Zusammenfassung
1. Häufigkeit und mikroskopische Anatomie der Coronarthrombose wurden bei 30 Infarktfällen und 28 akuten Herztodesfällen untersucht.
2. Bei 24 Infarktfällen (80%) und bei 12 akuten Herztodesfällen (40%) fanden sich obturierende oder stenosierende, bei je 4 weiteren Fällen flache parietale Thromben. Bei Infarktfällen überwiegen sekundär obturierende auf der Grundlage vorangegangener wandständiger Thromben (17 Fälle), bei akuten Herztodesfällen stenosierende parietale Thromben (8 Fälle).
3. Die verschließenden Thromben der Infarktfälle sind oft mindestens teilweise als typische Gerinnungsthromben fibrinreich.
4. Auf Grund der Sekundärveränderungen läßt sich das Alter der Thromben und Thrombusteile schätzen. Möglichkeiten und Grenzen dieser Methode werden aufgezeigt.
5. Protrahierter und rezidivierender Verlauf der Coronarthrombose ist häufiger als die Entwicklung in einem Schub. Die parietalen Thromben entstehen als Basis oft schon vor und die Verschlüsse nicht selten (vielleicht sogar bei einem Drittel der Fälle) erst nach dem_klinischen_ Infarktbeginn. Es ist zu vermuten, daß sich dann vielfach mit dem Verschluß auch die eigentliche Infarktnekrose erst_nach_ dem klinischen Infarktbeginn entwickelt (protrahierter Infarktverlauf bei progredienter Thrombose). Die antithrombotische Behandlung hat dann die Chance, den Verschluß und gleichzeitig den eigentlichen Infarkt zu verhindern.
6. Sekundäre Vergrößerungen der Thromben nach Infarktbeginn durch eine begrenzte proximale Apposition scheinen zwar relativ häufig zu sein (11 Beobachtungen). Ihre Bedeutung für die sekundäre Vergrößerung des Infarktes ist aber noch wenig geklärt.
Summary
Frequency and microscopic anatomy of coronary thrombosis were investigated in 30 cases of myocardial infarction and 28 cases of sudden heart death. In 24 cases of infarction (80%) and 12 cases of sudden heart death (40%) there were found occlusive or stenosing, in 4 further cases of both conditions flat mural thrombi. In cases of infarction there predominate secundary occlusions on the basis of a preceding mural thrombosis (17 cases), in cases of sudden heart deaths stenosing mural thrombi (8 cases) are prevailing. The occlusive thrombi of the infarction cases are at least partially consisting of a coagulated material rich in fibrin. The age of thrombi and parts of thrombi can be estimated on the basis of secundary changes. Possibilities and limitations of this method are demonstrated.
Coronary thrombosis develops more frequently in a protracted and recurring course than in a single phase. The mural thrombi as basis frequently arise before the clinical manifestation of infarction, whereas the occlusions appear in about one third of the cases only afterwards. It is supposed, that in the latter cases the real infarction necrosis also develops only after the clinical onset of infarction (“protracted course of infarction during progressive thrombosis”). This offers the chance to prevent the occlusion and at the same time the real infarction by antithrombotic therapy. Secundary enlargements of thrombi after the onset of infarction by means of a limited proximal apposition seem to be relatively frequent (11 cases). But their significance for the secundary enlargement of the anatomical infarction area is still uncertain.
