Background: Anxiety disorders are among the most common mental health concerns in people's lifest... more Background: Anxiety disorders are among the most common mental health concerns in people's lifestyles. There are many traditional psychological methods to manage anxiety. Heart rate is the most relevant biomarker of the autonomic nervous system among the various physiological markers. Hence, the increase in the heart rate is associated with a wide range of negative psychological events. Objectives: In this study, a wearable device called a "smart bracelet" was developed and tested to make mechanical oscillations similar to normal people's heart rates, and then the effect of it was evaluated among taekwondo athletes wearing this device before the matches. Methods: In order to evaluate the results, the heart rate was measured five minutes before the competition, with an average of 81 bpm in the group using the bracelet and 84.42 bpm in the group without the bracelet. The average heart rate in the group with the bracelet was 78.5, and without a bracelet was 92.14 bpm. The results of this study showed that wearable technology inducing mechanical waves is similar to normal people's heartbeat through the radial nerve, which reduces stress significantly and can prevent an increase in stress.

Case Presentation: This study aimed to report a 74-year-old man showing gradually progressive deterioration in his memory for five years. The patient developed trichotillomania (TTM) subsequent to his dementia. Neuropsychological examination indicated the deficits to be more predominantly in the frontal lobe. Conclusions: This study reviewed the literature on TTM in dementia case reports that had mostly investigated the cases of right-handed men aged > 65 years. TTM Patients with underlying disease had not any improvement. Although there was some heterogeneous evidence for the presence of brain abnormalities in individuals with hair-pulling behavior, no definitive conclusion was drawn. Beta-thalassemia major (TM), a chronic, genetically determined hematological disorder, has receiv... more Beta-thalassemia major (TM), a chronic, genetically determined hematological disorder, has received little investigation on the psychological aspects of the disease and the psychosocial adjustment of patients with this anemia. In the present study, the aim was to explore the nature of psychopathology according to age, sex, school performance, severity and complications of the disease in TM patients. Methods and Materials: A Cross-section descriptive study was conducted at the 14-Thalassemia Unit of different hospitals of Mazandaran province Iran in 2006,. Psychological aspects were evaluated by the Persian version of symptoms checklist-90-revised questionnaire. Information on relevant demographic characteristics, school performance, severity and complications of the disease was collected by one of the investigators who created the questionnaire. Analysis of data was done by Chi square method, t test and one way ANOVA. Results: 687 (41.08%) of the 1672 eligible patients with TM completed the questionnaires. The mean age of the participations was 21.93±5.90 years and with a range of 15-45 years. 363 (52.8%) were female, 602 (6.87%) were single, 465 (67.7%) had not higher educational level. In our study, 446 (64.9%) of TM patients had GSI > 0.7 as psychiatric patients, 141 (20.5%) had GSI between 0.4 to 0.7 as suspected psychiatric patients, and 100 (14.6%) had GSI < 0.4 as non-psychiatric patients. Conclusion: These findings show that beta-thalassemia major patients are at risk for psychiatric symptomatology and need appropriated psychiatric consults.

