These pains may sometimes point to serious underlying conditions such as malignancies, infections or injuries. However, majority of the cases may be due to 'growing pains', that have a benign and self-limiting course [1]. Growing pains, though considered benign, can cause considerable anxiety in the parents. Sometimes, the child wakes up in the middle of night with extreme agony, complaining of severe pain in the legs. There are no symptoms in the morning and pediatrician finds no abnormality on physical examination [2]. The pediatrician may be in a dilemma; should parents be simply reassured or the child has to be investigated thoroughly? This article reviews the current knowledge regarding the diagnosis, etio-pathogenesis and management of this fairly common but perturbing condition. DIAGNOSIS Growing pains are typically intermittent, nocturnal and poorly localized, usually occurring once or twice per week-though there is never a regular pattern. The design and study of two classes of noncompetitive acetylcholinesterase inhibitors (AChEIs) wh... more The design and study of two classes of noncompetitive acetylcholinesterase inhibitors (AChEIs) which also function as NSAID prodrugs are reported. The most potent AChEIs disclosed contain an aromatic alkyl-aryl linker between an NSAID and a lipophilic choline mimic and they inhibit acetylcholinesterase (AChE) in the submicromolar range. These agents have the therapeutic potential to dually target inflammation by releasing an NSAID in vivo and activating the cholinergic anti-inflammatory pathway via cholinergic up-regulation.The design and synthesis of dual-action anti-inflammatory and anticholinergic agents are reported. Several of the compounds studied noncompetitively inhibit acetylcholinesterase (AChE) in the submicromolar range and release a pharmacologically active NSAID upon hydrolysis.

This chapter discusses the Junjappa-I1a (JI)-heteroaromatic annulation, which is a new general α-... more This chapter discusses the Junjappa-I1a (JI)-heteroaromatic annulation, which is a new general α-oxoketene dithioacetals mediated inverse method for the synthesis of benzo/condensed heterocycles and related heteroaromatization processes. In principle, a wide variety of five and six membered heterocycles bearing a ring C–C double bond with an exocyclic methyl group can be used as allyl anion precursors for benzoheterocycles. The α-oxoketene dithioacetals employed in this work as three carbon 1,3-biselectrophilic components are drawn from various active methylene ketones. The corresponding α-oxoketene N,S- and O,S-acetals which are usually derived from α-oxoketene dithioacetals are also described. The chapter also discusses the reaction of 3-Methyl-5-1ithiomethylisoxazole with α-oxoketene dithioacetals—a general method for the synthesis of substituted and fused 1,2-benzisoxazoles. It also discusses Reaction of 1-N-carboxy-2-1ithiomethylindole Dianion with α-oxoketene dithioacetals—a new method for the synthesis of 2,3- substituted and fused carbazoles. A reaction of 3-lithiomethyl-2-methyl-l-phenylpyrazolin-5-one with α-oxoketene dithioacetals—a new general method for the synthesis of 1,2-disubstituted indazolones and their condensed analogs is also discussed. The chapter also presents the reaction of 6-lithiomethyl-l,3-dimethyluracil with α-oxoketene dithioacetals—one pot new general synthesis of substituted quinazolines; reaction of 1-methylpyrrole-2-acetonitrile with α-oxoketene dithioacetals—new efficient synthesis of highly substituted indoles; reaction of indole-3-acetonitrile with α-oxoketene dithioacetals—a new efficient general method for the synthesis of 1,2- substituted and condensed carbazoles; and reaction of thiophene-2-acetonitrile 57 and thiophene-3-acetonitrile with α-oxoketene dithioacetals—an efficient new general synthesis of highly substituted benzo[b]thiophenes.
