Background: Diagnosis of fibrosis on chronic hepatitis C still needs invasive method such as live... more Background: Diagnosis of fibrosis on chronic hepatitis C still needs invasive method such as liver biopsy. Aspartate Transaminase to Platelet Ratio Index (APRI) is a cheap and widely used assessment to indicate the presence of fibrosis in the liver. Insulin resistance as one an extra complication of chronic hepatitis C. We determined the correlation between degree of chronic hepatitis C severity based on APRI and the incidence of insulin resistance in chronic Hepatitis C patients. Methods: Degree of chronic hepatitis C severity was determined based on APRI and insulin resistance were determined using HOMA-IR. Correlation were analyzed using Chi-square and phi coefficient test, respectively. Results: Thirty chronic hepatitis C patients in this study consisted of in men was 16 male (53.3) and 14 female (46.7) patients with an average age of 51.63 ± 12.29. Frequency distributions of severity based on dominant APRI showed fibrosis in 18 patients (60) and no fibrosis in 12 patients (40). Insulin resistance distribution based on HOMA-IR showed no insulin resistance in 16 patients (53.3) and insulin resistance in 14 patients (46.7). There was a correlation between degree of chronic hepatitis C severity based on APRI and insulin resistance based on HOMA-IR with p = 0.006 and r = 0.491, respectively. Conclusion: There was a correlation between chronic hepatitis C severity based on APRI and insulin resistance based on HOMA-IR. © 2019, Yerevan State Medical University. All rights reserved.

Introduction: Tuberculous colitis and Crohn's disease (CD) are two disorders that possess similar... more Introduction: Tuberculous colitis and Crohn's disease (CD) are two disorders that possess similarities in clinical features and colonoscopy findings. Case presentation: The patient had previously undergone colonoscopy and mucosal biopsy in this case report, indicating chronic diarrhea and hematochezia. Based on the results from the colonoscopy, the patient was suspected for Crohn's disease. Meanwhile, the collected mucosal biopsy stained with Hematoxylin-Eosin suggested the early diagnosis of CD. Hence, patient was prescribed with sulfasalazine 2x500 mg for 6 weeks, where no clinical improvement was found afterward. Patients still complained nausea, vomiting, frequent diarrhea and bloody stools. The patient was later suspected of intestinal tuberculous and prepped for interferon-gamma release assay (IGRA) and stool Xpert MTB/RIF test. Both of the additional tests suggested that the patient was positive tuberculous. According to the collective facts and findings, the patient was diagnosed with intestinal tuberculous and prescribed with category 1 anti-tuberculosis. Conclusion: The patient was followed-up after nine months treated with anti-tuberculosis, where the negative result was obtained from the stool Xpert MTB/RIF test. In the following year after the therapy, the patient felt no complaints.

Inflammatory bowel disease (IBD) with a poor prognosis may be due to persistent colitis. Accordin... more Inflammatory bowel disease (IBD) with a poor prognosis may be due to persistent colitis. According to the latest guidelines, monitoring has become a part of the treatment process for colitis. Adequate monitoring of the patient's condition is necessary to determine the course of the disease to prevent the worsening of the condition and suppress the subclinical inflammatory process. This analytical study with a cross-sectional design was conducted to evaluate the activity of colitis using the results of C-reactive protein (CRP) and fecal calprotectin (FC) assays. FC levels were analyzed by ELISA, while CRP levels were analyzed using Siemens Flex particle-enhanced turbidimetric immunoassay. In 30 subjects with endoscopy and biopsy of colitis, 16 men and 14 women had a median age of 52.5 (18–70) years. Background Gastrointestinal manifestations of coronavirus disease 2019 (COVID-19) appear to be su... more Background Gastrointestinal manifestations of coronavirus disease 2019 (COVID-19) appear to be substantial. Fecal calprotectin is a promising biomarker in COVID-19 associated gastrointestinal inflammation; however, its role in the severity of COVID-19 remains limited. We conducted a study to analyze the relationship between the severity of COVID-19 and hypoxic intestinal damage. Methods We assessed the severity of 44 hospitalized COVID-19 pneumonia patients based on the PaO2/FiO2 (P/F) ratio. Inflammatory markers were measured from blood samples, and fecal calprotectin was obtained from stool samples. Crohn’s disease (CD) is a chronic debilitating inflammatory disease which mostly affect gastroint... more Crohn’s disease (CD) is a chronic debilitating inflammatory disease which mostly affect gastrointestinal tract, but due to its unique features, CD enables to affect extraintestinal organs. Pathophysiology of extraintestinal manifestations is still debatable as many experts propose immune-related hypotheses. It is still unpredictable which manifestation precedes another as studies ongoing. Diagnosing CD is difficult since no gold standards available, therefore clinicians must combine history taking, diagnostic modalities, and a good clinical judgement to diagnose CD. Treatment for CD is not only to treat disease activity, but also to prevent complications to preserve patients’ quality of life.

Over the past decade, significant advances have been made in the noninvasive assessment of liver ... more Over the past decade, significant advances have been made in the noninvasive assessment of liver fibrosis in patients with chronic liver disease. Transient elastography appears to be excellent in assessing liver fibrosis. For the interpretation of liver stiffness measurements, the doctor should know the disease clinically, biologically and morphologically and its parameters. In chronic liver disease, especially in chronic hepatitis C, the value of liver stiffness is strongly correlated with fibrosis stage according to the histology score. Patients with similar fibrosis but high alanine aminotransferase levels tend to have higher liver stiffness values, especially in chronic hepatitis B, and diagnostic performance for low-stage fibrosis can be affected when ALT is elevated. Transient elastography is an excellent tool for early detection of cirrhosis. Although TE cannot completely fulfill the need for liver biopsy, it can be used as an important noninvasive tool that allows the setting up of more efficient and custom management strategies for patients with chronic liver disease.