Medication induced hair loss is an occasional side effect of many psychopharmaceuticals. It can reduce medication compliance if not discovered and treated. We present a 18 year old female with migraine headache who developed hair loss after 3 months of receiving topiramate treatment. CASE REPORT: 18 year old female had been suffering from headache visited in psychiatric clinic. She agreed to a treatment with topiramate (50mg per day) for Migraine headache. 3 months later, the patient complained of significant hair loss. Topiramate tapered to 25 mg/day and stopped. Hair loss stopped after topiramate withdrawal. Two weeks after reintroduction of topiramate, hair loss developed again. The medication was stopped and hair loss stopped again. Fluoxetine is a Selective Serotonin Reuptake Inhibitor (SSRI) that exerts its anti-depressive eff... more Fluoxetine is a Selective Serotonin Reuptake Inhibitor (SSRI) that exerts its anti-depressive effect by blocking the presynaptic reuptake of the neurotransmitter serotonin, 5-hydroxytryptamine (5-HT). Although fluoxetine is usually considered safe for most patients, in the present case report, we describe a young patient with Mixed Anxiety and Depression Disorder (MADD) treated with fluoxetine 10 mg/day, who developed hypotension when the dosage was titrated up to 20 mg/day. After discontinuing the use of fluoxetine, the symptoms of hypotension improved. A temporal association and dose-dependent relationship between the hypotension and the use of fluoxetine was observed. To the best of our knowledge, this is the first case report that precisely associates regular doses of fluoxetine with the presence of hypotension. BACKGROUND Fathers' depression is an understudied subject, and the increasing participation o... more BACKGROUND Fathers' depression is an understudied subject, and the increasing participation of fathers on the care of their children, this is an issue of great importance. This study aimed to determine the relationship of paternal postpartum depression with prenatal and postpartum depression of mothers and their marital satisfaction. METHODS This longitudinal study was conducted among 352 expecting couples in 28-40 weeks of pregnancy and 6-8 weeks after delivery from April to September 2019. Eligible participants were selected from primary health care centers in northern Iran using a two-stage sampling method. Socio-demographic information, Edinburgh Postpartum Depression Scale, and ENRICH Couple Scale were used for data collection. The primary basis of data analysis was cross-lagged structural equation modeling to explore the underlying mechanism for paternal postpartum depression. RESULTS The results showed that a) maternal prenatal depression indirectly (βstand= 0.32, p =0.004); b) maternal postpartum depression directly (βstand = 0.56, p <0.001); c) paternal prenatal depression indirectly (βstand= 0.11, p =0.028) were associated with paternal postpartum depression. Also, marital satisfaction directly (βstand= -0.19, p =0.002) and indirectly (βstand= -0.11, p =0.007) had a relationship to paternal postpartum depression. LIMITATIONS Culturally-sensitive measures of marital satisfaction, especially in conservative context of developing countries, along with self-reported data of psychological problems may lead to under-reported findings. CONCLUSION The major contribution of marital satisfaction and maternal depression during prenatal period on the paternal postpartum depression, emphasizes on the prenatal period as the ideal time for fathers' mental health improvement.

BACKGROUND AND OBJECTIVE: Obsessive compulsive disorder (OCD) and bipolar disorder are common psy... more BACKGROUND AND OBJECTIVE: Obsessive compulsive disorder (OCD) and bipolar disorder are common psychiatric illnesses and there are many challenges in pathology and their treatment. In recent years, the role of opioid system in the pathology of these disorders has been considered. In this report, we present a case with simultaneous episodes of hypomania and OCD following opium use. CASE REPORT: The 65-year-old man, who was suffering from irritability, talkativeness, over spending and frequent washing following use of opium, visited in psychiatric clinic. After administration of 30 mg of methadone every day, all her symptoms improved. During 12 years of follow-up, we observed that her symptoms recurred when the patient was taking opium again. Few days after repeat prescription of methadone, these symptoms subsided. CONCLUSION: The findings of this study emphasize the role of opioid system in the pathology of obsessivecompulsive disorder and bipolar disorder, at least when there are comorbid.

The national survey was conducted on 30,532 children and adolescents aged 6-18 years. The psychiatric disorders were assessed by employing k-SADS-PL questionnaire and interviews carried out by 240 clinical psychologists with the participants and their parents. Data were analyzed by Chi-square test and logistic regression.Results: A total of 277 (0.97%) were diagnosed as people with substance use disorder which were further studied for comorbid disorders. Background: Zolpidem is a non-benzodiazepine drug, approved by FDA for sleep induction. Zolpidem ... more Background: Zolpidem is a non-benzodiazepine drug, approved by FDA for sleep induction. Zolpidem is thought to be a safer drug than benzodiazepines (BZD) because of no evidence of abuse or dependence potential, but several case reports of zolpidem abuse and dependence have been published along with a small number of cases demonstrating seizures after sudden zolpidem withdrawal. Case presentation: A 32-year-old unmarried woman suffering from major depressive disorder had been taking zolpidem for insomnia for more than 1 year. She began to take zolpidem alone without mixing other kinds of hypnotics, and 50 mg of zolpidem used to be initially effective in treating her insomnia. In some days the dose increased up to 100 mg per day. In the end, she had to discontinue zolpidem abruptly because she could not afford it anymore. After 2 days, she suddenly showed facial spasm, mouth opening, tonic-clonic seizure, and loss of consciousness for about 1-2 minutes. Post-ictal confusion with clouded consciousness, psycho-motor retardation, persisted in 1 day. EEG in wakefulness revealed intermittent, generalized, diffused alpha wave and diffused sharp waves, and suggested seizure waves in the patient. Conclusion: Our case suggested that the potential of zolpidem dependence and withdrawal seizure are also present in the Iranian population. The female-gender, high dosage and longterm use of zolpidem might be risk factors for the development of adverse effects.

