, nurses and pharmacists. There was an increase in the awareness and attitude of Health care professionals towards Pharmacovigilance. But inadequate training in methodology of reporting ADR was the main problem. There is a need to provide adequate basic training to all health care professionals by educational interventions through nearest Pharmacovigilance units. It should be of good quality as well as cost affordable.

The causes of drug resistances are varied. The primary resistance attributed to genomic instability while alteration in membrane permeability of drug and inability to reach the target sites, drug inactivation by enzymes, mutation and altered expression of target proteins there by affecting drug target interactions are some of the mechanism of acquired drug resistance. In addition to genetic changes there are epigenetic changes or non mutational mechanisms which occur quickly in response to environmental changes. Normal tissues never develop resistance to chemotherapy, because they possess intact genetic machinery. Obesity is a multi factorial disease which involves complex molecular pathways is basically due t... more Obesity is a multi factorial disease which involves complex molecular pathways is basically due to imbalance between energy intake and energy expenditure.Presumably other than genetic endowments, a series of biological events tip the fine balance of homeostatic mechanism of energy control and any shift in these regulatory pathways can result in obesity. Though the effect is not always predictable, once the homeostasis is altered it is very difficult to restore the balance. The primary regulating centre of energy intake is brain; the hypothalamus being the main integrating centre. The regulation of energy expenditure is still not clear. With very few pharmaceutical agents to curb this emerging public health problem, a better understanding of molecular mechanism of energy imbalance is well appreciated. This article gives an overview of molecular mechanism of patho physiology of obesity.

With the proven success among the novice and seniors, microteaching helps to promote real-time teaching experiences. The core skills of microteaching such as presentation and reinforcement skills help the novice teachers to learn the art of teaching at ease and to the maximum extent. The impact of this technique has been widely seen in various forms of education such as health sciences, life sciences, and other areas. The emerging changes in medical curricula by the Medical Council of India and the role of medical teachers envisage the need of this special training of teachers and monitoring of their skills for their continued efficient performance at any age. The alleged limitations of microteaching can be minimized by implementing this at the departmental level in several sequences. The author made literature search of research and review articles in various educational databases, journals, and books. From the reference list of published articles, books were also reviewed. This paper presents an outline of the various phases of microteaching, core teaching skills, implementation aspects, and the impact of microteaching on medical education.

Pharmacovigilance plays an imperative role in providing information about adverse drug reactions ... more Pharmacovigilance plays an imperative role in providing information about adverse drug reactions (ADRs) and drug safety in a hospital. Hence, it was considered worthwhile to study the surveillance of ADRs in inpatient departments (IPD). Our study aims to evaluate the frequency, type, characteristics, causality, and severity and avoidability of ADRs in the IPDs. This prospective observational study was done in the IPDs of a 550-bed tertiary care hospital in South India during the period April to September 2012. All the patients had experienced at least one suspected ADR after hospitalization and drug treatment; patients who are hospitalized due to suspected ADR(s) in the last 6 months were assessed after obtaining permission from the Institutional Research and Ethics Committee. All the ADRs were recorded on the appropriate, prescribed form for reporting ADRs designed by the Central Drugs Standard Control Organization. A total of 147 ADRs were reported with the use of 96 drugs used by 91 IPD patients. The maximum number of ADRs reported in our study was with antipsychotic drugs and the maximum number of patients reported rashes (n=30) followed by extrapyramidal symptoms (n=17). Causality assessment by the WHO scale and Naranjo&amp;#39;s algorithm showed 48 and 59 ADRs possibly related to drugs, respectively. Of this, 45 patients with ADR had moderate severity and 41 patients with ADRs were not preventable. The maximum number of ADRs reported in our study was with antipsychotic drugs and the most common ADR reported was rashes. Most of the ADRs reported had a possible causality, were moderate in severity, and were not preventable. The majority of patients had recovered from the ADRs.

The main objective of this article is to report a range of adverse effects ofCHOP regimen in patients with intermediate grade Non-Hodgkins Lymphoma (NHL). This randomized prospective study was conducted on de novo intermediate grade NHL patients, aged >20?60 years, receiving CHOP chemotherapyregimen. Out of 30 patients who satisfied the inclusion criteria, only 25 patients completed 6 cycles of chemotherapy and were evaluated for various toxicities. The main objective of this article is to report a range of adverse effects ofCHOP regimen in patients with intermediate grade Non-Hodgkins Lymphoma (NHL). This randomized prospective study was conducted on de novo intermediate grade NHL patients, aged >20?60 years, receiving CHOP chemotherapyregimen. Out of 30 patients who satisfied the inclusion criteria, only 25 patients completed 6 cycles of chemotherapy and were evaluated for various toxicities. Evaluation of all the 25 subjects showed that, in comparison with the baseline, most patients exhibited toxic damages.