According to previous epidemics, an increase in post-traumatic stress disorder (PTSD) has been seen until one year later. Objectives: Due to the importance of psychological issues secondary to COVID-19, in this study, the frequency of PTSD symptoms in the follow-up of patients with COVID-19 who were hospitalized in Sari was evaluated. Methods: In this cross-sectional study, 199 patients diagnosed with COVID-19 who were hospitalized in Sari were identified, and patients’ records were recorded. PTSD criteria were assessed based on the PTSD Checklist for DSM 5 (PCL-5). Data were analyzed using SPSS 16. Results: In the present study, the overall prevalence of PTSD was 19.1% (38 patients). Aim: This study was aimed to determine the relationship between sleep quality and mental health o... more Aim: This study was aimed to determine the relationship between sleep quality and mental health of veterans in comparison with that of their spouses and a control group. Methods: Thirty-seven chemical veterans with moderate to severe injury based on the standards of Foundation of Martyrs and Veterans Affairs were selected from Mazandaran province. Then, the spouses of 31 of them were selected and 15 of their close relatives also enrolled in the study as the control group. The three groups were evaluated by GHQ for their mental health and Pittsburg Sleep Quality Index (PSQI) for sleep quality. Collected data were analyzed using SPSS software 16 and descriptive and analytic statistical methods. Results: The mean score of mental health in veterans, their spouses and close relatives were 44.13 ± 14.4, 34.19 ± 15.2 and 21.73 ± 17.32, respectively. The mean scores of PSQI test in veterans, their spouses, and their close relatives were 10.94 ± 5.6, 8.7 ± 5.5 and 4.27 ± 1 that the difference was statistically significant among the three groups (P ≤ 0.0001). A positive relationship was seen between mental health and sleep quality in veterans and their spouses respectively (r = 0.4, P = 0.02) and (r = 0.83, P < 0.0001). Conclusion: Poor sleep quality in chemical veterans compared to their spouses and close relatives could be due to mental health, rather than chemical effects and chronic lung disease. Therefore, treatment of mental health might be an important step to improve the sleep quality of veterans.

A short form of the general health questionnaire (GHQ-12) has been widely used as an assessment tool of general and mental health. This study was designed to evaluate the properties of the Persian version of this questionnaire. Of 6140 subjects aged 10–90 years of a cohort study of Northern Iran, the relevant data of 5074 individuals aged ≥18 years were considered for analysis. Confirmatory factor analysis was conducted to evaluate the six models including one-dimensional model, one-dimensional model with correlated errors on negative items, two-dimensional model based on negative and positive statement items, two-dimensional model based on social dysfunction and psychological distress, three-dimensional model based on anxiety/depression, social dysfunction and loss of confidence and finally one-dimensional model with an additional factor for the negatively phrased items. The best fit indices belonged to one-dimensional model with correlated errors on negative items with χ2 = 686.8, df = 39, CFI = 0.953, RMSEA = 0.057 (95%CI: 0.054–0.061), SRMR = 0.0317, AGFI = 0.952, TLI = 0.920 and AIC = 764.8 and also three-dimensional model with χ2 = 860.7, df = 51, CFI = 0.941, RMSEA = 0.056 (95%CI: 0.053–0.059), SRMR = 0.0343, AGFI = 0.955, TLI = 0.923 and AIC = 915.0. In the comparison between these two best models we had ∆χ2 = 174.2, ∆df = 12, p-value < 0.001 in favor of the one-dimensional model. One-dimensional model with correlated errors on negative items showed the best fitting indices with our population.

MS with episode of psychosis is a rare entity, and to the best of our knowledge, no case has been reported from Iran till date. We report a case of MS with first-episode psychosis in a 27-year-old single man with no history of psychiatric disorder or drug abuse. The patient developed neurological symptoms after 3 months and was finally diagnosed as a case of MS. His symptoms started with behavioral dysfunctions and progressively resulted in depression. Subsequently, treatment was performed with citalopram 20 mg daily, risperidone 2 mg three times a day, and biperiden 2 mg three times a day; however, no improvements in the symptoms were observed. The mean age of the participations was 21.93±5.90 years and with a range of 15-45 years. 363 (52.8%) were female, 602 (6.87%) were single, 465 (67.7%) had not higher educational level. In our study, 446 (64.9%) of TM patients had GSI > 0.7 as psychiatric patients, 141 (20.5%) had GSI between 0.4 to 0.7 as suspected psychiatric patients, and 100 (14.6%) had GSI < 0.4 as non-psychiatric patients. Conclusion: These findings show that beta-thalassemia major patients are at risk for psychiatric symptomatology and need appropriated psychiatric consults.

We present a 18 year old female with migraine headache who developed hair loss after 3 months of receiving topiramate treatment. CASE REPORT: 18 year old female had been suffering from headache visited in psychiatric clinic. She agreed to a treatment with topiramate (50mg per day) for Migraine headache. 3 months later, the patient complained of significant hair loss. Topiramate tapered to 25 mg/day and stopped. Hair loss stopped after topiramate withdrawal. Two weeks after reintroduction of topiramate, hair loss developed again. The medication was stopped and hair loss stopped again. Although fluoxetine is usually considered safe for most patients, in the present case report, we describe a young patient with Mixed Anxiety and Depression Disorder (MADD) treated with fluoxetine 10 mg/day, who developed hypotension when the dosage was titrated up to 20 mg/day. After discontinuing the use of fluoxetine, the symptoms of hypotension improved. A temporal association and dose-dependent relationship between the hypotension and the use of fluoxetine was observed. BACKGROUND Fathers' depression is an understudied subject, and the increasing participation o... more BACKGROUND Fathers' depression is an understudied subject, and the increasing participation of fathers on the care of their children, this is an issue of great importance. This study aimed to determine the relationship of paternal postpartum depression with prenatal and postpartum depression of mothers and their marital satisfaction. METHODS This longitudinal study was conducted among 352 expecting couples in 28-40 weeks of pregnancy and 6-8 weeks after delivery from April to September 2019. Eligible participants were selected from primary health care centers in northern Iran using a two-stage sampling method. Socio-demographic information, Edinburgh Postpartum Depression Scale, and ENRICH Couple Scale were used for data collection. The primary basis of data analysis was cross-lagged structural equation modeling to explore the underlying mechanism for paternal postpartum depression. RESULTS The results showed that a) maternal prenatal depression indirectly (βstand= 0.32, p =0.004); b) maternal postpartum depression directly (βstand = 0.56, p <0.001); c) paternal prenatal depression indirectly (βstand= 0.11, p =0.028) were associated with paternal postpartum depression. Also, marital satisfaction directly (βstand= -0.19, p =0.002) and indirectly (βstand= -0.11, p =0.007) had a relationship to paternal postpartum depression. LIMITATIONS Culturally-sensitive measures of marital satisfaction, especially in conservative context of developing countries, along with self-reported data of psychological problems may lead to under-reported findings. CONCLUSION The major contribution of marital satisfaction and maternal depression during prenatal period on the paternal postpartum depression, emphasizes on the prenatal period as the ideal time for fathers' mental health improvement.

In recent years, the role of opioid system in the pathology of these disorders has been considered. In this report, we present a case with simultaneous episodes of hypomania and OCD following opium use. CASE REPORT: The 65-year-old man, who was suffering from irritability, talkativeness, over spending and frequent washing following use of opium, visited in psychiatric clinic. After administration of 30 mg of methadone every day, all her symptoms improved. During 12 years of follow-up, we observed that her symptoms recurred when the patient was taking opium again. Few days after repeat prescription of methadone, these symptoms subsided. CONCLUSION: The findings of this study emphasize the role of opioid system in the pathology of obsessivecompulsive disorder and bipolar disorder, at least when there are comorbid.

Zolpidem is thought to be a safer drug than benzodiazepines (BZD) because of no evidence of abuse or dependence potential, but several case reports of zolpidem abuse and dependence have been published along with a small number of cases demonstrating seizures after sudden zolpidem withdrawal. Case presentation: A 32-year-old unmarried woman suffering from major depressive disorder had been taking zolpidem for insomnia for more than 1 year. She began to take zolpidem alone without mixing other kinds of hypnotics, and 50 mg of zolpidem used to be initially effective in treating her insomnia. In some days the dose increased up to 100 mg per day. In the end, she had to discontinue zolpidem abruptly because she could not afford it anymore. After 2 days, she suddenly showed facial spasm, mouth opening, tonic-clonic seizure, and loss of consciousness for about 1-2 minutes. Post-ictal confusion with clouded consciousness, psycho-motor retardation, persisted in 1 day. EEG in wakefulness revealed intermittent, generalized, diffused alpha wave and diffused sharp waves, and suggested seizure waves in the patient. Conclusion: Our case suggested that the potential of zolpidem dependence and withdrawal seizure are also present in the Iranian population. The female-gender, high dosage and longterm use of zolpidem might be risk factors for the development of adverse effects.

Methods: In this cross-sectional study, 199 patients diagnosed with COVID-19 who were hospitalized in Sari were identified, and patients’ records were recorded. PTSD criteria were assessed based on the PTSD Checklist for DSM 5 (PCL-5). Data were analyzed using SPSS 16. Results: In the present study, the overall prevalence of PTSD was 19.1% (38 patients). Aim: This study was aimed to determine the relationship between sleep quality and mental health o... more Aim: This study was aimed to determine the relationship between sleep quality and mental health of veterans in comparison with that of their spouses and a control group. Methods: Thirty-seven chemical veterans with moderate to severe injury based on the standards of Foundation of Martyrs and Veterans Affairs were selected from Mazandaran province. Then, the spouses of 31 of them were selected and 15 of their close relatives also enrolled in the study as the control group. The three groups were evaluated by GHQ for their mental health and Pittsburg Sleep Quality Index (PSQI) for sleep quality. Collected data were analyzed using SPSS software 16 and descriptive and analytic statistical methods. Results: The mean score of mental health in veterans, their spouses and close relatives were 44.13 ± 14.4, 34.19 ± 15.2 and 21.73 ± 17.32, respectively. The mean scores of PSQI test in veterans, their spouses, and their close relatives were 10.94 ± 5.6, 8.7 ± 5.5 and 4.27 ± 1 that the difference was statistically significant among the three groups (P ≤ 0.0001). A positive relationship was seen between mental health and sleep quality in veterans and their spouses respectively (r = 0.4, P = 0.02) and (r = 0.83, P < 0.0001). Conclusion: Poor sleep quality in chemical veterans compared to their spouses and close relatives could be due to mental health, rather than chemical effects and chronic lung disease. Therefore, treatment of mental health might be an important step to improve the sleep quality of veterans.

A short form of the general health questionnaire (GHQ-12) has been widely used as an assessment tool of general and mental health. This study was designed to evaluate the properties of the Persian version of this questionnaire. Of 6140 subjects aged 10–90 years of a cohort study of Northern Iran, the relevant data of 5074 individuals aged ≥18 years were considered for analysis. Confirmatory factor analysis was conducted to evaluate the six models including one-dimensional model, one-dimensional model with correlated errors on negative items, two-dimensional model based on negative and positive statement items, two-dimensional model based on social dysfunction and psychological distress, three-dimensional model based on anxiety/depression, social dysfunction and loss of confidence and finally one-dimensional model with an additional factor for the negatively phrased items. The best fit indices belonged to one-dimensional model with correlated errors on negative items with χ2 = 686.8, df = 39, CFI = 0.953, RMSEA = 0.057 (95%CI: 0.054–0.061), SRMR = 0.0317, AGFI = 0.952, TLI = 0.920 and AIC = 764.8 and also three-dimensional model with χ2 = 860.7, df = 51, CFI = 0.941, RMSEA = 0.056 (95%CI: 0.053–0.059), SRMR = 0.0343, AGFI = 0.955, TLI = 0.923 and AIC = 915.0. In the comparison between these two best models we had ∆χ2 = 174.2, ∆df = 12, p-value < 0.001 in favor of the one-dimensional model. One-dimensional model with correlated errors on negative items showed the best fitting indices with our population.

MS with episode of psychosis is a rare entity, and to the best of our knowledge, no case has been reported from Iran till date. We report a case of MS with first-episode psychosis in a 27-year-old single man with no history of psychiatric disorder or drug abuse. The patient developed neurological symptoms after 3 months and was finally diagnosed as a case of MS. His symptoms started with behavioral dysfunctions and progressively resulted in depression. Subsequently, treatment was performed with citalopram 20 mg daily, risperidone 2 mg three times a day, and biperiden 2 mg three times a day; however, no improvements in the symptoms were observed.
